Opis:: Artykuł przedstawia wielokryterialną procedurę postępowania przy kwalifikacji pacjentów do zabiegu wymiany stawu biodrowego w procesie leczenia. Zastosowano metodę Electre Tri w hybrydowym systemie DSS 3.0. Artykuł składa się z opisu kompetencji ekspertów, drzewa decyzyjnego oraz modelowania rankingów i klasyfikacji podjętego problemu. Przykładem badawczym są pacjenci z różnym zaawansowaniem choroby zwyrodnieniowej, których stan określono w trakcie leczenia szpitalnego.
Advanced degenerative changes causing disturbances in biomechanics, disability and social isolation. A multitude of conditions of varying severity of the patient causes problem with determining the optimal timing of treatment. The classification of patients for surgery is complex. Published studies show a variety of parameters, variants and classification of evaluable patients with osteoarthritis of the hip. It notes the lack of proposals for assessing the performance in the field of biomechanics in one system, and with the classification of a sick patient. The paper presents the problem of the methodology for specific decision problems. The aim of the article was to develop (and verify) test procedure for the evaluation of osteoarthritis of the hip. Verified appropriate method of multi-criteria Electre Tri, which allowed to established patient qualifications for surgery or hip replacement program of physiotherapy or pharmacotherapy.

Opis:: Hip osteoarthritis is a serious clinical and social problem. The number of patients who suffer from degenerative changes in the hip joints and require endoprosthesis-plasty is constantly increasing. This paper presents physiotherapeutic activities based on Proprioceptive Neuromuscular Facilitation (PNF) which optimize a patient’s mobilization using the reserves in their body fully to make improvements in movement and to regain lost functions for achieving beneficial therapeutic effects. The aim of the study was to evaluate the influence of PNF therapy on changes in muscle strength, mobility, and gait pattern in patients after Total Hip Arthroplasty. The case described here regards a 63-year-old woman diagnosed with left hip osteoarthritis who had Total Hip Arthroplasty. The patient was examined twice before and after PNF therapy. The range of mobility of hip joints, level of pain, muscular strength and gait were assessed. Applied PNF therapy, including dynamic (eccentric, concentric) and static muscle training, post-isometric relaxation, stabilization and control in the stance phase, resulted in improved hip joint mobility, muscle strength, gait pattern and pain reduction in the patient. The case study demonstrates that a short (two-week) but intensive (over two hours per day) PNF therapy positively influenced selected motor functions after Total Hip Arthroplasty.

Opis:: Surface arthroplasty, also referred to as resurfacing, is a method of sparing hip arthroplasty, which is a surgical alternative usually reserved for younger, active patients. The introduction of successive generations of implants with metal-on-metal articulation resulted in increased interest in this method of treatment. Despite the unquestionable advantages and promising initial results, resurfacing has some limitations and disadvantages. One of them is the increase in the concentration of metal ions. Although they are necessary for the proper functioning of the human body, in large amounts they can lead to serious reactions of the soft tissues adjacent to the prosthesis, including metallosis, ALVAL (Aseptic Lyphocytic Vasculitis-Associated Lesion), or the formation of pseudotumours. Despite numerous studies and scientific reports, there is a need for further work on the relationship between the concentration levels of metal ions in people after resurfacing and the biological, causal mechanisms of the development of adverse reactions in the body.
Endoprotezoplastyka powierzchniowa, zwana kapoplastyką jest metodą oszczędnej endoprotezoplastyki stawu biodrowego, która stanowi alternatywę chirurgiczną zarezerwowaną zwykle dla młodszych, aktywnych pacjentów. Wprowadzanie kolejnych generacji implantów o artykulacji metal-metal spowodowało wzrost zainteresowania tą metodą leczenia. Pomimo niekwestionowanych zalet i obiecujących wyników początkowych, kapoplastyka obarczona jest jednak pewnymi ograniczeniami i wadami. Jedną z nich jest wzrost stężenia jonów metali, które choć niezbędne do prawidłowego funkcjonowania organizmu ludzkiego, w dużych ilościach mogą prowadzić do poważnych reakcji tkanek miękkich przylegających do protezy, w tym do metalozy, ALVAL (Aseptic Lyphocytic Vasculitis-Associated Lesion) czy tworzenia guzów rzekomych. Pomimo licznych badań i doniesień naukowych istnieje potrzeba dalszych prac nad związkiem między poziomami stężeń jonów metali u osób po kapoplastyce, a biologicznymi, przyczynowymi mechanizmami rozwoju niekorzystnych reakcji w organizmie.
