Opis:
The century preceding the social processes of transformation in Poland in the 1990s prompted via new state and church legislation the creation and equal functioning of public and private health care facilities, including those run by church bodies. The reactivated Church institution, Caritas, along with the Diocesan Caritas of Plock, brought to life new forms of care for the sick and those in need of medical care and their families. The health care crisis in Poland, as it were, forced the genesis of such activities.

Introduction and objective. Deficits of vitamin resources constitute a significant public health problem, especially among the elderly population. The aim of the research was to determine the level of vitamin 25 (OH) D and vitamins from group B in the chronically ill elderly in domiciliary care, depending on functional capacity and coexisting diseases. Materials and method. The pilot study included 137 patients staying in long-term domiciliary care. Samples of the participants’ venous blood was obtained for laboratory tests. Centrifuged serum was used to determine the level of the following biochemical parameters: vitamin 25 (OH)D, B12, folic acid and total protein, albumin, triglycerides, total cholesterol and HDL cholesterol. Assessment of the functional status of patients was made by using the Barthel scale. Results. More than 3⁄4 of the patients with functional deficit (according to Barthel’s score 0–85 points) were deficient in vitamin 25 (OH)D, while folic acid values were below the reference values in more than half of the patients. Respondents with lower functional efficiency were characterised by a reduced average value of vitamin 25 (OH)D and folic acid. Conclusions. The studied group of the chronically ill elderly was characterised by a deficiency of vitamin D3 and folic acid. Subjects with a functional impairment deficit show a reduced mean value of vitamin 25 (OH)D and folic acid in the blood serum, compared to the group of patients with higher mobility.

This study provides an overview of the legal aspects of long-term care (LTC) and institutional care for the elderly. It investigates whether LTC services for seniors could be recognized as a distinct human right, possibly through a United Nations convention. The study explores the existence and core components of the right to LTC and examines the minimum standards for its investigation, implementation, and enforcement. Additionally, it analyzes the specific rights of seniors that contribute to the right to care for dependent persons while also considering the intersectionality of this right and its relationship with other human rights. The study investigates the legal frameworks for protecting the right to LTC in various countries and assesses legal solutions derived from general legal instruments and special standards, particularly those concerning individuals with disabilities. Furthermore, it discusses proposed laws aimed at preserving the dignity of seniors and preventing unnecessary examinations and abuse. The study ultimately evaluates whether the right to LTC is an independent right or an extension of the rights to healthcare and social security.

Opis:
Purpose: To assess the degree of independence in physical activities of daily living for patients under long-term in-home nursing care; to evaluate any changes in physical activities of daily living at entry and after 90 days of in-home nursing care. Materials and methods: InterRAI-HC (Residental Assessment Instrument – Home Care) questionnaire was used as a tool according to the protocol and assessed patients at entry and again after 90 days. The participants consisted of 100 consecutive patients who were newly admitted to long-term in-home nursing care.Results: The analysis of total dependence showed that more than 50% of patients were entirely dependent in eight out of ten specified physical activities of daily living (except mobility in bed and food consumption). Intention to treat analysis was performed; the percentages of patients totally dependent in performing eating and mobility in bed actions increased significantly by the 90-day follow-up. Also a comparison of the mean value of independence for analyzed activities did not change significantly over the 90 days, with the only exception being the mean value of independence for dressing up the upper part of the body, which significantly improved after 90 days per protocol analysis. A high mortality rate is noted among these patients (15% by the 90 day follow-up). This probably resulted from the restrictive qualification criteria used in Poland for patients to receive this form of in-home healthcare service.Conclusions: The majority of patients who are under long-term in-home nursing care in Poland are entirely dependent when it comes to the physical activities of daily living.

Opis:
Social services represent a tool of social policy as well as a dynamic social system. This article focuses on a heterogeneous structure of social service providers and it gives rise to a debate of a network optimization of social facilities in the Slovak regions in the context of demographic changes as well as morbidity of inhabitants, social dependency due to various reasons, etc. The Prešov Region had been chosen to analyze the structure of social service providers in order to evaluate an actual situation and particularities in an existing social model of a given region. The analysis’ outputs showed inevitability of an access to deeply structured data and realization of multi-dimensional analyses that reflect demographic aspects, morbidity and social dependency of inhabitants in a given region.
Usługi socjalne stanowią narzędzie polityki socjalnej, a także dynamiczny system socjalny. Niniejszy artykuł skupia się na niejednorodnej strukturze dostawców usług socjalnych i daje podstawę do dyskusji o optymalizacji sieci placówek socjalnych w regionach Słowacji w kontekście zmian demograficznych, a także zachorowalności mieszkańców, zależności socjalnej z różnych przyczyn, etc. Region Preszów został wybrany do analizy struktury dostawców usług socjalnych w celu oceny rzeczywistej sytuacji i specyfiki w istniejącym modelu socjalnym danego regionu. Wyniki analizy wykazały nieuchronność dostępu do głęboko ustrukturyzowanych danych i realizacji wielowymiarowych analiz, które odzwierciedlają aspekty demograficzne, zachorowalność i zależność społeczną mieszkańców w danym regionie.
