Opis:
The article tackles the question of the emergence, as well as the characteristics, of the psychological mechanisms of addiction in individuals with prolonged and intensive alcohol intake, and indications for therapeutic work with such individuals in the course of a basic programme of therapy for alcohol addicts. Excessive, prolonged and intensive drinking leads to the emergence of addiction- specific psychological mechanisms which encompass the emotional sphere of the individual concerned, the cognitive sphere and the centre of personality i.e. the structure of Self. The emotional sphere witnesses the emergence of the addictive mechanism of regulating one’s emotions, the mind of an addict sees the creation of an illusion and denial mechanism, while the Self structure experiences the mechanism of diffusion and splitting. The combined effect of these mechanisms destroys an individual’s life and centres it around drinking. The author stresses the significance of therapeutic work on the ‘breaking down’ of these mechanisms and on solving those personal problems which constitute a constant source in activating the mechanisms. Therapeutic work in the basic therapy programme concentrates mainly on the understanding and acceptance of one’s addiction, coping with alcohol craving, recognising the destruction alcohol brings and admitting helplessness in relation to alcohol. Work on the mechanisms should commence in the first meeting with the patient, in order to prevent drink relapse.

Opis:
The article reviews the definition of the word ‘psychotherapy’ which, depending on the theoretical approach adopted, can be defined in a variety of ways, and which consequently comes down to the notion of help and therapeutic effect. Irrespective of whether the psychotherapy takes on an individual or group character, it is a specialised method of helping people with different mental disorders, its main aim being the levelling out of the disorder in question, the introduction of an individual’s development-oriented changes and his or her attainment of health and mental well-being in the broadest sense. The author focuses on the advantages of both individual and group therapy, especially in the treatment of addictions. She additionally indicates the factors which have an influence on the effectiveness of psychotherapy, while stressing that the best effects are achieved through consolidating individual and group psychotherapy into one coherent therapy programme.

Background. There is an increasing interest in the role of adipocytokines in cardiovascular pathophysiology. Aim. The aim of the study was to compare visfatin levels, a novel adipokine, in patients with heart failure (HF) due to the left ventricular systolic dysfunction with those in age- and body mass index (BMI) - matched healthy controls in relation to the parameters of glucose metabolism and high sensitivity C-reactive protein (hsCRP) levels. Material/Subjects and Methods. The study population consisted of 28 males with systolic HF referred for cardiopulmonary exercise testing, divided into two subgroups based on their NYHA class (HF patients NYHAI+II, n=17, and HF patients NYHAIII+IV, n=11), and 23 controls. The following indices were measured in a serum samples: visfatin, hsCRP, glucose and lipid metabolism parameters, and the insulin resistance index HOMAIR (homeostasis model assessment insulin resistance) was calculated. Results. Concentrations of visfatin and high-density lipoprotein cholesterol (HDL-cholesterol) in the HF subjects were significantly lower (p≤0.01) than in controls. The Kruskal-Wallis test showed significant differences between three groups (controls and both subgroups of heart failure patients) in mean levels of visfatin, hsCRP, glucose, HOMAIR and HDL-cholesterol. Conclusion. Serum visfatin concentrations in patients with systolic HF, particularly with more advanced NYHA classes, are significantly lower in comparison to healthy controls and are independent of age or anthropometric and metabolic parameters.
