99mTc-HIG can be used for early detection of inflammation and infection foci especially in immunodeficiency patients. The present study involves in-house preparation and evaluation of human immunoglobulin-hydrazinonicotinic acid (HIG-HYNIC) kit to be labeled with 99mTc. In this indirect labeling method, the structure and biological activity of the protein can be maintained. The kit consists of two vials A and B, the vial A contains 2 mg of HIG-HYNIC, and the vial B contains 10 mg of tricine and 1 mg of stannous chloride dihydrate (SnCl2.2H2O). The results revealed that the lyophilized kits were stable for at least 10 months, and showed a high quality from the points of physical, chemical, radiochemical and biological purities, and its validity for human injection. The labeling yield of HIG-HYNIC compound exceeded 98%. The application of the prepared kit was experimentally illustrated in animal models with artificially induced inflammatory lesions after E. coli injection into rat leg muscle. The biodistribution and imaging studies in rats have shown good localization in infected muscle (T) in comparison to the normal muscle (M) 4 and 24 h after injection. Ratios of 17 and 18 were achieved 4 and 24 h post injection, respectively. The results of biodistribution of 99mTc-HIG in human and initial clinical evaluation show that this agent is a good radiopharmaceutical for infection imaging.

Human gamma globulin can be labeled by a direct or indirect method of radiotracer incorporated in a protein molecule. In this indirect method hydrazinonicotinic acid (HYNIC) is used which saves the structure and biological activity of the protein. Our goal was the efficient labeling of the human gamma globulin and evaluation of its biodistribution in different organs which can be used on experimentally induced infection causing inflammation. Immune globulin is mixed with s-hynic and IgG-hynic is developed using sidle A-lyzer and stored at –20°C which can be used at least for six months and then Sn-tricine kit is prepared which is used for 99mTc labeling. Efficiency of 99mTc-IgG-hynic labeling at pH 6.4 was very much dependent on ligand (hynic) and coligand (tricine) presence in the reaction mixture. Radiochemical purity was more than 90% in the kits prepared. Serum stability study showed no decomposition of 99mTc from the complex. The biodistribution studies showed the highest percentage ID/organ in the blood, liver and kidney, respectively. A human gamma globulin was successfully labeled through hynic to 99mTc by an indirect method with high radiochemical purity.

The present work is a contribution to rescue the history of development of the application of 99mTc, widely used in nuclear medicine, to its use as tracer for the study of the transport of fi ne sediment in suspension, in water environment. It addresses the usefulness of its application in obtaining important parameters in environmental studies, illustrating them with some applications already performed and the results obtained. This kind of study, when associated with information on hydrodynamic parameters, for example, river, tidal, wind and wave currents, are powerful tools for the understanding and quantification of fi ne sediment transport in suspension. Fine sediment is an important vector in the transportation of heavy metals, organic matter and nutrients in water environment, and the quantitative knowledge of its behaviour is mandatory for studies of environmental impacts. Fine sediment labelled with 99mTc, can also be used to study the effect of human interventions, such as dredging of reservoirs, access channels and harbours, and the dumping of dredged materials in water bodies. Besides that, it can be used to optimize dredging works, evaluating the technical and economic feasibility of dumping sites and their environmental impact. It is a valuable support in the calibration and validation of mathematical models for sediment dynamics.

Background: Due to its use of ionising radiation, the field of nuclear medicine is a unique and significant part of medical diagnostics and patient treatment. The aim of this study was to assess the internal exposure of nuclear medicine employees to radioiodine 131I and technetium 99mTc as well as to assess the external exposure doses. Material and Methods: The radioiodine 131I and technetium 99mTc contents in the thyroid of staff members (about 100 persons) dealing with these radionuclides have been measured in four departments of nuclear medicine. The measurements were conducted with a portable detection unit for in situ measurements of radioiodine and technetium. High sensitivity environmental thermoluminescent dosimeters (TLD) were used to measure the external exposure dose. Results: The average values and ranges of radioiodine 131I activity measured in the thyroids of all of the medical units' employees were: 83 Bq (range: 70-250 Bq), 280 Bq (range: 70-4000 Bq), 275 Bq (range: 70-1000 Bq) for technical staff, nuclear medicine staff and hospital services staff, respectively. The mean value of technetium 99mTc content in the thyroids of nuclear medicine staff was approximately 1500 Bq (range: 50- -1800 Bq). External exposure dose rates were in the range of 0.5-10 μGy/h. Conclusions: The calculated average effective dose for particular person caused by the inhalation of radioiodine 131I is below 5% of 20 mSv/year (occupational exposure limit). Wprowadzenie: Ze względu na stosowanie promieniowania jonizującego medycyna nuklearna jest istotną i unikalną gałęzią metod diagnostycznych i leczniczych. Celem podjętych badań była ocena narażenia wewnętrznego pracowników zakładów medycyny nuklearnej na jod promieniotwórczy 131I i technet 99mTc oraz ocena dawek pochodzących z ekspozycji zewnętrznej. Materiał i metody: Pomiary zawartości radiojodu 131I i technetu 99mTc w tarczycy osób pracujących z tymi radionuklidami (ok. 100 osób) wykonano w 4 ośrodkach medycyny nuklearnej. Zastosowano przenośny zestaw detekcyjny mierzący radionuklidy in situ. Dawki pochodzące z ekspozycji zewnętrznej mierzono, wykorzystując środowiskowe dozymetry termoluminescencyjne (thermoluminescent dosimeters - TLD) o dużej czułości. Wyniki: Zawartość radiojodu 131I w tarczycy wynosiła średnio: 83 Bq (zakres aktywności: 70-250 Bq) w grupie personelu technicznego, 280 Bq (zakres: 70-4000 Bq) w grupie personelu medycznego i 275 Bq (zakres: 70-1000 Bq) w grupie personelu pomocniczego. Średnia zawartość technetu 99mTc w tarczycy wynosiła ok. 1500 Bq (zakres: 50-1800 Bq). Otrzymana moc dawek pochodzących z ekspozycji zewnętrznej mieściła się w zakresie 0,5-10 μGy/godz. Wnioski: Oszacowana średnia dawka skuteczna pochodząca z inhalacji radiojodu 131I stanowiła mniej niż 5% rocznego limitu dla narażenia zawodowego, które wynosi 20 mSv/rok. Med. Pr. 2013;64(5):625–630

Opis:
Monte Carlo simulations are more and more popular trend in nuclear medicine imaging. They were also widely used in radiotherapy and brachytherapy with use of more general simulation software. GATE (Geant4 Application for Tomographic Emission) is a platform developed on general Monte Carlo code designed for simulating and tracking the passage of particles through matter (Geant4) but is specifically designed for nuclear medicine imaging purposes. The aim of this work was to create and validate a model of specific gamma-camera (E.Cam DUET), used in our department, for further use in image analysis and dosimetry field, with GATE simulation platform. We have modeled in first step a point source in the air in three configurations of gamma camera (without collimator, with low-energy high-resolution (LEHR) collimator and with high-energy (HE collimator)), then we have created a phantom with three spherical sources of different sizes but with the same concentration of the radiopharmaceutical. Simulation results were compared to the measured values of the energy spectrum and images obtained on the detectors, and show a good agreement for the point source experiments and revealed some differences for the phantom studies. This results shows that some additional tests and refinements are required but also allows us to study the image degrading quality factors, such as septal penetration and to work towards eliminating them from the pictures.
