Press Up Exercises as an Alternative to Conventional Therapy of Radicular Symptoms in Patients with Low Back Pain

The study examines whether clinical and objective improvement can be achieved in patients with LBP (low back pain) with radicular symptoms using a 6-week exercise program based on press up exercises created from ADL (activities of daily living) movement patterns. This original study involved 10 men with acute L5 / S1 disc hernia. Before starting the exercise program, patients determined the intensity of pain VAS (Visual Analogue Scale), the intensity of paraesthesias (NSIP: numerical scale of paraesthesia intensity) and the location of paraesthesias. Magnetic resonance imaging (MRI) was used to objectify the extent of the disc hernia, spinal cord compression, and pressure acting on the dural sac. Patients completed -week exercise program consisting only of press up ADL movement patterns in a closed kinematic chain. At the end of the exercise program, patients determined the outcome values of clinical symptoms and underwent control MRI within 2 weeks at the latest. At the end of the study all patients showed a statistically significant reduction in pain intensity (p = 0.005), paraesthesia (p = 0.006). The pressure on the dural sac was reduced in all patients. One of the patients had a partial reduction of intervertebral disc hernia by 35.7 %. None of the patients had a change in spinal root compression. Conventional therapy should focus on influencing clinical symptoms that appear to correlate with dural sac compression. We dare to argue that reduction of hernia disc is not a sign of primary recovery in LBP patients with acute phase radicular symptoms.

Executive functioning significantly affects the functional rehabilitation progress in stroke patients. The current study are aimed to examine the impact of executive functions on functional recovery gain in a sample of right hemispheric (RH) frontal stroke patients. A total of 50 inpatients admitted to the Theseus Rehabilitation Center and diagnosed with RH frontal stroke were included in this study. The initial assessment of executive functioning was conducted using the Frontal Assessment Battery (FAB). Functional recovery of basic activities of daily living (ADLs) was computed by the difference between follow-up and baseline Barthel Index (BI) scores. Pearson’s analysis revealed that the FAB global scores significantly correlated with the BI gain scores. Additionally, multiple regression analysis showed a significant contribution of only motor programming to the basic ADLs recovery gain. The global status of executive function was found to be significantly associated with functional recovery gain, and motor programming was the strongest predictor in RH frontal stroke patients. These findings are discussed for the treatment rehabilitation prognosis.

Opis:
The Israeli educational system is dealing intensively for the last two decades in the assimilation of the inclusion approach of student with special needs (SwSN) in general education. The first part of the article presents the historical development of special education in Israel as a background to the presentation of the new amendment of Special Education Law – Amendment 11 (2018). The current stage of the Eleventh Amendment implementation aims to ensure the inclusion of SwSN in the general education by an allocation of budget for his needs according to a standard assessment of his functioning level, in addition to his disability. The SwSN parents’ will decide about their child placement according to the model of “The Parents’ Choice”, and the student’s budget will be transferred to the chosen educational framework according to the “Funding Follows the Child” principle. The second part of the article presents a case study of special education school in Israel for students with intellectual developmental disabilities that gradually assimilate the spirit of the law amendment from both aspects – standard assessment of the students functioning and parental partnership, as part of professional work processes carried out at school in Activities of Daily Living (ADL) field.

INTRODUCTION: Functional activity in post-stroke survivors is often impaired and has a significant negative impact on the patient, his/her family, community and society. MATERIAL AND METHODS: Pilot study – a two-group, before-and-after preliminary study was carried out to investi-gate the feasibility of conducting a larger randomized controlled trial investigation of the efficacy of Bobath-based poststroke neurorehabilitation. Patients were admitted to the neurological rehabilitation unit after ischemic stroke. Both groups (group I aged 40–55 years and group II aged 67–82 years received ten half-an-hour physiotherapy sessions over a two-week period. Measurements of the the Barthel Index, gait velocity, cadence and stride length were conducted twice: on admission, and after the last session of rehabilitation. RESULTS AND DISCUSSION: Statistically significant and favourable changes in the selected activities of daily living, gait velocity and stride length were observed in both age groups. CONCLUSIONS: This pilot study indicates a short-term benefit for using interventions based on the Bobath Concept. The findings presented suggest that Bobath-based rehabilitation may be considered an effective form of post-stroke rehabilitation in various age groups.
WSTĘP: Czynności funkcjonalne u osób po udarze są często obciążone deficytem, a przez to wpływają negatywnie na pacjenta, jego rodzinę, społeczność lokalną i całe społeczeństwo. MATERIAŁ I METODY: Badanie pilotażowe – ocena pacjentów przed i po terapii. Pacjenci po udarze niedokrwiennym mózgu zostali przyjęci na oddział rehabilitacji neurologicznej. Obie grupy (grupa I: 42–55 lat oraz grupa II: 67–82 lata) otrzymały 10 półgodzinnych sesji terapeutycznych w ciągu dwóch tygodni. Pomiar wskaźnika Barthel, prędkości chodu, tempa chodu oraz cyklu chodu został przeprowadzony dwukrotnie: przy przyjęciu oraz po zakończeniu rehabilitacji. WYNIKI: W obu grupach zostały zaobserwowane statystycznie znaczące pozytywne zmiany wybranych czynności codziennego życia, prędkości chodu oraz długości dwukroku. WNIOSKI: Badanie pilotażowe wskazuje krótkoterminowe korzyści wynikające z wykorzystania terapii opartej na koncepcji Bobath. Wyniki sugerują, że terapia taka może być wykorzystana jako efektywny sposób rehabilitacji poudarowej w różnych grupach wiekowych.

The paper aims at describing the last year of life of Polish people in comparison with previous years of their life. We include international context as well as gender differences in the analysis. A descriptive part of the paper is based on data from end-of-life interviews of the Survey of Health, Ageing and Retirement in Europe (SHARE). The second part – comparative analysis – in addition to the end-of-life interviews takes into account regular interviews conducted in previous waves. We find differences in the process of health deterioration in the last year of life by cause of death. In particular, individuals who died due to cancer were often relatively healthy in the preceding years of life, but experienced a dramatic decrease in health during the last year of life. Another finding is that time spent in the hospital significantly increases in the last year of life. Moreover, we find that number of daily activities’ limitations is positively associated with care received in the last year of life. The results show that the last year of life is often significantly different than preceding years of life both in Poland and other European countries.

Alzheimer disease is known for placing a great burden on caregivers which include the social, psychological, physical and/or economic aspects. The disease diminishes patients` independence in activities of daily living (ADL). Home care is usually preferred by patients and families. The role of the main caregiver is often taken by the spouse or a close relative. Dementia caregivers are subject to high rates of physical and mental disorders. ADL scales are useful for the evaluation of patients` capacity and their needs for care and burden. Those scales enable one also to arrive ar the outcome measure of rehabilitation. One can divide them into three parts: universal (generic) basic ADL scales, basic scales which are specific for Alzheimer disease and instrumental - extended (expanded) ADL scales. In this paper the most commonly used scales evaluating activities of daily living in Alzheimer disease, including as equally basic as well as instrumental ADL scales, have been presented. Due to the systematic review we were able to present their psychometric properties: content validity, internal consistency, construct validity, criterion validity, reliability, responsiveness, floor-and ceiling effects, and interpretability. Those scales that received positive ratings have been presented.
Starzenie się społeczeństw doprowadzi do wzrostu zapadalności na chorobę Alzheimera. W zaawansowanym stadium choroba uniemożliwia samodzielne wykonywanie nawet prostych czynności codziennych i osoba chorująca na chorobę Alzheimera wymaga stałej opieki. Ocena samodzielności osób z otępieniem jest bardzo ważna – pozwala ona na oszacowanie potrzeb dotyczących opieki nad chorymi, gdyż pociąga to za sobą koszty i obciążenie opiekunów. W artykule przeglądowym przedstawiono aktualne możliwości oceny samodzielności w wykonywaniu czynności Ŝycia codziennego u osób z otępieniem – służą do tego skale oceniające czynności podstawowe (ADL) i skale nazwane instrumentalnymi lub rozciągniętymi skalami ADL

The ability to function independently is crucial for successful aging, while an age through the development of involutional changes and risk of comorbidity promotes functional limitations. On average, 20% of people over 70 years of age need help in at least one of the 6 basic activities of daily living. The ability to cope in everyday life is affected by the reduction of physical fitness, sensory impairment and the effects of chronic conditions, falls and injuries. Deterioration of physical fitness is a consequence of muscle mass and strength reduction, impaired neuromuscular function resulted in slowing nerve conduction and reaction time, deterioration of balance, coordination and central transmission. Vision and hearing disorders are a consequence of aging and diseases. Cognition impairment, important for an independence, may be a consequence of degenerative changes in the CNS or dementia. Numerous studies have confirmed the relationship of involutional changes, accompanying diseases, falls and injures with limitations in daily functioning, and maintaining physical activity and implementation of a comprehensive geriatric assessment into geriatric care with an early diagnosis of deficits and implementation of intervention can prevent or delay the disclosure of disability.
