Background: Anticoagulants are used mainly in the prevention of venous thromboembolism. The frequency of haemorrhagic complications is correlated with patients’ age and level of International Normalized Ratio (INR). Spontaneous intramedullary spinal cord haemorrhage due to anticoagulation therapy is an extremely rare occurrence. A 62-year old male was admitted to hospital with acute abdominal pain and paraplegia. Because of persistent atrial fibrillation and previous history of ischaemic stroke, on admission the patient was treated with anticoagulant, INR – 15. MRI revealed an abnormal fusiform hyperintesity area in the spinal cord. ‘Haemo’ sequence confirmed the presence of hemosiderin deposits at the level of Th5 – Th7. Anticoagulation therapy might be a cause of spontaneous intramedullary spinal cord haemorrhage. Maintaining high levels of clinical suspicion and utilizing MRI with additional ‘haemo’ option may help in making the right diagnosis.

Wybudowane w latach 1928-1939 osiedle "Dyrekcja" ze względu na swoją powierzchnię, ilość i kubaturę budynków uważane było za drugą po Gdyni największą inwestycję okresu międzywojennego. Osiedle reprezentuje cały zespół wartości, wynikających z jego programu ideowego (symboliki państwowej), układu przestrzennego, spójnej formy architektonicznej zabudowy i charakterystyki technicznej budynków. Artykuł niniejszy ma na celu, próbę dokonania szeroko rozumianej oceny stanu założenia, po 80-latach od rozpoczęcia budowy.
District "Dyrekcja" built in the year 1928-1939 was regarded the second after the Gdynia largest investment of the period between world wars due to its size number of houses. The district represents the whole set of the values resulting the ideological programme (national symbolism), spatial composition, coherent architectural form of the buildings and their technical characteristics. The presented article is aimed as a complex evaluation of the district “Dyrekcja” after 80 years of its foundation.

Opis:
The protection of historical ruins requires solution of doctrinal and technical problems. Technical problems concern above all preservation of walls, which are exposed to the influence of atmospheric factors. The problem that needs to be solved in any historic ruin is securing of wall crests. Form of protection of the wall crests depends on many factors, mainly technical features of the wall and architectural and conservatory vision. The following article presents three aspects important for protection of wall crests. Firstly, analysis of features of the wall as a structure, secondly the characteristics of destructive agents, thirdly forms of protection of wall crests. In the summary of the following article, advantages and disadvantages of each method of preservation of the wall crests were presented.

The syndrome of inappropriate antidiuretic hormone secretion (SIADH) is an etiologically-diverse pathological condition resulting from the elevation in both ectopic and autonomic secretion of the antidiuretic hormone (ADH) by neoplastic tissue; excessive stimulation of the hypothalamic-pituitary axis, e.g. in pulmonary diseases, central nervous system (CNS) abnormalities, endocrine glands dysfunction or due to the use of some medications; intensification of renal ADH action by certain medications and action of substances chemically-related to vasopressin. The clinical characteristics of the syndrome are comprised of the presence of inadequately concentrated urine, hyponatremia, and hypo-osmolal blood serum, as well as weight gain. To show the variety of its causes and courses the article presents three cases of patients diagnosed with SIADH in the Department of Internal Medicine. In each of them the syndrome of inappropriate antidiuretic hormone secretion manifested in clinical laboratory tests in the form of hyponatremia.

Introduction. Lumbar disc herniation (LDH) is a common cause of back pain and leg pain. For more than 60 years, standard discectomy by fenestration has been used to treat LDH. In his study, three commonly used scales were used to assess the outcome after microdiscectomy, such as the Low Back Pain Rating Scale (LBPRS), Oswestry Disability Index (ODI) and the Visual Analogue Scale (VAS). To the best of the authors’ knowledge, this is the first study analyzing treatment results at 3 months after microdiscectomy. Materials and method. During almost 5 years, a total of 327 patients underwent microdiscectomy operations for single level disc disease. Of these, 286 patients were available for 3-mounth follow-up. All patients were operated on at the Neurosurgery Department in the Specialist Hospital in Sandomierz, Poland. Before surgery and 3 months after burgery, all patients were asked to complete LBPRS, ODI and VAS scales. The statistical analysis used was p2, the effect size indicator for ANOVA. Results. Significant improvement was observed in all scales used 3 months after surgery. Conclusions. There are new, minimally invasive percutaneous techniques available, such as percutaneous laser disc decompression and intradiscal injection of radiopaque gelified ethanol. There are also trials with platelet-rich plasma or mesenchymal stem cells injected into the disc, aimed at the restoration of healthy disc properties. The hypothesis that metabolic products of bacteria propionibacterium acnes can contribute to disc degeneration inspired attempts at antibiotic therapy This study was conducted on a large group of patients and confirmed that discectomy is the recommended method of surgery which produces good treatment results in 3-mounths follow-up.
