The paper consists of two parties. The former deals with the theoretical framework of the problem. On the one hand, attention is paid to selected elements of medical ethics, especially its connections with health economics. On the other hand, the authors discuss the notion and significance of prophylactic activities carried out by health insurance institutions. The latter part of the paper is based on an empirical material, which was made available by the Supervisory Office for Health Insurance in mid-March 2003 and concerns the costs of medical services incurred by all the regional health insurance institutions in Poland from 1999 to 2003. The costs of prophylactic health programmes and health policy programmes are analysed with the use of additional data obtained from the Financial Department of the Łódź Regional Sickness Fund. It has been calculated that the costs of prophylactic health programmes and health policy programmes per capita will range from 0.48 zł to 8.56 zł in 2003, depending on the health insurance institution. The share of these costs on total costs of medical services will range from 0.066 to 1.239%. The presented situation gives rise to a justified anxiety and makes one ask numerous questions connected with ethical dilemnas.

Alcohol withdrawal syndrome is one of the most serious disorders resulting from alcohol addiction. Moreover, this disorder may cause complications such as hallucinations including delirium. Doctors and paramedics dealing with patients very often face aggressive behaviour. The aim of the study was to investigate the scale of that phenomenon and characterize it. The study included the analysis of patients’ medical documentation with diagnosed alcohol abstinence syndrome at the time of admission. Research confirmed that the aggressive behaviour occurred in 25% of cases. It occurred most frequently in case of delirium, and was directed to the patient’s environment. Conclusions: 1) Aggressive behaviour is quite a serious problem that affected every fourth patient being tested. The reasons can be different, but the crew of ambulances and doctors from hospital wards must be attentive and prepared for all kinds of violent behaviour, not only towards them, but to the patient himself also. 2) Aggressive behaviour was observed more frequently in the event of alcoholic delirium. 3) Aggression demonstrated by the patients with alcohol abstinence syndrome is addressed in almost ⅔ of the cases to the environment of the patient.
Alkoholowy zespół abstynencyjny jest jednym z najpoważniejszych zaburzeń wynikających z uzależnienia od alkoholu. Dodatkowo w trakcie jego przebiegu może dojść do powikłań z wystąpieniem halucynacji oraz pełnoobjawowym majaczeniem alkoholowym włącznie. Lekarze i ratownicy medyczni w kontaktach z takimi pacjentami często spotykają się z agresywnym zachowaniem. Celem pracy było zbadanie skali wspomnianego zjawiska oraz próba jego scharakteryzowania. Badania polegały na analizie kart chorobowych pacjentów z rozpoznanym, w chwili przyjęcia, alkoholowym zespołem abstynencyjnym. Z badań tych wynikało, że zachowania agresywne występowały w 25% przypadków. Najczęściej agresja była skierowana na otoczenie pacjenta i dotyczyła majaczenia alkoholowego. Wnioski: 1) Agresywne zachowanie jest dość poważnym problemem, występującym u co czwartego badanego pacjenta. Przyczyny tego stanu bywają różne, a członkowie załóg karetek pogotowia i lekarze oddziałów szpitalnych muszą być uważni i przygotowani na różnego rodzaju gwałtowne zachowania, skierowane nie tylko na personel, ale także na samego pacjenta. 2) Agresywne zachowanie było obserwowane zdecydowanie częściej w przypadku majaczenia alkoholowego. 3) Agresja pacjentów z alkoholowym zespołem abstynencyjnym skierowana jest w blisko ⅔ przypadków na otoczenie pacjenta.

Objectives More and more countries introduce a total ban on smoking tobacco in public places. The aim of this work was to evaluate the effectiveness of “The Act of 8 April 2010 on amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and act on National Sanitary Inspectorate” and assess the frequency of complying with the smoking bans by the students of the Medical University of Lodz, Poland. Material and Methods Between 2007–2011, at the Social Medicine Institute of the Medical University of Lodz, a study using random survey was carried out involving students who were starting their studies at the Health Department of the Medical University of Lodz. The analysis of the collected material showed that 1038 people reported being smokers at the time of the study. Among that group, 530 students were included in the study prior to, and 508 after the introduction of the amendment. In order to verify their compliance with the smoking ban, the respondents were asked whether they smoked only in designated areas or wherever they wanted to. Results The ratio of people claiming they smoked anywhere they wanted to, disregarding the smoking ban, was 60% (N = 318) and after the amendment had been introduced, this ratio was 62.2% (N = 316), it increased by 2.2 percentage points. The observed difference was statistically irrelevant (Chi² = 0.530, p > 0.05). Conclusions The Act ”On amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and Act on National Sanitary Inspectorate” in Poland did not result in the expected changes in the frequency of complying with the smoking ban by the 1st year students.

It has been proven that outdoor and indoor air pollutants can cause adverse health effects and are able to promote the onset of atopic diseases. The current manuscript is focused on methodological issues. The aim of the study is to determine the effect of air pollution, urban environment, and urban heat islands (UHIs) on the occurrence of respiratory diseases including allergic rhinitis, asthma and wheezing in preschool children. The study group consists of 276 five-year-old children attending randomly selected kindergartens in the urban and rural areas of the Łódź Voivodeship. The questionnaire including data on the child’s state of health and socio-economic data will be filled by the caregivers. The children will undergo skin prick testing and the measurement of volatile organic compounds in exhaled breath. The key components of air pollution – particulate matter (PM) will be measured by personal meters. The PM sampling planned in the study will take 12 h for PM2.5 and for PM10 alike. Data on the level of outdoor air pollution will be collected based on the results obtained from air monitoring stations. The impact of air pollution, UHIs and the environment on the respiratory system and the presence of allergies in children, including chronic respiratory diseases, will be assessed. The project results will provide a scientific basis for the development of preventive programs in the population of children in the Łódź Voivodeship, adapted to the real health needs of society.

Objectives The objective of this study was to evaluate the association between breastfeeding duration and child neurodevelopment based on the Polish Mother and Child Cohort Study. Material and Methods The current analysis included 501 mother–child pairs. The analysis evaluating the association between the length of breastfeeding and child neurodevelopment considered the following variables: maternal age and body mass index, weight gain during pregnancy, parental level of education, marital status, socioeconomic status, child gender, birthweight, type of delivery, preterm delivery, pre- and postnatal exposure to tobacco constituents and child day care attendance. Psychomotor development was assessed in 1-year-olds on the Bayley Scales of Infant and Toddler Development. Results The length of breastfeeding correlated positively with maternal age at delivery (ρ = 0.13), maternal and paternal level of education (ρ = 0.2 and ρ = 0.14 respectively), birthweight (ρ = 0.1) and marital status (ρ = 0.16) (p < 0.05). A negative correlation between the length of breastfeeding and maternal smoking status during the first year after delivery (ρ = –0.19) and weight gain during pregnancy (r = –0.1) was observed (p < 0.05). The association between the duration of breastfeeding and child development was not statistically significant in the model with the inclusion of confounding variables. A significant association between language development and maternal level of education (p = 0.004), gender of the child (p = 0.0007) and maternal weight gain during pregnancy (p = 0.01) was found. A negative association between cognitive development and maternal salivary cotinine during pregnancy (p = 0.03) and a negative association between motor development and maternal smoking status during the first year after delivery (p = 0.007) were also found. Conclusions This study found no significant association between the duration of breastfeeding and child development after adjustment for confounders. Int J Occup Med Environ Health. 2019;32(2):175–84

Effects of environmental exposures in utero and in the first years of life on early life health and development is a growing research area with major public health implications. The main aim of this work has been to provide an overview of the next step of the Polish Mother and Child Cohort Study (REPRO_PL) covering exposure, health and neurodevelopment assessments of children at 7 years of age. Details regarding methodology of the follow-up of the children are crucial for cross-cohort collaboration and a full understanding of the future research questions. Phase III of the REPRO_PL cohort covers a follow-up of 900 children at the age of 7 years old. The questionnaire filled in by the mothers is composed of: socio-demographic, child exposure and home environment information, nutritional status and health data. In the case of 400 children, environmental (including collection of urine, saliva and buccal cells), health status and psychomotor assessments are performed. Health and development check consists of physical measurements, child health status assessment (including lung function tests, skin prick testing, an interview/examination by an allergist) and psychomotor development tests (the Strength and Difficulties Questionnaire and the Intelligence and Development Scales). The results of the study will become available within the next few years. Extension of the REPRO_PL cohort with examinations of children at the age of 7 years old may provide a better understanding of the relationship between environmental and lifestyle-related factors and children’s health and neurodevelopment; and may further strengthen scientific base for policies and interventions promoting healthy lifestyle.
