Opis:
The potential applications of loop heat pipes (LHPs) are the nuclear power space systems, fuel cell thermal management systems, waste heat recovery systems, medium temperature electronic systems, medium temperature military systems, among others. Such applications usually operate in temperature ranges between 500–700 K, hence it is necessary to develop an LHP system that will meet this requirement. Such a thermal management device require to meet various technical problems and challenges currently existing in the development of LHP working in medium temperatures, including: (1) selection of appropriate working fluid; (2) selection of appropriate LHP construction material; (3) construction of suitable test rig capable of testing at elevated temperatures; (4) development of new testing methods. Currently, there are no proven working fluids that can be used in LHPs in medium temperature ranges. Water can be applicable only at temperatures up to 570 K. Caesium can be applicable at temperatures above 670 K. Organic fluids usually tend to generate non-condensable gasses and/or decompose at elevated temperatures and their viscosity dramatically increases. For halides, most of them are very reactive or toxic and their full property data are not available or the majority of the physical properties are predicted, also live tests and their environmental impact data are not adequate. As for casing/LHP construction material, there are no full chemical compatibility tables with most of the medium temperature working fluids and the reactivity of fluids significantly limits the potential materials. Also, testing such an LHP is an endeavour as the reactivity of medium temperature fluids and the use of obscure metals create new challenges. Altogether creates multiple challenges in the development, testing, handling and operating of LHP in the medium temperature range.

Insulinomas are rare tumors, accounting for 1-2% of all neoplasms of the pancreas. Usually their treatment is not associated with any problems; however there is a small subset of problematic clinical cases. The authors present their own clinical experience with surgical treatment of insulinomas of the pancreas. The aim of the study was to conduct a retrospective analysis of patients with insulinomas of the pancreas who underwent surgical treatment at Department of General and Transplant Surgery Medical University in Łódź. Material and methods. The study included all patients who underwent surgical treatment at the Department between 2007 and 2013 for the tumor of the pancreas. Further retrospective analysis included all patients with tumors of the insulinoma type. The data was obtained from medical records, surgical protocols and histopathology reports. Results. The analysis included 530 patients who underwent surgical treatment for the tumor of the pancreas. Insulinoma was found in 10 (1.88%) patients (8 females, 2 males). An average age of patients who underwent surgical treatment was 47.5±13.8 years. An average size of the tumor was 1.6±0.5 cm. Six patients underwent extirpation of the insulinoma, while the other patients underwent distal resection of the pancreas. All patients underwent “an open surgical procedure”. The average duration of the surgical procedure was 55±45 minutes. Duration of the hospitalization in the analyzed group of patients was 7±5 days. Incidence of postoperative pancreatic fistulas was 10%. All insulinomas were benign. Conclusions. Insulinomas were rare among patients who underwent surgical treatment at the Department. They were benign and their treatment was unproblematic. However, there is a small group of cases that can be associated with problematic clinical situations. Thus treatment of patients with insulinomas should be conducted at specialist centers. Correct diagnostic and therapeutic management, involving close cooperation between multiple medical specialists, results in complete curing of majority of patients.

Cel: 1. Ocena wyników leczenia pacjentów po jednoczasowej plastyce przepukliny w bliźnie pooperacyjnej (Incisional Hernia Repair - IHR) sposobem onlay i abdominoplastyce u pacjentów po operacji ominięcia żołądka - RYGB ze znaczną utratą masy ciała. 2. Określenie różnic zmian jakości życia (Quality of Life - QL) u chorych bariatrycznych przed i po zabiegu konturującym. Materiał i metody: Analizą kliniczną objęto 20 chorych ze zniekształceniem powłok brzucha (po zabiegu RYGB i redukcji masy ciała) poddanych jednoetapowej IHR i abdominoplastyce. Oceniono wyniki operacyjnego leczenia, a także zmiany QL chorych przed i po operacji konturującej powłoki brzuszne (skale DAS24 i SF-36) w porównaniu do stanu przed zabiegami operacyjnymi. Wyniki: Powikłania w gojeniu rany (zakażenie, miejscowa martwica) i zapalenie płuc stwierdziliśmy u 2 osób, obecność surowiczaka u dwóch, natomiast zaburzeń czucia u czterech chorych. Po każdym etapie leczenia chirurgicznego stwierdzono poprawę QL we wszystkich jego aspektach. Wnioski: 1. Jednoczasowa operacja przepukliny brzusznej sposobem onlay z abdominoplastyką jest bezpieczną metodą poprawiającą funkcjonowanie pacjentów. 2. Wszystkie etapy leczenia bariatrycznego wpłynęły na stopniową poprawę jakości życia. 3. Wysoka wartość BMI chorych przed zabiegiem przepukliny brzusznej metodą onlay z abdominoplastyką zwiększa ryzyko powikłań, co wiąże się z wydłużonym okresem hospitalizacji.

The Background Delayed graft function (DGF) is a common complication following kidney transplantation and is associated with ischemia-reperfusion injury (IRI). Lymphocytes contribute to the pathogenesis of IRI and ischemia-reperfusion related to delayed graft function. Materials and Methods : 135 Caucasian patients received a kidney graft from deceased heart-beating organ donors. We divided patients into 2 groups - patients with the eGFR>=30 on the 21st day post-transplantation (n=36) and patients with the eGFR<30 on the 21st day post-transplantation (n=99) to assess kidney graft function. We measured serum creatinine levels on 1st and 2nd post-transplant day and preoperative levels of monocytes, lymphocytes, platelets, and neutrophils and their ratios. Results: We have found statistically significant differences between the eGFR<30 and the eGFR>=30 groups in the average lnLymphocytes (0.36 +/-0.6 vs. -0.016 +/-0.74, respectively p=0.004) lnNLR (1,27 +/-0.92 vs. 1.73+/-1.08 p=0.016) lnLMR (1.01 +/-0.57 vs. 0.73 +/-0.64 p=0.02), lnPLR (4.97 +/-0.55 vs. 5.26 +/- 0.67 p=0.023), and CCR 2% (-20.20 +/- 21.55 vs. -4.29 +/- 29.62 p=0.004. In the univariate analysis, factors of lnLymphocytes >=0.22 (OR=0.331 95%CI 0.151-0.728 p=0.006), lnLMR>=1.4 (OR=0.255 95%CI 0.072-0.903 p=0.034) were associated with a worse graft function, while lnNLR>=1.05 (OR=2.653 95%CI 1.158-6,078 p=0.021), lnPLR>=5.15 (OR=2.536 95%CI 1.155-5.566 p=0.02) and CRR2 (OR=3.286 95% CI 1.359-7.944 p=0.008) indicated a better graft function. Conclusion: A higher absolute lymphocyte count (lnLymphocytes) and lnLMR, as well as lower lnNLR and lnPLR were associated with a lower eGFR on the 21st day after kidney transplantation. In the multivariate analysis, CRR2 in combination with either lnLymphocytes, lnNLR or lnPLR improved the accuracy of detecting patients with poor graft function.

Purpose: 1. Evaluation of results after one-time incisional hernia repair (IHR) modo on-lay and abdominoplasty (Ab-pl) in patients after loss of weight following previous Roux-en-Y Gastric Bypass – RYGB. 2. Analysis of differences in quality of life (QL) changes in bariatric patients before RYGB, as well as before and after abdominal contouring operation. Material and Methods: Clinical analysis involved 20 patients with abdominal disfigurement (following RYGB, and massive weight loss) who underwent one-time IHR using on-lay method together with abdominoplasty. We estimated postoperative results, as well as changes in QL, before RYGB and before and after abdominal contouring procedure (based on DAS 24 and SF-36 scales) in comparison with stage before surgeries. Results: Complications - abnormal wound healing (infection, local necrosis) and pneumonia were found in two persons, seroma in two cases, whereas dysesthesia in four patients. We confirmed QL improvement in all aspects after each stage of treatment. Conclusions: 1. One-stage on-lay hernia repair and abdominoplasty is a safe method improving the functioning of patients. 
2. All stages of bariatric treatment resulted in gradual improvement of the quality of life. 3. High BMI in patients before onlay incisional hernia repair with abdominoplasty increases the risk of complications, which is connected with longer hospital stay.

Celem artykułu jest analiza znaczenia innowacji technologicznych w handlu elektronicznym, a także ocena, w jaki sposób mogą one wpływać na kształtowanie się sektora e-commerce. W artykule zaprezentowano wyniki własnych badań ankietowych przeprowadzonych wśród właścicieli e-sklepów, jak również e-konsumentów, zwracając uwagę na liczbę i stopień wykorzystywanych narzędzi przez przedsiębiorców oraz wymagania klientów wobec podmiotów prowadzących sprzedaż internetową. Analizy zagadnienia dokonano w kontekście charakterystycznych dla współczesnego wirtualnego rynku zjawisk, jak big data, konkurencyjność, komodytyzacja, rosnąca świadomość klientów.
The aim of this article is an analysis of the meaning of technological innovations in e-commerce as well as the review of how they might influence the shaping of the e-commerce sector. The article presents the results of an in-house survey research, conducted among the online store owners and customers. It focuses on the number of tools and the degree to which they are used by store owners as well as clients' demands towards sellers. The analysis of this issue was made in the context of the characteristic for the modern virtual market occurrences, such as big data, competitiveness, commoditization, increasing awareness of customers.
