    Introduction. Traumatic brain injuries (TBI) are a real social problem, with an upward trend worldwide. TBI is the leading cause of death and disability, especially among young men. Each year in Europe, and also in Poland, 243 per 100,000 individuals suffer from TBI. Assessing prognosis after traumatic brain injury is very important in order to help clinicians to make a decision about the implementation of specific methods of treatment, and to make communication with the patient and the patient’s family easier. Objective. The objective of this review was to present prognostic factors, to assess outcomes within a short time after a moderate to severe traumatic brain injury, as well as to predict functional outcome. The most important independent variables were: age, Glasgow Coma Scale (GCS) motor score, pupil response, Marshall CT classification and traumatic subarachnoid haemorrhage. Other important prognostic factors included hypotension, hypoxia, glucose, coagulopathy, haemoglobin and category of CT characteristic, such as midline shift, mass lesion, basal cistern. Conclusions. Gender and intraventricular haemorrhage did not have predictive value. This subject needs much more research in the area of new prognostic factors which would be better associated with outcome after traumatic brain injury.

    The objective of the study was to describe the characteristics of agriculture and forestry related injury cases admitted to an Emergency Department (ED), and to asses factors related to injury severity and hospital admission. Retrospective analysis of ED case records in Teaching Hospital No 1 in Lublin, from January 2004 to December 2005 were utilized. Inclusion criteria: patient >14 years old, with agriculture and forestry related injuries. Univariate and bivariate descriptive analyses and multiple logistic regressions were performed. 3791 cases were included, 63.1% males, 53.3% cases younger than 30 years and 47.1% of the patients sustained injuries related to machines or falls. After adjusting for age, sex and the presence of multiple injuries, animal related injuries, followed by machine related injuries and falls, were at higher risk of a more severe injury (OR: 1.77, 1.61 and 1.50, respectively). This groups also showed a higher likelihood of hospital admission (OR: 2.03, 192 and 2.00, respectively). Patients attended to in the ED during night hours (OR: 2.06) were also at a higher risk of hospitalization. It was concluded that animal related injuries, agriculture machine operators, and falls, besides accounting for two thirds of analyzed cases, are the mechanism of injury with a greater risk of a more severe injury and higher likelihood of a hospitalization.

    Despite urbanization of contemporary society and limitation on the environmental areas, the threat of animal attacks on people is still an important social and medical problem. Although they usually do not have serious consequences, they are nevertheless disturbing and alarming enough to make people seek professional, medical help. The aim of this study was to estimate the epidemiology and the risk rate of animal related injuries in the urban and rural population, as well as to select the characteristics of such injuries, which are distinguishable from injuries caused by other factors. The work presents a retrospective analysis of the medical records of 1,872 patients treated at the Department of Trauma and Emergency Department between 2001-2004, who suffered from animal related injuries. Our analysis led to the following conclusions: Most animal related injuries, reported to doctors, are mild injuries. The threat of animal attack is similar in urban and in rural areas, and concerns mostly men between the ages of 22-40. Injuries caused by large animals, such as a horse or a cow, require hospitalization and should be considered as high-energy injuries. The vast majority were patients who after the treatment could return to their social and occupational activities; the mortality rate in hospitalized patients with animal related injuries was higher then in other injuries, but the difference was statistically irrelevant.

    Introduction. Despite significant recent advances in the treatment of chronic lymphocytic leukaemia (CLL), most cases of the disease are still incurable. Treatment with monoclonal antibodies, such as ofatumumab, is one of the new therapeutic options. Objective. Retrospective analysis of the efficacy of ofatumumab in patients with chronic lymphocytic leukaemia (CLL) treated in the Haematooncology and Bone Marrow Transplantation Department of the Medical University of Lublin, Poland, during 2011–2013. Materials and method. The analysis included 5 patients (3 women and 2 men), aged 47–65, with Rai stage II-IV CLL, after a few lines of treatment. Three patients received ofatumumab in monotherapy and 2 patients received ofatumumab in combination with cyclophosphamide (50 mg/day) and dexamethasone (40 mg/day). All patients included in the study were diagnosed with an active form of leukaemia with symptoms such as lymphocytosis or massive lymphadenopathy. Results. All patients responded to the treatment. Within the first 8 weeks of the treatment, levels of white blood cells returned to normal in patients with baseline lymphocytosis (3 patients). An increase in platelet levels was reported in 3 patients. Haemoglobin levels were higher or comparable to the baseline values in all studied patients after the completion of immunotherapy. In the patient with massive lymphadenopathy and hepato- and splenomegaly, the size of the lymph nodes, spleen and liver decreased and neutrophil levels increased. Time of progression was 5–12 months, and in one patient partial remission has been maintained. The treatment was well-tolerated in most cases. Asymptomatic neutropenia and an infection with Candida glabrata were observed. Conclusions. Ofatumumab may be a new and safe therapeutic option for patients with CLL after a few lines of treatment.

    Ziarniniak Wegenera jest chorobą autoimmunologiczną, charakteryzującą się martwiczym i ziarniniakowym zapaleniem małych naczyń. Schorzenie to dotyczy górnych i dolnych dróg oddechowych, nerek, gałki ocznej. W 50% przypadków choroba zajmuje skórę, prowadząc do powstawania trudno gojących się owrzodzeń. W poniższej pracy przedstawiamy przypadek 23-letniej pacjentki, u której rozpoznano ziarniniaka Wegenera z zajęciem skóry małżowiny usznej prawej, z naciekiem na śluzówce jamy ustnej i asymetrią twarzy. W leczeniu, obok pulsów sterydowych oraz leczenia immunosupresyjnego, stosowano tlenoterapię hiperbaryczną, uzyskując bardzo dobry efekt terapeutyczny oraz całkowite ustąpienie zmian skórnych.
    Wegener’s granulomatosis is an autoimmune disease characterized by necrotizing granulomatous inflammation of small vessels. This condition concerns the upper and lower airways, kidneys, and eyes. In 50% of cases the disease affects skin, leading to the formation of non-healing skin ulcers. In this article, the case of a 23-year-old patient is presented who was diagnosed with Wegener’s granulomatosis on skin around the right outer ear, infiltration of the mucosa of the mouth and facial asymmetry. In treatment, apart from steroid pulses and immunosuppressive therapy, hyperbaric oxygen therapy was used, which led to a very good therapeutic effect, and total regression of skin lesions.

    The main objective of this paper is an attempt to reply to the question: how is the concept of dynamic competence understood by representatives of the manufacturing companies operating in the Polish agricultural machinery sector? Within the theoretical and design measures, a method of reconstruction and interpretation of literature implying the discussion in a group of intentionally selected experts operating in the examined sector will be used; as a result of the creative discussion, the key designates of was considered appropriate to determine the hierarchy of their importance. The task of deliberately selected respondents will be to make a choice as to which of the suggested dimensions of the dynamic competence best reflects the sense of this concept.
    Zasadniczym celem pracy jest próba odpowiedzi na pytanie: jak pojęcie kompetencji dynamicznych rozumiane jest przez przedstawicieli przedsiębiorstw produkcyjnych działających w polskim sektorze maszyn rolniczych? W warstwie teoretycznej i projektowej wykorzystana została metoda rekonstrukcji i interpretacji literatury implikująca dyskusję w grupie celowo dobranych ekspertów działających w badanym sektorze; w wyniku twórczej dyskusji wygenerowane zostały kluczowe desygnaty kompetencji dynamicznych. Na płaszczyźnie empirycznej za zasadne uznano ustalenie hierarchii ich ważności. Zadaniem celowo dobranych respondentów było dokonanie wyboru, które z zasugerowanych wymiarów kompetencji dynamicznych najbardziej oddają sens tego pojęcia.

    The main aim of this paper is to develop a model of key technical dynamic capabilities providing implementation flexibility of manufacturing companies of the Polish agricultural sector. Achieving the main target required to formulate sub-targets, to which the following have been included: the query of subject literature remaining in a direct relation to the topic of the research, which, in the authors’ intention, will find its expression in the developed definition of technical dynamic capabilities. At the design level, it is essential to search for the answer to the question: what technical capabilities imply dynamic flexibility of the Polish manufacturers of the agricultural machinery sector?, while, at the empirical level, to prioritize individual technical dynamic capabilities and determine what deficiencies in this area are characteristic for the studied companies.
    Zasadniczym celem badań jest opracowanie modelu kluczowych technicznych zdolności dynamicznych zapewniających elastyczność implementacyjną przedsiębiorstw wytwórczych polskiego sektora maszyn rolniczych. Osiągnięcie celu głównego wymagało zrealizowania celów pośrednich, wśród których wyróżniono: kwerendę literatury przedmiotu pozostającą w bezpośredniej relacji z tematem badań; w zamierzeniu autorów znalazło to swój wyraz w proponowanej definicji technicznych zdolności dynamicznych. Na płaszczyźnie projektowej istotne jest poszukiwanie odpowiedzi na pytanie: jakie techniczne zdolności dynamiczne implikują elastyczność polskich wytwórców sektora maszyn rolniczych? Natomiast na płaszczyźnie – empirycznej ustalenie hierarchii ważności poszczególnych technicznych zdolności dynamicznych oraz ustalenie jakie niedomagania w tym zakresie cechują badane przedsiębiorstwa.

    The objective of efficient management in the public sector is first of all the rational use of limited resources that sector has. It is only possible when the performance of activities is entrusted to qualified managers specializing in the management of public sector organizations. It also depends on the implementation of modern management methods, for example procedures which enforce the efficient use of material (especially financial) resources: it is so-called management technology. Health protection and the management of public health care system units is one of the key elements of the state policy carried out as part of its constitutional duty regarding the common good. Within the system of managing these resources, territorial self-governments, particularly communal self-governments, are responsible for performing the activities which have the fundamental importance for citizens. The problem discussed in this article is first of all the clarity of planning in self-governmental independent health care centers and the availability of information on the management of public financial resources. It presents the results of research carried out in selected independent public health care centers in Warmia and Mazury region. Apart from drawing certain conclusions, the aim of the article was to propose changes in the financial management system of self-government health care units, especially planning which involves the performance budget execution system of the revenue and expenditure plan.

    Cow’s milk has the highest share of total milk production and consumption in the world, including Poland. Milk is the most important source of readily available minerals, particularly calcium, in the human diet. The chemical composition of milk varies depending on genetic, environmental and physiological factors. Changes in the composition and physicochemical properties of milk affect its biological value and processing suitability. The objective of this study was to determine the effect of age of high-yielding Polish Holstein-Friesian (PHF) cows and lactation day on the calcium (Ca) and magnesium (Mg) content of milk. The average milk yield increased significantly during three consecutive lactations. The age of cows had a significant effect on the average Ca and Mg content of milk throughout lactation. Milk from younger cows had higher concentrations of the analyzed macronutrients. The concentrations of Ca and Mg in milk varied considerably depending on the stage of lactation. A substantial decrease in the milk Ca content was noted in the first stage of lactation. The calcium concentration in milk stabilized in the second month post partum, and it remained stable until the end of lactation. A decrease in the Mg content was noted until day 30 of lactation in milk from primiparous cows and cows in their second lactation. The oldest cows produced milk with the lowest Mg concentration, which resulted from a steady decrease in the milk Mg content until the fourth month of lactation. The Mg content of milk continued to increase from the fourth month until the end of lactation, regardless of a cow’s age. The Ca and Mg content of milk from the analyzed high-yielding cows was below the normal ranges (by approximately 24% and 6%, respectively). This is the reason why dairy products are often fortified with minerals.
