WSTĘP: Ze względu na znaczenie dla zdrowia publicznego problemu, jakim jest poznanie czynników psychologicznych mogących mieć znaczenie dla przebiegu choroby nowotworowej, zrealizowano badanie, którego celem była ocena czę-stości i uwarunkowań akceptacji choroby u pacjentów z rakiem jelita grubego w okresie bezpośrednio poprzedzającym interwencję chirurgiczną. MATERIAŁ I METODY: W badaniu kwestionariuszowym wzięło udział 200 pacjentów z rozpoznanym rakiem jelita grubego. W celu zidentyfikowania ostatecznych predyktorów mających wpływ na akceptację choroby nowotworowej wykonano analizę regresji logistycznej wraz z weryfikującą parametryzacją za pomocą automatycznej selekcji wstecznej. WYNIKI: Nieco ponad połowa badanych (56,5%) z rakiem jelita grubego w okresie bezpośrednio poprzedzającym interwencję chirurgiczną nie akceptuje swojej choroby. Wpływ na akceptację choroby miały zmienne: występowanie raka jelita grubego w rodzinie oraz zadowolenie z opieki medycznej. WNIOSKI: Częstsza akceptacja choroby nowotworowej jelita grubego występuje u pacjentów deklarujących zadowolenie z opieki medycznej oraz u pacjentów, u których rak jelita grubego występował w rodzinie.
INTRODUCTION: Because of the importance of the problem for public health, which is the knowledge of psychological factors that may affect the course of cancer, a study was carried out to assess the frequency and determinants of disease acceptance in patients with colorectal cancer in the period immediately preceding the surgical intervention. MATERIAL AND METHODS: 200 patients with colorectal cancer participated in the questionnaire study. In order to identify the final predictors influencing the acceptance of cancer, logistic regression analysis was performed along with the verification of parametrization using the automatic backward selection method. RESULTS: Slightly more than half of the respondents (56.5%) with colorectal cancer do not accept their disease in the period immediately preceding the surgical intervention. Among the determinants influencing the acceptance of the disease were the following variables: family history of colorectal cancer and satisfaction with medical care. CONCLUSIONS: More frequent acceptance of colorectal cancer occurs in patients who declare satisfaction with medical care and among patients with a family history of colorectal cancer.

The aim of the study: We evaluated the connection between the presence of the -2518 A/G MCP-1 as well as 190 G/A CCR2 polymorphic variants and colorectal cancer (CRC) occurrence. Material and methods: Study group consisted of subjects with different stages of CRC as well as healthy controls. Genotypes were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. Results: W observed an association between the colorectal cancer and the GG genotype of the -2518 A/G MCP-1 single nucleotide polymorphism. No statistically significant correlation was found between CRC and the 190 G/A CCR2 polymorphism. Conclusion: The results of this study support the hypothesis that polymorphism in the MCP-1 gene may contribute to the etiology of colorectal cancer.

Introduction: Colorectal cancer is one of the most common cancers in both sexes and is one of the leading causes of death in Poland and the world. An effective method to prevent the development of colorectal cancer (CRC) is the detection of polyps during a screening colonoscopy and their removal. Objective: The aim of the study was to evaluate results of screening for early detection of colorectal cancer by colonoscopy. Material and methods: The study was based on analysis of medical records of 7965 patients who underwent colonoscopy as part of the National Screening Program (NSP) for Early Detection of Colorectal Cancer in the 2000–2014 period. Results: Polyps were removed in 2900 (36.4%) patients, among whom 1885 (23.6%) had adenomas, which were more frequent in men (32.9%). Tubular adenomas were observed in 1685 patients (21.1%), tubulo-villous adenomas were detected in 157 patients (2%) and villous adenomas – in 43 (0.5%) of them. Sixty-three (0.79%) colorectal cancers were detected in various clinical stages, including adenocarcinoma in situ in 3 patients. Probability of 5-year survival rate of colorectal cancer amounted to 74% while 10-years survival was reached in 63% of patients. Conclusions: The detection rate of polyps, adenomas and carcinomas thanks to screening colonoscopy was 36.4%, 23.6% and 0.79% respectively, while the probability of 5-year survival was 74%. An improvement in the quality of colonoscopy was observed in subsequent years of the NSP, which translated into better detection of adenomas.

We determined the distribution of genotypes and frequencies of alleles of the (CA)n repeat polymorphism in intron 3 of the urokinase plasminogen activator receptor (uPAR) gene, uPAR antigen levels and microvessel density (MVD) in tumour and distant mucosa samples from 52 patients with colorectal cancer. The uPAR level was higher for patients with high MVD comparing to patients with lower MVD which may suggest that uPAR can be correlated with progression of colorectal cancer. The significant relationship between the high MVD and uPAR antigen level appeared to be independent of the (CA)n repeat polymorphism because no differences in the level of uPAR antigen between carriers of alleles were found. The received results, indicate that uPAR might be considered as a target in colorectal cancer patients' therapy.

Chalcones, as a large group of organic compounds, are widely implemented in various types of anti-cancer therapeutics. These plant metabolites are present in fruits, vegetables, spices, and have anti-tumor, anti-inflammation, immunomodulation, antibacterial and anti-oxidative activities, as well as many other pharmacological and biological effects. The aim of the present study was to investigate cytotoxic effects, type of cell death and mechanism of action of the newly synthesized vanillin based chalcone analogues, (CH1) and (CH2) on human colon cancer HCT-116 and noncancerous (control) MRC-5 cell lines. In order to compare effects of vanillin based chalcone analogues on investigated cell lines, as reference substances cisplatin (cisPt) and dehydrozingerone (DHZ) were used. Investigation of antitumor effect of chalcone analogues on HCT-116 cells was carried out by three methods MTT assay, flow cytometry and immunofluorescence analysis. The result of our investigation indicated that newly synthesized vanillin based chalcone analogues expressed powerful antitumor effect on cancer cells (HCT-116 cell line), while their effect on healthy cells (MRC-5 cell line) was not statistically significant. Vanillin based chalcone analogues caused overexpression and activation of mitochondrial Bax protein and caspase-3 in HCT-116 cells, indicating that their mechanism of antitumor action was mediated through activation of inner apoptotic pathway. These results indicate possible usefulness of CH1 and CH2 in antitumor therapy whether through its direct cytotoxic effect or as adjuvant therapy. Our results indicate possible usefulness of CH1 and CH2 vanillin based chalcone analogues in antitumor therapy.

Introduction. Minimally invasive techniques in colorectal surgery have become increasingly popular and are considered a standard of care in most surgical cenres. Locating the tumour during laparoscopic procedure can be technically challenging. Incorrect localization of the primary lesion may lead to a non-radical resection margin. The technique of endoscopic tattooing (ET) prior to surgery or endoscopic treatment is considered a useful tool. Various dyes can be used for this purpose, such as: Indian ink, methylene blue, indigocarmine, toluidine blue, isosulfan blue, haematoxylin and eosin, indoxin green. This procedure is recommended by international scientific societies (ASGE and ESGE). Objective. The purpose of the study is to review the current literature on the use of ET in large intestine tumour lesions. Materials and method. A MEDLINE literature search of English language articles addressing the use of ET to enable intraoperative tumour localization in colorectal surgery was performed to evaluate and summarize the feasibility of this technique. Results. The use of ET enables the easy and safe localization of colorectal tumurs during minimally invasive colorectal procedures. The percentage of complications is insignificant. Conclusions. The available literature proves the safety and benefits of using the ET prior to surgical or endoscopic treatment. ASGE and ESGE recommend the use of ET in marking tumours before surgical treatment, and the area after endoscopic resection for further evaluation.

ntroduction: A proper level of nutrition is significant in the period of convalescence in patients subject to major surgical procedures, particularly due to neoplastic disease. Bioelectrical Impedance Analysis, or BIA is a widely used method in assessing body mass composition. BIA measurement is easy, quick, cheap and repetitive. Material and methods: We assessed the body composition of 56 patients (25 women and 31 men) hospitalized and operated at the Department of General and Oncological Surgery of the Wroclaw Medical University in the years 2017–2018 using bioelectric impedance. Results: The average body weight loss in the 4th postoperative day was 1.32% of body mass and on the day of release from hospital – 4.23% of body mass in relation to body mass upon admission. The percentage of body fat (FM – Fat Mass) in patients admitted to the department is above the normal range. The change in body composition in hospitalized patients mainly concerns the amount of adipose tissue and the amount of extracellular and intracellular water (ECW – Extracellular Water; ICW – Intracellular Water). Conclusions: Bioelectrical impedance can be an easy and effective method of assessing body composition and its change in patients undergoing major surgery. Amongst the analyzed groups, patients operated for pancreatic cancer lose the largest percentage of body weight until discharge from the department. Loss of body mass mainly occurs as loss of fat mass (FM).

In Poland there there are about 15‑16 thousand cases of colon cancer per year. The health care system allows the treatment of patients with colorectal cancer in highly specialized hospitals, oncology centers and district hospitals. The results of treatment within different reference level differ. The aim of the study was to evaluate the results of surgical treatment of patients with colorectal cancer at a district hospitals compared with the results of highly specialized center. Material and methods. A retrospective study. The material consisted of 171 consecutively operated patients diagnosed with colorectal cancer treated in the Department of Surgery, District Hospital in Wołomin. The control group consisted of 200 patients treated surgically at the Department of General and Colorectal Surgery, University Hospital in Łódź. In both centers, the patients were operated on by surgeons with experience in operations on the large bowel. The demographic data, information on the type of indication (elective vs emergent), and the severity of the disease by AJCC / TNM scale were collected. In the district hospital there were patients with more advanced disease (p <0.001), older (p = 0.0001), and often operated under emergent indication (p = 0.0001). The telephone survey collected data on survival or the date of death of the patient and set the percentage of five-year survival. Results. The proportion of five-year survival in the study group and control group was respectively 46% and 71% (p <0.0001). The percentage of five-year survival among patients undergoing elective procedure in both centers were respectively for Wołomin and Łódź 58% and 73% (p = 0.008). The proportion of 5-year survival among “younger” patients (<70) was respectively in Wołomin and Łódź 64% and 81% (p = 0.004) for “older” patients with (> 70) 50% and 60% (p = 0.6747) Conclusions. Overall results of surgical treatment of patients with colorectal cancer in the district hospital are inferior to treatment results in a highly specialized center. The population treated in the district hospital is statistically significantly different in comparison to patients treated in highly specialized center. The following differences were captured: severity of the disease, age and type of indication (elective vs emergent). The diffrences has an influence on the outcomes. The five years survival for patients > 70 years undergoing elective procedure is not statistically different between the district hospital and highly specialized center.

The incidence of colorectal cancer (CRC) is increasing from year to year. Despite intensive research CRC etiology remains unknown. Studies suggest that at the basis of the process of carcinogenesis can lie reduced efficiency of DNA repair mechanisms, often caused by polymorphisms in DNA repair genes. The aim of the study was to determine the relationship between gene polymorphism Lys589Glu of EXO1 gene and modulation of the risk of colorectal cancer in the Polish population. Determination of the molecular basis of carcinogenesis process and predicting increased risk will allow qualifying patients to increased risk group and including them in preventive program. Material and methods. The material used in study was blood collected from 130 patients diagnosed with colorectal cancer. The control group consisted of 135 healthy people. Genotyping was performed by TaqMan method. Results. The results obtained indicate that the genotype Lys/Glu is associated with an increased risk of colorectal cancer (OR 1.811, 95% Cl 1.031-3.181, p = 0.038). Conclusion. On the basis of these results, we conclude that Exo1 gene polymorphism Lys589Glu may be associated with an increased risk of colorectal cancer.

Since the discovery of the first E. coli mutator gene, mutT, most of the mutations inducing elevated spontaneous mutation rates could be clearly attributed to defects in DNA repair. MutT turned out to be a pyrophosphohydrolase hydrolyzing 8-oxodGTP, thus preventing its incorporation into DNA and suppresing the occurrence of spontaneous AT→CG transversions. Most of the bacterial mutator genes appeared to be evolutionarily conserved, and scientists were continuously searching for contribution of DNA repair deficiency in human diseases, especially carcinogenesis. Yet a human MutT homologue - hMTH1 protein - was found to be overexpressed rather than inactivated in many human diseases, including cancer. The interest in DNA repair contribution to human diseases exploded with the observation that germline mutations in mismatch repair (MMR) genes predispose to hereditary non-polyposis colorectal cancer (HNPCC). Despite our continuously growing knowledge about DNA repair we still do not fully understand how the mutator phenotype contributes to specific forms of human diseases.

Purpose: The aim of the study was to determine the significance of carcinoembryonic antigen and matrix metalloproteinase 2 peritoneal washes and serum concentration in patients suffering from colorectal cancer concerning tumor staging and 5-year survival rate in these patients. Methods: 80 patients who underwent curative surgery for colorectal cancer were included in the study. Preoperative serum and intraoperative peritoneal washes CEA and MMP-2 concentrations were measured. Results: Regarding tumor penetration, CEA-s and CEA-p concentrations were higher in subsequent stages from T2 to T4. Both CEA-s and CEA-p concentrations were lower in T2 compared to T3 and T4. Significant difference of CEA-s and CEA-p was noted between T2 and T4 stages. MMP-2-s concentration was higher in T3 compared to T2, the highest MMP-2-p concentration was in T4, with no statistical significance. Regarding nodular status, a significant difference of CEA-s was noted between N0 and N1. For CEA-p, significance was found between N0 and N2 as between N1 and N2. MMP-2-s concentration was the highest in N1, MMP-2-p concentration was the highest in T4, with no statistical significance. The 5-year survival rate for all patients was 63.53%. There were significant differences in CEA-s and CEA-p concentrations between patients with negative and positive 5-year survival. Conclusion: Intraoperative peritoneal washes concentration of CEA may potentially serve as an important factor for more precise colorectal cancer staging. CEA-p and CEA-s concentrations correlate with survival rate in patients suffering from colorectal cancer and can be useful as an additional prognostic factor. The usefulness of MMP-2 measurement still requires further studies.

Assessing survival in cancer patients is a valuable source of information for patients and physicians alike, as well as for politicians who have a direct impact on the shaping of health policy and health systems. The aim of the present study was to assess the changes in the 5-year relative survival of colorectal cancer patients during 1995–2014. Materials and method. The data of 8,970 patients with colorectal cancer in the years 1995–2014, 5,033 males and 3,937 females aged 67.5 ± 11.7 from Świętokrzyskie Cancer Registry were used. Cases were classified according to the topographical codes ICD-O-3: C18.0-C18.9, C19.9, C20.9, C21.0-C21.2, C21.8. The end of follow-up was fixed at 31 December 2014. Four five-year calendar periods were defined. In each calendar period, relative survival rates using the Ederer II method were estimated separately for males and females. Results. In 2010–2014 (against 1995–1999), the absolute increase in the 5-year relative survival in males and females with colon cancer was the highest and reached 9.8 percentage point (p.p.) and 9.6 p.p., respectively. Patterns of survival for both colon and rectal cancer patients according to gender and age were very similar. Conclusions. In 1995–2014, an increase in the value of relative survival rates of males and females with colorectal cancer was observed. Systematic increase in funding in health care was a chance for reducing the burden of colorectal cancer by more widespread and equal access of effective early detection and cancer treatment.

Colorectal cancer (CC) in Poland is the type of cancer with the highest dynamics of disease growth and is epidemiologically related to age. The analysis involved 353 patients operated on due to CC in senile and old age and compared with younger patients. It was found that people at this age are more often diagnosed with CC They were more often women, the patients did not differ in the stage of cancer, while they were significantly more often qualified for surgery due to urgent indications. In patients with colonic cancer, the resectability and radicality of the procedures in comparison with patients with rectal cancer was significantly higher, while there were more complications and deaths in the 30-day follow-up in this group. The overall survival in senile and old age was significantly worse. In the first year of follow-up after surgical treatment of patients in this group, complications and deaths were more frequently observed. However, in patients who survived 12 months after the operation, the overall survival rate did not significantly differ.

Purpose: To evaluate the clinical significance of MMP-2 and TIMP-2 concentrations in the sera of patients diagnosed with colorectal cancer. Methods: The study group comprised 48 patients with colorectal carcinoma and 24 healthy controls. The serum concentration of MMP-2 and TIMP-2 proteins was evaluated by the ELISA method. Results: The mean level of MMP-2 in the sera of patients with colorectal cancer was 39.4 ng/ml and was significantly lower compared with the control (p<0.001). A decrease in TIMP-2 protein in the sera of patients with colorectal cancer was also observed where its mean level was 132.3 ng/ml. The concentration of MMP-2 and TIMP-2 did not correlated with any clinicopathological parameters, except for the patients’ age. In addition, the concentration of MMP-2 in the sera of patients with CRC correlated negatively with the number of white blood cells in the blood and the prothrombin index. The concentration of TIMP-2 correlated positively with potassium and urea concentration in the blood. Conclusions: The results of our study indicate lack of clinical usefulness of determining the levels of MMP-2 and TIMP-2 in the sera of patients with colorectal cancer. However, these proteins play an important role in the carcinogenesis of colorectal cancer.
