Opis:
Atmospheric pollution suspended in humid air is popularly known as ‘smog’. It is composed of dust particles of different sizes, as well as non-metal oxides, organic compounds, and heavy metals. Exposure to harmful substances suspended in the air – apart from, for example – smoking cigarettes, one of the modifiable factors leading to the development of respiratory diseases. There are six types of substances present in the air that have a negative impact on public health and result in significant consequences: ozone, particulate matter (PM) of different diameters – PM2.5µ, PM2.5–10 µ, PM10 µ, nitrogen dioxide, sulphur dioxide, carbon monoxide and lead. Particular attention is given to small dust particles (PM10 and PM2.5) because they can penetrate into the lower respiratory tract. Apart from describing the composition of smog and sources of air pollution, the article also discusses the impact of atmospheric pollutants on both development and aggravation of the symptoms of such respiratory tract diseases as asthma, chronic obstructive pulmonary disease, respiratory infections and lung cancer. Some of legal measures applied in different countries aimed at reducing exposure to noxious air pollutants are reviewed. The authors believe that the increased focus on risks arising from inhaling toxic air pollution may be a first step for developing systemic solutions aimed at resolving or, at least, decreasing those risks.

Lung cancer is one of the most lethal types of cancer due to a lack of proper treatment. The rare presence of molecular therapy targets forces the use of platinum-based drugs. Cisplatin, approved by the USA as an anticancer therapy in the 1970s, is still one of the most prominent therapies against lung cancer. Unfortunately, the biggest limitation of cisplatin-based therapy is the development of cisplatin resistance. Cancer cells overcome the vast DNA damage caused by the drug in a variety of ways such as detoxication and extracellular transport of the drug, enhanced repair mechanisms, omitting apoptosis and epigenetic alterations. Chemotherapy resistance is an issue that so far cannot be dealt with. Nevertheless, better understanding of the molecular pathways behind cisplatin resistance brings hope for better therapy outcomes in lung cancer patients.

Lung cancer is associated with an increased risk of venous thromboembolism, including pulmonary embolism. In some situations, clinical deterioration in patients with lung cancer accompanied by elevated pressure in the right ventricle, usually measured by commonly used echocardiography, may support the diagnosis of pulmonary embolism. However, there are many other causes of increased pressure in the right ventricle in such patients, for example: progression of cancer, pre-existing lung diseases, surgical resection of pulmonary tissue, pnemotoxicity of radiotherapy or concomitant diseases of the left heart. The article presents 2 clinical cases of patients with lung cancer, in which elevated pressure in the right ventricle was resulted from other causes and accompanied the progression of cancer, despite the clinical picture suggesting a pulmonary embolism. Increased pressure in the right ventricle and usually associated pulmonary hypertension, significantly worsen already poor prognosis of patients with lung cancer. The differential diagnosis should, therefore, take into account the whole clinical picture, excluding venous thromboembolism as an important cause of pulmonary hypertension, but also take into account other potential factors to be able to make the right diagnosis and implement optimal treatment as early as possible.

BackgroundThe authors’ aim was to study the dynamics of oxidative stress in experimental exposure to silica dust, to evaluate the histopathological findings in the phase preceding the formation of fibrous/fibrohyaline pulmonary nodules, and to assess the effects of curcumin administration.Material and MethodsThe research was performed on 48 male Wistar rats with an average weight of 320 g. Overall, 38 rats were instilled with a single dose of 0.3 ml suspension containing 30 mg of a SiO2/ml saline solution, and were sacrificed 30, 90 and 120 days after instillation; 14 of those sacrificed on days 90 and 120 also received curcumin. The control group included 10 animals which were instilled with a saline solution. Malondialdehyde (MDA), carbonyl proteins (CPs), total thiolic proteins (TPs) and reduced glutathione (GSH) were determined in blood and the lung tissue. The standard technique for pulmonary toxicology developed by Porter was applied to semi-quantitatively assess the histopathological findings.ResultsIt was found that MDA had increased significantly early on in both biological environments and remained elevated, and adding curcumin proved beneficial, while CPs only increased moderately in the lung tissue without a curcumin impact. Moreover, TPs dropped abruptly, significantly and persistently in the lung tissue and blood, and were not influenced by curcumin. Finally, GSH decreased significantly and intensely in the lung tissue and blood, with curcumin lowering the levels towards those found within the control group. The histopathological examination identified nodules of a cellular type, without any fibrosis, but with spots of associated lipoproteinosis. The early lesions in the airways and vessels were suggestive of a remodeling process. Curcumin diminished the occurrence of alveolitis but not the remodeling process.ConclusionsThe study confirms the early onset of oxidative stress in experimental silicosis. It also simultaneously and dynamically researches markers of oxidative stress in blood and the lung tissue. Curcumin proved beneficial on oxidative stress and lesions in the alveolar epithelia, but ineffective in preventing vascular and airway remodeling. Med Pr. 2021;72(3):239–47

The acid-base equilibrium is closely linked to gas exchange in the lungs, and respiratory exchange ratios are used to evaluate respiratory effectiveness and tissue oxygen levels. Acid-base indicators are determined in both arterial and venous blood samples. This study compares the usefulness of acid-base indicators of venous and arterial blood in monitoring the condition of horses with recurrent airway obstruction. Prior to treatment involving bronchodilating glucocorticoids, expectorant and mucolytic drugs, more pronounced changes were observed in venous blood (pH 7.283, pCO2 61.92 mmHg, pO2 35.541 mmHg, HCO3 - 31.933 mmHg, BE 2.933 mmol/l, O2SAT 58.366%, ctCO2 38.333 mmol/l) than in arterial blood (pH 7.309, pCO2 53.478 mmHg, pO2 90.856 mmHg, HCO3 - 28.50 mmHg, BE 3.133 mmol/l, O2SAT 93.375%, ctCO2 31.652 mmol/l), indicating compensated respiratory acidosis. The improvement of respiratory efficiency minimized acidosis symptoms in both venous blood (pH 7.365, pCO2 43.55 mmHg, pO2 47.80 mmHg, HCO3 - 30.325 mmHg, BE 3.050 mmol/l, O2SAT 80.10%, ctCO2 29.80 mmol/l) and arterial blood (pH 7.375, pCO2 39.268 mmHg, pO2 98.476 mmHg, HCO3 - 26.651 mmHg, BE 4.956 mmol/l, O2SAT 98.475%, ctCO2 28.131 mmol/l). Venous blood parameters were marked by greater deviations from mean values, both before and after treatment. Acid-base indicators determined in venous blood are indicative of respiratory disturbances, but they do not support a comprehensive evaluation of gas exchange in the lungs.

Ethylene phosphorodifluoridite (C2H4F4O2P2) (CAS No. 3965-00-2) is a colorless corrosive fuming liquid that is used as a stabilizer in the electrolyte of rechargeable batteries. There are no previous reports of toxic effects following exposure to this compound. A 28-year-old male complained of respiratory distress after accidental inhalation of ethylene phosphorodifluoridite for 30 min. The patient developed acute lung injury with noncardiogenic pulmonary edema and was treated with supportive management. The patient fully recovered and was discharged after 7 days without any significant sequelae. The patient’s symptoms were attributed to non-cardiogenic pulmonary edema caused by ethylene phosphorodifluoridite exposure. The case study showed that ethylene phosphorodifluoridite should be added to the list of chemicals that can cause acute lung injury.

Opis:
Measurements of dynamic surface tension were carried out in aqueous systems (water or 0.1 mM Triton X-100) comprising nanoparticles formed from chemically modified polyaldehyde dextran (PAD). The nanostructures, considered as potential drug carriers in aerosol therapy, were obtained from biocompatible polysaccharides by successive oxidation and reactive coiling in an aqueous solution. The dynamic surface tension of the samples was determined by the maximum bubble pressure (MBP) method and by the axisymmetric drop shape analysis (ADSA). Experiments with harmonic area perturbations were also carried out in order to determine surface dilatational viscoelasticity. PAD showed a remarkable surface activity. Ward-Tordai equation was used to determine the equilibrium surface tension and diffusion coefficient of PAD nanoparticles (D = 2.3×10-6 m2/s). In a mixture with Triton X-100, PAD particles showed co-adsorption and synergic effect in surface tension reduction at short times (below 10 s). Tested nanoparticles had impact on surface rheology in a mixed system with nonionic surfactant, suggesting their possible interactions with the lung surfactant system after inhalation. This preliminary investigation sets the methodological approach for further research related to the influence of inhaled PAD nanoparticles on the lung surfactant and mass transfer processes in the respiratory system.

Introduction. Air pollution is one of the most important issues of our times. Air quality assessment is based on the measurement of the concentration of substances formed during the combustion process and micro-particles suspended in the air in the form of an aerosol. Microscopic atmospheric particulate matters (PM) 2.5 and 10 are mixtures of organic and inorganic pollutants smaller than 2.5 and 10 μm, respectively. They are the main cause of negative phenomena in the earth’s atmosphere of Earth and human health, especially on the respiratory and cardiovascular systems. Particulates have the ability to cause permanent mutations of tissue, leading to neoplasms and even premature deaths. Nitrogen dioxide (NO2) is one of the main pollutants which arises mainly during the burning of fossil fuels. Based on numerous scientific researches, it has been proved that long-term exposure to NO2 could increase morbidity of cancer due to inflammatory processes increasing abnormal mutations. Materials and method. Data available in the Polish National Cancer Registry, Chief Inspectorate for Environmental Protection and Map of Health Needs in the Field of Oncology for Poland, WHO Air Quality Guidelines 2005 were analyzed. Air pollution was also evaluated: PM2.5, PM10, NO2, and compared with lung cancer morbidity. Results and conclusions. Based on the available data and literature, it can be concluded that in 2009–2017, on average, each Pole smoked ten cigarettes a day +/- 2. Therefore, it can be estimated that after 60 years everyone had 30 package-years of smoking, leading to a high risk of lung cancer and other smoking related diseases. Additionally air quality in Poland is not satisfactory, exceeding the standards presented in the WHO Guidelines 2005. It can be assumed that this may translate into an additional, independent continuous increase in morbidity and mortality dependent on smoking.

Sepsis is a serious medical problem and is one of the main causes of high mortality in intensive care units. Fifty percent of patients with severe sepsis will develop acute lung injury (ALI). Amentoflavone (AMF) is a polyphenolic compound possessing potent anti-inflammatory activities. The study aimed to explore the protective effects of AMF against ALI in cecal ligation and puncture (CLP)-induced septic rats. The results showed that AMF administration protected against septic ALI, as reflected by marked amelioration of histological injury of lung tissues and decrease of pulmonary edema in CLP-treated rats. AMF ameliorated CLP-induced increase of systemic and lung TNFα and IL-1β and binding activity of p65 NF-κB, indicating the inhibition of inflammation. Moreover, AMF prevented CLP-induced oxidative stress, as evidenced by increase of oxygen consumption rate, decrease of TBARS content, increase of SOD activity and GSH level in lung tissue of CLP-treated rats. CLP resulted in significant decrease of mRNA expression of Nrf2 and GCLc, which was inhibited by AMF. AMF-induced protective effects on ALI, inflammation, and oxidative stress were inhibited by lentivirus shRNA-mediated silence of Nrf2 and buthionine sulphoximine (BSO), an inhibitor of GSH synthesis. AMF increased Nrf2-binding activity in GCLc promoters in lung tissue of CLP-treated rats. The results suggested that AMF protected against ALI in septic rats through upregulation of Nrf2-GCLc signaling, enhancement of GSH antioxidant defense, reduction of oxidative stress and final amelioration of inflammation and histological injury of lung. The data provide new therapeutic options for the treatment of sepsis-associated ALI.

Artykuł dotyczy problematyki modelowania sygnałów pozyskanych podczas osłuchu płuc. Oprócz modelu normalnego oddechu zdrowego pacjenta, przedstawiono opracowany model świstów astmatycznych, jednego z najpopularniejszych objawów zaburzenia oddychania. Biorąc pod uwagę fakt, że świsty osadzone są na tle normalnego oddechu, w artykule przedstawiono metodykę wyliczania poprawnego stosunku sygnałów tonalnych świstu do sygnału normalnego oddechu. Przeprowadzono również analizę przedziałów częstotliwości tonów występujących w świstach rzeczywistych.
The paper addresses a problem of modelling lung sounds. Apart from typical human breath of a healthy person, the model of asthma wheeze, one of the most often breathing disorders, is presented. Since wheezes are embedded in normal human breath, the methodology of signal-to noise (wheeze-to-breath) ratio calculation is discussed in the paper. The frequency range of wheezes present in real recordings is investigated in this work, too. Section 2 specifies signals that are obtained during the chest auscultation. In Section 3 feature identification and signal modelling of normal lung sounds are made. In this section the distribution of signal samples and the mean signal spectrum are shown in the figures. Section 4 describes identification of real signal features and modelling of wheezes. This section presents histograms of SNR (Signal to Noise Ratio or Wheeze-to-Breath Ratio), number of tones in wheezes and tone frequencies for real signals and generated tonalities. The model verification results obtained with use of tonality detection algorithms are given in Section 6. The short conclusions and a list of references are at the end of the paper.

Introduction: Lung cancer is the most frequent malignancy in Poland and is the first cause of death in oncology. The aim of the study is to analyze selected parameters of inflammation in lung cancer. Material and methods: 48 patients with lung cancer were included in the study group and 30 persons in the control group. The group was divided according to the level of severity - 19 with low/moderate stage (I-IIIA), 29 with high (IIIB and IV) and to histopatho- logical type - 9 with small cell carcinomas, 39 with non-small cell carcinomas (therein 18 - adenocarcinomas and 21 squamous carcinomas). The concentrations of circulating immune complexes (CIC), C-reactive protein (CRP), neutrophils, lymphocytes and their associated indicators were examined: CRP/CIC, NLR (neutrophil/lymphocyte ratio), PLR (platelet/lymphocyte ratio). Results: The average parameter values were significantly higher among people with lung cancer. The median value of CIC was significantly higher in the group with advanced cancer (0.30 vs 0.23, p<0.01 compare to stage I-IIIA). The median PLR and platelet counts among patients with squamous cell carcinoma were significantly higher compared to adenocarcinomas (215 vs 138.5, p = 0.037, 388.0 thousand/ul vs 277.5 thousand/ul, p = 0.034). Conclusion: Based on the results of our work, we suggested the dominance of a specific humoral response in the more advanced stage of lung cancerogenesis. The process of carcinogenesis is associated with various activities of neutrophils and lymphocytes.
Wprowadzenie: Rak płuca jest najczęstszym nowotworem w Polsce i pierwszą przyczyną zgonów w onkologii. Celem pracy jest analiza wybranych parametrów stanu zapalnego w raku płuca. Materiał i metody: Grupę badaną stanowiło 48 chorych z rakiem płuca oraz 30 przypadków grupy kontrolnej. Chorych podzielono wg stopnia zaawansowania - 19 z niskim/ umiarkowanym stopniem zaawansowania (I-IIIA), 29 z wysokim (IIIB i IV), a także wg typu histopatologicznego - 9 z rakiem drobnokomórkowym, 39 z niedrobnokomórkowym (w tym 18 z gruczolakorakiem i 21 z rakiem płaskonabłonkowym). Zbadano stężenia krążących kompleksów immunologiocznych (KKI), białka C-reaktywnego (ang. C-reactive protein, CRP), neutrofilów, limfocytów oraz związanych z nimi wskaźników: CRP/KKI, neutrofil/limfocyt (ang. neutrophil/lymphocyte ratio, NLR), płytka krwi/limfocyt (ang. platelet/lymphocyte ratio, PLR). Wyniki: Średnie wartości badanych parametrów były istotnie wyższe wśród osób z rakiem płuca. Mediana wartości KKI była istotnie wyższa w grupie z zaawansowanym nowotworem (0,30 vs 0,23, p<0,01 w porównaniu ze stopniem I-IIIA). Mediany wskaźnika PLR oraz liczby płytek krwi wśród osób z rakiem płaskonabłonkowym były istotnie wyższe porównując do gruczolakoraka (215 vs 138,5, p = 0,037; 388,0 tys/ul vs 277,5 tys/ul, p = 0,034). Wnioski: Na podstawie wyników pracy zasugerowaliśmy dominację swoistej odpowiedzi humoralnej w bardziej zaawansowanym stadium kancerogenezy płuca. Proces kancerogenezy związany jest z różną aktywnością neutrofili i limfocytów.

Wstęp Skutki zdrowotne zawodowej ekspozycji na pył azbestu mogą się pojawić po wielu latach od 1. narażenia. Celem pracy jest ocena zależności zmian chorobowych w układzie oddechowym od czynników opisujących zawodową ekspozycję na pył azbestu w 1. badaniu oraz analiza czynników wpływających na progresję tych zmian w kolejnych badaniach. Materiał i metody Grupę badaną stanowiło 591 byłych pracowników zakładów azbestowych „Gambit” w Lubawce. Ocenie poddano wyniki badań lekarskich, badań rentgenowskich (rtg.) klatki piersiowej i parametrów sprawności wentylacyjnej płuc przeprowadzonych w latach 2001–2012. Wnioskowanie statystyczne przeprowadzono na podstawie dwustronnych testów istotności przy poziomie istotności 0,05. Wyniki Potwierdzono zależność występowania zmian chorobowych w układzie oddechowym od wielkości narażenia na pył azbestu. Wykazano wyższe ryzyko zmian śródmiąższowych wraz ze wzrostem stężenia kumulowanego pyłu azbestu – dla pracowników o najwyższym narażeniu skorygowany iloraz szans (odds ratio – OR) wynosił 1,63 (95% przedział ufności (confidence interval – CI): 0,99–2,71), natomiast dla zmian o stopniu zaawansowania kwalifikującym do rozpoznania azbestozy ryzyko było istotnie ponad 5-krotnie wyższe w porównaniu z zatrudnionymi w najniższej ekspozycji. Obserwacja zależności progresji zmian śródmiąższowych od narażenia na pył azbestu wykazała 4-krotnie wyższe ryzyko progresji zmian u pracujących w najwyższym narażeniu. Średnie wartości FEV1 (forced expiratory volume in 1 s – natężona objętość wydechowa pierwszosekundowa), FVC (forced vital capacity – natężona pojemność życiowa), FEV1/FVC (forced expiratory volume in 1 s to forced vital capacity – natężona objętość wydechowa pierwszosekundowa do natężonej pojemności życiowej) były istotnie niższe u pracujących w wyższym narażeniu na pył azbestu. Potwierdzono także wpływ nałogu palenia tytoniu na wystąpienie i progresję zmian śródmiąższowych płuc w badanej populacji. Wnioski Przeprowadzona analiza wyników profilaktycznych badań stanu zdrowia pracowników zatrudnionych w przeszłości w zakładach stosujących jako surowiec azbest chryzotylowy wskazuje na zasadność ich długoterminowej obserwacji klinicznej oraz promowania profilaktyki antynikotynowej w tej grupie byłych pracowników. Background The adverse health effects of occupational exposure to asbestos dust may occur several years after first exposure. The objective of the study was to assess the relationship between lesions in the respiratory system and the factors contributing to occupational exposure to asbestos described in the first medical examination as well as to analyze the factors responsible for the progression of these changes in further medical tests. Material and Methods The study group comprised 591 former workers of asbestos processing plant “Gambit” in Lubawka. The results of medical examinations carried out in 2001–2012 were assessed. Statistical inference was performed based on bilateral significance tests at the 0.05 level of significance. Results A higher risk of interstitial lung changes along with an increase in the cumulative concentration of asbestos was indicated; for the employees with the highest exposure, the adjusted odds ratio (OR) was 1.63 (95% confidence interval (CI): 0.99–2.71), while for changes with the severity degree qualifying for asbestosis diagnosis, the risk was significantly increased, over fivefold higher, compared to subjects employed in the lowest exposure. The analysis of the relationship between the progression of interstitial changes and the exposure to asbestos dust showed a fourfold higher risk of the progression in workers employed in the highest exposure. Mean values of FEV1 (forced expiratory volume in 1 s), FVC (forced vital capacity), FEV1/FVC (forced expiratory volume in 1 s to forced vital capacity) were significantly lower in the subjects working in a higher asbestos exposure. The effect of tobacco smoking on the occurrence of interstitial lung changes and their progression was also confirmed. Conclusions The results of prophylactic medical examinations of the health status of workers formerly employed in the plants using chrysotile indicate the importance andthe need for a long-term clinical follow-up and the promotion of anti-smoking prevention in this group of former employees. Introduction and objective. Occupational asbestos exposure is one of the major public health issues. Although asbestos use in Poland has decreased, asbestosis continues to remain an important health concern in the area of occupational medicine. The aim of the study is to perform a descriptive analysis of hospitalization cases in patients suffering from asbestosis in Poland. Materials and method. The authors used hospital discharge records to conduct a retrospective, population-based study. To estimate the asbestosis hospitalization rate, data from the Nationwide General Hospital Morbidity Study conducted by the National Institute of Public Health were used. Events were defined as in-patient hospital discharges during 2006–2016. 1,101 hospitalization records of 764 patients were included in the study. Results. In the study group of first-time hospitalized patients, the mean and median ages were 68.1 and 69 years, respectively; CI: 67.3–68.9; SD: 11.1, min-max:16–99 years. In this group, significant gender differences were observed (560 males vs. 204 female; P<0.001). No statistical differences were observed regarding the place of residence. During the observation period, 61 in-hospital deaths were reported (8% of all patients), 40 of whom (5.2 % of all patients) took place during the first-time hospitalizations. Asbestosis hospitalizations were mainly clustered in the south-western region, whereas the highest hospitalization rates were clustered in several areas of Poland. Conclusions. The authors believe this study to be the first evaluation of asbestosis in Poland made on the basis of the hospital morbidity database. The data presented may be helpful in comparative studies on the epidemiology of asbestosis across European countries.

Early physiotherapy is an important part of the comprehensive treatment of patients after major abdominal surgery (MAS). Accelerated mobilization should be safe and requires the use of appropriate techniques. Most of the physiotherapists and surgeons recommends using abdominal belts. Opponents claim that belts have an adverse effect on lungs ventilation. The aim of the study was to determine the effect of abdominal belt on lung ventilation efficiency in the early period after MAS. Material and methods. The study involved 20 patients after MAS. Including 9 women and 11 men, aged between 40 to 90 years (xˉ 66.7). In the scheduled 7 patients and urgent 13. All of them were in the early period after surgery. Dynamic spirometry was performed twice in the postoperative period. For the first time wearing a belt, and then without the belt in the same group. Evaluated the forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF). For the analysis uses the Wilcoxon matched-pairs test and Spearman’s rank correlations. P values <0.05 were considered significant. Results. The value of the ventilation indicators measured wearing abdominal belt were slightly lower than the values evaluated without the belt. The results are shown in percent predicted for age and gender, FEV wearing belt 52%, without belt 53%; FEV1 59% vs 61%; PEF 46% vs 51%. There were no statistically significant differences Conclusions. There were no significant negative influence of abdominal belt on lungs ventilation in early period after MAS. There was no correlations between age, body mass index and changes in ventilation indicators.

Objectives: The brick field industry is one of the oldest industries in India, which employs a large number of workers of poor socioeconomic status. The main aim of the present investigation is i) to determine the prevalence of musculoskeletal disorders among brick field workers, ii) to determine the prevalence of respiratory disorders and physiological stress among brick field workers compared to control workers. Material and Methods: For this study, a total of 220 brick field workers and 130 control subjects were selected randomly. The control subjects were mainly involved in hand-intensive jobs. The Modified Nordic Questionnaire was applied to assess the discomfort felt among both groups of workers. Thermal stress was also assessed by measuring the WBGT index. The pulmonary functions were checked using the spirometry. Physiological assessment of the workload was carried out by recording the heart rate and blood pressure of the workers prior to work and just after work in the field. Results: Brick field workers suffered from pain especially in the lower back (98%), hands (93%), knees (86%), wrists (85%), shoulders (76%) and neck (65%). Among the brick-making activities, brick field workers felt discomfort during spading for mud collection (98%), carrying bricks (95%) and molding (87%). The results showed a significantly lower p value < 0.001 in FVC, FEV₁, FEV₁/FVC ratio and PEFR in brick field workers compared to the control group. The post-activity heart rate of the brick field workers was 148.6 beats/min, whereas the systolic and diastolic blood pressure results were 152.8 and 78.5 mm/Hg, respectively. Conclusions: This study concludes that health of the brick field workers was highly affected due to working in unhealthy working conditions for a long period of time.
