Background. Gestational Diabetes Mellitus (GDM) is a glucose tolerance disorder posing health risks for both the mother and fetus. Lifestyle modifications are recommended to control glucose levels before pharmacotherapy. The aims of this study were 1) to examine the impact of combined GDM intervention on the glycemia levels and pregnancy outcomes in a woman with GDM; and 2) to explore what can help women adhere to the desired lifestyle changes recommended for GDM. Material and methods. This instrumental study uses a mixed-method approach, combining quantitative and qualitative methods. It focuses on a comprehensive lifestyle intervention, incorporating exercise, dietary protocol, maternal education, and support. One participant was enrolled, starting the intervention at the 24th week of pregnancy. Low glycemic index (<55%) and low glycemic load meals were recommended. Exercises were supervised and conducted regularly three times a week until delivery (48 sessions in total). Results. Glucose levels remained within normal range throughout the entire pregnancy, without the need for pharmacotherapy. The pregnancy resulted in a natural delivery of a healthy newborn with normal parameters. The participant completed the entire intervention, implementing the recommended lifestyle changes and avoiding complications associated with GDM. Conclusions. 1. Individualized and holistic care for women with GDM can help maintain normal glucose levels and increase the chances of positive pregnancy outcomes. 2. Key factors contributing to the success of the intervention were emotional support, education, and personalized care provided by specialists. 3. Implementing lifestyle changes during pregnancy may reduce the need for pharmacological interventions and complications related to GDM.
Wprowadzenie. Cukrzyca ciążowa (GDM) to zaburzenie tolerancji glukozy, które stwarza zagrożenie dla zdrowia matki i płodu. Zaleca się, aby kobiety wprowadzały modyfikacje stylu życia w celu kontrolowania poziomu glukozy zanim będzie potrzebna farmakoterapia. Celem pracy było zatem 1) zbadanie wpływu kompleksowej interwencji w stylu życia z GDM na poziom glikemii i wyniki ciąży u kobiety z GDM; oraz 2) zbadanie, co może pomóc kobietom w przestrzeganiu pożądanych zmian stylu życia zalecanych w GDM. Materiał i metody. W badaniu instrumentalnym wykorzystano podejście mieszane, łącząc metody ilościowe i jakościowe. Skoncentrowano się na kompleksowej interwencji w stylu życia dla ciężarnych z GDM, łączącej ćwiczenia fizyczne, interwencję żywieniową, edukację i wsparcie matki. Jako uczestniczkę przyjęto jedną kobietę. Interwencja rozpoczęła się w 24. tygodniu ciąży. Zalecano niski indeks glikemiczny (<55%) i posiłki o niskim ładunku glikemicznym. Ćwiczenia fizyczne odbywały się regularnie i były nadzorowane, trzy razy w tygodniu do momentu porodu (łącznie 48 sesji). Wyniki. Poziomy glukozy pozostały w normie przez całą ciążę, bez konieczności farmakoterapii. Ciąża zakończyła się porodem naturalnym zdrowego noworodka o prawidłowym parametrach. Uczestniczka zakończyła interwencję pomyślnie, wprowadzając rekomendowane zmiany w stylu życia i unikając powikłań związanych z GDM. Wnioski. 1. Zindywidualizowana i holistyczna opieka nad kobietą z GDM, może pomóc w utrzymaniu prawidłowych poziomów glukozy i pozytywnych wyników ciąży. 2. Czynnikami kluczowymi wpływającymi na sukces interwencji były wsparcie emocjonalne, edukacja i zindywidualizowana opieka specjalistów. 3. Wprowadzenie zmian w stylu życia podczas ciąży może przyczynić się do zmniejszenia konieczności interwencji farmakologicznych i powikłań związanych z cukrzycą ciążową.

Several chemical compounds found in plant products have proven to possess beneficial properties, being currently pointed out due to their pharmacological potential in type 2 diabetes mellitus complications. In this context, we studied the effect of Geranium robertianum L. (herb Robert) leaf decoctions in Goto-Kakizaki (GK) rats, a model of type 2 diabetes. Our results showed that oral administration of G. robertianum leaf decoctions over a period of four weeks lowered the plasma glucose levels in diabetic rats. Furthermore, the treatment with G. robertianum extracts improved liver mitochondrial respiratory parameters (state 3, state 4 and FCCP-stimulated respiration) and increased oxidative phosphorylation efficiency.

Based on overlapping structural requirements for both efficient aldose reductase inhibitors and PPAR ligands, [5-(benzyloxy)-1H-indol-1-yl]acetic acid (compound 1) was assessed for inhibition of aldose reductase and ability to interfere with PPARγ. Aldose reductase inhibition by 1 was characterized by IC50 in submicromolar and low micromolar range, for rat and human enzyme, respectively. Selectivity in relation to the closely related rat kidney aldehyde reductase was characterized by approx. factor 50. At organ level in isolated rat lenses, compound 1 significantly inhibited accumulation of sorbitol in a concentration-dependent manner. To identify crucial interactions within the enzyme binding site, molecular docking simulations were performed. Based on luciferase reporter assays, compound 1 was found to act as a ligand for PPARγ, yet with rather low activity. On balance, compound 1 is suggested as a promising lead-like scaffold for agents with the potential to interfere with multiple targets in diabetes.

Opis:
Background: Cardiovascular proteomics investigation reveals the characterization and elucidation of the novel therapeutic targets and strategies to prevent the development of heart failure associated diabetic complication by using 2DE and MS. Methods: The experimental animals were made diabetic with a single intraperitoneal injection of alloxan (150 mg/kg of bw). Albino rats were randomly divided into four individual groups: Group-I control (n=6), group-II alloxan-induced diabetic rats, untreated (n=6), group-III (n=6) and group-IV (n=6) alloxan-induced diabetic rats were treated with aqueous and ethanolic extracts of Cynodon dactylon for 15 days, respectively. Animals were euthanized to collect the heart tissues and blood samples. 2DE sample preparation, gel running and staining (n=6: each groups) were performed at the same time to avoid variation. The result of six gel images from each group were analyzed and evaluated as one match set with 2D software (P<0.05). Results: The above experiment revealed two up-regulated proteins in group-II i.e. NTF4 and ETFB. Conclusions: NTF4 is a neuro-protective agent for neuro-degenerative diseases. It will prevent diabetic secondary complications, such as diabetic polyneuropathy and cardiomyopathy. ETFB is active in the mitochondria, the energy-producing centres in cells. It is clear from the experiment that because of up-regulation of ETFB more energy is availabile and the electron transfer for heart during diabetes is possible, what leads to reduce the oxidative stress and free-radical formation. The up-regulated proteins reduced CVD that occurred just before overt hyperglycaemia due to administration of C. dactylon. This approach established the preliminary reference map for decoding cellular mechanisms linked between pathogenesis CVD and diabetes.

Purpose: Diabetes is a chronic disease that pays for a large proportion of the nation's healthcare expenses when people with diabetes want medical care continuously. Several complications will occur if the polymer disorder is not treated and unrecognizable. The prescribed condition leads to a diagnostic center and a doctor's intention. One of the real-world subjects essential is to find the first phase of the polytechnic. In this work, basically a survey that has been analyzed in several parameters within the poly-infected disorder diagnosis. It resembles the classification algorithms of data collection that plays an important role in the data collection method. Automation of polygenic disorder analysis, as well as another machine learning algorithm. Design/methodology/approach: This paper provides extensive surveys of different analogies which have been used for the analysis of medical data, For the purpose of early detection of polygenic disorder. This paper takes into consideration methods such as J48, CART, SVMs and KNN square, this paper also conducts a formal surveying of all the studies, and provides a conclusion at the end. Findings: This surveying has been analyzed on several parameters within the poly-infected disorder diagnosis. It resembles that the classification algorithms of data collection plays an important role in the data collection method in Automation of polygenic disorder analysis, as well as another machine learning algorithm. Practical implications: This paper will help future researchers in the field of Healthcare, specifically in the domain of diabetes, to understand differences between classification algorithms. Originality/value: This paper will help in comparing machine learning algorithms by going through results and selecting the appropriate approach based on requirements.

The activity of Cu,Zn superoxide dismutase in the fluid obtained from eye lens capsules after cataract surgery was investigated in samples obtained from patients with senile cataract and with senile cataract combined with diabetes mellitus. Two parameters were measured and compared: the frequency of occurrence of detected superoxide dismutase activity and the relative activity of the enzyme in samples derived from senile cataract patients versus those from the patiens affected additionally by diabetes mellitus. It was confirmed that the decrease of superoxide dismutase activity during cataract was additionally promoted by diabetes mellitus.

Introduction: According to the WHO, an estimated 422 million people are suffering from diabetes worldwide. Among them, the incidence of cholelithiasis is higher than in the healthy population. The aim of this literature review was to summarize the available evidence about acute cholecystitis in patients with diabetes. Materials and methods: This study adhered to the PRISMA guidelines. The course of hospitalization of patients with and without diabetes who underwent cholecystectomy due to acute cholecystitis was compared. Following information was abstracted from original studies: general study information, patient characteristics, complications, and recommendations for patients with diabetes. Results: Initial search provided 1632 results. After full text assessment, 40 studies met the inclusions criteria. Operative and postoperative complication rates were significantly higher among the diabetic patients. Diabetes is a risk factor for conversion from laparoscopic to open cholecystectomy method. The authors' opinions on elective surgery before the onset of acute cholecystitis symptoms are divided. Conclusions: Diabetic patients are at greater risk of developing complications. An individualized screening and treatment approach, as well as proper preparation of the diabetic patient for an elective cholecystectomy could have a positive effect on the outcome. However, the low quality of the data from the systematic review does not allow for meta-analysis, which is why we cannot draw strong conclusions.

Oxidative stress, capable of eliciting damage to various biomolecules including DNA, is a recognized component of diabetes mellitus and its complications. Metabolic syndrome (MetS) is associated with the development of type 2 diabetes mellitus (T2DM), as well as other unfavorable outcomes. The aim of this study was to elucidate the role of oxidative stress in the development of T2DM, by investigating association of oxidative DNA damage with metabolic parameters in subjects with MetS and early T2DM. Selected anthropometric and biochemical parameters of MetS, inflammation and oxidative DNA damage: body mass index (BMI), fatty liver index (FLI), waist circumference (WC), total cholesterol, HDL and LDL-cholesterol, gamma-glutamyl transpeptidase (GGT), uric acid, C-reactive protein (CRP), total leukocyte/neutrophil count, and urinary 8-hidroxy-deoxyguanosine (u-8-OHdG) were assessed in male subjects with MetS and both younger (≤55 years) and older (>55 years) subjects with T2DM of short duration without complications. BMI, FLI, WC, total and LDL-cholesterol and uric acid were higher, while the u-8-OHdG was lower in MetS group, when compared to older T2DM subjects. None of these parameters were different neither between MetS and younger T2DM, nor between two sub-groups of subjects with T2DM. Values of CRP, HDL-cholesterol, triglycerides, GGT, leukocytes and neutrophils were not different between all examined groups of subjects. Higher 8-OHdG in older subjects with T2DM suggests that both aging process and diabetes could contribute to the development of DNA damage. Oxidative DNA damage cannot serve as an universal early marker of T2DM.

Celem pracy była ocena aktywności fizycznej i sposobu wypoczywania pacjentów chorych na cukrzycą typu 2. Badania przeprowadzono w ośrodkach zdrowia w województwie małopolskim w 2008 roku. Anonimowe ankiety wypełniło 100 osób (66 kobiet i 34 mężczyzn) w wieku od 40 do 84 lat. Większość badanych odpoczywała biernie. Połowa ankietowanych wskazała na leżenie, oglądanie telewizji i słuchanie muzyki jako sposób wypoczynku. Znaczna większość chorych nie stosowała aktywności fizycznej jako niezbędnego elementu w leczeniu cukrzycy. Najczęściej (systematycznie) stosowaną formą aktywności fizycznej były: hodowla, ogrodnictwo i spacer. Zły stan zdrowia zniechęcał ankietowanych do podejmowania aktywności fizycznej.
The aim of the study was to evaluate physical activity and resting manners of type 2 diabetes sufferers. The research was conducted in health care centres in Małopolskie voivodship in 2008. 100 people (66 women, 34 men) aged between 40 and 84 filled in anonymous questionnaires. The majority of the respondents rested passively; half of the surveyed indicating lying, watching TV and listening to music as a way of resting. The majority of the ill did not embark upon physical activity as an essential element in treating diabetes. The most frequently (systematically) practised forms of physical activity were animal breeding, gardening and strolling. Poor health condition discouraged the surveyed from undertaking physical activity.

Cukrzyca to „epidemia XXI wieku”, szacuje się, że w 2011 roku na cukrzycę chorowało 6,7% populacji Polski, z czego jedna czwarta nie miała świadomości choroby. Tak duże rozpowszechnienie cukrzycy sprawia, że coraz więcej lekarzy staje przed problemem prawidłowej kwalifikacji pacjenta z cukrzycą do amatorskiego uprawiania sportu, w tym nurkowania. W niniejszej publikacji przedstawiono najważniejsze informacje dotyczące oceny ryzyka, dopuszczenia do amatorskiego nurkowania pacjentów z cukrzycą oraz proponowanych protokołów postępowania przed i podczas uprawiania tego sportu.
Diabetes is an "epidemic of the 21st century" and it is estimated that in 2011 6.7% of Poland's population suffered from diabetes, one quarter of which was unaware of the disease. Such a high prevalence of diabetes mellitus results in an increasing number of physicians being confronted with the problem of qualifying patient with diabetes to partake in amateur sports, including diving. This publication presents the most important information concerning risk assessment, admission of diabetic patients to amateur diving and some proposed protocols of management before and during sport practice.

Introduction. Quality of Life (QoL) of Polish women and men with type 1 diabetes (T1DM) and type 2 diabetes (T2DM) was analyzed, taking into consideration acceptance of the illness, the occurrence of depression, complications of diabetes, concentration of glycated haemoglobin, and demographic factors. Materials and method. The study was conducted among 115 patients with T1DM and 215 patients with T2DM aged 18–60. The patients were divided into women and men. The tool applied for studying QoL was the Polish language version of the Audit of diabetes-dependent QoL questionnaire(ADDQoL) comprising 2 questions related to the general QoL and 19 domains related to aspects of life. Each domain included 2 components: Impact and Importance, and their product determined the value of the weighted impact score. The Acceptance of Illness Scale, Beck Depression Inventory and studied demographic and clinical parameters were also applied. Results. Patients with both types of diabetes demonstrated a negative influence of the disease in all domains of ADDQoL. Values of the average weighted impact of ADDQoL showed significant associations with diabetic complications in T1DM and gender and depressive symptoms in T2DM. Diabetes negatively affects the QoL of diabetic patients in Poland, especially regarding freedom to eat and to drink and sex life in both genders in T1DM, and freedom to eat and drink, and feelings about the future in both genders, and working life and sex life in T2DM men. Conclusions. Risk factors for worse QoL are: the occurrence of diabetes complications in T1DM patients and male gender and depression in T2DM patients.

Background: Knowledge about diabetes mellitus (DM)-related complications, the direct and indirect costs incur red by patients with DM is still insufficient in Poland. Economedica DM-PL (Economedica) survey was a cross-sectional observational study that aimed to assess these issues. Material and methods: The survey was carried out in 2016 on a representative sample of 2032 diabetic patients recruited by 195 general practioners (GP) and 48 diabetologists (D) randomly selected from the national registry of physicians. Each GP and D recruited 8 to 10 patients with DM according to the specified protocol. The study included 1938 medical questionnaires (comple ted by the GPs and Ds), 1938 questionnaires about costs (completed by the patients) and 1532 treatment adherence questionnaires. Results: The Economedica survey provided new insights into DM epidemiological and cost data. 70% of patients with type-1 DM were diagnosed with the disease before the age of 40. For 76% patients with type-2 DM, it was diagnosed aeftr the age of 50. The average monthly out-of-pocket medical costs for DM patients in 2016 reached 66.2 EUR, while their non-medical costs were 36.8 EUR. Conclusions: The methodology applied in our study proved useful for providing new dimensions of DM epidemiological and cost data in Poland.

Background. There are few updated data on rates of hospital mortality of diabetic patients and length of their hospital stay on a country level. To determine such rates we provided analysis using claims data from a Nationwide General Hospital Morbidity Study carried out by the National Institute of Public Health – National Institute of Hygiene (NIPH- NIH) in Warsaw from 2010 to 2018. Objective. The aim of the study was to analyze the nine years changes of in-hospital morbidity and mortality in diabetic patients and length of hospital stay using a comparative approach by gender, age and place of residence. Material and methods. The data on all patients from general hospitals in Poland treated because of diabetes were taken from a nationwide database, kept since 1979 by the Department for Monitoring and Analyses of Population Health of NIPH-NIH. This database contains information gathered under the Statistical Research Program of Public Statistics. Hospitalization rates were used to evaluate the ‘hospitalized’ incidence of diabetes (number of hospitalization cases due to diabetes per year by the analyzed unit of population). In-hospital mortality was calculated as the percentage of deceased patients out of all patients hospitalized due to diabetes. Results. The number of cases and hospitalization rates of diabetic patients was rapidly declining by 18.8% for type 2 (E11) and 23.7% for type 1 (E10) diabetes. The downward tendency in the scope of hospitalization affected mainly older women and rural residents. Hospital mortality due to diabetes rose up dangerously to 3.77% exceeding the rates recorded eight years earlier. Conclusions. The recent reduction in hospitalization rates of people with diabetes in Poland may be associated with an unexpected increase in hospital mortality.
