There is a growing number of elderly patients, so it is necessary to create new standards of oncologic care for such individuals in order to provide them with the best possible treatment. An elderly woman was treated for locally advanced small-cell lung cancer. Due to the suspicion of coronary disease, arterial hypertension and age, anti-cancer treatment with carboplatin and etoposide was recommended. When carboplatin infusion came to a stop, signs of myocardial infarction in ECG as well as elevated levels of troponin I were reported. Originally, non-invasive treatment was introduced, but several days later three DES stents were placed in coronary arteries. An attempt was made to treat the patient with cisplatin and etoposide, after which respiratory failure, tumor lysis syndrome and pancytopenia occurred. That is why chemotherapy was discontinued at the time. The patient’s tumor area and brain was irradiated. 16 months later, she is still alive without signs of disease progression. New oncologic standards should be elaborated in order to ensure appropriate treatment for elderly patients.

was to evaluate the clinical spectrum of emergency surgery for acute abdominal disorders and their outcome. Material and methods. The study group comprised 1426 patients, aged between 10 and 92 years subject to emergency surgery, due to an acute abdomen during the period 2001-2004. Analysis comprised age, sex, concomitant diseases, ASA scale classification, postoperative diagnosis, type of surgery, complications, mortality and duration of hospitalization. Patients were divided into two age groups: <60 and ≥60 years. Results. Appendicitis was the most common diagnosis (52.9%) in patients under 60 years, while cholecystitis (32.5%) and ileus (30.9%) in patients over 60 years. Complications were observed in 14.8% patients, the most common being related with wound healing (5.6%). The mortality rate amounted to 5.7%. Mortality was most often associated with bowel obstruction (29.2%), surgery for acute bowel ischemia (25.5%), and bowel perforation (20.7%). The mean duration of hospitalization was 7.9 days. Conclusions. 1. In comparison to elective surgery, emergency abdominal operations, particularly in elderly patients are related with a higher mortality rate. 2. In elderly patients, the high mortality rate and substantial number of complications is associated with the advanced primary disease and severe coexisting comorbidities, which significantly reduce the overall health condition.

Introduction: Acute appendicitis (AA) is the most common indication for emergency surgery and it occurs more often in children and young adults than in patients of advanced age. AA in older patients is a challenging surgical problem because of its atypical presentation. This study was performed to determine whether the age of a patient impacts the outcomes and whether laparoscopic appendectomy (LA) is a feasible method for treatment of patients > 65 years. Material and methods: We performed a retrospective study of 355 patients with AA who were admitted to the Department of General, Minimally Invasive and Elderly Surgery in Olsztyn from 2014 to 2017. The patients were divided into three age groups: 18 to 40, 41 to 65 and >65 years. The histopathological diagnoses were divided into three types: simple AA, phlegmonous AA, and gangrenous AA. Results. LA was performed in 96% of young adults and in 67% of older patients. The patients older than 65 years had higher preoperative white blood cell counts, higher C-reactive protein (CRP) levels and had a longer length of stay (LOS) than younger patients (P=0.05, P=0.03, and P=0.03, respectively). We found a positive correlation among the CRP levels, open appendectomy (OA), and gangrenous appendicitis. Conclusions: Patients older than 65 years more frequently underwent OA than LA, had higher preoperative CRP levels and had a longer LOS than younger patients. Higher CRP levels were associated with a greater risk of gangrenous appendicitis. LA is a safe and feasible treatment method for patients older than 65 years.

Introduction. In Poland, it is estimated that HF is observed in approximately 0,7 million of patients. The aim of the study was to assess health behaviour in geriatric patients diagnosed with HF regarding pharmacotherapy and non-pharmacological treatment. Material and methods. The study group comprised 72 patients with diagnosed HF, over 65y. The study was performed using a questionnaire survey including 20 closed-ended questions. Results. The mean number of medicinal products prescribed by physicians for the treatment of HF and other diseases was 6.59±1.8. 50% of subjects were additionally taking medications and supplements for self-treatment, 63% declared they managed to take medications prescribed on their own. A diet is followed only by 50%, but only 18% limit their sodium intake. 46% still consume alcohol, and 10% of still smoke cigarettes. 46% declared the use of body weight monitoring, but none of them measured their body weight every day. Conclusion. Health behaviour of geriatric patients with HF in Poland indicates their low compliance with medical guidelines and it may explain a high hospitalisation rate and increasing costs of treatment.

The aim of the study was to evaluate the results of general surgical treatment of patients over 80 years of age.Material and methods. Three hundred and four patients aged 80 to 105 years with general surgical disorders, treated in 2005-2009, were retrospectively included in the study. The collected information included demographic data, coexisting diseases, the mode of admission, the diagnosis, the method and result of treatment, and also potential complications and 30-day mortality. The data were subjected to statistical analysis.Results. The study group included 186 women and 118 men. Two hundred patients (65.8%) were admitted in an emergency setting. The most common causes of immediate hospitalisation were: mechanical ileus (26.5%), gastrointestinal bleeding (22%), trauma (16%), and gall-bladder disorders (8.5%). The remaining 104 (34.2%) patients were operated upon on an elective basis. An emergency operation was required by 121 (60%) of the patients admitted in an emergency setting; the remaining ones were treated conservatively. Hernia plasties (27.5%), cholecystectomies (15.3%), colorectal resections (13.2%), strumectomies (11.2%) and endoscopies (6.1%) predominated among elective surgeries.The total number of complications and mortality were 19.4% and 14.5%, respectively. The number of complications and mortality were significantly higher in the group of patients admitted in an emergency setting (25.5% and 20.5%, respectively) than in patients admitted on an elective basis (8.7% and 2.9%, respectively), p<0.01.The mean duration of hospitalisation was 9.7 days (1 to 60 days), with a small difference between the groups of patients treated on an elective and emergency basis (8.5 and 10.4 days), p=0.181.Conclusions. The results of surgical treatment of elderly patients do not significantly differ from the results of treatment of the general population. Much worse results, coupled with a significant increase in mortality, are observed in patients admitted and treated on an emergency basis.

Purpose: To examine the relationship between depression and quality of life in elderly patients with glaucoma. Materials and methods: The study was conducted as a descriptive and cross-sectional research. The sample of the work consists of 130 patients aged 60 years and over who were followed up at Mersin University Hospital Ophtalmology Department between 01 October 2016 and 31 March 2017. The data were gathered with Personal Information Form, Geriatric Depression Scale and Modifiye GLAU-QOL 17 Glaucoma Quality of life. Number, percentage, mean, Pearson Correlation, Student`s t test, ANOVA test, Tukey and Games-Howell statistic were used in the evaluation of the data. Results: The mean age of the patients was 67.9±7.27. The mean score of geriatric depression scale of the patients was 15,23±5,52. It was determined that 16,2% of the patiens had possible depression and 62,3% had definite depression. The median Glau–QOL-17 Glaucoma Quality of Life Questionnaire subcale scores of the patients were as follows: daily living (3,58±3,25), driving (3,23±2,53), worry (5,26±3,47), self-assesment (6,4±3,07), psychology (4,08±2,74), feeling oppressed (4,39±2,35) and taking responsibility (3,56±1,86). The mean total score was 34.44±15.29. There was a negative and statistically significant relationship between the age of the patients and the scores of geriatric depression and the scores of age and quality of life. Conclusions: Elderly patients with glaucoma are at a major risk for depression and their quality of life is negatively affected.

Technical and pharmacological revolution in contemporary medicine has not resolved the ethical problems that seem to be more relevant today than ever before. Most of the papers that concern medical ethics focus on `great' problems, such as human genome sequencing, organizms cloning and quantitative or qualitative interference with life creation. This article, however, describes practical ethical problems that the doctors face in every-day practice. First of all, the problem of how thestandards of prophilaxis and treatment should be established when the costs exceed financial possibilities of a health system. Secondly, how to face the problem of rare diseases when the cost of a single patient treatment may be equal to the whole medical department budget. Thirdly, how to procede with elderly patients with multiple diseases qualified for invasive procedures. All of these examples share the common base: in these cases the technical equipment is at our disposal, yet we cannot or we do not want to use it.

The number of elderly patients undergoing liver transplantation (LT) is increasing worldwide. The aim of the study was to evaluate the impact of recipient age exceeding 60 years on early and long-term outcomes after LT. Material and methods. This study comprised data of 786 patients after primary LT performed at a single center between January 2005 and October 2012. Patients over and under 60 years of age were compared with respect to baseline characteristics and outcomes: postoperative mortality (90-day) and 5-year patient (PS) and graft (GS) survival. Associations between recipient age exceeding 60 years and LT results were assessed in multiple Cox regression models. Results. Recipients older than 60 years (n=107; 13.6%) were characterized by more frequent hepatitis C virus infections (p<0.001), malignancies (p<0.001), and cardiovascular comorbidities (p<0.001); less frequent primary sclerosing cholangitis (p=0.002) and Roux-en-Y hepaticojejunostomy (p<0.001); lower Model for End-stage Liver Disease (MELD; p=0.043); and increased donor age (p=0.012). Fiveyear PS of older and younger recipients was 72.7% and 80.6% (p=0.538), while the corresponding rates of GS were 70.3% and 77.5% (p=0.548), respectively. Recipient age exceeding 60 years was not significantly associated with postoperative mortality (p=0.215), PS (p=0.525) and GS (p=0.572) in multivariate analyses. The list of independent predictors comprised MELD (p<0.001) for postoperative mortality; malignancies (p=0.003) and MELD (p<0.001) for PS; and malignancies (p=0.003), MELD (p<0.001) and donor age (p=0.017) for GS. Conclusions. Despite major differences between elderly and young patients, chronological age exceeding 60 years alone should not be considered as a contraindication for LT.

Introduction. Medical care in geriatrics has its own specificity, and in order to build a successful doctor-patient relationship it is important while studying medicine to learn some functioning aspects of this age group. Objective. The aim of this study is to explore and describe the role of emotional intelligence in the attitudes of medical students towards elderly patients, taking into account their the place of residence. The place of residence has an impact on social relations and socialization, which could effect with disparities in behaviour towards elderly patients. Materials and method. Medical University of Lublin students from rural (N=71; M=23.44 SD=1.80) and urban (N=87; M=23.34; SD=1.38) areas took part in the study. Emotional Intelligence Questionnaire (INTE) and Attitude Towards Elderly Patients Questionnaire (ATEP) were used. Results. There are statistically significant differences between medical students from rural and urban areas in their attitude towards elderly patients (p=0.001), but no differences were observed between rural and urban residents in terms of emotional intelligence (general result, action factor and cognitive factor). Positive correlations were observed between action factor (r=0.322), cognitive factor (r=0.311) and general INTE result (r=0.358) and attitude towards elderly patients in the group of medical students from rural area. Conclusions. Medical students from rural areas are characterized by a more positive attitude towards elderly patients than medical students from urban areas. The predictors of attitudes towards elderly patients are the cognitive factor of emotional intelligence and the place of residence of medical students. The results give the opportunity to design a well-developed programme of a geriatric course which could be matched to the personal predispositions of students.

Medical documentation of 4680 patients of an individual nursing practice in Jarosław, Poland, was analyzed statistically. The inclusion criteria of the conducted research were patients age ≥60 years with type 2 diabetes diagnosed by a physician. The study patients were selected with the use of a diagnostic survey method, the authors’ own questionnaire, and medical documentation analysis. As a result, the study involved 170 patients. The chi-square test of independence and the Mann-Whitney test were applied to verify the differences between the variables. The majority of the studied patients were women (61.1%), in the 71–80 age group (57.6%). Irrespective of gender, diabetes was most often diagnosed in people in early senility, aged 60–70 years (62.9%). Most of the respondents received oral medications (59.4%); had a large waist circumference (92.9%), as well as high body mass index (95.3%). In the majority of the elderly, no hypoglycaemia (83.0%) or hyperglycaemia (77.6%) were recognized. However, numerous complications were diagnosed, namely dyslipidaemia (85.9%), hypertension (81.8%), macroangiopathy (90.0%), microangiopathy (71.2%), and ophthalmological diseases (25.9%). Most diabetics suffer from overweight or obesity, and long-term diabetes complications. Analyzing the incidence of diabetes and its complications should help to outline further actions that need to be taken to diagnose and prevent the disease in order to detect its complications early and prolong the patients’ survival as well as raise their quality of life.
Analizie statystycznej poddano dokumentację 4680 pacjentów Indywidualnej Praktyki Pielęgniarskiej w Jarosławiu. Kryterium doboru był wiek ≥60 r.ż. oraz rozpoznana przez lekarza cukrzyca typu II. Materiał badawczy uzyskano za pomocą metody sondażu diagnostycznego z wykorzystaniem autorskiego kwestionariusza ankiety oraz analizy dokumentacji medycznej. Do badań zakwalifikowano 170 pacjentów. Weryfikacji różnic między zmiennymi dokonano przy użyciu testu niezależności x2 oraz testu Manna-Whitneya. Wśród badanych przeważały kobiety (61.1%), w przedziale wiekowym 71-80 lat (57.6%). Niezależnie od płci cukrzyca najczęściej była rozpoznawana u osób we wczesnej starości, w wieku 60-70 lat (62.9%). Największa część ankietowanych była leczona lekami doustnymi (59.4%), u większości obwód tali był zbyt duży (92.9%), podobnie jak wynik BMI (95.3%). U większości seniorów nie występowała hipoglikemia (83.0%) i hiperglikemia (77.6%). Natomiast stwierdzono liczne powikłania w postaci dyslipidemii (85.9%), podwyższonych wartości ciśnienia tętniczego (81.8%), makro- (90.0%) i mikroangiopatii (71.2%) oraz chorób oczu (25.9%). Większość osób chorujących na cukrzycę cierpi na nadwagę i otyłość oraz ma późne powikłania cukrzycy. Analiza częstotliwości zachorowań na cukrzycę i jej powikłań powinna pomóc w wyznaczeniu dalszych działań w kierunku rozpoznawania, zapobiegania i wczesnego wykrywania tego schorzenia, co pozwoli chorym zwiększyć szanse na dłuższe życie i podnieść jego jakość.
