Opis:
Hearing impairment caused by noise, traditionally called – depending on the duration of exposure - acute or chronic acoustic trauma, includes, in addition to presbyacusis, the most common adult population of hearing impaired. In Poland - according to the report of the Central Statistical Office (GUS, 2011), the number of workers employed in NDN exceeded the noise level (85 dB) is about 200 thousand, the highest in the mining, metal and metal products production, textiles and wood production. According to the Regulation of the Council of Ministers of on June 30, 2009, on the list of occupational diseases (Journal of Laws No. 132, item 1115), it is defined as "bilateral permanent hearing loss of the cochlear or sensory-nerve type, expressed as an increase in hearing threshold of at least 45 dB in the ear better heard, calculated as an arithmetic mean for frequencies 1,2 and 3 kHz. Hearing impairments also occur in the military and police during field training and in combat where the source of acoustic injuries are firearms and pulse-inducing explosions (as in some industries) with high C peak levels (Lc peak) Time to rise to a maximum of <1 ms. The prevalence of loud music listening, particularly by personal stereo players, is also affecting children and adolescents with audiometric hearing loss, according to the World Health Organization (WHO) estimates of around 15-20%. The preventive action strategy is defined by the European Union legislation and the national implementing legislation that reduces or eliminates the risk and reduces (if not eliminated), taking into account available technical and organizational solutions to minimize the risk of hearing damage. If you can not reduce the noise levels with technical and organizational methods, you need individual hearing protectors. Ear protectors may be equipped with electronic systems with active noise reduction (which can improve low and medium frequency performance), adjustable attenuation (improves speech intelligibility and perception of warning signals), and wireless communication for verbal communication.

Opis:
Objectives: Vertigo is a very common symptom mainly caused by the lesion of vestibular system (peripheral or central) and often accompanied by some work-related diseases and occupational intoxications. The aim of this study was to assess the value of electronystagmography (ENG) and videonystagmography (VNG) for diagnosing vertigo of various origin. Materials and Methods: The study included four groups, 25 subjects each, of patients suffering from vestibular disorders of peripheral, central and mixed origin versus healthy controls. All were examined by means of ENG and VNG, using the bithermal caloric test with 30°C and 44°C air irrigations of the ears. The findings (frequency of induced nystagmus FRQ, its slow phase velocity SPV, canal paresis CP, directional preponderance DP, vestibular excitability VE) were analysed and compared. Results: In all patients with vertigo due to vestibular neuritis, barotrauma and kinetosis, significant CP, the important sign of peripheral site of vestibular lesion was identified both in ENG and VNG. None of the patients with central origin disorders showed CP in VNG; in the majority of cases DP was observed. However, in ENG we found CP in 5 patients with central origin disorders. There were no essential differences between ENG and VNG in measurements of FRQ and SPV except for higher values in VNG in controls and patients with mixed vertigo. Conclusions: The results suggest that the VNG should be recommended in preference as the valuable method to assess vertigo and to discriminate between the peripheral and the central vestibular lesions.

Opis:
The article consists of nine parts: introduction, seven sections and concluding remarks. The introduction indicates that the growth in the importance of knowledge, technological capacity and human capital brings about changes which pose a challenge for the component of any organisation which is its employees. The second part of the article characterizes the flexibility of working time, which increases satisfaction among employees. The following, third part discusses part-time work, paying particular attention to the benefits and the consequences of such an arrangement. The fourth part of the elaboration concerns fixed-term contracts, which are beneficial primarily to employers. The next, fifth part of this article discusses on-call employment, which due to its nature makes the employee dependent on the employer. Polish legislation does not include regulations regarding on-call employment. A new form of employment and work organisation is teleworking – the sixth section. It has many advantages of ergonomic nature because the employee enjoys considerable freedom in time and space organisation. A new form of work known as temporary employment is presented in the seventh section of the article. This form of employment is used in the case of seasonal, periodic, relatively ad hoc workers. The eighth section presents job rotation. The form of work in this system is a modern technique of company work organisation. This model combines two policies: training and employment. This form in no way hinders the operation of the company and on the contrary, its employees have a chance for continuous training, as well as improving their qualifications. In the concluding remarks, an ergonomic assessment of new forms of work organisation was made.

Opis:
Objectives: The aim of this study was to compare the parameters of vocal acoustic and vocal efficiency analyses in medical students and academic teachers with use of the IRIS and DiagnoScope Specialist software and to evaluate their usefulness in prevention and certification of occupational disease. Material and Methods: The study group comprised 40 women, including students and employees of the Military Medical Faculty, Medical University of Łódź. After informed consent had been obtained from the participant women, the primary medical history was taken, videolaryngoscopic and stroboscopic examinations were performed and diagnostic vocal acoustic analysis was carried out with the use of the IRIS and Diagno- Scope Specialist software. Results: Based on the results of the performed measurements, the statistical analysis evidenced the compatibility between two software programs, IRIS and DiagnoScope Specialist, with the only exception of the F4 formant. The mean values of vocal acoustic parameters in medical students and academic teachers, obtained by means of the IRIS software, can be used as standards for the female population not yet developed by the producer. When using the DiagnoScope Specialist software, some mean values were higher and some lower than the standards specified by the producer. Conclusions: The study evidenced the compatibility between two measurement software programs, IRIS and DiagnoScope Specialist, except for the F4 formant. It should be noted that the later has advantage over the former since the standard values of vocal acoustic parameters have been worked out by the producer. Moreover, they only slightly departed from the values obtained in our study and may be useful in diagnostics of occupational voice disorders.

Opis:
Introduction. This work aimed at assessing effects of surgical treatment and post-operative rehabilitation of chronic hypertrophic laryngitis by videolaryngostroboscopy and the GRBAS scale. Material and methods. The examinations were carried on 50 patients of both sex, including 16 women (32.0%) and 34 men (68.0%) at the age of 19-66, surgically treated due to oedema hypertrophic lesions in the larynx. The voice was assessed by the perceptive GRBAS scale, a subjective voice self-assessment questionnaire, and through pre- and post-therapy videolaryngostroboscopic tests, which were also conducted after phoniatric rehabilitation (30 days after the operation). Physical therapy (iontotherapy with hydrocortisone and 1% solution of CaCl2) and respiratory and phonatory exercises were introduced in the rehabilitation. Results. The GRBAS scale indicated that the patients with the Reincke’s oedema and vocal fold polyp showed greater voice improvement after the therapy and rehabilitation than those with chronic hypertrophic laryngitis. Contrary to the assessment carried out before the surgical procedure, the hoarseness level, rough voice, breathing voice, weak voice and tense voice mainly ranged between 0 and 1 scale. In stroboscopy a correct marginal shift and occlusion of the glottis were noticed in each case after eliminating lesions like Reincke’s oedema and vocal fold polyp (merely in 6.0% we found a disorder in the marginal shift). In the above patients their voice formation was unrestrained, 69.0% and 75.0% respectively, mid-location of the voice was normal in 92.0% and 88.0%, normal phonation time in 85.0% and 94.0%, and accurate voice setting in 77.0% and 75.0%. In stroboscopy the worst results of the voice assessment were obtained in the laryngeal papilloma patient. Conclusions. The videolaryngostroboscopy seems a perfect diagnostic method in larynx diseases, both in qualifying patients for a surgical treatment and monitoring effects of the treatment and post-operative rehabilitation.
