Strata w wypadku wymiany polega na tym, że poprzedni etap, który został usunięty, katastrofą czy świadomie, jest jak karta z książki wyrwana. Nowoczesna ingerencja w zabytkowy budynek musi szanować poprzednie karty, poprzednie nawarstwienia. Czasami może nawet pewne braki stron uzupełnić albo raczej wskazać. Mądre postępowanie nie kieruje się w takim wypadku wyłącznie poleceniami służb konserwatorskich. Empatia dla zapisów architektury/księgi to wyzwanie dla twórców, szczególnie architektów. Pokażemy taki proces na przykładzie kamienicy o numerze Rynek Główny 25 w Krakowie, która jest przykładem starannego unowocześniania struktury i przestrzeni z zachowywaniem czytelnych śladów. Jest także ukłonem teraźniejszości ku przyszłości nowego ducha tej przestrzeni.
Architecture is the art of building, that is, gathering durable materials and agglomerating them in such a way as to produce a work that is durable, functional, and beautiful (as described by the Vitruvian triad: firmitas, utilitas, venustas). The completion of construction marks the beginning of the building's duration. The duration of architecture almost always involves making changes. It would be wonderful to let architecture last indefinitely, with its original shapes, textures, and materials preserved. Technical progress, changes in needs, natural and historical accidents, and many other factors make it necessary to introduce changes in the object. Persistence of architecture is a constant change and it is good if they are created by EVOLUTION. Layering is like writing a book, page by page. It then allows you to "read" the architecture and with it the history of the place and the people. Layering is more interesting than exchanging. The loss in the case of replacement is that the previous stage that was removed, whether by disaster or intentionally, is like a page torn out of a book. A modern intervention in a historic building must respect the previous pages, the previous layering. Sometimes it can even fill in some missing pages or rather point them out. The wise course of action in such a case is not guided only by the orders of conservation services. Empathy for architectural/book records is a challenge for creators, especially architects. We will show such a process on the example of the tenement house number 25 Market Square in Krakow, which is an example of careful modernization of structure and space while preserving legible traces. It is also a nod of the present to the future of the new spirit of this space.

In September2009, gas and rock were ejected from the roof of the excavation at the Rudna copper mine of KGHM Polska Miedź S.A. The ejection occurred as a result of an unsealing of a small gas trap locateda few metres above the roof of the excavation within the dolomites of the first Zechstein cyclothem (PZ1). In order to identify the nature of the reservoir rock for natural gas, microscopic observations and X-ray microtomography were performed and compared with the results of the acoustic anisotropy tests. The obtained results indicate that, based on well-recognized petrological rocks, the results of acoustic anisotropy can be successfully applied as diagnostic for the recognition of rocks of various structures, textures and porosities.

Gruźlica wciąż stanowi poważny problem epidemiologiczny na całym świecie. Ponad 130 lat wytężonej pracy naukowców na całym świecie nie przyniosło zapewniającej protekcję szczepionki przeciwgruźliczej, szybkich i pewnych metod diagnozowania tej choroby oraz skutecznych sposobów leczenia gruźlicy wywoływanej przez izolowane ze wzrastającą częstością wielolekooporne prątki gruźlicy. Wywołujące gruźlicę prątki Mycobacterium tuberculosis są Gram-dodatnimi pałeczkami tlenowymi o unikatowej w świecie bakterii ścianie komórkowej, która zbudowana jest, poza peptydoglikanem i polisacharydami, z nietypowych glikolipidów i lipidów, w tym długołańcuchowych kwasów mykolowych, będących silnymi modulatorami układu odpornościowego człowieka. Diagnozowanie gruźlicy oparte jest na hodowli prątków na podłożach bakteriologicznych, obok której wykonywane jest badanie bakterioskopowe plwociny, radiologiczna ocena zmian w klatce piersiowej, tuberkulinowy test skórny, czy też testy molekularne (AMPLICOR Mycobacterium tuberculosisis Xpert MTB/RIF) lub metody wykorzystujące mykobakteriofagi (FastPlaque oraz ReporterPhage). Istotnym problem epidemiologicznym są uśpione (latente) zakażenia prątkami gruźlicy, które wykrywa się testami interferonowymi (QuantiFERON®-TB Gold Plus oraz T-SPOT.TB). Mimo znacznego postępu wciąż brakuje szybkich, wiarygodnych i tanich testów diagnostycznych gruźlicy.
Tuberculosis (TB) remains a serious epidemiological problem throughout the world. More than 130 years of hard work of scientists around the world has not delivered fully protective vaccine for TB, fast and reliable methods for diagnosing and effective treatment of tuberculosis caused by multi-drug resistant Mycobacterium tuberculosis (MDR-TB), isolated recently with an increasing incidence. MDR- TB are Gram-positive, aerobic bacilli with the unique bacterial cell wall, which is built not only of peptidoglycan and polysaccharides, but also of unusual glycolipids and lipids, including long-chain mycolic acids, which are potential modulators of the human immune system. Diagnosis of tuberculosis is based on the culture of mycobacteria on bacteriological media, alongside which are carried out: bacterioscopy of sputum, radiographic assessment of changes in the chest, tuberculin skin test, or molecular test (AMPLICOR Mycobacterium tuberculosisis Xpert MTB/RIF), and tests with mycobacteriohages (FastPlaque and ReporterPhage). The important epidemiological issue are dormant (latent) mycobacterial infections, which can be detected by interferon-γ release assays (QuantiFERON®-TB Gold Plus and T-SPOT.TB). Despite considerable progress we are still lacking high-speed, reliable and low-cost diagnostic tests for tuberculosis.

Tuberculosis (TB), an infectious disease caused by Mycobacterium tuberculosis (M.tb), remains a leading public health problem in most parts of the world. Despite the discovery of the bacilli over 100 years ago, there are still many unanswered questions about the host resistance to TB. Although one third of the world's population is infected with virulent M.tb, no more than 5-10% develop active disease within their lifetime. A lot of studies suggest that host genetic factors determine the outcome of M.tb-host interactions, however, specific genes and polymorphisms that govern the development of TB are not completely understood. Strong evidence exists for genes encoding pattern recognition receptors (TLR, CD14), C-type lectins, cytokines/chemokines and their receptors (IFN-γ, TNF-α, IL-12, IL-10, MCP-1, MMP-1), major histocompatibility complex (MHC) molecules, vitamin D receptor (VDR), and proton-coupled divalent metal ion transporters (SLC11A1). Polymorphisms in these genes have a diverse influence on the susceptibility to or protection against TB among particular families, ethnicities and races. In this paper, we review recent discoveries in genetic studies and correlate these findings with their influence on TB susceptibility.

A perianal fistula is a pathological canal covered by granulation tissue connecting the anal canal and perianal area epidermis. The above-mentioned problem is the reason for the patient to visit the surgeonproctologist. Unfortunately, the disease is characterized by a high recurrence rate, even despite proper management. The aim of the study was to determine the current condition of perianal fistula treatment methods in everyday surgical practice, considering members of the Society of Polish Surgeons. Material and methods. 1523 members of the Society of Polish Surgeons received an anonymous questionnaire comprising 15 questions regarding perianal fistula treatment in everyday practice. Results. Results were obtained from 807 (53%) members. After receiving answers, questionnaire results were collected, analysed, and presented in a descriptive form. Conclusions. Study results showed that most Polish surgeons choose the fistulectomy/fistulotomy method. Considering treatment of perianal fistulas the most important issue is to find the correct, primary fistula canal. Further methods should be individually selected for each patient. One should also remember that every fistula is different. Surgical departments that operate a small number of perianal fistulas should direct such patients to reference centers.

The sex and age of the physician performing the digital rectal examination (DRE) procedure is one of the parameters influencing patients’ comfort. It is postulated that the stress related to DRE during admission to the surgical ward may affect the compliance. The aim of the study was to characterize patients’ preferences according to their sex, age, socioeconomic status and according to DRE-related variables. Material and methods. Patients admitted to the Department of General and Colorectal Surgery at Medical University in Łódź between October 2014 and June 2015 were asked to complete a questionnaire regarding their sex, age, ethnic background, socioeconomic status and preference for a physician performing the DRE during admission to the surgical ward. Results. The study enrolled 225 patients, 52% (n=117) women and 48% (n=108) men. Most patients 73.3% (n=244) expressed no preference for sex of the physician performing the DRE during admission, while 22.7% (n=51) preferred a same-sex physician. Analysis showed that the age, female sex, lower education status, small amount of comorbidities and number of hospitalizations, and no previous colonoscopy experience were all associated with preference for a same-sex physician. Conclusions. In our study most patients expressed no preference for sex of the physician performing the DRE during admission to the surgical ward. However, over one-tenth of patients reported such preferences. Most of these patients preferred a same-sex physician. It is important to offer these patients the choice of physician performing the DRE. Addressing patients’ preferences may improve the atmosphere in the clinical environment, reduce stress, and facilitate better treatment.

The aim of the study was to assess patients’ awareness of the prevention and treatment of colorectal cancer. Material and methods. Patients diagnosed with colorectal cancer, hospitalised at the Department of General and Colorectal Surgery of the Medical University in Łódź during the period from January 2015 to April 2015, were asked to complete a questionnaire concerning their families’ medical case record, factors predisposing them to the development of colorectal cancer, the tests applied in diagnostics, and the treatment process. The questionnaire comprised 42 closed-ended questions with one correct answer. A statistical analysis of all answers was carried out. Results. The study group consisted of 30 men and 20 women aged 27–94 years old. A strong, statistically significant negative correlation between a patient’s age and his/her awareness of the prevention and treatment of colorectal cancer was noted (p<0.001; r= -0.51). The study demonstrated a statistically significant relationship between the occurrence of neoplasms in a patient’s family (p=0.009) or, more specifically, the occurrence of colorectal cancer (p=0.008), and the awareness of the prevention programme. The women’s group was characterised by statistically significantly greater awareness of colonoscopy as a screening examination (p=0.004). Conclusions. Patients need more information on colorectal cancer, its risk factors, prevention, the treatment process, and postoperative care. Lack of awareness of the colorectal cancer issue can be one of the major factors contributing to the high incidence of this disease.

Bacillus Calmette-Guérin (BCG) and pertussis vaccines have been found to be insufficient and their further improvement is required. In order to develop improved vaccines, a better understanding of the main pathways involved in the host's protective immunity to the pathogens is crucial. We address the question as to whether the balance between pro- and anti-inflammatory cytokine production might affect the host responses to BCG and diphtheria-tetanus toxoids-whole cell pertussis (DTwP) vaccines. The study population consisted of 118 healthy people, age range 18-30 years, who had been subjected to BCG and DTwP vaccination according to the state policy. Tuberculin skin testing (TST) revealed a delayed type hypersensitivity (DTH) to PPD (purified protein derivative) in 53% volunteers. The variability in development of the BCG-driven DTH to tuberculin prompted us to address a question as to whether Th1/Th2 polarization is involved in the lack of skin responsiveness to PPD. PPD-stimulated blood lymphocytes from TST+ participants produced significantly more IFN-γ and less IL-10 than lymphocytes from TST- volunteers. However, TST- volunteers' sera contained more anti-pertussis IgG but not anti-diphtheria toxin IgG. Mycobacterial antigens and particularly PPD induced a higher expression of HLA-DR and co-stimulatory CD80 receptors on DCs from TST+ than TST- participants. BCG but not PPD pulsed DCs from TST- volunteers produced significantly more IL-10. Mycobacterial antigen stimulated DCs from TST+ volunteers induced a more intense IFN-γ production in co-cultures with autologous lymphocytes than the cells from TST- participants. Differences among the types of dendritic cell activities contribute to development of tuberculin reactivity in BCG vaccinated volunteers.

Introduction: Proctocolectomy with ileal pouch-anal anastomosis is the gold standard in the surgical treatment of patients with ulcerative colitis, familial adenomatous polyposis and other colorectal diseases requiring colectomy. The treatment consists in removing the large intestine and creating an intestinal reservoir from the last ileum loop and then anastomosing the intestinal reservoir with the anal canal. Like any surgical procedure, RPC-IPAA also carries the risk of complications, both early and late. Late postoperative complications include sexual dysfunction. Aim: The main goal of the following work is to assess the quality of life and sexual activity in patients having undergone the RPC-IPAA procedure at the General and Colorectal Surgery Clinic. Material and methods: The study group consisted of patients aged 19–79 who had been subjected to RPC-IPAA procedures at the General and Colorectal Surgery Clinic in years 2010–2019. The study was conducted on the basis of a survey consisting of 50 questions about the social and mental condition, medical history and previous treatment as well as the quality of sexual life before and after surgery. The scale used for the assessment of the quality of sex life consisted of 5 grades: very low, low, medium, high, very high. Thirty subjects (21 men and 9 women) took part in the survey. Ulcerative colitis (86.6%) was the most common reason for qualification for restorative proctectomy among the examined patients; less common reasons included familial adenomatous polyposis (13.3%) and synchronous colorectal cancer (3.3%). A vast majority of the surgeries had been performed after 10 years’ duration of ulcerative colitis, and the intestinal reservoir had been functioning for over a year at the time of the examination. In addition, the effect of taking steroids and the impact of early postoperative complications on the quality of sex life of patients was assessed. Results: High or very high sexual activity before surgery was reported by 46% of patients whereas low or very low quality was reported by 13%. The rest of the responders assessed their pre-operative sexual activity as average. After surgery, 23% of patients rated their sexual activity as high or very high while 36.6% of patients rated it as low or very low (P = 0.07). It was also noted that taking corticosteroids before surgery decreased the quality of sex life after surgery (P = 0.07 for activity, P = 0.04 for quality). None of the women surveyed used artificial moisturizing of intimate places during sex. Only 1 person stated that they started using artificial moisturization of intimate places after the procedure (P = 0.5). None of the men surveyed had used pharmacological agents to help them obtain an erection before surgery while as many as 33% of responders reported the need for their use after surgery (P = 0.008). Other postoperative sexual dysfunctions were also registered, such as dyspareunia (13.3%), sensory disorder within the intimate region, fecal incontinence, and urinary incontinence. Conclusions: To sum up, sexual activity and quality of sexual life deteriorated after RPC-IPAA in our patients.
