This study presents a comprehensive synoptic analysis of one of the most violent storms recorded in recent years in the northwestern part of Poland, which occurred on August 11, 2017. Its development took place ahead of a waving cold front in the tropical air mass, downstream of the upper-level trough. The thunderstorms formed over Lower Silesia in the afternoon and moved towards Gdańsk Pomerania to occur over the Baltic Sea after midnight on August 12, 2017, where they gradually disappeared. As the thunderstorms moved through this area, they ranged from single convective cells and unorganized multicell storms through supercell thunderstorms to mesoscale convective systems in the form of bow echo squall lines and the mesoscale convective vortex (MCV). The convective system, evolving over time, fulfilled the derecho criteria. Its development was related to the presence of both the upper and mid-level jet stream, which supported the formation of a strong rear inflow jet (RIJ) in the rear part of the convective system, being one of the main factors generating the formation of a bow echo squall line with strong wind gusts. The maximum wind gusts recorded on August 11, 2017, are among the highest in the history of Polish measurements and amounted to 42 m/s in Elbląg, 36 m/s in Chrząstów, 35 m/s in Gniezno, and around 30 m/s at several other stations.

Effect of a bicarbonate-buffered peritoneal dialysis solution on clinical and laboratory indices of dialysis adequacy

Background: Biocompatible fluids were introduced to improve dialysis and patient outcome in end-stage renal disease. However, being buffered with lactate, they may insufficiently correct metabolic acidosis, and lead to changes in peritoneum structure. Bicarbonate buffered fluids might mitigate these complications. The aim of the study was to evaluate the influence of a bicarbonate dialysis fluid on clinical and laboratory indices of dialysis adequacy. Material and methods: 20 prevalent peritoneal dialysis (PD) patients created two groups. One group continued on lactate fluid, the other changed to bicarbonate solution. Clinical and laboratory indices of dialysis adequacy were evaluated at baseline, and at six weeks intervals for 24 weeks. Results: In bicarbonate group, pH was 7.36±0.03, HCO3 22.1±1.8 mmol/l at baseline and 7.36±0.04 and 21.2± 2.3 mmol/l at 24 weeks, while in controls pH equaled 7.35±0.12, HCO3 22.2±1.4 mmol/l, at baseline, and 7.40±0.03, and 22.3±1.8 mmol/l, at 24 weeks, with no significant differences. Dialysis adequacy (urea Kt/V), urine output and dialysis ultrafiltration did not differ between the groups, either at baseline or at study termination. Conclusions: Bicarbonate PD solution appears to be similar to standard fluid in the impact on hydration status and on acid/base balance. Longitudinal studies are needed to assess long-term advantages of these biocompatible solutions.

Introduction: Vitamin D deficiency is a worldwide public health problem. The objective of this survey was to assess the undergraduate students’ vitamin D status and knowledge about this vitamin. Materials and methods: An online multi-choice survey was designed and launched in Northern Poland (Gdańsk region). The first part of the survey assessed diet, supplementation, UV radiation exposure (UVE) and general health of respondents. The second part was a vitamin D knowledge test (vitamin D optimal level, deficiency-related diseases). 1766 student volunteers responded to the survey: 369 male and 1397 female. Data was divided according to the respondents’ sex and university affiliation. Appropriate parametric or non-parametric statistical tests were used with statistical significance set at p<0.05. Results: Regular consumption of vitamin D-rich food was high, except for fish (only 18; 22%). High number of participants did not declare any type of supplementation (43; 44%) and only occasionally were exposed to UV (77%; 80%). The most frequently recognized disease linked to vitamin D was osteoporosis. Medical University students obtained higher test scores (4,55), however this did not correlate with healthy vitamin D habits. Conclusion: Undergraduate medical and non-medical students have unsatisfactory vitamin D status and poor understanding of its function and impact on health, which implies the need for changes in the educational program.
