Opis:
We have evaluated the activity of acid aspartic protease in 195 strains of Candida isolated from the oral cavity of three groups of patients. The first group comprised patients with cancer of the larynx qualified for surgery, the second- patients with neoplastic disease (Hodgkin's disease, lymphoma, acute granulocytic leukaemia, lymphatic leukaemia, lung cancer, multiple myeloma, stomach cancer, breast cancer) who were not treated, the third group- patients with neoplastic diseases treated by chemotherapy. The strains of fungi were differentiated using API 20C and Api 20C AUX tests according to the protocol adopted at the Department of Medical Parasitology and Biology, Medical University of Łódż. The activity of acid protease was studied by Staib method in Rózga modification. Almost all strains showed high and very high proteolytic activity. The rang of proteolysis zone of Candida strains from the three groups of patients varied from 2,5 to 12,5 mm. We have found the mean proteolytic zones of strains isolated from groups Iand III differed statistically significantly (p<0,001). Similarly, statisticall sihnificant difference was seen between these parameters for groups II and III (p<0,05), while there was no difference between strains from group I and II.

Relationship between activity of hydrolases in fungi isolated from vagina of pregnant women and selected symptoms. Pregnancy is considered a factor of vulvovaginal mycosis. Secretion of hydrolases is an important determinant of Candida virulence. Thus, the aim of the study was to found the relationsbip between activity of 19 hydrolases in fungi isolated from vagina of pregnant women and symptoms of mycosis. 251 pregnant women were examined. Samples were collected from vagina and cultured on Sabouraud media. Activity of hydrolytic enzymes was evaluated using API ZYM (bioMerieux) test. Fungi were found in 20.1% of vaginal samples. Symptoms were detected in 45.8% of wornen. Only 32.1% of women with discharge and 26.4% with pruritus had mycosis. Out of the 19 examined hydrolazes, 13 active enzymes were detected in fungal strains. We found for the first time the relationsbip between activity of fungal esterase lipase and the presence of vaginal discharge in pregnant women.

In mycoses congenital – nonspecific innate as well as acquired immunity (involving neutrophiles, monocytes, macrophages, dendritic cells and lymphocytes) both play important roles in host defence. Th1 lymphocytes release cytokines (IL-2, IL-12, IFNg) and stimulate cytotoxic cells and neutrophiles to destroy fungal cells. Th2 lymphocytes, on the other hand, suppress cellular immunity by releasing the cytokines IL-4, IL-6 and IL-10 which counter regulate the secretion of IL-2, IL-12, IFNg and depress the activity of macrophages. Cellular mechanisms play essential roles in host responses to fungal infections. Dysfunction of T lymphocytes and a reduction in their number are typically observed in patients with mycotic diseases. There occurs a reduction of both T lymphocyte populations and the T-helper to T-suppressor cell number ratio, and these are of critical importance in explaining the diminished IgA production and enhanced adhesion of fungal cells to the surface of host cells as well as in facilitating the intrusion of fungi throughout the skin and mucous membranes. The specific immunological reaction, associated with the synthesis of antibodies against fungal cell wall or cytoplasmic antigens, is of little significance in protective immunity, but nevertheless has a rather important role to play in diagnosis as well as in supporting phagocytosis by inhibition of fungal cell adherence. In patients with mycoses, typically low blood serum level of the immunoglobulin class G and A and low sIgA in saliva are observed. A detailed understanding the nature and function of the immune system in mycoses is necessary to enable improvements in pharmacotherapy with antifungal antibiotics and chemotherapeutics, as well as to treatments based on immunotherapy and vaccination.

It is known that fungi representing different genera and species can cause organ-limited or systemic infections after disrupting of the natural defense mechanisms in a human organism. The treatment of mycoses still encounters considerable difficulties. Therefore, in vitro assessment of the susceptibility of fungal strains to the antifungal agents now in use and to new drugs is needed more urgently than ever before. It should be emphasized that we treat the fungal susceptibility to antifungal drugs as a quantitative feature of the strain examined. The aim of the presently reported study was the evaluation of the antimycotic action of two azole compounds – miconazole and itraconazole (Janssen) against 205 Candida strains isolated from the various biological specimens of two groups of patients – hospitalized (group 1) and examined in outpatient clinic (group 2); differentiation of species and codes of these strains; analysis of dose-response curves and parameters of polygons of the azoles minimal inhibitory concentrations (MIC). The susceptibility to miconazole and itraconazole was estimated with the agar diffusion test on 3% Sabouraud’s agar – the method developed in our laboratory, using several different concentrations of the drug, which made the plotting of dose-response curves possible. The lowest concentration inhibiting the growth of fungal strain (MIC) was calculated using a transformed equation of rectilinear regression according to Kadłubowski. Species and fungal codes of isolated strains were evaluated according to the guidelines worked out in our department with the use of different media and biochemical tests (bioMérieux). Among 89 strains isolated from the hospitalized patients, six species of the genus Candida genus were found; one strain belonged to Trichosporon cutaneum species. The most frequently encountered species was Candida albicans (73%), which significantly dominated over C. tropicalis, C. parapsilosis, C. glabrata, C. lipolytica and C. famata. All strains from the second group of patients belonged to C. albicans species. In all C. albicans strains from both groups of patients, the most frequent assimilation code (2576174) was found. The miconazole MIC values for Candida strains isolated from the group 1 were characterized by a wide range of variation, from 0.0247mg/l to 6.826 mg/l, from group 2 – 0.0277 to 0.719 mg/l. The itraconazole MIC values were 0.011 to 2.813 mg/l, and 0.0103 to 0.718 mg/l, respectively. The analysis of mean values (x) of miconazole and itraconazole MICs and other parameters allowed us to find that the strains isolated from the patients of group 1 were significantly less susceptible to both drugs in comparison with the strains of the group 2 patients. Also, C. albicans strains from this group of patients had a significantly lower (x) MIC in comparison to the mean values for the most of Candida species isolated from the hospitalized patients (P<0.001). In conclusion, we have found that the most Candida strains from both groups of patients were susceptible to the examined antifungal agents. The strains isolated from the outpatient clinic patients were generally more susceptible especially to itraconazole in comparison with strains from hospitalized patients.
