Opis:
Numerous evidences suggested that the hepatitis B virus (HBV) was recognized as an important factor in the development of hepatocellular carcinoma (HCC). Dickkopf-1 (DKK1) recently was reported to be involved in the progress of HCC. HBV may regulate DKK1 expression in hematoma carcinogenesis. Here, we demonstrated that HBV could regulate DKK1 promoter activity which resulted in upregulation of its mRNA and protein expression in several HBV existing cell lines, and HBx played a prominent role in this process. Transcription factor binding site search result showed that there is a SP1 site in DKK1 promoter region. Luciferase assay showed that overexpression of SP1 could increase DKK1 promoter activity in a dose dependent manner. Accordingly, siRNA inhibition of SP1 expression reduced DKK1 promoter activity and decreased the expression of DKK1 protein.

Opis:
The case reports about a 75-years-old man without a previous medical history, in whom a heart failure NYHA III de novo was diagnosed together with persistent atrial fibrillation and hepatocellular carcinoma in clinical stage IV. Based on echocardiography and computed tomography there were pulmonary hypertension in course of lymphangiosis carcinomatosa as well as diffuse metastases in the abdomen diagnosed. Before the treatment initiation the patient was classified 3 in WHO performance status. After an improvement in control of the rhythm frequency and the heart failure treatment stabilisation with a β-blocker, an ACE-inhibitor, spironolactone and furosemide, the patient’s performance status improved to WHO 2. He was further disqualified from surgical procedures due to the advanced clinical stage of the oncological disease. Considering high probability of further cardiotoxic influence of sorafenib on the heart failure despite its satisfactory control, the patient was assigned to palliative chemotherapy with FOLFOX. Parallel he was strictly followed up cardiologically in an outpatient clinic what certainly supported the oncological treatment. The patient survived 32 weeks from the first hospitalization and the progression free survival was 12 weeks from the chemotherapy initiation.

Opis:
Additional approaches to control malignancies are needed due to the emerging trends in the incidence of cancer of different organ sites. Due to the high frequency of hepatocellular carcinoma (HCC) and its poor prognosis, preventing HCC is an urgent priority. To explore the antioxidant and apoptotic pathways of grape seed extract (GSE) we induce HCC experimentally by diethylnitrosoamine (DEN) and treated the animals with low and high doses of GSE. The results indicate good therapeutic possibilities for GSE use in treatment of HCC., This was evidenced via regression of liver enzymes' function (ALT&AST), the HCC markers; α-fucosidase, α-fetoprotein and carcinoembrionic antigen (CEA) in HCC groups treated with the grape seed extract. Also, tumor necrosis factor (TNF-α) showed a significant decrease using GSE in HCC bearing animals. Regarding the apoptotic pathways of GSE, we found a significant down regulation of apoptosis enhancing nuclease (Aen), Bcl2-associated X protein (Bax), B-cell translocation gene 2(Btg2), Cyclin G1 (Ccng1) and Cyclin-dependent kinase inhibitor 1A (Cdkn1a) gene expression in HCC+GSE groups as compared to HCC bearing group. In the same trend, the necrotic/apoptotic rates were significantly higher in the HCC groups treated with GSE vs. the HCC bearing group. Finally, the 8-OHdG/2-dG generation decreased by 73.8% and 52.9% in HCC+GSE at low and high doses, respectively. Based on these encouraging observations, grape seed extract could be a promising natural remedy for attenuating hepatocellular carcinoma that has a great future in approaches directed towards control of HCC.

The control and treatment of chronic diseases is a major public health challenge, particularly for patients suffering from liver disease. In this paper, we propose a frame to estimate survival and death probabilities of the patients suffering from liver cirrhosis and HCC in the presence of competing risks. Database of the admitted patients in a hospital in Delhi has been used for the study. A stochastic illness-death model has been developed describing two liver illness states (Cirrhosis and HCC) and two death states (death due to liver disease and death due to competing risk). Individuals in the study were observed for one year of life at any age xi. The survival and death probabilities of the individuals suffering from liver cirrhosis and HCC have been estimated using the method of maximum likelihood. The probability of staying in the cirrhotic state is estimated to be threefold higher than that of developing HCC (0.64/0.21) in one year of life. The probability of cirrhotic patient moving to HCC state is twice (0.21/0.11) the probability of dying due to liver disease. HCC being the severe stage, the probability of patient dying due to HCC is three times that of cirrhosis. Markov model proves to be a useful tool for analysis of chronic degenerative disease like liver cirrhosis. It can provide in-depth insight for both the researchers and policy makers to resolve complex problems related to liver cirrhosis with irreversible transitions.

Dysregulation of miRNA is widely involved in human cancers, including hepatocellular carcinoma (HCC). Array data for miRNAs indicated that miR-331-3p might be one of the disorderly expressed miRNAs in HCC cell lines, but the function of miR-331-3p in HCC remains unclear. In this study, quantitative real time polymerase chain reaction (qRT-PCR) results indicated that miR-331-3p was up-regulated in HepG2.2.15 cells, Ad-HBV-HepG2 cells and pCH9/3091transfected SMMC7721 cells compared with their control group, respectively. miRNA target prediction software was used, and VHL was found to be one of the target genes of miR-331-3p. qRT-PCR and western blot analysis indicated VHL expression was decreased when miR-331-3p was over-expressed and increased when miR-331-3p was inhibited in SMMC7721 cells. The luciferase reporter activity was inhibited in SMMC7721 cells when co-transfected with miR-331-3p expression vector and VHL 3'-UTR wild type vector and increased in HepG2.2.15 transfected with miR-331-3p inhibitor compared to its control group respectively. When co-transfected with miR-331-3p expression vector and VHL 3'-UTR mutated type vector in SMMC7721 cells the luciferase reporter activity was recovered. All of these results show that HBV up-regulated miR-331-3p expression in HCC cell lines and miR-331-3p could inhibit VHL expression by directly targeting its 3'-UTR. This provided useful information in exploring the mechanism of HCC induced by HBV infection.

Intraabdominal bleeding is a serious surgical problem, particularly in elderly patients following an anticoagulant therapy. It occurs, that abdominal haemorrhage is the first symptom of hepatocellular carcinoma, mostly in Asians and Africans, occasionally in Europeans. The article's author shows a case of an elderly man, treated by Acenocumarolum, in a haemorrhagic shock. During an operation a bleeding liver tumor was revealed. The surgical team performed an effectual tumor resection and stoped the bleeding.

The article is an attempt at diagnosing training needs of the employees of units operating in health care sector in the Podkarpacie Province. In times of permanent changes affecting each sphere of economy, providers of health care services cannot afford to remain outside this trend. Improving qualifications, adaptability of the offer, influencing its quality, and above all, the awareness of the necessity of these changes, have become an element which is fully integrated also with this sphere of public sector operations. Taking into account the above, the article verifies not only training needs articulated by employees of Health Care Centers (HCC) operating in the Podkarpacie Province, but also the way they are perceived by the managers of these centers, the ability to define training needs and their compatibility with characteristic features of analyzed HCCs. Therefore special emphasis has been placed on demonstrating the variety of diagnosed training needs with reference to such criteria as the size of analyzed centers, the market serviced by them, and their location. These determinants allowed us to conduct a complex analysis of conditions and structure of voiced need for subject training, and as a consequence, contributed to diagnosing the expectations of the health care sector concerning initiatives improving the quality of public services in the health care services area.
