Objectives The aim of this study was to evaluate the relationship between high air temperatures and occupational injuries (OIs) occurred during the summer seasons 2000–2013 in agricultural workers from the Autonomous Province of Trento (APT), North-Eastern Italy. Material and Methods Data about OIs for the APT from 2000 to 2013 occurring during the warm season (N = 7325) was provided by the National Institute of Insurance for Occupational Illness and Injury. Daily average and daily maximum temperatures values for the specific geographical site of events were retrieved. Daily temperatures were then assessed in 3 time lags: for the day of the event (lag 0), and for the previous 24 h (lag 1) and 48 h (lag 2). Daily temperatures were then categorized in 3 exposure groups (< 75th, 75–95th and > 95th percentiles). The risk of OIs was assessed as odds ratio (OR) calculated through a Poisson regression model controlled for age, sex, ethnicity and time period, and assuming OI rates for days on which temperature was comprised in < 75th percentile exposure groups as the referent ones. Results Estimated incidence of OIs during the study period was 3.4±2.3 events/day. The peak of work-related accidents occurred on days characterized by severe thermal conditions, and in particular during heat waves (incidence rate ratio = 1.09, 95% confidence interval (CI): 1.02–1.17, p = 0.0165). Days having temperatures higher than 95th percentile, assessed as daily average, both on current days (OR = 1.119, 95% CI: 1.008–1.242) and in lag 1 (OR = 1.125, 95% CI: 1.013–1.249), as well as daily maximum temperatures, were at the highest risk of work-related injuries (OR = 1.144, 95% CI: 1.029–1.272). Conclusions In conclusion, presented findings recommend policymakers to develop appropriate warning/alert systems for agricultural workers regarding high environmental temperatures. Introduction Increasing evidence suggests that idiopathic pulmonary fibrosis (IPF) occurs more often in subjects previously exposed to wood dusts than in non-exposed subjects. Here we report 2 cases of the IPF among workers prolongedly exposed to high levels of hardwood dusts. Case report The case No. 1: An 83 year-old male former smoker, retired joiner developed mild dyspnoea and chronic dry cough over the period preceding the examination. Pulmonary function tests (PFT) identified a mild restrictive pattern and diffusion capacity for carbon dioxide (CO₂) that was severely impaired (57% of predicted value). High resolution computer tomography (HRCT) identified bilateral, subpleural basal reticular opacities in honeycombing, without any nodules or ground-glass opacities. The case No. 2: A 73 year-old male retired joiner, never smoker, presented a 3-year history of progressive breathlessness and non-productive cough in mild hypoxemia. Pulmonary function tests suggested a moderate restrictive pattern in severely impaired diffusion capacity for CO₂ (54% of predicted value). High resolution computer tomography identified diffuse peripheral reticular opacities and honeycombing of lower fields, with apico-basilar gradient. Both cases received diagnosis of the idiopathic pulmonary fibrosis. Discussion The pulmonary fibrosis is a common feature of several diseases and may be induced by inflammatory disorders following inhalation of organic and inorganic dusts (e.g., asbestos, silica), and several reports suggest that many cases of the IPF may be in fact secondary to occupational dust exposure as in the case reports we present here. Conclusions Occupational exposure to wood dusts may be a risk factor for the IPF. Unfortunately, exposure reconstruction is frequently inconsistent and anamnesis often misses other causes of the pulmonary fibrosis (e.g., extrinsic allergic alveolitis). Med Pr 2015;66(5):739–747
Wstęp Coraz więcej dowodów wskazuje na częstsze występowanie samoistnego zwłóknienia płuc (idiopathic pulmonary fibrosis – IPF) u osób wcześniej narażonych na pyły drewna niż u osób nienarażonych na nie. Niniejszy artykuł przedstawia 2 przypadki IPF u pracowników długotrwale eksponowanych na wysokie stężenia pyłu drewna. Opis przypadków Przypadek nr 1: U 83-letniego mężczyzny, byłego palacza i emerytowanego stolarza, w ciągu 6 miesięcy poprzedzających badanie rozwinęły się lekka duszność oraz przewlekły suchy kaszel. Badania czynnościowe płuc wykazały łagodne zaburzenia wentylacji typu restrykcyjnego oraz poważne upośledzenie dyfuzji dwutlenku węgla (CO₂) (57% wartości prognozowanej), natomiast tomografia komputerowa wysokiej rozdzielczości – obustronne podopłucnowe siateczkowate zacienienia u podstawy płuc w postaci obrazu plastra miodu, bez żadnych guzków ani zacienień typu matowej szyby. Przypadek nr 2: Badania czynnościowe płuc 73-letniego mężczyzny, nigdy nie palącego emerytowanego stolarza, z postępującą od 3 lat dusznością, suchym kaszlem oraz łagodną hipoksemią, wykazały umiarkowane zaburzenia wentylacji typu restrykcyjnego oraz poważne upośledzenie dyfuzji CO₂ (54% przewidywanej wartości), natomiast tomografia komputerowa o wysokiej rozdzielczości – rozproszone obwodowe siateczkowate zacienienia oraz obraz plastra miodu w dolnych polach ze szczytowo-podstawnym gradientem. W obu przypadkach zdiagnozowano samoistne zwłóknienie płuc. Omówienie Zwłóknienie płuc jest wspólną cechą kilku chorób i może być wywołane przez procesy zapalne wskutek wdychania organicznych lub nieorganicznych pyłów (np. azbestu, krzemionki). Wiele doniesień, w tym przedstawione w niniejszej pracy, wskazuje na liczne przypadki IPF, w których etiologii narażenie zawodowe na pyły miało charakter drugorzędny. Wnioski Narażenie zawodowe na pyły drewna jest czynnikiem ryzyka występowania IPF. Niestety odtworzenie rzeczywistej ekspozycji zazwyczaj jest trudne, a przeprowadzany wywiad często pomija pozostałe przyczyny powstania zwłóknienia płuc (np. alergiczne zapalenie pęcherzyków płucnych). Background Carpal tunnel syndrome (CTS) is a musculoskeletal condition that often impairs the fitness to work. Our aim is to retrospectively evaluate the association between physical exposures in meat processing industry in Northern Italy and the CTS, taking into account non-occupational factors. Material and Methods A cross-sectional study was performed to include 434 workers (236 males, 198 females, 37.0±10.6 years old, working age: 12.6±10.8 years) from meat processing industries. Signs and symptoms were collected at the compulsory occupational medical surveillance. Occupational risk factors were assessed through a questionnaire and direct assessment by investigators. Adjusted odds ratios (ORadj) for factors of interest were estimated through binary logistic regression. Results Diagnosis of the CTS was reported for 61 out of 434 subjects (14.1%) for an incidence of 11.3/1000 person- years. In general, signs and symptoms for the CTS were associated with the following demographic factors: smoking history (OR = 1.909, 95% confidence interval (CI): 1.107–3.293), previous traumas of the upper limb (OR = 3.533, 95% CI: 1.743–7.165), hypothyroidism status (OR = 7.897, 95% CI: 2.917–21.38) and, in the case of female participants only, previous pregnancies (OR = 2.807, 95% CI: 1.200–6.566) as well as a personal history of oral contraceptive therapy and/or steroidal replacement therapy (OR = 11.57, 95% CI: 4.689–28.56). The carpal tunnel syndrome cases were associated with the following occupational factors (> 4 h/day): forceful hand exertion (ORadj = 3.548, 95% CI: 1.379–9.131), repeated trauma of the hand (ORadj = 3.602, 95% CI: 1.248– 10.395), repeated movements of the wrist (ORadj = 2.561, 95% CI: 1.100–5.960). Conclusions Increasing levels of hand activity and force were associated with the increased CTS prevalence among participants. Recommendations have to be provided in order to reduce occupational exposure to these risk factors and improve medical surveillance. Med Pr 2017;68(2):199–209

Objectives: Very little is known regarding the toxicokinetics of inhaled zinc, in particular in the case of female workers and for modern, low exposure settings. Our aim is to evaluate the relationship of external zinc levels to those of serum and urine for female workers. Material and Methods: Eleven female workers (age: 41.7±8 years old, body mass index (BMI): 23.5±4.2 kg/m2) in a galvanizing plant were investigated. Exposure assessment consisted of personal/environmental air samples, and measurement of zinc in serum (collected at the end of first shift of the working week (T1)) and urine, collected before the first shift of the working week (T0), T1 and at the end of the last shift of the working week (T2). Results: Both environmental and personal air samplings for zinc and zinc compounds were below the recommended by the German Research Foundation (Deutsche Forschungsgemeinschaft – DFG) limit values of 2 mg/m3 (7.34±2.8 μg/m3 and 8.31±2.4 μg/ m3, respectively). Serum (118.6±20.9 μg /dl) and urine zinc levels were within reference values for female Italian subjects: the latter increased from 56.4±33.5 μg/dl at T0, to 59.8±37.0 μg/dl at T1, and ultimately 65.4±34.4 μg/dl at T2, but no significant trend was found. End of shift (Spearman’s correlation coefficient p value = 0.027) and differential excretion of urinary zinc (both: T0 vs. T1 and T0 vs. T2) were correlated with airborne zinc concentration (p = 0.002 and 0.006, respectively). Conclusions: In general, our data suggests that urine may be a useful medium also for female in order to assess zinc exposure. Further studies are required in order to evaluate whether differential excretion may be useful for the biomonitoring of zinc exposure in the workplaces also for male workers. Objectives Carpal tunnel syndrome (CTS) is the most common nerve entrapment syndrome, which since the beginning of the seventies has been linked to the keyboard and visual display unit (VDU). The objective of this study was to investigate the prevalence and personal factors associated with CTS in female VDU workers in Italy. Material and Methods Participants in this study were female adult subjects, working ≥ 20 h/week (N = 631, mean age 38.14±7.81 years, mean working age 12.9±7.24 years). Signs and symptoms were collected during compulsory occupational medical surveillance. The binary logistic regression was used to estimate adjusted odds ratios for the factors of interest. Results Diagnosis of CTS was reported in 48 cases (7.6%, 11 of them or 1.7% after a surgical correction) for the incidence of 5.94/1000 person-years. In general, signs and symptoms of CTS were associated with the following demographic factors: previous trauma of upper limb (adjusted odds ratio (ORa) = 8.093, 95% confidence interval (CI): 2.347–27.904), history (> 5 years) of oral contraceptives therapy/hormone replacement therapy (ORa = 3.77, 95% CI: 1.701–8.354) and cervical spine signs/symptoms (ORa = 4.565, 95% CI: 2.281–9.136). Conclusions The prevalence of CTS was similar to the estimates for the general population of Italy. Among personal risk factors, hormone therapy, previous trauma of the upper limb and signs/symptoms of the cervical spine appeared to be associated with a higher risk of CTS syndrome. Eventually, the results reinforce interpretation of CTS in VDU workers as a work-related musculoskeletal disorder rather than a classical occupational disease. Objectives Work-related musculoskeletal disorders (MSD) are quite frequent in healthcare workers (HCWs), but data about MSD in home-based healthcare workers (HHWs) are lacking. In this study we describe the prevalence of MSD among Italian HHWs. Material and Methods A case-control study was carried out among 300 random-selected female HCWs, the sample comprising 100 HHWs, 100 HCWs with a low exposure to patient handling (MAPO – Movimentazione e Assistenza Pazienti Ospedalizzati – Movement and Assistance of Hospital Patients index 0–5) and 100 HCWs with high exposure to patient handling (MAPO index ≥ 5.01). As a negative control group, 200 visual display unit workers were also randomly selected. Musculoskeletal disorder cases were collected using a standardized case definition. A multivariate logistic regression analysis was performed comparing the MSD prevalence in the 4 groups. Results The overall prevalence of MSD was 17% in the reference group and 28.3% for HCWs. HHWs and HCWs with MAPO index ≥ 5.01 had similar prevalence of neck pain (9% and 11%, respectively), whereas lumbosacral pain prevalence was higher in the HHWs group (31%), with similar results in residential HCWs groups (21% in MAPO index 0–5 group and 25% in MAPO index ≥ 5.01 group). HCWs of group MAPO index ≥ 5.01 and HHWs showed the higher prevalence of upper limb complaints, with a prevalence of 20% and 10%, respectively. In multivariate regression analysis, prevalence of MSD complaints was quite similar in HHWs (adjusted odds ratio ($\text{OR}_\text{adj}$) = 2.335, 95% confidence interval (CI): 1.318–4.138) and in HCWs of the group MAPO ≥ 5.01 ($\text{OR}_\text{adj}$ = 2.729, 95% CI: 1.552–4.797). Conclusions The prevalence of MSD in the examined HCWs was relatively high, with HHWs appearing as a particularly high-risk group for lumbosacral back pain. In higher exposed HCWs, upper-limb symptoms were particularly frequent, probably reflecting the different tasks required to manage residential and homebased patients. In conclusion, this study reaffirms the high prevalence of MSD in HHWs, seemly similar to high-risk residential groups. Int J Occup Med Environ Health 2017;30(2):291–304

Background To investigate whether exposure to phenolic resins (PR) is associated with quantitative olfactory disorders (QOD), a cross-sectional study of self-reported olfactory impairment (SROI) was performed in occupationally exposed subjects. Material and Methods Sixty-six workers (45 males, 21 females) at the age (mean ± standard deviation) of 39.8±10.15 years old were divided into 3 exposure groups on the basis of biological exposure indices (BEI) for urinary phenols. It was asked whether the sense of smell has been normal or abnormal during the recent 2 months: the participants were eventually divided into self-reported normosmic, hyposmic, hyperosmic groups. Results Prevalence of the SROI was 45.5%, with 21 (31.8%) workers complaining about the hyposmia, 12 (18.2%) – anosmia and 9 (13.6%) – hyperosmia. In univariate analyses, female sex was associated with the SROI and the hyperosmia. Highly exposed workers showed the SROI more frequently (odds ratio (OR) = 4.714; 95% confidence interval (CI): 1.077–20.626) than those not exposed (reference) or low exposed (OR = 1.333; 95% CI: 0.416–4.274). In multivariate analyses, female sex was the main risk factor for the SROI (adjusted odds ratio (ORa) = 5.622; 95% CI: 1.525–20.722) and the hyperosmia (ORa = 25.143; 95% CI: 2.379–265.7) but a high exposure to phenol (ORa = 11.133; 95% CI: 1.060–116.9) was the main risk factor for the anosmia. Conclusions This study has found slight evidence among the cross-section of chemical industry workers that the exposure to phenol may be associated with the SROI. On the other hand, self-reporting of the QOD may be biased by personal factors and further research with objective measurement is therefore required. Med Pr 2016;67(2):173–186
Wstęp Celem badania przekrojowego było określenie zależności między zawodową ekspozycją na żywice fenolowe (phenolic resins – PR) a ilościowymi zaburzeniami węchu (quantitative olfactory disorders – QOD), tj. zgłaszanymi przez pacjentów upośledzeniami węchu (self-reported olfactory impairment – SROI). Materiał i metody Sześćdziesięciu sześciu pracowników (45 mężczyzn, 21 kobiet) w wieku (średnia ± odchylenie standardowe) 39,8±10,15 roku podzielono na 3 grupy na podstawie wartości oznaczanych w moczu wskaźników narażenia biologicznego (biological exposure indices – BEI) dla fenoli. Następnie w zależności od zaburzeń węchu – ich braku lub występowania w ciągu ostatnich 2 miesięcy – badanych podzielono na grupy: normosmiotyczną, hiposmiotyczną i hiperosmiotyczną. Wyniki Upośledzenie węchu zgłosiło łącznie 30 badanych (45,5%), w tym hiposmię – 21 osób (31,8%), anosmię – 12 osób (18,2%) i hiperosmię – 9 osób (13,6%). Analiza jednoczynnikowa wykazała, że istnieje zależność między płcią żeńską a występowaniem SROI i hiperosmii. Ponadto SROI było częstsze u pracowników z grupy wysokiego narażenia zawodowego na fenole (iloraz szans – odds ratio (OR) = 4,714; 95% przedział ufności – confidence interval (CI): 1,077–20,626) niż z grupy niskiego narażenia (OR = 1,333; 95% CI: 0,416–4,274) i pracowników nienarażonych (grupa referencyjna). Także analiza wieloczynnikowa wykazała, że płeć żeńska jest głównym czynnikiem ryzyka SROI (skorygowany iloraz szans – adjusted odds ratio (ORa) = 5,622; 95% CI: 1,525–20,722) i hiperosmii (ORa = 25,143; 95% CI: 2,379–265,7), natomiast wysokie narażenie na fenol – anosmii (ORa = 11,133; 95% CI: 1,060–116,9). Wnioski Badanie dostarczyło słabych dowodów na istnienie zależności między narażeniem pracowników przemysłu chemicznego na fenol a występowaniem SROI. Na wyniki badania ilościowych zaburzeń węchu (QOD) przeprowadzonego z użyciem ankiety mogły wpłynąć czynniki indywidualne, dlatego koniecznie są dalsze badania z zastosowaniem obiektywnych metod pomiaru. Background Epidemiological studies have shown that employees working with visual display units (VDU) are more likely to complain about musculoskeletal disorders (MSDs). The aim of this study has been to evaluate associations among MSDs and individuals and work-related factors. Material and Methods A total of 1032 VDU workers were assessed about their personal (i.e., age, working history, smoking history, physical activity) and work-related factors (i.e., predominant job tasks performed, work posture). Work environment was evaluated regarding fulfillment of the standard ISO 9241-5:1998. The investigation required a direct observation of participants (in order to accurately assess the prevalence of MSDs) and workstations. Adjusted odds ratios (ORa) were calculated by means of the logistic regression model. Results Prevalence of MSDs was relatively high (53%). In general, MSDs were significantly associated with female sex (OR = 2.832, 95% confidence interval (CI): 2.178–3.683), age ≥ 50 years old (OR = 2.231, 95% CI: 1.236–4.026), longer exposure to VDU, both as working history (10–14 years: OR = 1.934, 95% CI: 1.301–2.875; ≥ 15 years: OR = 2.223, 95% CI: 1.510–3.271) and working time (30–39 h/week: OR = 1.537, 95% CI: 1.087–2.273). Inappropriate workstation design was confirmed by the multivariate analysis as a risk factor for MSDs (ORa = 2.375, 95% CI: 1.124–5.018). Conclusions Musculoskeletal disorders were significantly associated with individual factors as well as characteristics of work environment. Wstęp Badania epidemiologiczne wykazały, że osoby pracujące przy monitorach ekranowych (visual display units – VDU) częściej skarżą się na zaburzenia mięśniowo-szkieletowe (musculoskeletal disorders – MSDs). Celem badania była ocena związku między MSDs a cechami indywidualnymi pracowników i czynnikami związanymi z pracą. Materiał i metody Badanie przeprowadzono wśród 1032 osób używających VDU w pracy. Zebrano dane dotyczące cech indywidualnych badanych (wiek, staż pracy, historia palenia, aktywność fizyczna) i czynników związanych z wykonywaną pracą (najczęstsze czynności, pozycja ciała). Oceniono też zgodność środowiska pracy z normą ISO 9241-5:1998. W celu dokładnej analizy występowania MSDs przeprowadzono bezpośrednią obserwację osób badanych i ich stanowisk pracy. Skorygowane ilorazy szans (adjusted odds ratios – ORa) obliczono z zastosowaniem modelu regresji logistycznej. Wyniki Częstość występowania MSDs była stosunkowo wysoka (53%). Zaobserwowano statystycznie istotną zależność między MSDs a płcią żeńską (OR = 2,832, 95% przedział ufności (confidence interval – CI): 2,178–3,683), wiekiem powyżej 50 lat (OR = 2,231, 95% CI: 1,236–4,026) i dłuższym narażeniem na VDU – zarówno w wyniku dłuższego stażu pracy (10–14 lat: OR = 1,934, 95% CI: 1,301–2,875; ≥ 15 lat: OR = 2,223, 95% CI: 1,510–3,271), jak i większego wymiaru czasu pracy (30–39 godz./tydzień: OR = 1,537, 95% CI: 1,087–2,273). Analiza wieloczynnikowa potwierdziła, że nieodpowiednio zorganizowane stanowisko pracy jest czynnikiem ryzyka MSDs (ORa = 2,375, 95% CI: 1,124–5,018). Wnioski Występowanie zaburzeń mięśniowo-szkieletowych jest istotnie związane z indywidualnymi cechami pracownika i właściwościami środowiska pracy. Odpowiednio zorganizowane stanowiska pracy mogą znacznie zmniejszyć częstość występowania MSDs u osób pracujących przy VDU. Med. Pr. 2016;67(6):707–719

Objectives This study aims to characterize personal attitudes and knowledge of a sample of Italian occupational physicians (OPhs) towards immunization practice in the case of healthcare workers (HCWs). Material and Methods A total of 90 OPhs (42.2% of males, 57.8% of females, mean age of 50.1±8.3 years old) compiled a structured questionnaire through a telephonic interview. They were asked about the official Italian recommendations for HCWs, their general knowledge of vaccine practice, their propensity towards vaccines (both in general and about specific immunizations), their risk perception about the vaccine-preventable infectious diseases. Eventually, a regression analysis was performed in order to identify factors predictive for vaccine propensity. Results Only 12 out of 90 subjects correctly identified all the 7 recommended immunizations. The hepatitis B virus (HBV) vaccine was correctly identified by 95.6% of the sample, and was also associated with the more positive attitude and the more accurate risk perception. Influenza vaccine had the lowest acceptance (75.9%). Eventually, pertussis, measles, parotitis and varicella vaccines were insufficiently recognized as recommended ones (all cases < 50% of the sample). General knowledge of vaccine and knowledge of official recommendations were significantly correlated with the attitude towards immunization practice (r = 0.259, p = 0.014 and r = 0.438, p < 0.0001). In the regression analysis general knowledge (unstandardized coefficient (B) = 0.300, 95% confidence interval (CI): 0.090–0.510, p = 0.006) and risk perception (B = 0.579, 95% CI: 0.155–1.003, p = 0.008) were significant predictors of the propensity to vaccinate. Conclusions Vaccinations gaps in HCWs may found their roots in OPhs incomplete knowledge of evidence-based recommendations. Specific training programs and formations courses should then be planned.
