L’impegno a favore del nascituro e uno dei segni vitali e dei dilemni delia civiltà contemporanea. Si quarda all’uomo prima della nascita per intenderlo fondamentalmente come persona o per sottrargli tale diritto connaturato. E ciò si riflette sulla coscienza sociale in senso lato e sui riferimenti dei singoli individui                      e gruppi sociali e professionali. Nell’ordine naturale viene prima la famiglia, comunità - comunione di individui arricchita dall’arrivo di una nuova persona e, non semplicemente di un essere. L’ambiente d’amore che dovrebbe contrassegnare la famiglia e un fattore essenziale per la concezione del nascituro. Intenderlo in quanto persona trova direttamente riscontro nella realtà delle relazioni giuridiche interne alla famiglia e anche dei fattori estrinseci della sua complessa collocazione legislativa. Altro nucleo, stavolta professionale, e rappresentato dal personale medico e paramedico a tutti i suoi livelli, categorie e competenze. II suo rapporto con il nascituro e un campo vasto, che plasma in misura straordinaria che l’atteggiamento della società in questa materia. II mondo della medicina ha spesso un’influenza preponderante sugli altri campi della vita. Pertanto è molto grande la responsabilità che ricade su infermiere, ostetriche e medici. Ciascuno nel proprio ambito di conoscenza, esperienza e competenza dovrebbe mettersi definitivamente al servizio del nascituro. Si può dire che qui è messa alla prova la purezza del servizio medico per la persona. Tutti questi campi e queste dimensioni devono trovare riscontro nella legislazione, nei suoi artefici e nei suoi esecutori. In tale ambito si avverte il bisogno cogliere il carattere di servizio della legge, spede nei confronti del nascituro.

On 24 April 2011 entered into force the directive of the European Parliament and of the Council No 2011/24/UE of 9 march 2011 on the application of patients' rights in cross-border healthcare. The article analyses the duty of Member States connected with cross-border medical care, types and limits of using cross-border medical care and problems evoked by refund of medical expenses. The subject of this study is also evaluation of the preamble and the normative text of the directive No 2011/24/UE against the regulation of the European Parliament and Council (EC) No 883/2004 of 29 April 2004 on the coordination of social security systems. The new directive provides clarity about the rights of patients who seek healthcare in another Member States and supplements the rights that patients already have at European Union level through the legislation on the coordination of social security systems.

The share of privately paid medical care is steadily increasing. Polish patients pay immediately for single services and may also privately conclude the contracts for medical insurance or subscription (steady access to complex private medical care). The aim of this paper is to draw the reader's attention to the possibilities a Polish patient has of obtaining complex private medical care. The paper describes the range of services which are guaranteed for all patients insured by National Health Fund (financed from public resources) and contracts of private health insurance and medical subscription.

The purpose of this opinion is to explain two legal issues relating to rules of concluding such contracts and control their execution by the National Health Fund. The author claims that the National Health Fund can shape content of contest notice and intention of obligations relationship to provide more than minimum required by the law number of employees in institution, where such health services are supplied. In the author’s view, control of the execution of contracts for performance of basic health services, carried out at night or during the holidays is allowed in any time without prior notice to the performer.

Theoretical background: Quality management and medical service improvement in district hospitals enables to achieve a number of internal and external benefits. It is a long-term process of conscious transformation of the existing state into a desired and accepted one by the concerned stakeholders. The diagnosis of the medical service organizations’ performance is an important aspect in the context of building a hospital of the future which is focused on quality, development and improvement. Purpose of the article: The article reveals the positions of hospital managers (directors) on the current level of the quality management maturity service improvement of the medical facilities they manage. Research methods: A diagnostic survey was conducted using a paper questionnaire. The form was handed in person and filled in the presence of the author. This provided an inducement for analysis and conclusions. The study was performed in 2022. Main findings: The level of implementation of the quality management system (QMS) principles in the surveyed hospital organizations, despite the variability, uncertainty, complexity and ambiguity of the environment, was evaluated at 3.39. This means that the practice of the QMS is widely established, but not in most areas. Therefore, the scope of the principles cannot be considered as complete. The result is different for the assessed maturity of quality excellence in the selected accreditation standards. The surveyed district hospitals evaluated their maturity level at 3.69. Thus, the respondents considered that the maturity level to be close to 70%. They identify the practice as typical (only some exceptions are deviations from it). The surveyed hospitals also have evidence of continuous improvement after at least one year. Another important aspect concerned maturity research in relation to innovation. The surveyed entities determined this parameter at the level of 3.50, which corresponds to approximately 66%. The highest level of maturity achieved the hospitals in the operational skills in conditions of disruption. They assessed the adaptation abilities and taken actions towards sustainable development at the level of 3.82 (72%). These results prove that the district hospitals in the Wielkopolska voivodeship take steps toward excellence, but their level of excellence in 2022 is not full (as it does not apply to 100% of occurrences, and the practice is also not implemented in entire hospitals, without exceptions).

Health care sector restructuring process that Poland is undergoing during last few years, extensively determines changes seen in whole health care sector. This applies also to nursing professionals. Direction of those, in relation to coming and anticipated dynamic growth of social needs in quantity and quality of health services not only in Poland, but a whole World, may and should be of concern. A need for an answer to a question, if it truly happens in practice and therefore formulate a proper conclusion, was a reason for following aim of this paper: an analysis and evaluate of changes in the structure of employment among nursing professionals, most of all in the context of increasing health care needs of Polish society.

In the Second Republic of Poland health protection was insufficiently developed. There were very few doctors, nurses and hospitals. Only people who were insured or the wealthy had access to medical treatment. The majority of citizens, especially rural resi-dents, could not use health care. The health policy of the state was passive for many years. Changes appeared only in the mid-thirties. The aim of health policy was a public health service for all citizens. The authorities declared their support for this idea on a number of occasions and the parliament accepted the law of public health in June 1939. It was the basis for the construction of common access to healthcare in Poland. To achieve this aim, health centres were built and the compulsory medical practice for the doctors was introduced in villages. The beginning of the Second World War prevented the enforcement of this Act. However, the attempt to create a public health service made in the Second Republic of Poland was a proof of progress which has been made in state health policy.

Secondary data on medical services (private and public sector perspective) was also analyzed. The publication was completed with a summary and conclusions.

This review includes information about the number of private health insurance and medical subscriptions in Poland, as well as the level of increases in premium rates, and the scale of health promotion and prevention conducted by Polish employers. The aim of this article is to present the current situation on the supplementary private medical market, which is involved in employees’ healthcare, and the challenges that both employers and medical providers with health insurers are now facing, and also to present the reasons why prevention and health promotion are important factors of healthcare under private health insurance. For the purpose of this review, scientific publications devoted to health promotion at the workplace were used, along with statistical data presented in the studies released by the Polish Insurance Association, the Polish Social Security Institution, the World Health Organization and the Organization for Economic Co-operation and Development. The literature search was carried out using the electronic databases of PubMed. Search terms included medical subject headings and free text words. No year of publication restriction was imposed. The conducted analysis shows that the demand for private medical care is rising. Employers are willing to invest in private medical care for their employees, although the scale is much lower among small and medium-sized entrepreneurs due to fiscal burdens. Given the rising demand and an insufficient number of specialists, access to medical services is deteriorating, and the premium rates and costs are rising. More employers are investing in health promotion at the workplace in order to decrease absenteeism and presenteeism, lower the utilization of medical packages, and improve their attractiveness on the market. Although the interest in private medical care is rising, employers should focus on health promotion and prevention at the workplace, and adapt their actions to the employees’ current needs and health issues. The health care system in Poland or any other country should ensure an equal and unrestricted availability of the health service to its population. This idea, though being noble and harmonious with the country’s constitution, seems to differentiate small administrative regions of the Silesian voivodeship. Therefore the presented paper is an attempt to estimate and analyse conditions of the health care system activity in this region. Those regions were systematized based on a structure of health care system. Also, a homogeneous groups of regions were created. The analysis is based on the cluster analysis.

Organizacja przemysłowej służby zdrowia w latach 1945–1955 odznaczała się swoistą dynamiką, na co wpływały różnorodne czynniki, wśród których za najważniejsze można uznać: uwarunkowania historyczne, polityczne, przemiany zachodzące w służbie zdrowia, rozwój społeczny, gospodarczy. Jej tworzenie po 1945 roku przebiegało etapami. Pierwszy, rozpoczęty jeszcze w 1944 roku, miał charakter wstępny, przygotowawczy. W tym to bowiem okresie wyznaczono główne kierunki działań na lata następne. Ponadto przygotowano określone rozwiązania ustawodawcze, które zasadniczo wpłynęły na organizację służby zdrowa, w tym przemysłowej. Drugi etap przypadł na początek lat pięćdziesiątych. Czas ten przyniósł właściwą organizację przemysłowej służby zdrowia, której zasadniczy rozwój nastąpił w okresie planu sześcioletniego, kiedy to ochrona zdrowia robotników nabrała szczególnego znaczenia. Objęci nią w sposób szczególny zostali robotnicy zatrudnieni w strategicznych sektorach przemysłowych. Przemiany zachodzące w tworzeniu przemysłowej służby zdrowia dały tło do zaprezentowania organizacji ochrony zdrowia robotników na Pomorzu Zachodnim w analizowanym okresie.
The organisation of industrial health service in the years 1945–1955 may be characterised as dynamic. Various factors were at work, among which the most important ones are the following: historical and political conditions, changes taking place in the system of health service, and the socio-economic development. After 1945 the system was created in stages. The first one, initiated still in 1944, was preliminary, preparatory. It was then when the main guidelines for the subsequent years were set down. In addition, the appropriate laws were prepared, which influenced significantly the organisation of the health service, including its industrial branch. The second stage occurred at the beginning of the 1950s. Then the industrial health service was properly organised; and the full-scale development proceeded during the six-year plan, when the health service for workers acquired special significance. The system covered especially workers employed in strategic industrial sectors. The changes in the industrial health service was a background for presenting the organisation of the health service in Western Pomerania in the period in question.

Additionally, she analyses typology of hospitals and its references to modularity and standardisation. The author also investigates container construction and presents some examples from Europe and Poland.
