ObjectivesIntoxication with novel psychoactive drugs poses a significant challenge for medicine due to diagnostic difficulties, complex clinical patterns resulting from the polyethiology of poisoning, and the risk of fatal complications.Material and MethodsThe authors of the present work have made a retrospective review of medical records of the patients hospitalized at the Toxicology Unit (TU), diagnosed with poisoning by novel recreational drugs.ResultsIn 2008–2013, 576 patients diagnosed with that type of poisoning were admitted to the TU. Of those, 192 (33.3%) patients were positive for ethanol in addition to the most popular toxins found in the material collected from the patients, such as tetrahydrocannabinol (50 [8.6%] patients) and amphetamine (35 [6.1%] patients). The most frequent clinical symptoms observed on admission were: anxiety (433 [75.1%] patients), agitation (275 [47.7%] patients), and complaints associated with the circulatory system (225 [39.1%] patients). In the subgroup of 104 (18.0%) psychiatrist-interviewed people, a relationship was found between the suicidal ideation reported during history-taking and the following factors: earlier psychiatric treatment (p = 0.000), financial problems (p = 0.015) and prolonged unemployment (p = 0.022).ConclusionsThe observed clinical pattern may be associated with sympathomimetic syndrome due to poisoning by novel psychoactive substances. Preliminary results of this research show that financial problems and a history of psychiatric treatment may enhance suicidal ideation in patients after acute poisoning by psychoactive substances.

W Polsce współczynnik zapadalności na gruźlicę jest wyższy niż średnia dla krajów Unii Europejskiej, Norwegii czy Islandii, natomiast gruźlica pochodzenia zawodowego jest w naszym kraju rzadko rozpoznawana. W pracy zaprezentowano pierwszy w Polsce przypadek rozpoznanej gruźlicy płuc pochodzenia zawodowego u pracownika domu pogrzebowego. Rozpoznanie etiologii zawodowej omawianej choroby stwarzało wiele problemów diagnostycznych i stało się możliwe po wnikliwej analizie historii choroby pacjenta i dostępnego piśmiennictwa. Pozwoliło to na wykazanie związku między zakażeniem a wykonywaną pracą. Uznano, że przenoszenie ciała i zajmowanie się zwłokami podczas przeprowadzania ich toalety pośmiertnej może powodować wydalenie zalegającego w płucach powietrza resztkowego i generować potencjalnie zakaźne aerozole. Mimo częstego występowania gruźlicy w populacji ogólnej nadal sporadycznie rozpoznawane są przypadki zawodowe tej choroby, szczególnie wśród pracowników domów pogrzebowych. Med. In Poland the incidence rate of tuberculosis is higher than the average for countries of the European Union, Norway or Iceland. On the other hand, occupational tuberculosis is still rarely recognized in our country. In this paper we present the first in Poland- to our best knowledge – case of pulmonary manifestation of this disease acquired at work in a funeral director. The recognition of the occupational etiology of the discussed illness gave rise to an array of diagnostic problems and became feasible after a thorough analysis of the patient's history and literature data, which allowed finding the relationship between infection and performed job. We speculated that the transfer of the dead body and handling of cadavers may expel residual air from the lungs of the deceased. Although tuberculosis occurs frequently in the general population, the recognition of work-related cases of this disease is still rare, especially among funeral directors. Med Pr 2014;65(3):429–435

Objectives: The epidemiology of acute poisonings, observed in Toxicology Unit in Łódź, is the main objective of the study. The authors present frequency of respective kinds of poisoning cases and associated mortality. They discuss also the structure of poisonings with regard to type and causative agents over a period between 2003 and 2007. Materials and methods: The data used for the analysis were obtained from National Poison Information Center in Łódź. They were divided into two groups. The first one comprised information about patients treated at the Toxicology Unit (TU), whereas the other one included cases treated in other hospitals and only consulted by TU staff. Results: Analysis of the data shows that overdosage of medications was the most frequent cause of hospitalization during 2006-2007. It constituted also the leading cause of deaths due to poisoning during 2003-2005. However, it was superseded during the subsequent period of 2006-2007 by intoxication with alcohols which was associated with the highest number of deaths in the latter period. Situation changed dynamically also with regard to the intention of poisoning. Suicidal poisonings constituted the largest group in 2003, whereas poisonings caused by abuse, including dependence-related abuse, were the most frequent in subsequent years. Patients between 15 and 30 years old formed the largest group among the total number of poisonings. Conclusions: The results of our study on epidemiology of acute poisoning in Łódź between 2003-2007 show that poisoning is a frequent cause of admission to hospital and constitutes a major health problem in Łódź district because of large number of hospital admissions, associated mortality and the high proportion of patients at young age.

Do regionalnego ośrodka zatruć (ROZ) przyjęto 45-letniego pacjenta z powodu omyłkowego zatrucia pochodną dimetyloaniliny w miejscu pracy. Substancja toksyczna trafiła do organizmu drogą doustną. Podczas hospitalizacji podstawowym sposobem leczenia było podawanie błękitu metylenowego jako klasycznej odtrutki stosowanej w takim przypadku. W trakcie intensywnej opieki i leczenia u pacjenta zaobserwowano masywną hemolizę wewnątrznaczyniową, wymagającą leczenia preparatami krwiopochodnymi, oraz cechy uszkodzenia wątroby w postaci cholestatycznego uszkodzenia wątroby i żółtaczki. W Klinice Toksykologii pacjent przebywał 3 tygodnie. Opisany przypadek wskazuje na możliwość wystąpienia ciężkich powikłań narządowych w przebiegu zatrucia nawet niewielką ilością substancji chemicznej. Med. Pr. 2017;68(6):795–801
A 45-year-old male patient was admitted to the Regional Poison Center because of poisoning with dimethyloaniline contained in a toxic resin-curing dimethyl aniline-based formulation ingested inadvertently. Intoxication happened at workplace. The patient was then transferred to the Toxicology Clinic, where he stayed for 3 weeks. During the hospitalization, the primary method of treatment involved administration of methylene blue, which is the antidote of choice in such cases. During the intensive care and treatment of the patient massive intravascular hemolysis was seen. In that case treatment with blood products was required. He also showed signs of liver dysfunction due to cholestatic liver damage and jaundice. The reported case shows that severe organ damage may result from poisoning with even a small amount of the toxicant. Med Pr 2017;68(6):795–801

ObjectivesThe aim of the research was to analyze the nature of changes and tendencies observed in the frequency, circumstances and causes of acute intentional poisonings within the agglomeration of Łódź, Poland. The study related to adults.Material and MethodsAs the research material, medical records of patients hospitalized due to an acute poisoning in the Department of Toxicology in Łódź were used. Information on the hospitalized patients was gathered in the database created in accordance with the assumed criteria and an approved system of the collection of variables describing the issues of intentional acute poisonings.ResultsIn the analyzed period, within the group of 18 918 adult patients hospitalized due to an acute poisoning, male patients prevailed, accounting for 57.17% of the whole group. Intentional poisonings constituted 40.19% of all poisonings, with women dominating within that group of patients – accounting for approximately 58.63%. The largest group was formed by young people, 18–35 years old, accounting for 43.22%. The average age of the intentionally poisoned patients showed an upward tendency; this tendency concerned women to a larger extent. Medications played the most powerful role in acute intentional poisonings. Within the whole population, they were responsible for 97.27% of intoxications. The analysis of the dynamics of changes in the percentage of patients referred for further mental treatment revealed that the number of patients continuing hospital treatment in psychiatric wards tended to grow.ConclusionsObservations indicating that women are more likely to attempt intentional poisoning, and that the number of patients with psychiatric disturbances tended to grow in the analyzed period, demonstrate an urgent need for preventive measures to be taken in the local communities.

This study aims to present a case of acute mercuric chloride poisoning at a potentially lethal dose treated with the antidote – 2,3-dimercapto- 1-propanesulfonic acid (DMPS) and continuous renal replacement therapy (CRRT) combined with CytoSorb. A 21-year-old woman was admitted to a hospital with abdominal pain, vomiting, and suspected gastrointestinal bleeding after taking 5000 mg of mercuric chloride for suicidal purposes. Due to the patient deteriorating general condition and multiple organ damage, on the third day she was transported to the Clinic of Anaesthesiology and Intensive Care (CAaIC), Łódź, Poland. Laboratory tests confirmed features of acute kidney injury and high mercury levels in the blood (1051 μg/l) and urine (22 960 μg/l) – DMPS therapy and CRRT combined with CytoSorb were instituted. Due to nervous system complaints (headache, dizziness), a lumbosacral puncture was performed – the mercury concentration in the cerebrospinal fluid (CSF) was 5.45 μg/l. During a colonoscopy, significant diagnostic abnormalities revealed features of colonic mucosal necrosis. The treatment resulted in a decrease in subjective complaints, decreased mercury levels in biological material, and improved parenchymal organ function. On the 15th day of therapy, the patient was transferred to the primary care center for further treatment. The case confirms the possibility of improvement of patient condition following ingestion of a potentially lethal dose (5 g) as a result of the initiation of appropriate therapy even on the third day. The presence of mercury in CSF confirms that inorganic mercury compounds (mercuric chloride) can pass through the blood-brain barrier after oral ingestion.

Introduction. The development of patient rights and increasing number of lawsuits based on medical malpractice make medical doctors constantly improve knowledge regarding the acceptability of changing the scope of operation. This is particularly important when patients have expressed their refusal to changing the scope and type of procedure (ESTP) or no informed consent (IC) has been obtained from the patient prior to the procedure. Method. The method of study comprised content analysis of existing legislation. The current jurisprudence and doctrine were confronted with existing regulations. An algorithm of correct formal procedures was elaborated. Results. The doctor has right to realize ESTP when the patient has not given the consent to it or the refusal expressed by him was not fully conscious. Healthcare providers are absolutely prohibited from realizing ESTP when patient objected to any changes being thoroughly informed by the physician prior to the operation. When patient refuses possible ESTP, the doctor has the right to withdraw from performing surgery but should inform the patient about other places, where a similar procedure can be provided.

Praca jest przeglądem literatury omawiającej skutki zdrowotne środowiskowej i zawodowej ekspozycji na glin (Al) i jego związki. Szczegółowo omówiono w niej źródła narażenia, drogi wchłaniania i metabolizm. Przedstawiono także rolę Al i jego związków w etiologii niektórych chorób związanych zarówno z narażeniem środowiskowym, jak i ekspozycją w środowisku pracy. Toksyczność Al najczęściej dotyczy układów nerwowego, kostnego, krwiotwórczego, moczowego i oddechowego. Zwiększony efekt działania aktywnych form tlenu może stymulować powstawanie złogów amyloidowych. W obrazie klinicznym wytapiaczy aluminium dominowały: ataksja, zaburzenia pamięci, upośledzenie myślenia abstrakcyjnego i stany depresyjne. Związki glinu powodują zaburzenia sprawności nie tylko psychicznej, ale również motorycznej.
This work features a review of literature discussing the health effects of environmental and occupational exposure to aluminum (Al) and its compounds. The sources of exposure, absorption routes and metabolism are discussed in detail. The role of Al and its compounds in the etiology of some diseases, related to both environmental exposure and exposure in the work environment, is also presented. Aluminum toxicity most often affects the central nervous system, the skeletal system, the hematopoietic system, the urinary tract and the respiratory system. An increased effect of active forms of oxygen can stimulate the formation of amyloid deposits. The clinical image of aluminum smelters was dominated by ataxia, memory impairment, impaired abstract thinking and depressive states. Aluminum compounds have a detrimental effect not only on the psychic but also motor skills.

Energy production and storage has become a pressing issue in recent decades and its solutions bring new problems. This paper reviews the literature on the human and environmental risks associated with the production, use, and disposal of increasingly common lithium-ion batteries. Popular electronic databases were used for this purpose focused on the period since 2000. Assessment of the toxicological and environmental impact of batteries should then have a holistic scope to precede and guide the introduction of appropriate safety measures. In this short review the authors will try to touch upon this complex subject and point out some important issues related to an unprecedented development of lithium ion batteries-powered world. Given the multi-billion dollar business with the risks associated with the development of new technologies requires careful consideration of whether the balance of profits and losses is beneficial to humans and the planet. Introduction and aim. Night nursing care ensures well-being, peace, and security allowing a patient to enter into the deep sleep phase. This study aims to analyze the relationship between an assessment of caring and therapeutic services and the opinion of in-patients on their experience and satisfaction with night nursing care. Material and methods. A questionnaire developed by the authors and the Newcastle Satisfaction with Nursing Scale were used in this study. This study was conducted in the eastern region of Poland in the Podkarpackie province. The study included 585 patients. Results. Tasks analyzed resulting from care and therapeutic functions, in the patient’s opinion, had a significant impact on the overall assessment of nursing care. Meeting the expectations regarding night rest (experience of nursing care) and satisfaction showed the strongest correlation. Tasks resulting from therapeutic functions showed an average correlation with the exception of planned medical care. Conclusion. The level of implemented health services by nurses at night significantly influences the overall assessment of the quality management system in health care.

Background Acute pesticide poisonings constitute an important toxicological problem in numerous countries. This report refers to patients treated for poisonings at the Toxicology Unit, Łódź, Poland, in the period 2004−2014. Material and Methods Data to be analyzed were obtained from medical records of hospitalized people. A group of 24 301 patients aged ≥ 15 were selected. In the group of 149 people poisoned with pesticides (0.61% of all poisoning cases), there were 40 women and 109 men. Logistic regression analysis was used to evaluate the impact of age on suicide attempts using pesticides. Results Suicide attempts significantly more frequently concerned younger people. In this type of behavior, coumarin derivatives were significantly more often used than organophosphorus compounds or pesticides classified as “others.” The patients with suicidal pesticide poisonings stayed in the ward significantly longer than those poisoned unintentionally. Conclusions In the analyzed population inhabiting an area with a high degree of urbanization, in terms of the place of residence and employment, pesticide poisonings were more common in men than in women. Suicide attempts using pesticides more frequently concerned younger people. Information found in this study may prove useful for education purposes, and also in informing clinicians. Opis:
Wstęp Celem pracy jest analiza regulacji prawnych, które odnoszą się do obowiązku zachowania tajemnicy lekarskiej i możliwości jej uchylenia poprzez przekazanie informacji na temat stanu zdrowia pacjenta jego pracodawcy. Realizacja celu głównego wymaga udzielenia odpowiedzi na następujące pytania: Czy pracodawca może uzyskać szczegółowe dane na temat stanu zdrowia pracownika, wykraczające poza zamieszczoną w zaświadczeniu lekarskim informację o istnieniu lub braku przeciwwskazań do wykonywania pracy? Czy lekarz ma prawo uchylić tajemnicę lekarską dotyczącą stanu zdrowia pracownika, jeśli choroba rozpoznana poza badaniami profilaktycznymi w sposób bezpośredni utrudnia/uniemożliwia wykonywanie pracy na dotychczasowym stanowisku? Materiał i metody Materiał do niniejszej pracy stanowią obowiązujące w Polsce regulacje prawne odnoszące się do obowiązku zachowania tajemnicy lekarskiej i określające zakres informacji na temat stanu zdrowia pacjenta, które mogą zostać przekazane jego pracodawcy. Wyniki Lekarz nie ma prawa przekazać pracodawcy informacji na temat przeciwwskazań do wykonywania określonej pracy, jeżeli do ich uzyskania doszło w trybie udzielania świadczeń zdrowotnych niekwalifikujących się do katalogu badań wstępnych, profilaktycznych lub kontrolnych. Podstawy przekazania pracodawcy przez lekarza informacji nie może stanowić wyjątek od obowiązku zachowania tajemnicy lekarskiej wskazany w art. 40 ust. 2 pkt 1–6 Ustawy z dnia 5 grudnia 1996 r. o zawodach lekarza i lekarza dentysty. Wymieniono tam enumeratywnie przypadki dopuszczalnego uchylenia tajemnicy zawodowej, co uniemożliwia stosowanie powyższego artykułu w przypadkach innych niż ściśle określone w jego treści. Wnioski Polskie prawo definiuje nieliczne przypadki, w których dopuszczalne jest przekazanie pracodawcy informacji dotyczących stanu zdrowia pracownika. W większości sytuacji uzyskanie informacji uzależnione jest od przeprowadzenia badań w ściśle określonym trybie. Gdy do stwierdzenia przeciwwskazań do wykonywania określonej pracy dochodzi podczas udzielania świadczeń zdrowotnych niemających związku z badaniami pracowników sensu stricto, informacja ta nie może zostać przekazana pracodawcy. Background The aim of this study is to analyze the legal regulations that pertain to the duty of doctor-patient privilege and the possibility of its repeal on the plane to provide information about the patient’s health status to his employer. Material and Methods The material constitutes the current Polish law regulations relating to the medical duty of doctor–patient privilege as well as provisions defining the scope of information on the patient’s health status that may be transferred to a patient’s employer. Results The doctor has no right to provide the employer with information on contraindications to perform a specific job, if the information was obtained in the mode of providing health services that are not eligible for the catalogue of preliminary/preventive/control examinations. The basis for the doctor providing information to the employer cannot be an exception to the obligation of medical confidentiality, which is governed by art. 40 § 2 pts 1–6 of the Act of Medical Profession, under which cases of admission of professional secrecy were listed, which makes it impossible to use the cited article in cases other than strictly specified in its content. Conclusions Polish law specifies the circumstances in which it is possible to provide an employer with the medical information on the patient’s health. However, there are regulations that provide for the information on contraindications to perform a certain type of work or job to be transmitted if the information has been obtained in a manner other than as provided in the ordinance on medical examinations of employees. ObjectivesThe variety of clinical presentation on the topic of carbon monoxide (CO) intoxication ranges from slight headache to coma or death. YouTube allows patients to search not only for entertainment but also medical advice. Therefore, the aim of this study was to evaluate the content and quality of YouTube videos concerning CO poisoning as a source of knowledge for non-medical audience.Material and MethodsOn the December 8, 2020 a YouTube search was conducted for the following phrases: “carbon monoxide poisoning,” “carbon monoxide symptoms,” “CO poisoning,” “carbon monoxide asphyxiation,” “carbon monoxide intoxication” using the “incognito mode” and without attachment to Google Account. The search results were set as: “default” in the YouTube browser. The first 50 results were taken into consideration. Two raters, a specialist in emergency medicine and a specialist in clinical toxicology rated videos with Quality Criteria for Consumer Health Information (DISCERN), Global Quality Score (GQS) and Journal of the American Medical Association (JAMA). “VidIQ Vision for YouTube” plug-in was used.ResultsNinety-five videos were included. The interclass coefficient for DISCERN, GQS and JAMA scores were: 0.8, 0.74 and 0.62 reaching good and moderate reliability. The mean DISCERN/GQS/JAMA was 28.1 (SD 7.9), 2.5 (SD 0.8) and 1.1 (SD 0.7) respectively. Higher DISCERN/GQS/JAMA had videos providing information on: exposure time, treatment options, hyperbaric chamber indications as well as physician speaker (p < 0.05). Video Power Index was higher when the video contained animations and presented patients own history of CO exposure but not influenced the DISCERN/GQS/JAMA scores. Videos providing misleading information had a higher like ratio.ConclusionsThe overall video quality was poor indicating inappropriate educational and informative value for patients who search information about carbon monoxide poisoning.

ObjectivesThe COVID-19 pandemic has forced emergency services to implement new standards of practice around the world. The dynamic and unpredictable nature of many clinical situations has placed emergency service personnel in direct danger of contracting the disease. This work uses a validated survey developed for the study to assess the predictors of stress that paramedics, nurses and doctors experience in the face of the COVID-19 pandemic.Material and MethodsThe study group included 955 medical staff, and the level of significance adopted for statistical analysis was p = 0.05. Non-parametric Mann-Whitney and Kruskal-Wallis tests were used to analyze the qualitative variables divided into groups. The selection of tests was carried out based on the distribution of variables, verified using the Shapiro-Wilk test. In order to determine the predictors that caused the feelings of stress, it was necessary to use the linear regression model.ResultsDuring the COVID-19 pandemic, stress among emergency medical personnel has increased considerably due to new factors that did not previously exist. The predictors of stress in the professional environment include the fear of contracting COVID-19, a decrease in the level of safety while conducting emergency medical procedures, and the marginalization of treatment for patients not suffering from COVID-19. Additional socio-demographic factors that increase stress among emergency medical personnel are being female and working in the nursing profession. Appropriate training, the supply of personal protective equipment and opinions on the preparedness of the system to deal with the outbreak of the pandemic did not affect the level of stress among health service personnel.ConclusionsThe factors that can be considered to act as predictors of occupational stress include the fear of contracting COVID-19, a decrease in the level of safety and security while conducting emergency medical procedures, and the marginalization of patients not suffering from COVID-19.

Objectives: To evaluate the risk factors for the development of occupational allergy to birds among Polish zoo garden keepers. Methods: A total of 200 bird zookeepers employed in the Polish zoo gardens in Łódź, Warsaw, Gdańsk, Chorzów and Płock and exposed occupationally to bird allergens were examined using a questionnaire, skin prick tests (SPTs) to common allergens and bird allergens, spirometry and cytograms of nasal swab. The level of total IgE in serum and serum-specific IgE to parrot, canary, pigeon feathers and serum were also evaluated. Results: Eight percent of bird zookeepers were sensitized to at least one of the bird allergens. The most frequent allergens yielding positive SPT results were D. farinae — 32 cases (16%), D. pteronyssinus — 30 cases (15%) and grass pollens (16.5%). In the studied group, allergen-specific IgE against bird allergens occurred with the following frequency: 87 (43.5%) against canary feathers and/or serum, 80 (40%) against parrot feathers and/or serum and 82 (41%) against pigeon feathers and/or serum. Occupational allergy was diagnosed in 39 (26.5%) cases, occupational rhinitis was present in 22 (15%) cases, occupational asthma in 20 (13.6%) subjects, occupational conjunctivitis in 18 (12.2%) cases, whereas occupational skin diseases in 11 (7.5%) cases. More eosinophils were found in nose swab cytograms among bird zookeepers with occupational airway allergy. Conclusions: The findings indicate that occupational allergy to birds is an important health problem among zoo bird keepers in Poland.

Opis:
Toxic myocardial injury can be misdiagnosed as a myocardial infarction, resulting in the patient undergoing standard treatment for cardiac rehabilitation. However, such inadequate therapeutic strategies can lead to cardiovascular complications including dilated cardiomyopathy. This study presents a case of a 65-year-old man after accidental ingestion of organic solvents (toluene and xylene), whose condition demonstrated all the criteria for diagnosis of myocardial infarction. The qualitative determinations of the above mentioned volatile organic compounds (VOCs) in whole blood were carried out using a headspace sampling by means of gas chromatography. Cardiac catheterization revealed no specific coronary lesions, only a muscular bridge causing a 30–50% stenosis in the middle of the circumflex branch of the left coronary artery.

Objectives The aim of this study has been to assess the characteristics of acute poisoning deaths in Poland over a period of time 2009–2013. Material and Methods The analysis was based on the data obtained from the patient records stored in toxicology departments in 6 cities – Łódź, Kraków, Sosnowiec, Gdańsk, Wrocław and Poznań. Toxicological analyses were routinely performed in blood and/or urine. Major toxic substances were classified to one of the following categories: pharmaceuticals, alcohol group poisonings (ethanol and other alcohols), gases, solvents, drugs of abuse, pesticides, metals, mushrooms, others. Cases were analyzed according to the following criteria: year, age and gender of analyzed patients, toxic substance category and type of poisoning. The recorded fatal poisonings were classified according to the International Classification of Diseases. Results The record of 261 deaths were retrospectively reviewed. There were 187 males (71.64%) and 74 females (28.36%) and the male to female ratio was 2.52. Alcohol group poisonings were more frequently responsible for deaths in men compared to all poisonings, 91.1% vs. 71.6%, respectively (p < 0.05), and pharmaceutical agents were more frequently responsible for deaths in women, 47.4% vs. 28.4%, (p < 0.05). Methanol was the most common agent in the alcohol group poisonings, accounting for 43.75% (N = 49), followed by ethylene glycol, 39.29% (N = 44), and ethanol, 16.96% (N = 19). Conclusions Epidemiological profile data from investigation of poisoning deaths in Poland may be very useful for the development of preventive programs. Among various publications regarding end-of-life care in Poland there is one piece of important research that shows a difficult situation for family caregivers in end-of-life care in Poland, who often lack recognition and support and face a lonely struggle, especially in home care settings (Janowicz, 2019a). The Polish Government published documents in support of family caregivers for the first time in 2019, recognizing respite care and allocating money to some of them. Poland has successfully implemented British standards of hospice and palliative care, making it the best in Central-Eastern Europe; the same could be done in supporting carers in family settings (Krakowiak, 2020a). We have already learnt and benefited from the experience of the British organisation Carers UK, who have been operating successfully for more than 50 years and working towards inclusion of formal and informal care (Klimek, 2020). But how can we move forward in helping those who face loneliness and feelings of helplessness as family carers? We can learn from those who have already developed tools and created strategies supporting family caregivers. Exploring the educational strategies of supporting organizations from the UK, will help to point towards possible solutions to this social and educational challenge in Poland, helping to reduce the loneliness of carers in the home care settings. Most families still feel isolated, while most of our local communities do not support those who care, often for many months and years. Social educators and social workers need to tackle the questions of loneliness and isolation that many family caregivers face. First steps have been made and first publications issued, but more robust strategies and practical solutions are needed. Newest facts and figures from Carers UK documents and Best Practice In Supporting Carers by Carer Positive Employer in Scotland (2020) will help to show existing strategies used for and by employers. Among many existing initiatives this one regarding combining care and work could be very important to recognise the needs of working carers, sharing their job with the duty of constant care at home. Action is urgently needed in Poland, where many people do a full-time job alongside caring at home. Recognition of family carers’ needs by their workplace, support from employers and flexibility in working hours is still a rare exception, and it should be changed. The Covid-19 pandemic has fully exposed the problems of carers of dependent people around the world and also in Poland, especially difficult for those who combine care with work.
