Most existing quasi-orthogonal space time block coding (QO-STBC) schemes have been developed relying on the assumption that the channel is flat or remains static during the length of the code word symbol periods to achieve an optimal antenna diversity gain. However, in timeselective fading channels, this assumption does not hold and causes intertransmit-antenna-interferences (ITAI). Therefore, the simple pairwise maximum likelihood decoding scheme is not sufficient to recover original transmitted signals at the receiver side. To avoid the interferences, we have analyzed several signal detection schemes, namely zero forcing (ZF), two-step zero forcing (TS-ZF), minimum mean square error (MMSE), zero forcing – interference cancelation – decision feedback equalizer (ZF-IC-DFE) and minimum mean square error – interference cancelation – decision feedback equalizer (MMSE-IC-DFE). We have proposed two efficient iterative signal detection schemes, namely zero forcing – iterative interference cancelation – zero forcing – decision feedback equalization (ZF-IIC-ZF-DFE) and minimum mean square error – parallel interference cancelation – zero forcing – decision feedback equalization (MMSE-IIC-ZF-DFE). The simulation results show that these two proposed detection schemes significantly outperform all conventional methods for QOSTBC system over time selective channel.

The present study was carried out to develop a valid and reliable scale for assessing enthusiasm among teachers teaching at the school level. Methods: After reviewing the literature and discussing with experts, an item pool of 115 statements was created. The items were organized into seven dimensions of teacher enthusiasm based on factors generated after performing exploratory factor analysis. These seven dimensions were: Interest and Engagement (IE), Interaction with Students (IWS), Passion for Teaching (PT), Creativity and Innovation (CI), Professional Development (PD), Subject Knowledge Enrichment (SKE), and Professional Commitment (PC). The responses of teachers were taken on five points Likert scale. The standardization of the scale was completed on 558 teachers, who were teaching at Primary, Secondary and Senior Secondary level schools in the state of Haryana in India. Multi-stage random sampling was used to select the sample. Results: Item analysis was done by calculating t-value and r-value for each statement; twenty items were deleted and 95 items were retained. Factor analysis (principal component methods) generated seven factors behind the teacher enthusiasm scale which explain 44.02% of the total variance together. Cronbach's Alpha of the final teacher enthusiasm scale was .957. The final teacher enthusiasm scale contains 78 items. Discussion: The findings of the present study suggest that it would be a useful tool for future research and will help in assessing the enthusiasm of the teachers. The results have demonstrated that this teacher enthusiasm scale is a reliable and valid tool. Implications for practitioners: The teacher enthusiasm scale would be useful for different stakeholders (government, school administration and management) as they can evaluate the enthusiasm of teachers while selection process and due weightage can be given to the enthusiastic candidates. Enthusiastic teachers can work effectively for children with special needs along with normal students.

INTRODUCTION: Elderly COVID-19 patients admitted to the intensive care unit (ICU) are at high risk of an inflammatory syndrome, hypercatabolic reaction, malnutrition, and physical immobilization. This may result in loss of muscle mass and pulmonary infection leading to prolonged ventilatory support. Factors responsible for muscle mass loss in ICU are (1) microcirculatory disturbances, (2) presence of systemic inflammatory response syndrome (SIRS), (3) sepsis (4) drugs (corticoids, neuromuscular blockers) having inhibitory activity on the nervous system, neuromuscular junction and muscle itself. Mechanism of muscle atrophy in critically ill elderly patients include an imbalance between protein synthesis and degradation. Interventions to manage muscle atrophy for the patients admitted to ICU is also extrapolated to mechanically ventilated COVID-ARDS patients. PURPOSE: Early recognition of factors contributing to intensive care unit acquired weakness (ICUAW) in COVID-19 patients, inflammation, high catabolic phase, steroid use, and paralysis. The potential interventions to target these specific mechanisms and ameliorate muscle dysfunction in COVID-19 patients. CONCLUSIONS:Intensive care unit acquired weakness (ICUAW) in critically ill COVID-19 patients is due to severity of illness, co-morbidities, muscle unloading, or ICU treatments, a systemic reaction circulating within the body, or combinations therein. Furthermore, the availability of a culture model of ICUAW could facilitate in expediting the diagnosis of ICUAW and fast track the discovery of putative treatments. We recommend NIV or HFNC ventilation or early weaning from invasive mechanical ventilation in critically ill COVID-19 elderly patients.

Chronic kidney disease (CKD) is characterized by loss of kidney function and generally it is associated with several alterations in pulmonary functions, including restriction, obstruction, and impaired diffusion capacity. It became challenging to maintain oxygenation in a CKD patient associated with COVID 19 on non-invasive ventilation (NIV). In patients with end stage renal disease (ESRD), several factors likes, fluid overload, anaemia, immune suppression, extraosseous calcification, malnutrition, electrolyte imbalance, and acid-base disorder affecting the lungs indirectly. Here we are reporting NIV failure in two COVID 19 patients during dialysis. Both patients were stable on NIV before starting dialysis and underwent oxygen desaturation and shifted to invasive mechanical ventilation during hemodialysis. The possible mechanism of the oxygen desaturation is alveolar hypoventilation in the absence of hypercapnia, which may be due to the removal of CO2 from the body by some route (dialysate) other than the lungs.

The reflection and refraction phenomenon of propagation of waves in couple stress micropolar thermoviscous elastic solid media with independent viscoelastic and micropolar properties have been studied. The structure of the model has been taken such that the plane interface is divides the given media into two half spaces in perfect contact. Here, we find that there are five waves, one of them is propagating independently while others are set of two coupled waves travelling with different speeds. Energy ratios, reflection and refraction coefficients relative to numerous reflected and refracted waves have been investigated when set of two coupled longitudinal waves and set of two coupled transverse waves strike at the interface through the solid medium. The inequality of energy ratios, refraction coefficients and reflection coefficients are evaluated numerically and presented graphically under three theories of thermoelasticity, namely, Green-Lindsay theory (GL), Lord-Shulman theory (LS), Coupled theory (CT) versus angular frequency and angle of incidence.

The linear minimum mean square error (LMMSE) channel estimation technique is often employed in orthogonal frequency division multiplexing (OFDM) systems because of its optimal performance in the mean square error (MSE) performance. However, the LMMSE method requires cubic complexity of order O(N 3 p ), where Np is the number of pilot subcarriers. To reduce the computational complexity, a discrete Fourier transform (DFT) based LMMSE method is proposed in this paper for OFDM systems in the frequency selective channel. To validate the proposed method, the closed form mean square error (MSE) expression is also derived. Finally, a computer simulation is carried out to compare the performance of the proposed LMMSE method with the classical LS and LMMSE methods in terms of bit error rate (BER) and computational complexity. Results of the simulation show that the proposed LMMSE method achieves exactly the same performance as the conventional LMMSE method, with much lower computational complexity.

This paper investigates the differential binary modulation for decode-and-forward (DF) based relay-assisted free space optical (FSO) network under the effect of strong atmospheric turbulence together with misalignment error (ME). The atmospheric fading links experience K-distributed turbulence. First we derive novel closed form expression for average bit error rate (BER) and outage probability (OP) in terms of Meijer’s G function. Further, the OP of differential DF-FSO system with multiple relays is derived. We also analyze the asymptotic performance for the sake of getting the order of diversity and the coding gain. The power allotment term is utilized to examine the effect of different power allotment techniques on BER and OP. The simulation results have been used to validate the derived analytical results.

The onset of stationary convection in thermal instability of porous layer saturating a Jeffrey nanofluid is studied. The behaviour of nanofluid is described by a Jeffrey fluid model and the porous layer is assumed to follow Darcy’s law. Due to the presence of the Jeffrey parameter and nanoparticles, the momentum-balance equation of fluid is modified. The linear stability analysis and normal modes analysis method are utilised to derive the dispersion relation for the Rayleigh number in terms of various parameters for free-free boundaries. The effects of the Jeffrey parameter, Lewis number, modified diffusivity ratio, nanoparticles’ Rayleigh number and medium porosity on the physical system are discussed analytically and graphically.

Non-invasive techniques for the assessment of respiratory disorders have gained increased importance in recent years due to the complexity of conventional methods. In the assessment of respiratory disorders, machine learning may play a very essential role. Respiratory disorders lead to variation in the production of speech as both go hand in hand. Thus, speech analysis can be a useful means for the pre-diagnosis of respiratory disorders. This article aims to develop a machine learning approach to differentiate healthy speech from speech corresponding to different respiratory disorders (affected). Thus, in the present work, a set of 15 relevant and efficient features were extracted from acquired data, and classification was done using different classifiers for healthy and affected speech. To assess the performance of different classifiers, accuracy, specificity (Sp), sensitivity (Se), and area under the receiver operating characteristic curve (AUC) was used by applying both multi-fold cross-validation methods (5-fold and 10-fold) and the holdout method. Out of the studied classifiers, decision tree, support vector machine (SVM), and k-nearest neighbor (KNN) were found more appropriate in providing correct assessment clinically while considering 15 features as well as three significant features (Se > 89%, Sp > 89%, AUC> 82%, and accuracy > 99%). The conclusion was that the proposed classifiers may provide an aid in the simple assessment of respiratory disorders utilising speech parameters with high efficiency. In the future, the proposed approach can be evaluated for the detection of specific respiratory disorders such as asthma, COPD, etc.

Hypernatremia is a frequent and potentially life-threatening condition in hospitalized patients. It is a disorder of water metabolism and is usually defined as a plasma sodium concentration above 145 mEq/L. The treatment of severe cases of hypernatremia might be challenging in patient with feeding jejunostomy. The average person’s GI tract secretes and reabsorbs around 7 L of gastrointestinal fluid (including 3-5 L originating above the pylorus alone). These 7 L of gastrointestinal fluid will significantly dilute whatever we consume or infuse. However, this dilution of enteral feed may be bypassed in by feeding jejunostomy. There are very few cases reported on extreme hypernatremia. We present a case of severe hypernatremia in patient with feeding-jejunostomy, managed successfully with the recommended correction.

In this work, a real-time label-free microwave sensing mechanism for glucose concentration monitoring using a planar biosensor configured with an inset fed microstrip patch antenna has been demonstrated. A microstrip patch antenna with the resonating frequency of 1.45 GHz has been designed and is fabricated on the Flame Retardant (FR-4) substrate. Due to the intense electromagnetic field at the edges of the patch antenna, edge length has been used as the detecting area where the sample under test (SUT) interacts with the electromagnetic field. The Poly-Dimethyl-Siloxane (PDMS) with the trench in the centre has been employed as the sample holder. Here, the SUT is the glucose dissolved in DI (de-ionized) water with the concentration range of 0.2 to 0.6 g/mL. The dielectric constant dependency on the glucose concentration has been used as the distinguishing factor which results in a shift in the S-parameter. The experimentally measured RF parameters were observed closely which showed the shift in S11 magnitude from -40 to -15 dB and resonant frequency from 1.27 to 1.3 GHz w.r.t the SUT solution of 0.2 to 0.6 g/mL with linear regression coefficient of 0.881, and 0.983 respectively.

Introduction and aim. Reduction in child morbidity and mortality is result of upgradation of infrastructure and quality of public health care services. India alone accounts for 30% of the global neonatal deaths occur in India that occurs due to preventable cause. So, the aim of this study was to assess the pattern and status of neonatal mortality observed in SNCUs of Uttar Pradesh, India. Material and methods. Descriptive study was conducted based on secondary data obtained from sick new born care units (SNCU) online database from 89 Government-supported SNCUs of Uttar Pradesh, from April 2014 to March 2016. Data obtained included age, weight, sex, diagnosis, and outcome. Results. 22933 neonates admitted in SNCU were included in study with 14269 (62.2%) were males and 8664 (37.8%) females. Majority of the subject (20070; 87%) were in 0-5 days old age group. Most (72.5%) of admitted new-born improved and discharged. Low birth weight was significantly (χ2 = 1334.2, p<0.001) related with outcome. Birth asphyxia contributed to maximum (36.11%) numberof deaths, followed by respiratory distress syndrome (25.21%), sepsis (15.38%), prematurity and extremely low birth weight (5.8%). Conclusion. Improved antenatal care, improved access to health facility, timely referral of high-risk cases, capacity building, intensive interventional management can reduce neonatal mortality and its complications. Study also warrants, in-depth community-based qualitative study to identify gender-specific, equity issues.
