We wish to predict when an accident or tragedy will occur, and reduce the probability of its occurrence. Maritime accidents, just like all the other crashes and failures, are stochastic in their occurrence. They can seemingly occur as observed outcomes at any instant, without warning. They are due to a combination of human and technological system failures, working together in totally unexpected and/or undetected ways, occurring at some random moment. Massive show the cause is due to an unexpected combination or sequence of human, management, operational, design and training mistakes. Once we know what happened, we can fix the engineering or design failures, and try to obviate the human ones. We utilize reliability theory applied to humans, and show how the events rates and probability in shipping is related to other industries and events through the human involvement. We examine and apply the learning hypothesis to shipping losses and other events at sea, including example Case Studies stretching over some 200 years of: (a) merchant and fishing vessels; (b) oil spills and injuries in off-shore facilities; and (c) insurance claims, inspection rules and premiums. These include major losses and sinkings as well as the more everyday events and injuries. By using good practices and achieving a true learning environment, we can effectively defer the chance of an accident, but not indefinitely. Moreover, by watching our experience and monitoring our rate, understand and predict when we are climbing up the curve. Comparisons of the theory to all available human error data show a reasonable level of accord with the learning hypothesis. The results clearly demonstrate that the loss (human error) probability is dynamic, and may be predicted using the learning hypothesis. The future probability estimate is derivable from its unchanged prior value, based on learning, and thus the past frequency predicts the future probability. The implications for maritime activities is discussed and related to the latest work on managing risk, and the analysis of trends and safety indicators.

Celem jest minimalizowanie strat, które mogą wyniknąć z nieprzewidzianych zdarzeń.
This article discusses the importance of risk management and transportation efficiency in the supply chain. The supply chain involves multiple stages from planning and purchasing raw materials, through production, warehousing, transportation, distribution, and delivery to the end customer. Nowadays, supply chains have become increasingly complex and global, leading to an increased risk of unforeseen events. Managing risk in the supply chain involves identifying, assessing, controlling, and monitoring risk at each stage of the process. The goal is to minimize losses that may arise from unforeseen events.

The aim of the article is to present the important issues of managing occupational hazards in the production process for a selected company. To test the indicated area, the authors of the article used a method of analysis and causes of dangerous situations (Ishikawa diagram) and the method of analysis of manual workers (Risk Score). Analytical methods applied can be helpful in increasing awareness of safety among workers.

Opis:
The article presents some questions on bank systems in such high developed countries like Germany or the United States of America. The following problems are described:1. organization and main functions of a bank system2. management of a bank risk3. evolution of financial innovations as security techniques4. American and German bank systems' profiles The author has choosen the subjects in order to present the following message: some Polish legal regulations on financial sector functioning are still strongly connected with the old, central and bureaucratic system and even despite the new Act on Polish National Bank and another one on Bank Law it is difficult to develop our financial system in a proper way. It seems to be clear that Polish bank law must be changed. But the question is: whetherthe present law can be modified or, maybe, it is necessary to look for a completly new one. The author suggest that it could be based on one of European countries bank law. French, German or English models of bank system could be adjusted to Polish conditions. On the other hand, the Polish bank law of the interwar period could be also modified and brought closer to the present time. The author studies the experiences and directions of evolution that have been existing so far and he tries to find the answer on the question:: which model of a bank system would be the best for Poland. The bank and economic literature describe the following three basic types of bank systems existing in the world:1. English and American model2. German and Japanese one3. French one that draws from the both mentioned above Knowing the advantages and disadvantages of all of them, it seems that adoption of the German model would be the best solution for our country. It is becouse of tradition (in the iterwar period our bank system was modeled on a German one), direct neighbourhood and the fact that Germany will be our greatest economic partner in the near future. However, we can observe, that European bank systems areovercome by American models determining the standards of contemporary banking in the whole world. So, Polish bank sector must be sensitive to any institutional innovations of American banks. It is also clear that: “The chance for introducing the German model of bank system in Poland is hardly possible without the new, well-trained staff. If we do not choose the target financial system knowingly, it will be automatically pushed to the French model” (J. K. Solarz, Searching for a new model of Polish bank system, Warsaw, September 1994).

Opis:
The aim of this publication is to determine the correlates of creative and transgressive behavior in the context of the model of the determinants of creative management. The aim of this study was to verify empirically the relationship between the symptoms of creative features (cognitively and characterlogically) and indicators of transgressive behavior. The determinants of subjective qualities of creative and transgressive behavior are analyzed in this article. The author has adopted the assumption about a role of transgressive behaviors in dynamics of creative behaviors (creative resources). The following set of methods has been applied in the research: 1) Questionnaire of Creative Behavior KANH (Popek, 2001; Popek, 2010); 2) Scale to Measure Transgression (STr), (Studenski, 2006). Activity, predominance, courage and high selfesteem – main indicators of functioning a man with a high level of subject creative features – have proved to be significantly correlated with transgressive indicators of behavior. Subject of creative features expressed in a creative attitude coexists with transgressive behaviors: pro-creative motivation in searching for changes, nonconformism, focus on activity and overcoming problems, openness and courage to take up new tasks, innovation and acceptance of novelty. The resulting configurations of features in a range of variables (characteristics of creative and transgressive behavior) correspond to factors conditioning the success in business and management. To a little extent the previous studies took into account the subjective features recognized as creative (in the cognitive and characterological sphere) and their relationship with predisposition for transgressive behavior. The obtained results of research could contribute to the existing scientific achievements in the field of management of important determinants and correlates with subjective features ensuring success. It is defined by a transgressive action – as a kind of activity type to be changed. Subjective characteristics of the creative (creative assets) increases the efficiency of the action.
