    Artykuł przedstawia koncepcję profesjonalizmu jako odpowiedź na ważne pytanie poli-tyki zdrowotnej o metodę organizowania społecznych relacji, w tym stosunków władzy. Przedstawione zostały zmiany, jakie dokonały się w podejściu do profesjonalistów medycznych, poczynając od tradycyjnej koncepcji wychwalającej profesjonalizm, dalej jej krytykę, przez uznanie profesjonalizmu za narzędzie regulujące stosunki społeczne i organizacyjne, a także za ideologię, a następnie ideę opieki zarządzanej i konsumeryzm medyczny, kończąc na kryzysie profesjonalizmu i jego odrodzeniu, w postaci nowego profesjonalizmu. Pokazano wpływ na zmiany relacji lekarz-pacjent, gdzie w miejsce biernego pacjenta pojawił się aktywny klient, a pozycja lekarzy jako niezależnej profesji uległa osłabieniu.
    This article presents the concept of professionalization as the response to important questions of health policy on methods of organizing social relations, including the relations of political power. The author presented changes in the attitude of medical professionals, starting with the traditional conceptions, which extolled professionalism. Then the author presented their criticism, including considering professionalism to be the tool regulating social and organizational relations, ideology and medical consumerism, ending with the crisis of professionalism and its rebirth in the form of a new professionalism. Finally the author discussed the impact of changing relations between doctors and patients; a passive patient was replaced by an active client and the doctor's position as an independent professional decreased.

    In this work, new materials based on the mixture of hyaluronic acid and albumin from chicken eggs have been studied. Tests were carried out to determine the molecular weight of the tested hyaluronic acids. The properties of hyaluronic acid were investigated and significant differences were found in the mechanical properties of the tested compound, depending on its molecular weight. It was found that egg albumin can be combined with hyaluronic acid and thin films can be obtained. Spectrometric tests were performed both for pure compounds and for mixtures of hyaluronic acid with chicken egg albumin. IR spectroscopy showed that interactions between hyaluronic acid and egg albumin are mainly by hydrogen bonds, as the shifts in the main bands in IR spectra were observed. The addition of egg albumin to hyaluronic acid leads to the decrease of its mechanical properties. The deterioration of the mechanical properties of polymer films from HA-albumin mixtures may be due to interactions between compounds which were shown in the IR spectra. The thin films based on hyaluronic acid and egg albumin blend can be used as adhesive materials in biomedicine and cosmetics. Both biopolymers are biocompatible and biodegradable so we can expect a biocompatible and biodegradable material for potential application as adhesives.

    For the last three decades, an increasing interest in new materials based on blends of two or more polymers has been observed. Fish skin collagen and chitosan are constantly highly popular among scientists. This study aimed to obtain thin films from mixtures of low and medium molecular weight chitosan with fish collagen in three different ratios and examine their features for potential use in medicine and cosmetic industry. Polymer blends in ratios 25:75, 50:50, and 75:25 were made to obtain thin films. The infrared spectroscopy, mechanical properties study, contact angle measurements, topographic imaging, and swelling test were used to characterize the features of the films. A statistical appraisal of the results was conducted with the Q-Dixon’s test. The infrared spectroscopy analysis showed that in the IR spectra of the examined biomaterials, there are shifts in the bands positions proving intermolecular interactions between collagen and chitosan in the blends. The mechanical properties in the mixtures were different from those of a single biopolymer film. Hydrophilicity and polarity of the blends decrease with the increasing collagen content, which may suggest that the adhesion to the skin will be enhanced. The surface topography of the obtained films varies depending on the ratio of biopolymers in the mixtures. The swelling tests indicated that chitosan absorbs more water than collagen. The properties of the films made of collagen-chitosan mixtures vary depending on the molecular weight of chitosan and the content of each biopolymer in the blend.

    Effects of the EU Directive on Patient’s Rights in the sphere of patients etitlements, providers liabilities and services organization, coordination and financing The so called cross border care directive of European Union follows the long process of European Court of Justice judgments concerning regulation of patient’s right in respect to health services delivered outside the country of health insurance, specifically the planned procedures or treatment not available in the country of patient origin. At this moment the directive is still being transformed into domestic systems however, there are still important obstacles and problems caused by the implementation process. The paper concerns some specific aspects of the above mentioned implementation results, showing also the wider context of the previous attempts focused on the matter solving in EU, mostly describing the legal background and crucial political approaches. It briefly describes such questions like: pre-authorization requirements, subjective obstacles regarding implementation, national contact points problems, need for the legislation novelisation, base reimbursement problems, national systems protection, providers responsibility and some coordination of services aspects in relation to the described directive.

    Opis:
    Contemporarily the increasing role of marketing in the process of hospital management may be observed due to the lastly undertaken processes of systemic changes. The paper describes this phenomena on the basis of the Children’s University Hospital of Kraków, one of the leading polish centers in pediatrics. Authors analyze the case of a public clinical hospital asking if marketing activities should be used in such case. The issues focused on public relations and relations with the most important stakeholders are taken into account with a specific regard to the process of the hospital development, potential support for the hospital’s budget and for the future perspectives of such important provider. The case study explicitly illustrates the importance of different marketing techniques and actions for the process of effective management. The final conclusion underlined hereby, is the existing necessity of making publicly visible the vital role of such hospitals as well as building the positive and publicly accepted image of relationship and cooperation amongst stakeholders. This may be obtained with a great help and contribution of marketing knowledge and practices, specifically public relations and relationship marketing,

    Problemem wielu systemów ochrony zdrowia jest niedobór kadr medycznych, przede wszystkim lekarzy i pielęgniarek. Ich liczba, kompetencje i uprawnienia warunkują dostępność usług medycznych i ich jakość. Zapotrzebowanie na usługi medyczne zwiększa się wraz z postępującym procesem starzenia się populacji, wzrostem liczby zachorowań na choroby przewlekłe i ciągłymi reformami systemów zdrowotnych. Koszty pracownicze w ochronie zdrowia są kosztami najbardziej obciążającymi system, dlatego trzeba tworzyć efektywne zespoły pracowników sektorowych, korzystając z dostępnych zasobów. Wymienione czynniki skutkują racjonalizacją zatrudnienia lub nadawaniem nowych uprawnień medycznych i okołomedycznych nowym grupom profesjonalistów, czego wynikiem jest zjawisko krzyżowania się kompetencji (skill mix). Dobrze przygotowane i wdrożone krzyżujące się kompetencje pozwalają poprawić jakość opieki nad pacjentem, zwiększyć jego zadowolenie i uzyskać lepsze wyniki kliniczne. W procesie krzyżowania się kompetencji dochodzi do zamiany sprawowanych dotychczas ról. Gdy jedni profesjonaliści rozbudowują istniejące role, inni są zobowiązani do przyjęcia niektórych aspektów poprzedniej roli. Przy krzyżowaniu się kompetencji często pojawiają się też nowe role, nowe zawody, których zakres opracowuje się tak, aby pasowały one do obowiązującej praktyki. Najczęściej krzyżujące się kompetencje dotyczą personelu pielęgniarskiego i położniczego oraz lekarzy i ratowników medycznych. Aby przeciwdziałać niedoborowi lekarzy w Polsce, wprowadzono zmiany, których celem jest zwiększenie dostępu do świadczeń medycznych: m.in. pielęgniarki i położne uzyskały prawo do wypisywania recept i ordynacji medycznej, ratownicy medyczni – prawo do wykonywania medycznych czynności ratunkowych i świadczeń zdrowotnych, a fizjoterapeuci – prawo do realizacji samodzielnej wizyty fizjoterapeutycznej. Powstał także nowy zawód – koordynator medyczny.
    An important problem faced by many healthcare systems is the shortage of medical staff, and in particular doctors and nurses. Their number, competences and qualifications determine the level of availability and quality of medical services. Unfortunately, the demand for medical services is increasing, along with the progressive aging of the population, as well as the increase in the incidence of chronic diseases and frequent reforms of health systems. Employee costs related to healthcare are the most burdensome for the system; therefore, based on the available resources, it is necessary to create effective teams of sector employees. This results in rationalizing employment, or providing new medical and about medical competencies to new groups of professionals, which gives rise to the skill mix phenomenon. A well-prepared and implemented skill mix contributes to improving the quality of patient care, increased patient satisfaction and better clinical outcomes. In the process of mixing of competences, the roles that have been exercised so far are being changed. While some professionals are expanding their existing roles, other employees are required to accept some aspects of the previous roles. In Poland, in order to counteract such negative trends (the shortage of doctors), changes have been introduced to increase access to medical services (e.g., nurses and midwives being vested with the right to issue prescriptions and medical ordinances, paramedics – with the right to perform medical emergency services and provide healthcare services, and physiotherapists – with the right to conduct independent physiotherapeutic visits). A new profession of a medical coordinator has also been introduced.

    Cel: Systematycznie podejmowany wysiłek fizyczny stanowi jeden ze sposobów przeciwdziałania negatywnym skutkom cywilizacji i nasilającym się z upływem lat zmianom inwolucyjnym. Rekreacyjny, określany mianem zdrowego styl życia, w którym aktywność fizyczna stanowi intergralny element, jest coraz bardziej popularną i pożądaną wartością dla współczesnych społeczeństw. Cel pracy stanowiła ocena zadowolenia z życia i analiza motywów podejmowania aktywności fizycznej przez klientki klubu fitness w wieku 50-55 lat.Materiał i metody: Sondażem diagnostycznym objęto 60 kobiet w wieku 50-55 lat, w tym 30 aktywnych fizycznie, uczęszczających na zajęcia oferowane przez studio fitness FIT HOUSE w Rzeszowie (grupa I) i 30 niepodejmujących regularnej aktywności fizycznej (grupa II). Do analizy wyników wykorzystano testy: Chi-kwadrat Pearsona (χ2) i U Manna-Whitney’a.Wyniki: Kobiety aktywne w klubie fitness częściej deklarowały zadowolenie z własnego życia (p<0,001), wyglądu (p<0,001) i kontaktów towarzyskich (p=0,034). Stwierdzono, że 70% respondentek z grupy I podejmowało wysiłek fizyczny z uwagi na chęć poprawy sprawności fizycznej a 63% grupy w celu poprawy sylwetki.Wnioski: Uczestnictwo w zajęciach fitness warunkuje stopień zadowolenia z własnego życia, wyglądu zewnętrznego i kontaktów towarzyskich kobiet w wieku 50-55 lat. Głównym motywem podejmowania wysiłku fizycznego w klubie fitness jest chęć poprawy sprawności fizycznej i uzyskania zgrabnej sylwetki.
    Aim of the study: Regular physical activity is one of the ways to prevent the negative effects of civilization and age-related involutiory changes. Recreational, referred to as an healthy lifestyle, in which physical activity is an integral element, is increasingly popular and desirable value for the modern society. The aim of this study was evaluation of life satisfaction and analysis motives of taking physical activity by fitness center clients aged 50-55.Material and methods: The study involving the method of diagnostic survey included 60 women aged 20-30, including 30 physically active, taking part in the classes offered by FIT HOUSE fitness club in Rzeszow (I group) and 30 who did not regularly participate in any physical exercises (II group). The collected research results were analyzed with the use of a non-parametric test of independence Chi-square (χ2 ) and Mann-Whitney U test.Results: Women working out in fitness club have more often declared their life satisfaction (p<0,001), satisfaction with own appearance (p<0,001) and relationships (p=0,034). We found that 70% of respondents from the first group were taking part in physical exercises due to a desire of improve the physical ability and 63% of group, in order to improve their silhouette.Conclusions: Participation in fitness clasess determines the degree of life satisfaction, apperance and social contacts of women aged 50-55. The main motive of physical effort in the fitness club is desire to improve physical efficiency and slim silhouette.

    Nazwisko Adama Bochenka znane jest wszystkim polskim lekarzom i studentom medycyny od ponad 100 lat. Dzieło, które stworzył, pomaga budować fundamenty wiedzy kolejnych pokoleń medyków aż do dnia dzisiejszego. Anatomia człowieka Bochenka jest książką niezwykłą. Liczne ilustracje oraz charakterystyczny dla autora lekki styl pisania sprawiają, że wiedza z pozoru tak nieprzystępna i rozległa staje się przejrzysta i łatwiejsza do zrozumienia. Pozycja ta jest przykładem klasycznego, obszernego podręcznika anatomii, na jakiego napisanie niewielu tylko autorów na świecie potrafiło się zdobyć. Przez ostatnie 100 lat publikacja ta wprowadzała kolejne pokolenia lekarzy nie tylko w tajniki anatomii, ale i całej medycyny. Mimo upływu czasu, dzieło Bochenka wciąż jest uważane za podręcznik wiodący na wszystkich uczelniach medycznych w Polsce, czego niezbitym dowodem są ukazujące się regularnie nowe jego wydania. Z okazji setnej rocznicy śmierci wybitnego polskiego anatoma pragniemy przypomnieć pokrótce jego życiorys, a także przybliżyć historię powstawania największego z jego dzieł.
    The name of Adam Bochenek have been familiar to every physican and medical student in Poland for a hundred years. For all those years, the book he have written, has been introducing new generations of future doctors not only to anatomy but to the medicine in general. Bochenek’s The Human Anatomy is an extraordinary book. Typical for the author, descriptive way of writing as well as numerous drawings make all this vast and inaccessible knowledge more clear, lucid and sometimes even addictive. It is an example of a classical, extensive handbook of anatomy. Despite the passage of time, The Human Anatomy is still the leading textbook for all medical universities in Poland, and its new editions coming out every few years are the best way to prove it.

    Introduction: Third molars (TMs) are the most frequently impacted teeth due to the frequent lack of space in the dental arch resulting in their malposition or inability to erupt. Partially erupted TMs that cause recurrent inflammatory conditions must be removed. The aim of this study was to assess TM position on panoramic radiographs. Materials and methods: We evaluated 200 panoramic radiographs of patients 18-72 years of age. Teeth were assessed in terms of the presence of dental follicle, cervix/root ratio and root development stage. Maxillary TMs were assessed using the Archer and Pell and Gregory classifications, whereas the mandibular ones according to Pell and Gregory, Winter, IAN and Pederson classifications. Results: 622 TMs were assessed. In the maxilla, the most common type was A-positioned, vertically angulated TM with completely formed root/roots. In the mandible, the most common type was A1-positio- ned, mesioangular TM with completely formed root/roots and without enlarged follicle. According to Pederson’s index, 59.44% TMs were moderately difficult to extract. Most roots were in contact with inferior alveolar nerve. Conclusions: The use of the classifications mentioned above is helpful in assessment of the surgery difficulty level. In the long term this allows to increase the predictability of the procedure and minimize the intra- and post-operative complications.
