Opis:
Cloud-based computational environments can offer elastic and flexible services to wide audiences. Małopolska Educational Cloud was originally developed to support the day-to-day collaboration of geographically scattered schools with universities which organized online classes, led by university teachers, as an amendment to face-to-face teaching. Due to the centralized management and ubiquitous access, both the set of services provided by MEC and their usage patterns can be adjusted rapidly. In this paper we show how – during the COVID-19 pandemic – the flexibility of Małopolska Educational Cloud was leveraged to speed up the transition from in-class to remote teaching, both in the classes and schools which were already involved in the MEC project, and newly added ones. We also discuss the actions that were required to support the smooth transition and draw conclusions for the future.

Opis:
A new solution to the problem of frequency estimation of a single sinusoid embedded in the white Gaussian noise is presented. It exploits, approximately, only one signal cycle, and is based on the well-known 2nd order autoregressive difference equation into which a downsampling is introduced. The proposed method is a generalization of the linear prediction based Prony method for the case of a single undamped sinusoid. It is shown that, thanks to the proposed downsampling in the linear prediction signal model, the overall variance of the least squares solution of frequency estimation is decreased, when compared to the Prony method, and locally it is even close to the Cramér-Rao Lower Bound, which is a significant improvement. The frequency estimation variance of the proposed solution is comparable with, computationally more complex, the Matrix Pencil and the Steiglitz-McBride methods. It is shown that application of the proposed downsampling to the popular smart DFT frequency estimation method also significantly reduces the method variance and makes it even better than the least squares smart DFT. The noise immunity of the proposed solution is achieved simultaneously with the reduction of computational complexity at the cost of narrowing the range of measured frequencies, i.e. a sinusoidal signal must be sufficiently oversampled to apply the proposed downsampling in the autoregressive model. The case of 64 samples per period with downsampling up to 16, i.e. 1/4th of the cycle, is presented in detail, but other sampling scenarios, from 16 to 512 samples per period, are considered as well.

Opis:
The aim of this study was to verify improved, ensemble-based strategy for inferencing with use of our solution for quantitative assessment of tendons and ligaments healing process and to show possible applications of the method. Methods: We chose the problem of the Achilles tendon rupture as an example representing a group of common sport traumas. We derived our dataset from 90 individuals and divided it into two subsets: healthy individuals and patients with complete Achilles tendon ruptures. We computed approx. 160 000 2D axial cross-sections from 3D MRI studies and preprocessed them to create a suitable input for artificial intelligence methods. Finally, we compared different training methods for chosen approaches for quantitative assessment of tendon tissue healing with the use of statistical analysis. Results: We showed improvement in inferencing with use of the ensemble technique that results from achieving comparable accuracy of 99% for our previously published method trained on 500 000 samples and for the new ensemble technique trained on 160 000 samples. We also showed real-life applications of our approach that address several clinical problems: (1) automatic classification of healthy and injured tendons, (2) assessment of the healing process, (3) a pathologic tissue localization. Conclusions: The presented method enables acquiring comparable accuracy with less training samples. The applications of the method presented in the paper as case studies can facilitate evaluation of the healing process and comparing with previous examination of the same patient as well as with other patients. This approach might be probably transferred to other musculoskeletal tissues and joints.

Opis:
Idiopathic hypertrophic pachymeningitis (IHPM) is a rare disaster, with still unknown aetiopathogenesis. There are above 100 described cases of IHPM in the medical literature till 2010 year of which majority was reported in last 10 years. We would like to present a case of 52 years old woman admitted to Department of Neurology and Epileptology, Medical University of Lodz in August 2010. The disease was started with weakening of visual acuity in 2007 year in left eye. Because of this she was hospitalised in ophthalmology ward and after that at neurological ward, but the reason for weakening of visual acuity was unclear. She had had numerous CT scans of head, which were correct always. In May 2009 she was given an cardiostimulator because of cardiologic disease, which excluded ability to future MRI examination. In August 2009 she was diagnosed a total atrophy of left optical nerve and beginning of atrophy of right optical nerve.In February headaches change from occasional to every day pains. After she was admitted to hospital the CT scan of head showed cerebral haematoma. Although the existing symptoms of pressure to the right lateral ventricle with its displacement but she was not qualified to the operation by neurosurgeon. In August 2010 after another hospitalisation because lack of evolution of haematoma and increasing displacement she was transferred to the Neurosurgery Department. During the surgery stated thicker dura mater about 7 mm, which fragments were sent to pathomorphological examination. They showed changes of a fibrogranular nature. Many other laboratory test were made to discover cause of IHPM and they were negative, that proof the idiopathic hypertrophic pachymeningitis diagnosis.
Idiopatyczny przerost opony twardej to rzadkie schorzenie o niejasnej patogenezie. W literaturze światowej do 2010 roku opisano około 100 przypadków, z czego większość w ostatnim dziesięcioleciu. Niniejszym chcieliśmy przedstawić przypadek 52-letniej kobiety, która została przyjęta do Kliniki Neurologii i Epileptologii Uniwersytetu Medycznego w Łodzi w sierpniu 2010 roku. Choroba rozpoczęła się osłabieniem ostrości widzenia oka lewego w 2007 roku. Z tego powodu pacjentka była hospitalizowana na oddziale okulistycznym, a następnie neurologicznym, jednakże nie ustalono rozpoznania przyczynowego. Miała wielokrotnie wykonywane badanie CT głowy, które wypadało prawidłowo. W maju 2009 roku z powodu zaburzeń kardiologicznych implantowano chorej kardiostymulator, co w przyszłości wykluczało możliwość wykonania MRI. W sierpniu 2009 roku stwierdzono już całkowity zanik nerwu II oka lewego oraz zaczynający się zanik nerwu II po stronie prawej. W lutym 2010 roku bóle głowy zmieniły swoją częstość – ze sporadycznych stały się codzienne. Po przyjęciu pacjentki do szpitala w 2010 roku badanie CT głowy ujawniło krwiaka przymózgowego. Mimo istniejących objawów ucisku prawej komory bocznej, z jej przemieszczeniem, chora nie została zakwalifikowana do zabiegu. Po ponownej hospitalizacji w sierpniu 2010 roku, z powodu braku ewolucji krwiaka i narastających przemieszczeń pacjentkę przeniesiono do Kliniki Neurochirurgii. W trakcie zabiegu stwierdzono pogrubiałą oponę twardą (około 7 mm), której fragmenty przesłano do badania histopatologicznego. Wykazało ono zmiany o charakterze włóknisto-ziarnistym. U chorej wykonano szereg badań laboratoryjnych w celu ustalenia przyczyny przerostu opony twardej, które wypadły negatywnie, co wskazuje na idiopatyczny przerost opony twardej.

The major problem of total parenteral treatment consists in the balancing of the source and dose of the nutritional mixture, so as to not deepen malnutrition with a positive impact on the patients’ organism. The aim of the study was to evaluate selected factors that induce hepato-billiary complications in patients treated by means of parenteral nutrition at home. Material and methods. The retrospective study comprised 70 patients with biochemistry performed every three months. Considering statistical analysis patients were allocated to four groups, depending on the period of treatment. Group A analysis results were based on the activity of aminotransferases, group B on the activity of bilirubin. Both groups A and B were additionally divided into group I where we assigned normal values of control lab results, and group II with improper results after treatment. Results. Differences between groups Ia vs IIa were presented on the basis of the daily supply of glucose: mean- 2.52 vs 3.49 g/kg (p=0.000003), glucose/lipids ratio: mean- 3.76 vs 4.90 g/kg (p=0.0001), daily non-protein energy: mean- 16.73 vs 21.06 kcal/kg (p=0.0001). Differences between groups Ib vs IIb were presented on the basis of the daily supply of glucose: mean- 2.76 vs 3.46 g/kg (p=0.0007), glucose/lipids ratio: mean- 3.98 vs 5.13 g/kg (p=0.01), daily non-protein energy: mean-17.96 vs 20.36 kcal/kg (p=0.04). Based on the above-mentioned analysis the main goal in the prevention of hepatic complications should lead to the reduction of the dose of glucose. Increased glucose supply leads to increased number of hepato-billiary complications. Conclusions. Based on obtained results we were able to conclude that in case of liver complications associated with parenteral nutrition, proper management consists in the modification of nutritional mixtures (reduction in the daily glucose supply and change in the proportions of extra-protein energy). Such management has the greatest clinical effect. When determining the composition of the nutritional mixture one should adjust the glucose supply, so as to offset both sources of extra-protein energy
