hospital service in Poland
Opis:
W wyniku tych działań cztery lata później do nowego szpitala przyjęto pierwszych pacjentów. Był to 42 szpital w Królestwie Polskim. Funkcjonował przez półtora wieku, do czasu wybudowania w roku 1982 nowej placówki.
The authors present in their publication a historical development of hospitals in Kutno. The first mentions of Kutno appear in the documents from 1161. Kutno received town rights in 1432. During this period, the owner of the town established two hospitals, one at the Chapel of the Holy Spirit, and the second at the Chapel of St. Leonard. Those institutions were completely destroyed in 1753 in the big fire in town. In the eighteenth and nineteenth centuries, Kutno was the stopping place of numerous troops, which organized hospitals for their own needs. However, the military doctors employed in those hospitals also treated the inhabitants of the town and surrounding countryside. In 1809 a hospital in the nearby town of Gostynin, which in this period was the only institution of this kind in the area, was completely destroyed by fire. As a result of that event, in 1840, the residents of Kutno undertook efforts to build a new hospital in their town. Four years later, owing to their actions, the new hospital admitted first patients. That was the 47* hospital in the Kingdom of Poland. The hospital operated for one and a half century, until 1982 when a new facility was constructed.

Opis:
The work of the Holy Spirit Hospital and Church in Lublin was secured by using the property they owned and by the income obtained from its commercial part. They were „steady” and depended on the managers of the hospital. The hospital account books from the years 1548-1556 and 1604-1628 show that there was a change in this respect caused by funding a Carmelite monastery on the hospital grounds in 1610, as in the middle of the 16th century the income came mostly from the hospital's suburban estates, whereas at the beginning of the 17th century it mainly came from buildings within the town (from 43 to 25% the proportions changed to 12 to 69%). Besides the steady income the hospital obtained means from numerous donations and charities received mainly by testament, that were an expression of individual piety. The great amount of these small legacies – that not always had the form of money, – rarely recorded in the source materials, or not recorded at all, made it possible to finance the current charity work without any breaks. Acquiring income from legal acts (among others, testaments, agreements, judgments), that resulted from support given to the hospital by town authorities is easier to follow. In the middle of the 16th century the unsteady income amounted to 6-30% of all the money obtained by the hospital, and at the beginning of the 17th century already to 28-53%. This proportion influenced both reconstruction of the hospital buildings and the range of the charity work undertaken by the hospital.

Opis:
In this publication the review of the literature points to the four main directions of research relating to innovation in their organizations. They are mainly of interest in the economic and technological aspect, treating hospitals as providers of information systems and comprehensive medical services. At the same time the importance of nontechnological innovation category was pointed out, such as organizational, social, management, and services and relational. It was emphasized that in today's world you cannot make the proper development of the health sector, one of the most modern in medical technology, but also should take into account other types of innovation. Such an approach seems to be best to continue the modernization of the current state of health care, improve the quality of life and increase the availability of new treatments to patients that respond to their needs.

In the management of a modern organization Information Technology takes a special place, as to a large degree information circulation is possible and carried out just through its use. Taking advantage of Information Technology becomes a necessity forced by the environment of an organization―starting with a compulsory clearance performed partly electronically and finishing with the need of having e-mail addresses and web pages of a company being essential from the marketing point of view. Used information technologies, and related to them procedures as well as change processes do not depend on the fact that we deal with an economic or non-economic organization, a production company, a company providing services or a non-profit company. The aim of the article is to show the case study of a hospital, where the process of creating and implementing information technology has been described as an example, and creating an information system which supports the process of managing this institution.

Opis:
Theoretical background: Quality management and medical service improvement in district hospitals enables to achieve a number of internal and external benefits. It is a long-term process of conscious transformation of the existing state into a desired and accepted one by the concerned stakeholders. The diagnosis of the medical service organizations’ performance is an important aspect in the context of building a hospital of the future which is focused on quality, development and improvement. Purpose of the article: The article reveals the positions of hospital managers (directors) on the current level of the quality management maturity service improvement of the medical facilities they manage. Research methods: A diagnostic survey was conducted using a paper questionnaire. The form was handed in person and filled in the presence of the author. This provided an inducement for analysis and conclusions. The study was performed in 2022. Main findings: The level of implementation of the quality management system (QMS) principles in the surveyed hospital organizations, despite the variability, uncertainty, complexity and ambiguity of the environment, was evaluated at 3.39. This means that the practice of the QMS is widely established, but not in most areas. Therefore, the scope of the principles cannot be considered as complete. The result is different for the assessed maturity of quality excellence in the selected accreditation standards. The surveyed district hospitals evaluated their maturity level at 3.69. Thus, the respondents considered that the maturity level to be close to 70%. They identify the practice as typical (only some exceptions are deviations from it). The surveyed hospitals also have evidence of continuous improvement after at least one year. Another important aspect concerned maturity research in relation to innovation. The surveyed entities determined this parameter at the level of 3.50, which corresponds to approximately 66%. The highest level of maturity achieved the hospitals in the operational skills in conditions of disruption. They assessed the adaptation abilities and taken actions towards sustainable development at the level of 3.82 (72%). These results prove that the district hospitals in the Wielkopolska voivodeship take steps toward excellence, but their level of excellence in 2022 is not full (as it does not apply to 100% of occurrences, and the practice is also not implemented in entire hospitals, without exceptions).

Injuries are the leading cause of death before the age of 40 years, and the third most common incidence of death worldwide after cardiovascular diseases and cancer. The aim of the study was to determine the number and type of chest injuries, based on EMS (Emergency Medical Service) documentation in the district of Otwock, with particular emphasis on patient age and gender at the time of injury. Material and methods. Analysis considered data obtained from medical rescue teams of Otwock County in 2009 concerning chest injuries. Results. The study group comprised 166 cases of chest injuries. Chest injuries were more often diagnosed in male patients. Most accidents occurred in the afternoon (between 1 and 6pm), and in the summer and winter seasons. Motor vehicle accidents and falls from heights were the most common cause of chest injuries, while the largest number of cases involved superficial chest injuries. Conclusions. Chest injuries accounted for 12% of all medical rescue team interventions, due to injuries, most often connected with superficial contusions of the chest wall. Rib fractures are usually caused by blunt chest injuries, most often relating to the V-VIII ribs. Fractures of the I-III ribs are rare and are evidence of a significant injury. Due to the flexibility of the thoracic wall, fractures in children are less common, as compared to the adult population. Most chest injuries occur in the afternoon during increased patient activity.

Background. In Poland, as in the world at large, Emergency Departments (EDs) face a formidable problem as they are overloaded with an excessive number of patients. It was decided to investigate whether there are any factors which determine patient admittance rates to the observation and consultation areas of EDs. Objectives. The aim of this study is to determine whether differences in the rates of patient visits to emergency rooms (observation and consultation areas) on workdays and on weekends, are significantly determined by sociodemographic variables, patients’ beliefs and knowledge about the functioning of EDs and primary healthcare facilities (PHFs) or health-related variables. Material and methods. A total of 164 patients from the ED of the University Clinical Hospital in Opole were examined. The diagnostic survey method was employed, using the Satisfaction with Life Scale (SWLS), the Hospital Anxiety and Depression Scale (HADS) and an original questionnaire of the authors’ own design. Results. Neither age (p = 0.059), sex (p = 0.687), marital status (p = 0.585), place of residence (p = 0.423), employment status (p = 0.401), the presence of chronic diseases (p = 0.936) nor a lack of trust in primary care physicians (p = 1.000) determined the ED admittance rates on weekdays versus weekends. People who did not know where to seek medical help at night and on national holidays were more likely to visit the ED on weekends than on weekdays (28.5%; 23% vs 10.98%; 9, p = 0.010). Conclusions. It is difficult to define the characteristics of healthcare service recipients visiting the ED in terms of whether the admittance is a on workday or a weekend day. During each shift, the ED staff should be prepared to receive patients of different sociodemographic backgrounds and health statuses, possessing different levels of knowledge and beliefs about the functioning of EDs and PHFs

Opis:
Theoretical background: By deciding to purchase insurance the entity which does so expresses its demand for money should it suffer adverse consequences of a fortuitous event – that is to say, an event which the entity concerned has identified and whose occurrence it estimated as probable. There are very limited possibilities to learn about the specifics of insurance public hospitals buy. There are also no sources of data that could shed closer light on this, whether coming from the market or gleaned through surveys. As public finance entities, public hospitals are required to follow applicable public procurement laws in purchasing insurance, although that obligation may be waived under certain circumstances. Whatever the procedure hospitals choose to use in their purchases, certain purchase documentation will be generated over its course, which can be a valuable source of detailed information about the coverage bought. Purpose of the article: The main objective of this study is to determine the ways in which public hospitals can buy insurance, what kind of documents are generated in the process, and what scope of information they contain. Research methods: In order to reach the stated objective, a body of literature and legal regulations was analysed, and an empirical study was conducted. The underlying premise in the study was that purchasing procedures followed by public hospitals provide insight into their purchasing decisions, and thereby into the reported demand for insurance. Main findings: The study opened up a way for an assessment of the possibilities and limitations involved in gaining insight into insurance purchasing decisions based on the purchase documentation.
