We present the case of a 18-year-old man, who was admitted to the Maria Sklodowska-Curie Memorial Cancer Centre and Institute of Oncology in Warsaw in 2007 year after non-radical resection of synovial sarcoma located in right axilla. The staging was established as unifocal limited disease after diagnostic procedures. The patient was treated with combined therapy consists of neoadiuvant chemotherapy, radiotherapy, surgery and adjuvant chemotherapy. In 2009 year we noted locoregional recurrence of disease and after dissemination to the lungs and locoregional recurrence was diagnosed again. Eight line of chemotherapy was given and a number of metastasectomy of lungs was performed because of recurrence of pulmonary metastases. At present on the grounds of progression of disease – metastases to bones and recurrence of metastases to lungs patient is receiving a new drug – pazopanib, as non-standard therapy. As a result of this therapy a thirteen months stabilisation of disease is achieved.
W pracy opisano przypadek 18-letniego mężczyzny, który zgłosił się do Centrum Onkologii im. Marii Skłodowskiej-Curie w Warszawie w 2007 r. po nieradykalnej operacji wycięcia mięsaka maziówkowego w okolicy prawego dołu pachowego. Po przeprowadzeniu diagnostyki określającej zaawansowanie miejscowe i wykluczeniu rozsiewu procesu nowotworowego pacjenta zakwalifikowano do leczenia skojarzonego pod postacią chemioterapii neoadiuwantowej, radioterapii, leczenia chirurgicznego i uzupełniającej chemioterapii. W 2009 r. doszło do wznowy miejscowej, a w kolejnych latach do rozsiewu choroby do płuc oraz do następnej wznowy miejscowej. Pacjent otrzymał łącznie 8 linii chemioterapii i został poddany kilku metastazektomiom z powodu nawrotowych przerzutów do płuc. Obecnie z powodu progresji choroby, pod postacią przerzutów do kości i nawrotu przerzutów do płuc, pacjent jest leczony nowo zarejestrowanym lekiem w terapii mięsaków – pazopanibem, w ramach chemioterapii niestandardowej, w której wyniku uzyskano trwającą już 13 miesięcy stabilizację choroby.

BackgroundWhen assessing physical activity (PA), particular attention should be paid to medical university students who are taught to be health care professionals (HCPs) responsible for maintaining health in humans. However, different studies have shown that HCPs exhibit the same unhealthy behaviors as the general population. This study analyzed PA among medical university students of different faculties and their adherence to current PA recommendations.Material and MethodsData from 216 medical university students of physiotherapy, dietetics and pharmacy, including males (N = 44) and females (N = 172), the mean age of 22.3±1.8 years, were collected. The International Physical Activity Questionnaire in its long form (IPAQ-LF) was used to assess and classify PA behaviors. The results were analyzed in accordance with World Health Organization recommendations regarding PA.ResultsOver 60% of all the students were classified as active during all-day activity. However, while analyzing PA in different domains, the same shares of all the students were still insufficiently active during leisure time, and so they did not meet the recommendation of >75 min/week of vigorous PA, >150 min/week of moderate PA or an equivalent combination. All the students self-reported PA mainly in the work and transport domains. Generally, physiotherapy students were the most active and performed PA with higher intensity.ConclusionsThis study revealed a low level of leisure time PA among the students, and no habit of regular PA. Some changes in medical education should be suggested to include physical education as a long-term subject in medical school curricula. Future research is needed to investigate the exercise barriers that students perceive, which can guide future interventions aimed at improving their PA, and thereby impact on the quality of health care which they will provide.

Opis:
Tracheal secretion leakage might lead to ventilator-associated pneumonia. Standard tracheal tube cuffs are cylindrical in shape. Bronchial tree aspiration is observed in the presence of leakage past tracheal tube cuffs. The new TaperGuard tracheal tube has a cuff in the shape of a cone, preventing from the development of microtubules and microaspiration. The aim of the study was to compare standard tracheal tube cuffs with TaperGuard tubes, in terms of protection from microaspiration under general anesthesia. Material and methods. The observational study evaluated incidents of leakage during general anesthesia with intubation and mechanical ventilation in patients with significant obesity. The study group comprised 20 patients with the BMI >40 kg/m2 subjected to elective surgery under general anesthesia with intubation and mechanical ventilation, randomly divided into two subgroups: standard tracheal tube and TaperGuard tube. Results. In 4 of 10 patients with standard tracheal tubes we observed leakage around the cuff. In case of TaperGuard tubes no such leakage was observed. Conclusion. TaperGuard tubes protect against microaspirations, while standard tracheal tubes have no such properties.

Noninvasive ventilation (NIV) is a technique of mechanical ventilation which does not require invasive airway management, i.e. intubation or tracheostomy. In emergency medicine Continuous Positive Airway Pressure (CPAP) is used often. A new method of NIV is Impedance Threshold Device (ITD). Breathing through an ITD is utilized to raise blood pressure in hypotensive patients. Aim of the study was to compare haemodynamic effects of NIV ITD and NIV CPAP. Material and methods. This study involved a group of 25 healthy volunteers. NIV was performed using ResQGARD ITD and CPAP Boussignac. Ventilation time was 25 minutes for each mask in each participant. Every three minutes parameters were collected: SpO2, BP and HR. There was a one hour interval in between ventilation with each mask. CPAP pressure was set at a level of 8 cm H2O and the mean inspiratory resistance of the ITD was 7cm H2O. Collected parameters were subjected to ANOVA statistical analysis. Results. Absolute comparison of BP, HR and SpO2 values did not reveal statistically significant differences between the masks. However considering blood pressure levels at entry, ventilation through an ITD significantly raised BP. Ventilation with NIV CPAP did not change significantly BP. Conclusion. Ventilation through an ITD device significantly improve haemodynamic function, whereas CPAP ventilation had no significant effect on it.
