Neuropathic pain and chronic pain constitute an interdisciplinary problem on the border of medicine, psychology, sociology and economics. While it seems to be underestimated, the scale of this problem will continue to increase due to the population aging and the growing incidence of lifestyle disorders. People employed in various occupational sectors may also wrestle with these disease units, which affect the quality of their life, mental health and work productivity. A narrative review provided an overview of neuropathic pain and chronic pain, and their relationship to such factors as job type, work absenteeism and productivity decline, as well mental well-being. A systematic literature search was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines to identify appropriate literature by searching the electronic databases: PubMed/MEDLINE, Pain Journal and the Cochrane Database of Systematic Reviews. Studies were published in Polish, English and French. Research shows an increasing number of musculoskeletal diseases in professionally active people, which lead to disability or provoke work absences. However, sickness presenteeism and/or absenteeism caused by pain not only leads to economic burdens, but also to burnout, fatigue and depression syndromes in employees. These disorders may require specialized effective interventions to support the return to work or maintaining employment despite experiencing pain. Every patient with chronic or neuropathic pain should be correctly assessed to determine the best method of treatment and its effectiveness.

Śródoperacyjna identyfikacja nadczynnej tkanki przytarczyc stanowi jedno z największych wyzwań dla chirurgów wykonujących paratyreoidektomie. Przełomem w chirurgii przytarczyc może stać się nowa metoda śródoperacyjnego radiomapowania nuklearnego. Jest ona oparta na pomiarze emisji promieniowania gamma w czasie rzeczywistym przez tkanki w polu operacyjnym po uprzednim podaniu 99mTc MIBI (99mTc-methoxyisobutylisonitrile - 99mTc-metoksyizobutylizonitryl), wychwytywanego i akumulowanego wybiórczo przez tkankę przytarczyc. Celem artykułu była analiza wpływu zastosowania śródoperacyjnego radiomapowania nuklearnego na czas trwania zabiegu paratyreoidektomii. Badania przeprowadzono metodą analizy retrospektywnej jedno- i wieloczynnikowej w grupie 20 chorych poddanych zabiegom paratyreoidektomii, uwzględniając potencjalne czynniki kliniczne wpływające na czas zabiegu jako wejściowe zmienne niezależne. Jednoczynnikowa analiza statystyczna wykazała, że zastosowanie śródoperacyjnego radiomapowania nuklearnego skraca czas trwania zabiegu o średnio 60,38±7,47 min (p = 0,0459). Natomiast nadczynność wtórna, konieczność wykonania częściowej sternotomii oraz reoperacja przedłużały średni czas trwania zabiegu o odpowiednio: 88,44±40,21 min; 98,06±57,91 min oraz 74,33±33,92 min (p = 0,0112; p = 0,0409 oraz p = 0,0234; odpowiednio). Analiza wieloczynnikowa z zastosowaniem modelu regresji wielorakiej zidentyfikowała radiomapowanie nuklearne, sternotomię oraz reoperację jako zmienne niezależne istotnie statystycznie wpływające na czas trwania paratyreoidektomii. Uzyskane wyniki badań wykazały, że śródoperacyjne radiomapowanie nuklearne jest skuteczną metodą wspomagającą postępowanie chirurgiczne w zabiegach paratyreoidektomii. Dzięki ukierunkowaniu działania operacyjnego in vivo oraz zapewnieniu potwierdzenia tożsamości nadczynnej tkanki gruczolaka ex vivo natychmiast po jego resekcji metoda ta pozwala znacznie skrócić czas trwania paratyreoidektomii.
Thorough localization and confirmation of hyperactive parathyroid tissue is one of the most difficult tasks for surgeons performing parathyroidectomies. Therefore, the need for an intraoperative aid during parathyroidectomy is currently widely accepted. An instant detection of radioactivity after 99mTc MIBI injection, which is accumulated in parathyroid glands, called nuclear radio-mapping, has become a novel technique applied in these operations. We aimed to assess the utility of this new technique as one of potential factors influencing the duration of parathyroidectomy. We retrospectively analyzed the influence of chosen clinical factors on the duration of the surgical procedure, using univariate and multivariate statistical methods in a group of 20 patients who underwent parathyroidectomy. The results of univariate analysis showed that using nuclear radio-mapping decreased the mean duration of parathyroidectomy by mean 60.38±7.47 min (p = 0.0459). However, in patients with secondary hyperparathyroidism, sternotomy and reoperation, the mean procedure duration increased by mean 88.44±40.21 min; 98.06±57.91 min and 74.33±33.92 min (p = 0.0112; p = 0.0409 and p = 0.0234) respectively. Multivariate analysis showed that nuclear radio-mapping, sternotomy and reoperation independently influenced the duration of parathyroidectomy. Concluding, nuclear radio-mapping not only guides the resection in vivo, but also allows to confirm the identity of removed parathyroid ex vivo and to decrease the duration of parathyroidectomy. This technique is a useful and efficient intraoperative adjunct in parathyroid surgery.

The paper presents the results of coal milling tests in the aspect of slurry fuel preparation. The tests were carried out with the use of vibrating mill with a working chamber of 47.5 dm3. The influence of milling time, amount and composition of grinding aids on the degree of fineness and particle size distribution have been investigated. It was found that the efficiency of the grinding process (in this type of milling device) depends primarily on the milling time and the share of grinding aids and – to a lesser extent – on their polydispersity. The study allows to conclude that the grinding time, composition and the share of used grinding aids enable to control the final grain size of coal, which has an impact on apparent viscosity of coal water slurry.

The article presents the results of studies of torrefied willow (Salix viminalis L.) compaction. Densification tests were performed using a hydraulic press with a maximum pressure of 216 MPa. The effect of basic parameters of the briquetting process (pressure and temperature) on mechanical parameters of manufactured briquettes were determined. On the basis of the research, it was found that the increase in pressure and temperature of the densification process increases the density and strength of pressed briquettes. The positive effect of temperature is particularly noticeable at lower pressing pressures (36 MPa – 72 MPa). In the case of a temperature of 300 C, the increase in a pressure from 144 MPa to 216 MPa resulted in the decrease in the density and strength of the briquette. It was also found that the briquettes manufactured at this temperature are characterized by lower density and strength than the briquettes obtained at a temperature of 200 oC.

W artykule zaprezentowano rezultaty testów peletyzacji odpadowego pyłu antracytowego z wykorzystaniem trzech rodzajów lepiszczy skrobiowych, przeprowadzone w Instytucie Chemicznej Przeróbki Węgla. Surowcem stosowanym do badań peletyzowania była drobnoziarnista frakcja antracytu pochodząca z jednego z zakładów przemysłowych przetwarzających antracyt. Celem przeprowadzonych testów było określenie możliwości peletyzowania odpadowego pyłu antracytowego oraz określenie wpływu udziału stosowanego lepiszcza na parametry fizyko-mechaniczne wytworzonych peletów. Stwierdzono że aplikacja 2,5% lepiszcza skrobiowego umożliwia uzyskanie peletów o zadowalającej wytrzymałości mechanicznej.
The results of pelletization tests of waste anthracite dust with three types of starch binder, which were performed at the Institute for Chemical Processing of Coal have been presented. The raw material used in pelletization tests was fine-grained anthracite fraction derived from one of the industrial plants processing anthracite. The aim of the research was to determine the possibilities of waste anthracite dust use in terms of its pelletization and determination of the influence of the binder amount on the physico-chemical properties of the obtained pellets. It was stated out that the application of 2.5% starch binder allows obtaining pellets with adequate mechanical strength.

Introduction and objective. Continuous subcutaneous apomorphine (APO) treatment is one of the 3 therapeutic options for advanced Parkinson’s disease (PD), in addition to deep brain stimulation (DBS) and intrajejunal levodopa. Data from previously performed studies show that few PD patients can achieve APO infusion as monotherapy. The current pilot study presents the authors’ experience in achieving APO monotherapy. Materials and method. During the last 2 years, 9 patients with APO were treated in the Department of Neurology of the Medical University of Lublin; each patient was offered a 5-day duration APO treatment as monotherapy. The main indication for the APO therapy was advanced PD with motor fluctuations and the patient’s non-agreement for DBS therapy. Mean age of treated patients – 65.11 years, mean disease duration – 7.67 years, mean Hoehn-Yahr – 2.67, mean L-dopa equivalent before APO treatment – 1751.11 mg, mean daily dose of apomorphine as monotherapy – 106.11 ± 14.09 mg. Results. All treated patients managed to achieve APO monotherapy. A statistically significant reduction was found in the duration of the ‘off’ states in the observed PD patients on APO monotherapy (p<0.05). No significant improvement was observed in the III motor score of the UPDRS on APO treatment, compared to optimized oral therapy used before APO treatment. Conclusions. APO monotherapy can be achieved in advanced PD, and seems to be a good therapeutic option for this group of patients, especially in that it allows a significant reduction in the off-time which significantly simplifies the drug regime. Nevertheless, hospital admission with experienced neurologist supervision is recommended when establishing a PD patient’s APO monotherapy.

Multiple sclerosis is a chronic inflammatory disease which, due to the destruction of the fibres of the central nervous system in the process of demyelination, leads to numerous neurological symptoms and progressive disability. The disease is autoimmune, which means that myelin is destroyed by the patient’s own cells, caused by improper functionong and regulation of the immune system. Multiple sclerosis affects more and more people and is therefore a significant clinical problem which, to a large Entent, affects women, especially in childbearing age This presents a big challenge for carrying out pregnancy while continuing the therapy, ensuring the safety of the foetus and simultaneously achieving the best possible therapeutic effect. The decision whether the therapy should be continued or whether it should be eliminatedis usually made according to assessment of the possible gains and losse. Despite the lack of clear indications, there are many studies proving the relative safety of the use of individual but not all the drugs during pregnancy. Pregnancy, however, has a fairly good impact on the development of multiple sclerosis, and that safety considerations, especially those concerning the growing foetus, force a decision to change or completely suspend the therapy. In-depth research on the already available and emerging therapeutic pathways in multiple sclerosis bring hope for increasingly better results in the future in the treatment of pregnant patients with multiple sclerosis.

Introduction. Alzheimer disease (AD) is one of the best-known diseases. It is neurodegenerative disease characterized by gradual memory loss and dysfunction of behaviour. The pathogenesis of this disease is unclear. Icariin (ICA) is a flavonoid found in a Chinese medicinal herb. It is recognised for a wide range of biological and medical activities: anti-tumour and anti-inflammatory, and also has an impact on the nervous system: stimulates neuroproliferation and prevents neuron’s apoptosis. ICA may have a potential role in AD disease treatment. Objective. The purpose of this article is to review current knowledge about mechanisms and use of ICA in AD treatment. State of knowledge. AD does not present established pathomechanism, about which there are some hypotheses. Each hypothesis mentioned and checked in this review, with result that ICA may have a potential role. One well-known hypothesis is that about amylolytic which is associated with amyloid precursor protein (APP) gene mutation, which leads to amyloid beta (Aβ) protein accumulation and to occurenc of the disease. In this hypothesis, ICA may inhibit Aβ protein aggregation or alter APP expression. ICA may stifle neuronal apoptosis and promote neurogenesis and neuromodulation. According to other hypotheses, ICA could also impact on iron overload in the brain, harmful for neurons hyperhomocysteinaemia, disorder of the intracellular calcium management. Modification of theTau protein structure is another one theory of ICA action. Flavonoid from China may also have an influence on axonal transport. Conclusions. According to the literature there is no single mechanism of ICA action in slowing down AD progress. A wide range of therapeutic points is demonstrated by the broad effect of this substance. Further research is mandatory.

Objective. The aim of this study is to evaluate neurological scales, as well as biochemical and radiological parameters measured on day 10 after ischemic stroke (IS), according to their value as predictors of the long-term outcome. Materials and method. 45 patients were assessed according to the Barthel Index (BI) and National Institute of Health Stroke Scale (NIHSS) on day 10, and according to Modified Rankin Scale (mRS) 3 months after the onset of IS. On day 10 of IS, the serum level of C-reactive protein (CRP), albumin, D-dimers (DD), S100BB and Tau proteins was measured and the volume of ischemic focus assessed with the use of Computed Tomography (CT). The patients were divided into groups with good outcome (GO) and mRS 0–2, and with bad outcome (BO) and mRS 3–6. Results. NIHSS and BI scores (p<0.001), the volume of ischemic focus (p<0.01), CRP (p<0.01) and albumin level (p<0.05), but not DD, S100BB and Tau protein levels evaluated on day 10, correlated with mRS after 3 months since IS onset. Patients from the BO group were observed to have lower BI (p=0.001), higher NIHSS (p<0.01) and CRP levels (p<0.05), and bigger volume of ischemic focus (p<0.05) measured on day 10 of IS. In the GO group, there were more patients with atherosclerotic etiology (p=0.02 x2=7.856). Regression analysis showed that only the BI score assessed on day 10 of IS can predict the outcome after 3 months assessed by mRS (OR=1.102, 95%, CI:1.01–1.203; p=0.001). Conclusions. BI assessed on day 10 has a predictive value for the outcome evaluated by mRS 3 months after the onset of IS.
