Alcohol consumption and related abuse are causes of mortality and morbidity worldwide (WHO). Consumption of alcohol in the upcountry region usually increased in amount rather than in any other part of Sri Lanka. It is essential to know the mode of alcohol consumption among various types of consumers. That helps to conduct a well-planned program for the prevention and control of this catastrophic social pathology. The main scope of this study is to identify the mode of alcohol consumption and related health issues, at the same time to expose the associating factors that lead to alcohol consumption. Selected 50 adult males have been interviewed from the study area using a semi-structured questionnaire. The study participants were selected through the purposive sampling method. Gathered data have been analyzed using Excel and discussed. According to the research outcomes, we have found that most of the participants agreed that they are consuming locally made alcohol and beer. 70% of the consumers have different health issues, 58% have family problems and financial struggles due to alcohol consumption. About 40% alcohol dependent had clinical signs of chronic illness. 70% of the consumers drank alcohol outside and during their work time. Alcohol consumers stated their associating factors of consumption as the climate, society, family problems, poverty, and physical soreness. According to the questionnaire survey, 74% of the participants were identified for daily consumption of alcohol. The most considered matter is their starting age of alcohol consumption is so young, and this study emphasizes the need for public education regarding the effects and illness of alcohol consumption at a young age. Proper awareness and guidance could help the youngsters to overcome the addiction. Public places and workplaces are the main areas to conduct awareness and intervention programs.

The problem at work that is being focused on the adolescent’s developmental tasks in families with alcohol problems, especially in families with treated alcoholics. In present work was hypothesised that treatment of alcoholic fathers would be the one of the important protective factors in families with alcoholism. The first question addressed was whether children of treated alcoholics have been differed from children of drinking alcoholics according to the developmental tasks. The next question addressed was whether the specific family context was associated with the adolescent’s development. Participants included 91 children age 17-18. The research sample comprised three groups: 31 children of drinking alcoholics, 30 children of treated alcoholics, and 30 children of non-alcoholic parents. In general, the results of the present study suggest that the process treatment of the alcoholic parents was found to be a significant protective factor in alcoholic families.

Background. Osmolalities can be useful markers for determining whether given beverages are suited for maintaining an adequate hydration of the body. Losing 2% of body water relative to body mass reduces the efficiency of body function when undertaking physical effort by around 20%. Deficiencies in water intakes approaching 5-8% of body mass, double the impairment to the body’s physical and mental functioning, whereas at a level of 10% the body becomes incapable of performing any sort of physical effort. For such reasons the body’s hydration status is vital to its functioning. Objectives. To asses osmolalities as measured in various types of commercially available mineral waters and non-alcoholic beverages containing different amounts of extracts. Materials and Methods. Test materials were commercially available mineral waters (of low, medium and high mineral content) along with juices, nectars and drinks that are isotonic, energising and those described as being ‘light’ and sparkling. Osmolality was measured by the 800CL Osmometer instrument from TridentMed whilst the RL-type refractometer was used for determining extract values. Results. Isotonic drinks were found to have the same osmotic pressures as bodily fluids at 275 – 295 mOsm/kg water. The osmotic pressure in mineral waters depended on the extent of mineralisation and ranged from 13 mOsm / kg water (low mineral content) to 119 mOsm/kg water (high mineral content). Low osmolalities were also found in ‘light’ drinks (from 29.3 to 34 mOsm/kg water). Juices, nectars, energising drinks and colas typically have high sugar contents and have high osmolalities ranging 492 – 784 mOsm / kg water. Statistical analysis demonstrated significant associations (p < 0.05) between osmolalities and extract content in beverages as well as between osmolalities and mineral content in mineral waters. Upon factor analysis, it was possible to group the tested drinks according to similar osmolalities and extract content. Conclusions. Osmolalities measured in beverages are a marker that permits drinks to be classified into groups according to their tonicity and their ability to ensure that the body is properly hydrated; this becoming vital in cases when the body requires rapid body fluid replenishment.
Wprowadzenie. Wartość osmolalności napojów może być wykorzystana jako wskaźnik ich przydatności do właściwego nawadniania organizmu. Utrata wody w wysokości 2% w stosunku do masy ciała obniża wydolność fizyczną o około 20%. Niedobór wody sięgający 5-8% masy ciała powoduje dalsze zaburzenia wydolności fizycznej i psychicznej, a przy stracie wody do 10% masy ciała człowiek jest niezdolny do wykonywania jakiegokolwiek wysiłku fizycznego. Dlatego też kontrola stanu nawodnienia organizmu jest bardzo istotna. Cel. Celem badań była ocena zmierzonej osmolalności różnych wód mineralnych (nisko, średnio i wysoko zmineralizowanych) oraz soków, nektarów, napojów izotonicznych, energetyzujących, „light” i gazowanych zawierających różne zawartości ekstraktu ogółem (oBx). Materiał i metody. Materiał do badań stanowiły komercyjnie dostępne wody mineralne (nisko, średnio i wysoko zmineralizowane) oraz soki, nektary, napoje izotoniczne, energetyzujące i „light” oraz gazowane o różnej zawartości ekstraktu. Osmolalność oznaczono przy wykorzystaniu osmometru 800 CL firmy Trident Med., natomiast zawartość cukrów mierzono refraktometrem typu RL. Wyniki poddano analizie statystycznej wykorzystując program Statistica v. 12. Wyniki. Stwierdzono, że napoje izotoniczne charakteryzowały się typowym dla płynów ustrojowych ciśnieniem osmolalnym (275-295 mOsm/kg H2O). Ciśnienie osmolalne wód mineralnych zależało od stopnia ich mineralizacji i wahało się od 13 mOsm/kg H2O (nisko zmineralizowane) do 119 mOsm/kg H2O (wysoko zmineralizowane). Niską osmolalność wykazywały także napoje typu „light” (od 29,3 do 34 mOsm/kg H2O). Soki, nektary, napoje energetyzujące oraz napoje typu „cola” charakteryzujące się wysokim udziałem cukrów wykazywały wysoką osmolalność wynoszącą od 492 do 784 mOsm/kg H2O. Analiza statystyczna wyników wykazała, że istnieje istotna zależność (p < 0,05) pomiędzy osmolalnością a zawartością ekstraktu ogółem w napojach, a w wodach mineralnych pomiędzy osmolalnością a zawartością składników mineralnych. Analiza czynnikowa pozwoliła na pogrupowanie badanych napojów produkowanych przemysłowo wg podobnej osmolalności i zawartości ekstraktu ogółem na grupy o podobnej osmolalności i zawartości ekstraktu ogółem. Wnioski.. Osmolalności zmierzone w napojach są markerami, które pozwalają klasyfikować napoje według grup w zależności od ich toniczności i ich zdolności do zapewnienia, że organizm jest odpowiednio nawodniony. Staje się to niezbędne w przypadkach, gdy organizm wymaga szybkiego uzupełnienia płynu ustrojowego.

Several studies suggest that cytokines and neutrophils play an important role in the pathogenesis of acute pancreatitis (AP). the aim of the study was to assess the systemic release of proinflammatory cytokines and WBC (white blood cells) count with differential in patients with acute alcoholic pancreatitis (AAP) and to characterize the differences between patients with mild and severe forms of the disease. Material and methods. Thirty-five patients with the mild form of acute alcoholic pancreatitis (MAAP) were compared to 11 patients with severe acute alcoholic pancreatitis (SAAP). Serum levels of IL-6, IL-8, IL-12p40 and WBC differential count were measured every second day during the first week after admission. Results. During the course of the study, the average level of IL-6 was significantly (p<0.05) higher in patients with SAAP than in patients with the mild form of the disease (MAAP). Serum levels of IL-8 and IL-12p40 on admission were higher in patients with SAAP than in patients with MAAP but the difference was not statistically significant. Of all the types of WBCs, neutrophils were significantly (p<0.05) elevated the entire time in SAAP patients when compared to patients with MAAP on 5th and 7th day from admission to hospital. conclusions. Patients with SAAP had significantly higher proinflammatory cytokine IL-6 levels and neutrophil counts than patients with MAAP. The results suggest that proliferation and overstimulation of this subset of leukocytes might contribute to the development of the systemic inflammatory response in patients with SAAP.

Overuse of spirits is one of the biggest problems of the modern world. It is a very complex and difficult to eradicate phenomenon. Destructive consequences are carried for both the addict and the whole family. It is especially harmful for social, intellectual and emotional development of the child, which resides in a pathological environment. Parenting skills of parents with drinking problem are severely weakened and inconsistent. Children of alcoholics live in a tense atmosphere and everyday life is unpredictable for them. Troubling alcohol consumption interferes functioning, communication and trust of the family. The purpose of this article is to present the problems of alcoholism in a family with alcohol issues.

The article concerns the child from an alcoholic family in a pre-school environment. The author presents a theoretical introduction on definitions important for understanding the subject, then addresses the subject of social pathology as a factor disturbing the durability of the family system. The topic of social roles in the alcoholic family and their impact on the child’s socialization process continue to arouse interest. Another issue raised in the article is the topic of co-dependence, starting from the general definition and understanding of the concept and ending with the impact of this phenomenon on the child’s functioning. The issue analyzed and reflected is as follows: a child from an alcoholic family as a student – in particular in relationships with teachers and peers. The author reflected moreover on the possible difficulties a child in a pathological family may encounter in the school environment, as well as on suggested practical solutions and a support network that may help the individual and its immediate environment to fight against difficulties and to motivate change. The article has an introduction and ending, which aptly sums up the issues dealt with.

Aim of the study was to evaluate the effect of supplementation of high gravity media with mineral compounds and myo-inositol, at concentration which would be obtained as a result of degradation of phytates present in raw material during alcoholic fermentation. The process of alcoholic fermentation was conducted under laboratory conditions in a 72 h system at 37°C with the use of S. cerevisiae D-2 strain. Calcium chloride proved to be the most effective of all supplements tested. Final ethanol concentration increased by 1.2% v v−1  and the yield of process increased by ca. 7 dm−3  ethanol 100 kg−1  of starch in comparison with control. Selective supplementation with KH2 PO4,  ZnSO4  and MgSO4  also increased the ethanol concentration, but the effect was accompanied by a deterioration in composition of volatile products. The hydrolysis of phytate complexes with microbial phytases can be an alternative solution to supplementation of HG mashes presented in this work.

Background: Non-alcoholic fatty liver disease (NAFLD) is one of the most common liver diseases, influenced by various risk factors associated with metabolic disorders. Currently there is no specific effective treatment for NAFLD. Probiotics have been extensively researched for their health benefit Probiotic Supplementation for NAFLD. Methods: A simple literature review was performed based on searches via PubMed, ScienceDirect and Google Scholar, using the keywords "probiotic," "microbiota," "non-alcoholic fatty liver disease," "metabolic disorder," and "therapy." Results: Research on the use of probiotics for NAFLD demonstrated improvement in liver function and histology. However, the literature is inconsistent regarding the probiotics’ influence on the NAFLD risk factors. Probiotics can be an alternative therapy for NAFLD through the ability to modulate the microbiota of the gastrointestinal tract. Conclusion: Probiotics can be an alternative therapy in NAFLD patients, however more extensive studies are needed to ensure the safety of this approach.

Through years beer became one of the best known alcoholic beverages in the world. For some reason e.g. healthy lifestyle, medical reasons, driver’s duties, etc. there is a need for soft drink with similar organoleptic properties as standard beer. There are two major approaches to obtain such product. First is to interfere with biological aspects of beer production technology like changes in mashing regime or to perform fermentation in conditions that promote lower alcohol production or using special (often genetic modified) microorganism. Second approach is to remove alcohol from standard beer. It is mainly possible due to evaporation techniques and membrane ones. All these approaches are presented in the paper.

The aim of the study was to determine serum concentrations of afamin and adropin in patients with alcoholic liver cirrhosis and to define their correlation with the stage of disease. The study included 99 patients with alcoholic cirrhosis from the region of Lublin, (Eastern Poland). Liver cirrhosis was diagnosed based on clinical features, history of heavy alcohol consumption, laboratory tests and abdominal ultrasonography. The control group consisted of 20 healthy individuals without liver disease who did not abuse alcohol. The serum afamin and adropin concentrations were determined using ELISA kits. The concentration of afamin was found to be significantly lower in patients with compensated alcoholic liver cirrhosis, i.e. P-Ch B (85.1±40.6 μg/ml) and P-Ch C (56.4±32.3 μg/ml) individuals, compared to the control group (135.9±43.6 μg/ml); p-value was <0.01 and <0.001, respectively. As far as adropin is concerned, a reverse relationship was demonstrated: the highest concentration was found in patients with P-Ch C (11.7±5.7 ng/ml) cirrhosis. Furthermore, the above concentration was significantly higher compared to patients with P-Ch A cirrhosis (7.2±2.8 ng/ml; p<0.05) and controls (7.5±2.6 ng/ml; p<0.05). The concentration of afamin decreases with the severity of alcoholic liver cirrhosis, which most likely results from impaired hepatic synthesis. Otherwise, the higher the stage of disease according to the Child-Pugh score, the higher the concentration of adropin.

Gynaecomastia is uni- or bilateral breast enlargement in males associated with benign hyperplasia of the glandular, fibrous and adipose tissue resulting from oestrogen-androgen imbalance. Asymptomatic gynaecomastia is a common finding in healthy male adults and does not have to be treated, while symptomatic gynaecomastia might be the symptoma of many pathological conditions and requires meticulous diagnosis and therapeutic management. The commonest causes of gynaecomastia in the Polish population include liver cirrhosis and drugs used to treat its complications. The current study presents the case of severe painless gynaecomastia in a patient with decompensated alcoholic liver cirrhosis, treated with spironolactone because of ascites. Breast enlargement assessed a IIb according to the Simon’s Scale or III according to the Cordova-Moschella classification, developed slowly over the two-year period of low-dose spironolactone therapy The course and dynamics of disease are described and the main mechanisms leading to its development discussed. The importance of effective treatment of patients with severe gynecomastia is emphasized as the disease may result in significant psychosocial problems.

Introduction; p to 300 million people have the body mass index (BMI) greater than 30 kg/m2. Obesity is the cause of many serious diseases, such as type 2 diabetes, hypertension, and non-alcoholic fatty liver disease (NAFLD). Bariatric surgery is the only effective method of achieving weight loss in patients with morbid obesity. Objectives: The aim of the study was to assess the impact of bariatric surgery on non-alcoholic fatty liver disease in patients operated on due to morbid obesity. Material and Methods: We included 20 patients who were qualified for bariatric procedures based on BMI > 40 kg/ m2 or BMI > 35kg/m2 with the presence of comorbidities. The average body weight in the group was 143.85kg, with an average BMI of 49.16kg/m2. Before the procedure, we evaluated the severity of non-alcoholic fatty liver disease in each patient using the Sheriff-Saadeh ultrasound scale. We also evaluated the levels of liver enzymes. Follow-up evaluation was performed twelve months after surgery. Results: Twelve months after surgery, the average weight was 102.34 kg. The mean %WL was 33.01%, %EWL was 58.8%, and %EBMIL was 61.37%. All patients showed remission of fatty liver disease. Liver damage, evaluated with ultrasound imaging, decreased from an average of 1.85 on the Sheriff-Saadeh scale, before surgery, to 0.15 twelve months after surgery (p < 0.001). As regards liver enzymes, the level of alanine aminotransferase decreased from 64.5 (U/l) to 27.95 (U/l) (p < 0.001), and the level of aspartate aminotransferase decreased from 54.4 (U/l) to 27.2 (U/l). Conclusions: Bariatric procedures not only lead to a significant and lasting weight loss, but they also contribute to the reduction of fatty liver disease and improve liver function.

Opis:
Non-alcoholic fatty disease (NAFLD) is a liver disorder that affects up to 30% of the population, mainly in Western countries. It is estimated that up to 75% of NAFLD patients will develop a more aggressive form of the disease, non-alcoholic steatohepatitis (NASH). NAFLD can lead to fibrosis and liver failure; however, it is difficult to diagnose NAFLD due to its non-specific symptoms. Unfortunately, there is no treatment available for this disease. The risk factors of NAFLD are obesity and insulin resistance (IR). The molecular factors that seem to play an important role in the pathogenesis of NAFLD are oxidative stress as well as impaired DNA damage repair processes; a great body of evidence confirms an association with the base excision repair (BER) pathway. The activity of BER is decreased in patients with NAFLD and in animal models of this disease. In order to better understand the underlying basis of the disease, knowledge should be broadened in the area of DNA repair in NAFLD.

Introduction. Non-alcoholic fatty liver disease (NAFLD) is excessive fat build-up in the liver due to causes other than alcohol use. Aim. To evaluate the clinical efficacy and safety of Liv.52 DS tablets in the management of NAFLD. Material and methods. Prospective, interventional clinical study conducted on 60 patients of both sex, aged between 18-65 years, confirmed with NAFLD from clinical examination, laboratory test, ultrasound findings and those willing to give informed consent. All patients received Liv.52 DS at a dose of 2 tablets twice daily for 2 months. All patients were evaluated at baseline, end of 1st month, and end of 2nd month for liver function tests, hepatomegaly by ultrasound, NAFLD Fibrosis Score, lipid profile, hematology and biochemical investigations. Results. Study data was analyzed with GraphPad Prism Software Version 6.07. Data of those patients who completed the study was considered for analysis. Significant improvement in hepatomegaly, liver enzymes was observed. NAFLD fibrosis score revealed no progression of liver fibrosis due to NAFLD during the study period. No abnormal lab values were recorded and there were no adverse events reported during the study. Conclusion. Study concludes that Liv.52 DS is safe and beneficial in individuals suffering from NAFLD.

Opis:
Oleic acid (OA) is widely used in pathology studies of hepatocellular lipid deposition. Identifying the effects of different solvents on OA-induced liver lipid deposition would be beneficial for studies on hepatocytes. We treated BRL 3A cells with OA dissolved in different solvents. After 12 h incubation, cell viability was assessed using MTT assays. Reactive oxygen species (ROS), triglyceride (TG) and total cholesterol (TC) counts, and the expression level of glucose regulated protein (GRP78), sterol regulatory element binding protein (SREBP-1C) and fatty acid synthase (FAS) were analyzed. Water, PBS and DMSO were disadvantageous to the dissolution of OA and did not cause an OA-induced response in hepatocytes. In the alcohol+OA-treated cells, the severe ER stress, oxidative stress and cellular fat deposition were significantly increased. BSA promoted cell growth and the cells treated with 1.2% BSA+OA showed a lower grade TG and endoplasmic reticulum stress compared with KOH+OA and alcohol+OA treatments. KOH had no significant influence on BRL 3A cells viability. When treated with OA dissolved in KOH, BRL 3A cells showed a typical hepatocyte damage. KOH was considered the suitable choice for an OA solvent for BRL 3A cells in hepatic lipidosis research.
