The aim of the study was to analyze indications and results of the first one thousand liver transplantations at Chair and Clinic of General, Transplantation and Liver Surgery, Medical University of Warsaw.Material and methods. Data from 1000 transplantations (944 patients) performed at Chair and Clinic of General, Transplantation and Liver Surgery between 1994 and 2011 were analyzed retrospectively. These included 943 first transplantations and 55 retransplantations and 2 re-retransplantations. Frequency of particular indications for first transplantation and retransplantations was established. Perioperative mortality was defined as death within 30 days after the transplantation. Kaplan-Meier survival analysis was used to estimate 5-year patient and graft survival.Results. The most common indications for first transplantation included: liver failure caused by hepatitis C infection (27.8%) and hepatitis B infection (18%) and alcoholic liver disease (17.7%). Early (< 6 months) and late (> 6 months) retransplantations were dominated by hepatic artery thrombosis (54.3%) and recurrence of the underlying disease (45%). Perioperative mortality rate was 8.9% for first transplantations and 34.5% for retransplantations. Five-year patient and graft survival rate was 74.3% and 71%, respectively, after first transplantations and 54.7% and 52.9%, respectively, after retransplantations.Conclusions. Development of liver transplantation program provided more than 1000 transplantations and excellent long-term results. Liver failure caused by hepatitis C and B infections remains the most common cause of liver transplantation and structure of other indications is consistent with European data.

Opis:
Nowadays, in civil aviation, issues related to improving efficiency, reducing the costs of air operations as well as the negative impact of air transport on the environment are of increasing importance. These ideas allow the formulation of the paradigm relating to the development of air transport - ‘more Efficiently, more Economically, more Eco-friendly - 3E’. The article presents in a cross-sectional and synthetic way research conducted by leading scientific centres around the world as well as prototype aviation constructions designed by companies from the aviation industry. Benefits and disadvantages of future propulsions, such as purely electric, hybrid and distributed propulsions, were presented. Conclusions were formulated regarding further possible directions of civil aviation development, taking into account the improvement of its efficiency as well as economic and ecological indicators.

This paper is focused on multiple soft fault diagnosis of linear time-invariant analog circuits and brings a method that achieves all objectives of the fault diagnosis: detection, location, and identification. The method is based on a diagnostic test arranged in the transient state, which requires one node accessible for excitation and two nodes accessible for measurement. The circuit is specified by two transmittances which express the Laplace transform of the output voltages in terms of the Laplace transform of the input voltage. Each of these relationships is used to create an overdetermined system of nonlinear algebraic equations with the circuit parameters as the unknown variables. An iterative method is developed to solve these equations. Some virtual solutions can be eliminated comparing the results obtained using both transmittances. Three examples are provided where laboratory or numerical experiments reveal effectiveness of the proposed method.

Opis:
The paper is focused on the forthcoming IEEE 802.11ax standard and its influence on Wi-Fi networks performance. The most important features dedicated to improve transmission effectiveness are presented. Furthermore, the simulation results of a new transmission modes are described. The comparison with the legacy IEEE 802.11n/ac standards shows that even partial implementation of a new standard should bring significant throughput improvements.

The reported prevalence of periampullary duodenal diverticula varies between 9 and 32.8%. The aim of the study was to evaluate the prevalence of periampullary diverticula in the studied population and establish whether their presence influence the risk of choledocholithiasis and the risk of Endoscopic Retrograde Cholangio Pancreatography (ERCP) related complications. Material and methods. The study group of 3788 patients who underwent ERCP between 1996 and 2016at the 2nd Department of General Surgery Jagiellonian University Medical College in Kraków were analyzed. The group comprised of 2464 women (mean age 61.7 years) and 1324 men (mean age 61.8 years). The patients were divided into two groups. Group A included patients in whom there were no periampullary diverticula detected. Group B included patients in whom the opening of the bile duct was in the vicinity of a duodenal diverticulum. Results. There were 3332 patients included in group A (2154 women and 1178 men) and 456 patients in group B (310 women and 146 men). The prevalence of periampullary duodenal diverticula in the analyzed group was 12.8%. The presence of stones or biliary sludge was diagnosed in 1542 patients (47.6%) in group A and 290 patients (68.1%) in group B. Recurrence of choledocholithiasis occurred in 4.5% of patients (70/1542) in group A and 10.3% of patients (30/290) in group B. Complications occurred in a total of 76 patients in group A (2.3%) and 22 patients in group B (4.8%). Conclusions. The presence of choledocholithiasis and the risk of ERCP related complications are significantly higher in the group with duodenal diverticula.

Opis:
Analysis of the possibility of using a rotary engine based electric generator to propell a powered sailplane. The paper presents analysis of utilising Wankel type enine as a power input for an electric generator in the motor glider propulsion system. This generator would be a part of the propulsion system of a hybrid motor glider using the AOS 71 motor glider airframe. In the research, the rotational characteristics of the LCR 407ti wankel engine were determined experimentally. Driving torque run, power and fuel consumption were determined as a function of engine speed. The obtained results are presented in diagrams. The conceptual diagram of the hybrid drive is presented. The electric generator was selected and its effectiveness, as well as the effectiveness of entire propulsion system was assessed from the motor glider's performance point of view. Basing on the research conducted, conclusions were drawn and there were indicated the objectives and directions of further research on hybrid propulsion with specific aerodynamic and mass limitations of the aircraft.

Cel: Scharakteryzowanie przebiegu klinicznego oraz czynników prognostycznych ostrego poprzecznego zapalenia rdzenia kręgowego (acute transverse myelitis – ATM) oraz guzów śródrdzeniowych (intramedullary spinal cord tumors – ISCTs) u dzieci. Materiał i metody: Badana grupa składała się z 11 dzieci z ATM oraz 22 pacjentów z ISCTs, hospitalizowanych w Oddziale Neurologii Dziecięcej oraz Oddziale Neurochirurgii Dziecięcej w ciągu ostatnich 10 lat. U wszystkich pacjentów przeprowadzono badanie neurologiczne oraz badanie MRI. U dzieci, u których podejrzewano ATM, dodatkowo wykonano analizy płynu mózgowo- rdzeniowego. Badanie kontrolne wykonano po średnio 50 miesiącach w przypadku dzieci z ATM i średnio po 55 miesiącach w ISCTs. Wyniki: Dzieci w obu grupach prezentowały podobne objawy, jednak zaburzenia czucia były częstsze w grupie pacjentów z ATM. Płeć, wiek oraz średnia liczba zajętych segmentów rdzenia kręgowego nie wpływały znamiennie na wyniki w badaniu kontrolnym w obu grupach. Większość dzieci z ATM, które nie prezentowały objawów w badaniu kontrolnym, cechowała dobra odpowiedź na wstępne leczenie steroidami. Wnioski: Ze względu na podobny obraz kliniczny, zarówno ATM jak i ISCTs powinny być rozważane podczas diagnostyki różnicowej zaburzeń z zakresu rdzenia kręgowego. Zaburzenia czucia wydają się być bardziej specyficzne dla ATM. Dobra odpowiedź na wstępne leczenie steroidami u dzieci z ATM powinna być brana pod uwagę jako korzystny czynnik rokowniczy.
Purpose: To characterize the clinical course, outcomes and prognostic factors of acute transverse myelitis (ATM) and intramedullary spinal cord tumors (ISCTs) in children. Methods: The studied group consisted of 11 children with ATM and 22 children with ISCTs, hospitalized in the Departments of Pediatric Neurology and Pediatric Neurosurgery in the last 10 years. The patients underwent a neurological examination and MRI. CSF analyses were performed in children with a suspicion of ATM. Follow up lasted about 50 months in the ATM and 55 months in ISCTs. Results: Patients in both groups presented similar symptoms. A sensory loss was most common in the group of ATM children. Sex, age and the mean number of injured segments presented no significant influence on the results in the follow up in both groups. Most of the ATM children with no residual symptoms presented a good response to the initial steroid therapy. Conclusions: Because of similar symptoms both ATM and ISCTs should be taken into account in differential diagnosis of spinal cord disturbances. Sensory loss seems to be more specific for the ATM. A good response to the initial steroid treatment in the group of ATM children could be considered as a positive prognostic factor.

Opis:
The paper concerns pull-off strength results of epoxy coatings without and with five diffe- rent fillers. Polymer coatings were applied to a steel substrate that was degreased and/or pretreated by the means of abrasive blasting using electrocorundum and cast steel shot with different grain sizes. The roughness profile and the basic roughness parameters were determined. The results showed a decisive effect of substrate preparation on the coating adhesion.

The first Affad was the one we saw when the archaeological sites there were first investigated at the beginning of the century. The second Affad, which is the region that we have been exploring in the past 15 years, bore many signs of modern Sudanese culture encroaching upon the desert. In 2009, an asphalt road cut through the desert and shortly thereafter, the Debba bridge and power lines were constructed, the latter coming from a hydroelectric power station on the Fourth Cataract. Affad 3.0 is what the location looks like today—extensive industrial-scale farms on terraces too far away for traditional agriculture. The investment has already caused irreversible destruction to the archaeological heritage. Cattle+ in the title of this article refers to new data on large ruminants. The discovery of auroch remains and the Neolithic cattle data are both extremely important proxies for the adaptation strategies of people inhabiting the Southern Dongola Reach in prehistory.

In this study, optical microscopic analyses were applied to evaluate the thermal maturity, characteristics of solid bitumen, and other organic matter finely dispersed in Oligocene shales of the Menilite Formation in the Iwonicz-Zdrój–Rudawka Rymanowska Fold (IRF) and Bóbrka–Rogi Fold (BRF) of the Central Carpathian Synclinorium of the Silesian Nappe, Outer Carpathians, Poland. The investigation was carried out at two-unit depths of the shallow and deeper D-1 sections (430 m – IRF and 4,300 m – BRF) and outcrop samples (BRF). The mean random huminite reflectance values indicate immature conditions with respect to hydrocarbon generation in samples from the D-1 shallow section (VRo ≈ 0.40%) and in the outcrop samples (VRo = 0.36%). The degree of thermal maturity of the organic matter from a depth of about 4,300 m – BRF based on random vitrinite (VRo ≈ 0.80%) and solid bitumen (BRo ≈ 0.65%) reflectance measurements is associated with the “oil window” for petroleum generation. The organic components dispersed in the examined Menilite Formation samples are typical for hydrocarbon-prone organic matter, suggesting the dominant kerogen type II. The potential precursor maceral for solid bitumen occurring in the examined samples from the deeper D-1 sections is largely the alginite maceral.
