Opis:
The sources of free radicals and antioxidant defence systems in living organisms are reviewed. Attention is focused on the role of dietary lipid oxidation products and antioxidants to maintain balance between factors promoting free radical mediated oxidation and the antioxidant defence mechanisms. Particular attention is paid to the influence of intake of vitamins with oxidative properties (vitamin C, vitamin E and ß-carotene) and polyphenols compounds in the prevention of coronary heart disease and cancer.

Opis:
The expectation that computer networks combined with the Internet and associated technologies will be common in all businesses of the future has resulted in a flurry of investment in ICT around the globe, and in wide interest in the Internet, e-commerce and e-business. For example the Malaysian government has created a high-speed data highway, a federal capital that is a 'wired city', and a 'paperless hospital' [1]. The UK Government too has recently approved plans for major ICT projects [5, 10]. These projects are in healthcare, and involve large financial sums. It is important that the use of these public funds leads to successful outcomes. This paper analyses a large successful ICT project that uses e-business technologies. The aim of the paper is to identify the success factors in this completed project to see if these pointers indicate favourable prospects for the outcome of these new healthcare projects.

A fundamental pre-requisite for the establishment of a scaleable regional health information network (RHIN) is the development of an architectural framework and tools for the integration of specialized autonomous systems and e-health service platforms supported by an underlying health information infrastructure (HII). In this context, HYGEIAnet, which is the RHIN of Crete in Greece, has identified and utilized a number of critical software components enabling integrated access to clinically significant information, based on an open architecture addressing successfully the various interoperability challenges at hand. HYGEIAnet provides the framework for the reuse of standardized common components and public interfaces, thus enabling integrated and personalized delivery of healthcare.

Continuing medical education is considered as a very important aspect in the development of skills of practicing physicians and the introduction of new concepts and developments in health care provision. Studies have shown that conventional CME techniques including self-studying and lecture attending have limited impact on clinical practices. The world wide web provides a very efficient and cost-effective delivery system for conveying anywhere and anytime multimedia information to large user groups. Although there are a number of CME web sites available today, the majority of them are static and text-based thus not offering interactive functionality and multiple content. To exploit modern telematics technologies and within the framework of the Leonardo da Vinci programme, the REMEDIES project developed an interactive web-based distance-training infrastructure. The REMEDIES system incorporates two training modules - a web-based training platform and a teleconference training platform. The system has been validated in terms of technical performance and user functionality in a controlled target group comprising of young medical doctors and medical students in the specific areas of radiology and laparoscopic surgery. The results of the trial have demonstrated the compliance of the system to the defined functional requirements and the potential usefulness to the defined training goals. Trial results have also demonstrated the need of extension of the educational material within the platform especially for the purposes of continuous education.

Opis:
The scenarios of cooperation between personal healthcare systems are discussed in this paper. Current telemedical solutions for home care provide the patient with a seamless diagnosis based on selected vital parameters. The application of two or more specialized systems monitoring the same subject rises the question of possible interference and cooperation. The cooperation between home care and wearable systems is of particular interest, because of their complementary features. Considerations of various cooperation scenarios led to the specification of three cooperation levels with regard to the required software integration. The paper presents also the design of a cooperation protocol for two prototype surveillance systems, enabling the use of communication resources of the stationary system to carry the messages of the wearable system over the wired transmission channel.

Opis:
ICT (Information and Communication Technology) may be powerful source of the innovation in health care in many areas, but change of the model of care delivery and of the relations between participants of the health care market is the most visible trace. The access to relevant information became a prerequisite to patient empowerment in his or her relation with health care professionals. Nowadays, the patient is expected to take active role in care processes and abandon the attitude of a passive recipient of medical services. There are also many challenges for health care professionals. Apart from the requirement of partnership relations with patients, health care professionals are obliged to follow evidence-based guidelines in their practice. The quality assurance strategies became inherent element of clinical activities. E-health environment brings a set of tools which can support all players active in health care domain in smooth adoption of new policies and requirements. The paper focuses on the issues of enhancement of access to health-related information by patients and health care professionals, medical information management and patient safety achieved with e-health technologies. A new model of health care delivery, especially for chronic care, based on the intensive use of e-health systems is proposed.

W artykule prezentowane są zmiany, jakie dokonują się w relacjach lekarz – pacjent pod wpływem rozwoju nowych technologii komunikowania, a zwłaszcza Internetu. Autor przedstawia wybrane modele, których tendencją jest odchodzenie od paternalizmu tradycji Hipokratesowej i rezygnacja z władzy lekarza na rzecz poszerzenia autonomii, podmiotowości i odpowiedzialności pacjenta. W realnych sytuacjach na kształt relacji lekarz – pacjent mają wpływ uwarunkowania polityczne, gospodarcze, społeczne. Powstanie telemedycyny i wykorzystanie Internetu w opiece zdrowotnej zmienia tradycyjny układ zależności oparty na władzy-wiedzy. Pojawiają się pojęcia charakteryzujące nowe sytuacje komunikacyjne, np. e-pacjent, e-zdrowie, e-lekarz, które zmieniają kontekst relacji lekarz – pacjent. Internet popularyzuje wiedzę medyczną, pozbawiając ją „aury tajemniczości”, staje się przestrzenią nowych relacji i form komunikacji o zdrowiu, chorobie i medycynie
The article presents changes taking place in the relationship between a physician and a patient after the advent of new communication technologies, particularly the Internet. The author presents selected models, which tend to move away from paternalistic tradition of Hippocrates and resign from power of physician towards more patient’s autonomy, subjectivity and responsibility. In real life situations, the relationship between physician and patient is shaped by political, economic and social conditions. The advent of telemedicine and use of the Internet in the health care system change the traditional system of dependencies based on power-knowledge. New notions that characterize new communication situations emerge, e.g. e-patient, e-health, e-doctor, which change the context of relationship between a physician and a patient. The Internet popularizes medical knowledge depriving it of “aura of mystery”. It becomes the space of new relations and forms of communication about health, disease and medicine.

Guidelines developed in line with Evidence-Based Medicine paradigm usually take form of narrative recommendations formulated on the basis of available evidence resulting from clinical trials and other types of studies. For past ten years there were developed several Formal Guidelines Representation languages that attempted to express them in form of algorithms or processes. Analysis of specific requirements related to e-health and in particular telemonitoring systems indicated, that a process oriented formalization of guidelines is in this case inappropriate, as such systems should be reactive, event driven and enable adaptation to cope with the quality of entered data. In this paper we describe an approach to guidelines implementation in the e-health system. Contrary to various FGR languages developed earlier the decision support will be based on the set of fuzzy rules that on the technical level will be implemented as XQuery transformations of XML data. We discuss this approach on an example related to the management of bronchial asthma, as customization of the system to this disease was selected as one of the proof-of-concept exemplifications of the system.
Wytyczne medyczne tworzone zgodnie z paradygmatem medycyny opartej na dowodach mają formę tekstowych rekomendacji popartych opisem przeprowadzonych badań klinicznych i obserwacji. W ostatnich dziesięciu latach rozwijano szereg języków formalnej reprezentacji wytycznych, za pomocą których przedstawiano je w postaci algorytmów lub procesów. Analiza wymagań związanych z systemami typu e-health, a zwłaszcza systemów telemonitorowania wskazała, że procesowo zorientowana formalizacja wytycznych jest dla nich niewłaściwa, ponieważ takie systemy powinny być reaktywne, sterowane zdarzeniowo i powinny mieć wbudowane mechanizmy adaptacji pozwalające na dostosowanie się do zmiennej jakości wprowadzanych danych. W procesie telemonitorowania zaproponowano wykorzystanie rozmytych reguł, które będą zaimplementowane jako transformacje XQuery dla danych XML. Podejście to jest dyskutowane na przykładzie astmy oskrzelowej, ponieważ wsparcie dla tej choroby zostało wybrane jako przykład wdrożenia systemu.

Telecommunications technology is making its way into operating rooms by new developments in e-health. However, conflicts arise with existing legal principles regarding data protection. This paper deals with key elements of the interactions between data protection and evolution in e-health. The scope will be the digital operating room, where different health services and activities converge through networked technology, raising a number of privacy-related issues. For instance, the patient's health records and tools for recording surgical procedures could be integrated within the same platform, potentially leading to sensitive personal data linkage. Also the possible duration and reason of storage of surgical recordings, is a matter that remains largely unresolved in current practice. First, this paper will analyze the data exchanges of the digital operating room. As these will include personal patient data, it must be assessed whether and how the European framework on data protection can apply. Second, the regulatory regime of the manufacturers of the devices of the digital operating room will be analyzed. Can the current legal framework relating to e-health provide for suitable regulation for such devices? Drawing from experience gained in research projects, this paper aims to provide practical answers to often theoretical questions.

This article describes the idea of introducing information technologies to hospital care and provides an overview of their implementation in Poland as one of the European Union member states. Special emphasis is put on Warsaw hospitals. The current state of their hospital information systems is further described based on the data recently collected by the Polish Supreme Audit Office. Additionally, the article includes two case studies, which give insight into everyday application of hospital information systems in a general hospital and a specialist institute.
