The health care entities use information from the accounting system in the management processes. New functioning conditions of the health care units require the access to information among others for the purpose of planning and control. This means that the information should concern both the past and the future. The purpose of this article is to identify the determinants of management accounting in the health care units.

The article focuses on the issues of wastewater management systems in the voivodal local government health care departments in the Lubuskie Voivodship, 2007-2010. Based on the conducted research, 65% of the public health care units subordinate to the Marshal’s Office of the Lubuskie Voivodship show changes in the financial condition of these units which are determined by the wastewater management systems.
Artykuł podejmuje problematykę funkcjonowania systemów zarządzania gospodarką ściekową w wojewódzkich samorządowych jednostkach ochrony zdrowia regionu lubuskiego, w latach 2007-2010. Na podstawie przeprowadzonych badań, wśród 65% funkcjonujących publicznych jednostek ochrony zdrowia podległych Urzędowi Marszałkowskiemu Województwa Lubuskiego, ukazuje zmiany kondycji finansowej tych jednostek determinowane przez systemy gospodarki ściekowej.

Objectives: The purpose of this article is to discuss the regulations on and the functioning principles for internal audits in the Polish health care system. Additionally, this study presents selected results of empirical research with regard to the effectiveness of internal audits in university hospitals. Research Design & Methods: To determine the research problems related to the functioning of the internal audit systems in selected university hospitals the authors analysed specialist literature and reviewed earlier studies addressing this question. On the basis of the analysis of such literature and applicable legislation the authors prepared an anonymous questionnaire and distributed it among internal auditors from a number of university hospitals in the voivodship (region) of Western Pomerania, including the city of Szczecin, and in the city of Poznań. Findings: This paper discusses the legal basis and functioning principles for internal audits in the Polish health care sector. On the basis of a pilot survey study carried out in selected university hospitals in Poland the authors evaluated internal audit efficiency. This study has shown that the perception of the auditor’s role and auditing efficiency varies significantly in comparable institutions. These conclusions should be considered as an inspiration for more advanced studies. Implications / Recommendations: The constantly growing scope of health care institutions bound by the internal audit obligations shows that such audits are indeed necessary and demonstrates the efficiency of internal audits in the public sector. Contribution / Value Added: The authors point to potential further research on internal audit efficiency in the Polish health care system.

W artykule zaprezentowano przegląd systemów zarządzania jakością i ryzykiem możliwych do zastosowania w publicznych jednostkach ochrony zdrowia. Obligatoryjność stosowania standardów kontroli zarządczej oraz chęć podnoszenia jakości świadczonych usług powodują, że często mamy do czynienia z wielością rozwiązań, które niejednokrotnie przy braku ich integracji powielają wymagania w stosunku do kadry zarządzającej i pracowników. Integracja wydaje się konieczna do podniesienia efektywności systemów zarządzania jakością i ryzykiem, a wskazanie obszarów podlegających temu procesowi stanowi właściwy wstęp do działań w tym zakresie.
The article presents an overview of quality management systems and the risk potential for use in public health units. Mandatory use of standards, management control and the desire to improve service quality, make it often that we have to deal with a multitude of solutions and in the absence of their integration duplicate the requirements for management and employees. Integration seems to be necessary to improve the effectiveness of quality management systems and risk management, and identification of areas subject to this process is an appropriate introduction to activities in this field.

Due to its specific nature, the provision of medical services is associated with considerable risk; and it is extremely important to diagnose and manage this same risk. Healthcare entities should be covered by appropriate instruments that reduce risk and support quality. In addition to legal regulations that must be met by entities providing such services, there are systemic solutions that are being implemented by more and more healthcare entities. The article analyses the risk that occurs in the management of a health care unit. In the first part of the article, the literature on the topic is reviewed, whereas what follows thereafter analyses risks in the management of a healthcare unit, including risk factors and risk allocation. The conclusion points to the risks which play the greatest role in the management of medical service providers; and enumerates some of the most important issues of risk management in medical entities.

Purpose: The aim of the article is to present the theoretical assumptions of the model of professional competencies in health care units. It was assumed that competences are a multidimensional concept and require an integrated approach that allows for the construction of a competency model that reflects their real complexity. A list of professional competencies will be presented, which will be subject to empirical verification in the course of future research by the authors in order to identify key competencies. Design/methodology/approach: The proposed lists of professional competencies (six domains) was created and are based on the analysis of healthcare competencies models the study of the literature - and the Authors’ observations of the analyzed entities. Findings: Presented model of professional competencies in health care units contains six domains with sub-competencies. The importance of assessing competences is undeniable. Competence recognition offers a way to develop workforce planning and career opportunities of practicing medical staff. Having an instrument that identifies existing competences and those that need to be acquired becomes significant for distinguishing the singularity of actions for a professional practice which is safe, humane and with no risk to the client, the medical staff or the health care organization. Originality/value: An identification the professional competencies of health care units managers significantly shaping competences of such organizations especially relevant in pandemic time.

A list of managerial competencies will be presented, which will be subject to empirical verification in the course of future research by the authors in order to identify key competencies. Design/methodology/approach: The proposed lists of managerial competencies (six domains) were created and are based on the analysis of healthcare competencies models - the study of the literature - and one of the Authors’ observations of the analyzed entities (as a consultant). Findings: Presented model of managerial competencies in health care units contains six domains with 32 competencies. These domains capture the dynamics and complexity of health care unit’s manager’s role and reflect the dynamic realities in health leadership today. Originality/value: An identification the managerial competencies of health care units managers significantly shaping competences of such organizations especially relevant in pandemic time.

Opis:
A presentation of the regulations concerning the protection of personal data at health care units is a purpose of the work. Medical data i.e. sensitive data constitute the special category of personal details (sensitive ones) which concern medical condition, information about the genetic code or addictions. A general prohibition on the processing of sensitive data exists, except for the situation, when provisions of the law allow it. In the legal status being in force processing both information referring directly to the medical condition of man, and information the average recipient can acquire these data is forbidden. Processing sensitive personal details without the written consent of the person which they concern, is possible only in the objective of protection of medical condition, providing medical services or curing patients by persons being engaged professionally in curing or with providing other medical services, provided there are created full guarantees of the protection such data.. Medical data gathered by the health-service units must be provided with the full legal protection, predicted in the act from 29.08.1997 about the protection of personal data. For creating appropriate conditions of storing medical documentation a manager of the health care unit is held responsible.

The paper discusses the problem of operation of medical waste management systems in the lubuskie voivodship local government health protection institutions 2007-2011. Na podstawie przeprowadzonych badań, wśród 65% funkcjonujących publicznych jednostek ochrony zdrowia podległych Urzędowi Marszałkowskiemu Województwa Lubuskiego, ukazuje endogeniczne zmiany ich systemów gospodarki odpadami determinowanych przede wszystkim kondycją finansową sektora usług medycznych.

Opis:
The necessity of the hospitals status change in Poland was the hot issue for quite a long time. The problems of hospitals debts, local governments role, state (public) ownership problems, management weaknesses, resources lack, humans resources problems and many others have been stressed in public debates, often by different sides and political parties. The present Ministry of Health from the very beginning proposed the formal status change concerning ownership of health care units, mainly hospitals, as a first step for the system transformation in this respect. The proposal of the new legislation called the Health package included such approach. One of the most important laws in the package was the 'Hospitals privatisation', subsequently vetoed by the President representing the political opposition. The next step toward the transformation was the governmental Plan B, proposed also by Ministry of Health, criticized for the improper legal instruments and infectivity of the proposed strategy. The new legislation, described in this paper, is a consequence of the process and the main issue of the analysis undertaken hereby.

Jakość staje się jednym z zasadniczych elementów skuteczności funkcjonowania placówki ochrony zdrowia na konkurencyjnym rynku. Głównym celem artykułu ,jest prezentacja oceny jakości obsługi pacjenta na przykładzie wybranego podmiotu leczniczego. Problematyka badawcza skupia się przede wszystkim wokół czasu oczekiwania na rejestrację i przeprowadzenie badania, sposobu odnoszenia się personelu lekarskiego i pielęgniarski ego do pacjenta i rodziców, a także na dostępności do sanitariatów i świetlicy. W prowadzonym badaniu wykorzystano pierwotne (ankieta) i wtórne (materiały wewnętrzne) źródła danych. Badaną zbiorowość stanowią pacjenci wskazanego podmiotu leczniczego.
Quality becomes one of the essential elements of effective functioning of the health care facilities in a competitive market. The main purpose of this article is to assess the quality of patient care for a selected entity hospital. The issue of research focuses primarily on the waiting time for registration and examination, a way of relating to medical and nursing staff to patients and parents, as well as the availability of toilets and common room. The conducted study used primary (questionnaire) and secondary materials (internal) data sources. The study community are the patients indicated entity hospital.

Wysoki standard obsługi oraz komfort pacjenta stają się obecnie jednymi z zasadniczych elementów skuteczności funkcjonowania zakładu leczniczego na konkurencyjnym rynku. Głównym celem artykułu jest analiza oceny standardów obsługi „małych pacjentów” na przykładzie wybranych podmiotów leczniczych na terenie Gliwic – w kontekście całościowego zarządzania podmiotem leczniczym. Problematyka badawcza skupia się przede wszystkim wokół wyposażenia kącika dziecięcego, nagradzania dziecka, miejsca do przewijania, podejścia personelu oraz systemu rejestracji. W przeprowadzonych badaniach wykorzystano metodę badawczą w postaci ankiety. Badaną zbiorowość stanowią pacjenci wybranych zakładów leczniczych.
High standard of the service and comfort of the patient become one of the essential elements of effective functioning of the health care facilities in a competitive market. The main purpose of this article is to assess the standards of the service of “little” patient care for a selected health care units – in the context of the overall management of the medical entity. The issue of research focuses primarily on the equipping the children's corner, awarding to the child, the place for the scrolling, tricking the staff and the registration system. The conducted study used research methods in the character of the questionnaire form. The study community are the patients indicated health care units.

Introduction. Cervical cancer is the fourth neoplasm in women with respect to incidence. In Poland, both cervical cancer incidence and corresponding mortality are gradually decreasing. Despite these improvements, the epidemiological situation significantly deviates from European standards. Poland has one of Europe’s lowest five-year survival rates at 54.1% for patients diagnosed in 2000–2002, compared to the European mean value of 62.1%. Objective. The aim of this study is to present health policy programmes related to HPV vaccinations run by local self-government units in 2009–2016. Materials and method. The research is based on analysis of already existing data developed by provincial governors and annual information reviews on health-policy programmes implemented by local self-government units presented to the Ministry of Health. All the programmes that included HPV vaccinations have been subjected to analysis. Results. In 2009–2016, local government units implemented a total of 1,204 health policy programmes that covered HPV vaccinations. Under these programmes, 2.05% of girls aged 10–14 were vaccinated. Percentage-wise, these were communes that contributed the most financially to the HPV vaccination programmes, whereas the counties the least. Conclusions. Local self-government’s programmes covering HPV vaccinations conform with the trends outlined in strategic documents on fighting neoplastic diseases. It is possible that the availability of HPV vaccination was limited for girls living in rural communes. Differences in the number of programmes, number of vaccinated girls and the financial outlays allocated for the implementation of HPV vaccination programmes in particular provinces, may be determined by the epidemiological situation in a given region, measured by the incidence rate of cervical cancer.

Jednym z przejawów polityki zdrowotnej realizowanej na poziomie lokalnym są samorządowe programy zdrowotne. Celem pracy jest określenie celów i działań jednostek samorządu terytorialnego w obszarze programów polityki zdrowotnej, ze szczególnym uwzględnieniem aspektów organizacyjnych i finansowych. Badanie zostało przeprowadzone na próbie 405 jednostek samorządu terytorialnego z terenu całego kraju i oparte na ankiecie internetowej (CAWI). Jak wynika z badań własnych, jedynie niespełna 27,5% urzędów realizuje obecnie jakikolwiek program polityki zdrowotnej, 91,1% urzędów realizuje PPZ, korzystając z własnego budżetu, ale jednocześnie 46,4% urzędów przeznacza na ten cel niespełna 0,1% całości środków finansowych. Zaledwie 27,9% badanych jednostek pozyskuje dodatkowe środki w ramach dofinansowania działań przez NFZ. Problemy z realizacją programów polegają na trudnościach ze znalezieniem realizatora programu oraz brakiem wykwalifikowanej kadry na poziomie urzędu do jego przeprowadzenia. Obok niewielkich nakładów finansowych, skuteczność działań obniżają także uwarunkowania organizacyjne i trudności kadrowe. One of the manifestations of health policy implemented at the local level are self-government health programs. The aim of the work is to define the goals and activities of Local Government Units in the area of Health Policy Programs, with particular emphasis on organizational and financial aspects. The study was conducted on a sample of 405 Local Government Units from all over the country and was based on an online survey (CAWI). As it results from own research, only less than 27.5% of offices currently implement any health policy program, 91.1% of offices implement health programs using their own budget, but at the same time 46.4% of offices spend less than 0.1% of all funds for this purpose. financial. Only 27.9% of the surveyed units obtain additional funds as part of financing activities by the National Health Fund. Problems with the implementation of programs consist in difficulties in finding a program implementer and the lack of qualified staff at the office level to carry it out. In addition to the above-mentioned difficulties, the effectiveness of actions is also reduced by organizational conditions and staffing difficulties.

Opis:
Opracowywanie, finansowanie i wdrażanie programów polityki zdrowotnej (PPZ) znajduje się wśród licznych zadań z zakresu zdrowia publicznego, właściwych dla władz publicznych. Wszystkie oddziały wojewódzkie Narodowego Funduszu Zdrowia (OW NFZ) w latach 2017-2018 rozpatrzyły pozytywnie 290 wniosków jednostek samorządu terytorialnego (JST) o dofinansowanie działań w ramach PPZ na kwotę ponad 8,7 mln zł. W roku 2018 w skali kraju najwięcej środków OW NFZ zostanie przeznaczona na dofinansowanie PPZ w obszarze szczepień ochronnych. Analiza danych finansowych uwidoczniła różnice regionalne w zakresie wysokości środków przeznaczanych na ten cel przez poszczególne OW NFZ, a wśród ich przyczyn znajdują się zarówno te natury ekonomicznej, jak i formalnej. Odnosząc się do opisanych w opracowaniu zmian legislacyjnych w obszarze możliwości dofinansowania PPZ, należy wskazać z dużym prawdopodobieństwem, iż spowodują one zmniejszenie zaangażowania JST w realizację samorządowych programów zdrowotnych.
The development, financing and implementation of health policy programs is among numerous public health tasks. All branches of the national health fund (NHF) in 2017- 2018 considered positively 290 applications of local self-government (LSG) units for co-financing activities carried out under health policy programs (HPP) for the amount of over PLN 8.7 million. In 2018, the most funds of the NHF will be spent on co-financing the HPP in the area of preventive vaccination. The analysis of financial data revealed regional differences in the amount of funds allocated for this purpose by individual NHF branches, and among their causes are both economic and formal ones. Referring to the legislative changes described in the study in the area of the possibility of co-financing the HPP, it should be indicated with high probability that they will result in a decrease in the involvement of local government units in the implementation of self-government health programs.
