Opis:
Background Laparoscopic cholecystectomy is one of the most commonly performed operation. Various methods for securing the cystic artery and cystic duct are described in literature. We aim to compare intra-operative and early post-operative outcomes of laparoscopic cholecystectomy using polymeric locking Hem-o-lok clips versus metallic ligaclips . Patients and Methods Retrospective study of prospectively maintained single institutional data including all consecutive patients who underwent elective laparoscopic cholecystectomy from 2013 to 2018. Patients in whom metallic ligaclips were used were grouped as Group I and those with Hem-o-Lok were grouped as Group II. The early post-operative outcomes of the two groups were compared. Results Total 1496 patients were included in the study; 836 patients in Group I and 660 in Group II. Study included 29.1% males and 70.9% females with mean age of 43.6 years. Hem-o-lok clip was better in securing wide cystic duct compared to metallic clips. Metallic clip failed to secure 8 out of 44 wide cystic duct compared to 0 out 70 with Hem-o-lok clips (p=0.002). The post-operative outcomes of both groups were comparable. There were no cystic duct leak, post- operative bleeding or major bile duct injuries in either group. Conclusion Use of Hem-o-lok clip is safe in laparoscopic cholecystectomy due to ease of application and security. Hem-o-lok is more useful in patients with thick and wide cystic duct which are difficult to secure with metallic clips with low risk of leak. The paper is concerned with strength analysis of multi-clip joints. Such structures enable visible growth of the carrying capacity and make them elements easily assembled by the clamping jaws and a single-clamp press. Each joint exerts an appreciable influence upon the safety level of the whole structure. A numerical analysis was performed to assess the effect of clearance between a sleeve and rope upon the joint quality. Phenomena emerging in the area between the clips were analysed as well. Some strength tests related to the process of drawing the wire rope out of the sleeve were also investigated. Changes in the stiffness of the clamping sleeve were analysed in view of the influence of the aforementioned factors upon the carrying capacity of joints and the sleeve material effort. Experimental investigations were conducted as well aiming at the determination of the smallest possible number of clips or the effective length of joints necessary for carrying the loads higher than the admissible load of the wire rope.
W pracy przeprowadzono analizę złącz wielozaciskowych. Umożliwiają one osiągnięcie większej nośności złącza poprzez zastosowanie szczęk zaciskających i prasy do zacisków jednokrotnych. Złącza mają istotny wpływ na poziom bezpieczeństwa całej konstrukcji, w której są zainstalowane. Przeprowadzono analizę numeryczną oceniającą wpływ wielkości luzu między tuleją a liną, analizę zjawisk zachodzących między poszczególnymi zaciskami, analizę procesu wyciągania liny oraz zmiany sztywności tulei zaciskanej. Wykonane następnie badania doświadczalne umożliwiły wyznaczenie najmniejszej liczby zacisków lub długości efektywnej złącza niezbędnej do przeniesienia obciążenia większego od nominalnego obciążenia lin.

In this paper, we investigate how data transmissions may be affected by various types of optical interference introduced into the fiber on purpose, via a clip-on coupler. The research proved that transmissions may be jammed completely using inexpensive equipment readily available on the market, provided that the attacker has physical access to the fiber optic cable. The most dangerous attacks rely on a typical, slowly modulated communication laser with a rectangular waveform. This study urges further research aimed at counteracting such attacks.

Background: A late clip slippage from the previously properly secured cerebral aneurysm is rarely observed. To date these complications have not pooled and evaluated using systematic review methodology. The objective was to report factors attributed to the late slippage of the aneurysm clip in the postoperative period. Material and methods: All causes of postoperative clip slippage were systematically reviewed and analysed according to PRISMA Individual Patient Data protocol. Medline (PubMed), Embase, Cochrane, ISI Web of Knowledge and Google Scholar were searched for all relevant cases. Results: Systematic review of the literature yielded 105 original cases proving slipped clip in the postoperative period. The slipped clip caused bleeding in 53.8% of patients. The putative cause of clip slippage was provided in only 34.7% of the published cases. If a single clip was used, then complete clip slippage was noted more often (p=0.04). Multiple clipping and clip-wrapping techniques were postulated as ways to prevent postoperative clip slippage. Conclusions: The reason for late slippage of the aneurysm clip remains unexplained by most authors. Based on systematic reviewing, the use of tandem of clips prevents their late migration off the aneurysm. Clipping with wrapping or use of a single clip reinforced by any wrapping material seems a more durable solution.

The article presents the results of measurements of the “b” dimension of the SB4 spring clips carried out using three measurement methods that differ in the method and width of the base plate contact. The results obtained are presented and the uncertainty of measurement for each method is estimated. Attention was paid to the issues of the obtained dispersion of the value of the “b” dimension. The tolerated dimensions are discussed. The proposed measurement methods can help to identify shape mismatches of spring clips.

В данной статье рассматривается клиповый иконоидный текст как один из новых видов текстов, создаваемых и функционирующих в Интернете. Основными составляющими иконоидных текстов являются мемы. Подчеркивается, что интерес к исследованию мемов велик, но при этом мемы и мемосфера слабо изучены. Указывается, что ученые приписывают интернет-мемам черты мемов, которые выделил Ричард Докинз в своей книге Эгоистичный ген (1976), но многие интернет-мемы ими не обладают. При уточнении понятия интернет-мем учитывается ряд особенностей мемов. Например, что мемы используются не только в виртуальной, но и реальной действительности; что мемы могут создаваться не основе шаблона и без него; что мемами могут являться картинки, слова, знаки и т.д.; что мемы могут содержать другие мемы, а также могут быть анимированными. Представляется общая типология интернет-мемов. В соответствии с ней мемы разделяются на визуальные, аудиальные и аудиовизуальные. Приводится также классификация мемов с визуальной и вербальной частью, которые получают названия иконоид и текстонсоответственно. Введение этих понятий необходимо для точного описания главных компонентов мемов. Ранее Катажина Марак обращала внимание на то, что отсутствует лексика для точного описания мемов. Обращается внимание на основные функции клипового иконоидного текста, т.е. релаксационную, интерактивную и когнитивную. Выделяются общие и отличительные признаки клипового иконоидного текста и музыкального видеоклипа. В работе раскрывается механизм создания иконоидных текстов, выявляются их особенности и описываются главные признаки.
The article discusses iconoid text as one of the new types of texts created and functioning on the Internet. Since the main components of iconoid texts are memes, the concept of an Internet meme is clarified. It is emphasized that the interest in the study of memes is immense, but at the same time, memes and the memosphere are understudied. It is pointed out that scientists attribute to Internet memes the features of memes that Richard Dawkins highlighted in his book The Selfish Gene (1976), but many Internet memes do not bear those traits. When clarifying the concept of an Internet meme, several features of memes are taken into account. For example, that memes are used not only in virtual reality, but also in real life; that memes can be created with and without a template; that memes can be pictures, words, signs, etc.; that memes can contain other memes, and can also be animated. The presented general typology of Internet memes divides them into visual, audio and audio-visual categories. A classification of memes with a visual and a verbal part, which are called the iconoid and the texton, is presented. The introduction of these concepts is vital for an accurate description of the main components of memes. Earlier, Katarzyna Marak emphasized the fact that there is no vocabulary for the exact description of memes. The main functions of the clip iconic text, i.e. relaxing, interactive and cognitive are accentuated. The general and distinguishing features of the clip iconoid text and music video clip are highlighted. The work reveals the mechanism for creating iconoid texts and identifies their specifics and describes their main characteristics.

The aim of the study was to present the first long-term results on the clinical use of compression anastomosis clips (CAC) in upper and lower gastrointestinal tract anastomoses. Material and methods. The study included 50 patients who underwent anastomosis of the upper (n = 32) or lower GI tract (n = 18) with the use of CAC. In the period of 6‑7 months after the surgery, patients underwent endoscopic examination and computed tomography evaluation of the anastomosis. Each anastomosis was evaluated macro and microscopically. The width of anastomoses was evaluated using a 4-point-scale for grading stenosis. Results. Of the 50 patients who underwent anastomosis with compression anastomosis clip, 28 (56%) patients reported to the follow-up examination within 190‑209 days of the execution of the anastomosis. Among the 22 patients who did not report to the study, 18 (36%) patients died within 91‑154 days from the execution of the anastomosis (mean 122 days), 4 (8%) patients were impossible to contact after discharge from hospital. Two mild stenoses (I0) were diagnosed; 1 of them was found in the gastroenterostomy and 1 in Braun enteroenterostomy. Microscopic changes were diagnosed in 4 anastomoses (3 gastroenterostomies, 1 Braun enteroenterostomy). Anastomoses were well-formed and wide, scars in the line of anastomoses were thin. Conclusions. During the period of 6 months after the anastomoses performed using CAC have been formed, they were evaluated as unobstructed and functioning properly; therefore, they can be safely performed within the upper and lower gastrointestinal tract.

Hráči počítačových her často vyžadují transformaci již existujícího anglického výrazu, jenž se ve hře objevuje, do jejich rodného jazyka. Cílem příspěvku je určit způsob, jakým jsou původní anglická substantiva převáděna do českého gramatického systému. Teoretická část referuje o pozici angličtiny ve světě, pojednává o filozofii gramaticko-lexikální adaptace obecně a rovněž nastiňuje základní principy dané hry. Dále pak popisuje již existující slang českých hráčů počítačových her. V praktické části jsou podstatná jména analyzována z hlediska slovotvorby, sémantické motivace a širšího situačního kontextu. U každého analyzovaného substantiva je uveden typický příklad jeho užití ve hře. Výsledky výzkumu jsou ověřeny v rámci rozboru praktické části příspěvku.
Computer game players often express the need to convert the existing English expression occurring in the game to their native language system. The aim of this article is to explore the way the original English nouns are converted to Czech grammatical system. In the theoretical part, the position of English language in the world, the philosophy of the grammatical-lexical adaptation in general and the game itself are suggested. Furthermore, the principle of the existing computer game slang of the Czech players is introduced. Subsequently, in the research part, the nouns are analysed in regard to their word-forming process, semantic motivation and wider situational context. The analysed nouns are provided with a typical example of their common usage in the game. The research outcomes are verified within the analysis of the research part.

The paper presents the characteristics of the packaging market in Poland. Was presented the essence and aspects involved in packaging, and the main functions of packaging. Defined the concept of packaging and are given legal regulations in accordance with the applicable rules on the need for policies to reduce waste resulting from the production of packaging. The article also indicated trends in the packaging innovation. The article presents the operating characteristics of Pol-Mare Sp. Ltd in the area of international security, transport of explosives and military equipment. Pol-Mare Ltd. It is a national leader in providing specialized freight forwarding services in land transport sea and air. The article shows the specificity of international carriage of cargoes in Poland.

System przytwierdzenia sprężystego szyn do podkładów jest bardzo odpowiedzialnym elementem w budowie drogi szynowej. W artykule omówiono charakterystykę pracy systemu przytwierdzenia typu SB oraz przedstawiono wyniki analizy wpływu zastosowania różnych rodzajów przekładek podszynowych na pracę całego mocowania oraz jego trwałość eksploatacyjną. Stwierdzono, że dobór przekładki podszynowej w zależności od zastosowanej łapki sprężystej ma kluczowy wpływ na trwałość systemu i zapewnienie odpowiedniego mocowania szyny.
Rail fastening system is a very responsible part in constructing a railway track. The article describes the characteristics of SB fastening system performance and presents the analysis of the influence derived from under rail pads for the performance of the whole fastening system and its reliability. It has been stated that an accurate selection of under rail pad depending on the used rail clip has a key importance for high durability of the system and ensuring a correct fixing of the rail.

Nowadays, civilization diseases, such as hypertension, are one of the biggest global health problems. In 2017 the threshold for hypertension diagnosis was set at 130/80 mmHg, which resulted in its increased prevalence, reaching nearly 50% of the human population. Therefore, strategies for hypertension prevention and treatment have been recently extensively developing. Nonetheless, growing body of factors which can affect blood pressure and induce hypertension is constantly prompting researchers to conduct experiments in this field. For this purpose, animal models seem to be appropriate and necessary. The present report reviews current findings related to hypertension types and causes. It also presents the main guidelines for high blood pressure prevention and describes different experimental models introduced to be carried out in such studies.

W artykule opisano obowiązujące wymagania dla łapek sprężystych SB4 stosowanych w drogach kolejowych, przed dopuszczeniem łapek do eksploatacji. Scharakteryzowano dwa podstawowe aspekty dotyczące łapek: aspekt materiałowy (skład chemiczny, rodzaj obróbki cieplnej, mikrostruktura, odwęglenie) oraz aspekt kształtu geometrycznego.
