Graves’ orbitopathy is a rare autoimmune disorder characterized by the inflammation of orbital tissues. The course of disease can be described in terms of its activity and severity. Aim: The aim of our study was to determine the factors affecting the activity and severity of Graves’ orbitopathy, as well as to identify the predictive factors of poor response to glucocorticoid treatment followed by orbital irradiation. Methods: We performed a prospective observational study of 214 patients with Graves’ orbitopathy who were divided into two groups depending on the treatment they had previously obtained for their Graves’ disease. They received i.v. methylprednisolone pulses followed by orbital radiotherapy. They were examined and had their TSH, TRAb and FT4 levels evaluated prior to treatment and after 1, 6 and 12 months. Results: A pre-treatment TRAb concentration higher by one unit (U/L) implied a mean increase in the relative risk of active orbitopathy by 4.7% (p = 0.0362). A TRAb concentration higher by one U/L 1 month after treatment implied a mean increase in the relative risk of moderate-to-severe and severe GO by 8.7% (p = 0.0167) 6 months after treatment. As regards poor response to treatment, patients with moderate-to-severe and severe Graves’ orbitopathy on admission carried a higher risk of being non-responders. Each point scored on the NOSPECS scale prior to treatment increased the relative risk of the patient being a non-responder by 30%. Conclusions: Patients with higher TRAb levels have a higher risk of active Graves’ orbitopathy and moderate-to-severe and severe Graves’ orbitopathy. Monitoring TRAb serum concentration in those patients is of great importance. Patients with more severe Graves’ orbitopathy carry a higher risk of being poor responders to immunosuppressive treatment. Therefore, careful monitoring of patients with Graves’ orbitopathy and their early referral to specialized centers is essential.

It has been proven that outdoor and indoor air pollutants can cause adverse health effects and are able to promote the onset of atopic diseases. The current manuscript is focused on methodological issues. The aim of the study is to determine the effect of air pollution, urban environment, and urban heat islands (UHIs) on the occurrence of respiratory diseases including allergic rhinitis, asthma and wheezing in preschool children. The study group consists of 276 five-year-old children attending randomly selected kindergartens in the urban and rural areas of the Łódź Voivodeship. The questionnaire including data on the child’s state of health and socio-economic data will be filled by the caregivers. The children will undergo skin prick testing and the measurement of volatile organic compounds in exhaled breath. The key components of air pollution – particulate matter (PM) will be measured by personal meters. The PM sampling planned in the study will take 12 h for PM2.5 and for PM10 alike. Data on the level of outdoor air pollution will be collected based on the results obtained from air monitoring stations. The impact of air pollution, UHIs and the environment on the respiratory system and the presence of allergies in children, including chronic respiratory diseases, will be assessed. The project results will provide a scientific basis for the development of preventive programs in the population of children in the Łódź Voivodeship, adapted to the real health needs of society.

Niniejszy artykuł prezentuje problematykę rozwoju obszaru wsi posiadającej dziedzictwo kulturowe objęte ochroną w formie Rezerwatu Ludowej Architektury, na przykładzie słowackiej wsi Osturnia. Analiza aktualnego stanu badanego obszaru została przeprowadzona pod kątem relacji zapisów obowiązującego, lokalnego planu zagospodarowania wobec regulacji, wynikających z ochrony konserwatorskiej. Swoje wnioski, obejmujące możliwe scenariusze rozwoju, autorki oparły na analizach sytuacji podobnych wsi słowackich, wspierając je tezami naukowymi z zakresu problematyki przemian obszarów wiejskich.
The article presents the subject matter of the development of the territory of a village that possesses cultural heritage that has been placed under conservation in the form of a Folk Architecture Preserve, on the example of the Slovakian village of Osturňa. An analysis of the current state of the investigated territory was performed, with a focus on the relationships between the provisions of the applicable local spatial development plan and the regulations that are applicable on account of conservation. The authors based their conclusions, which include possible development alternatives, on analyses of the situation of similar Slovakian villages, backing them up with academic theses from the field of the transformation of rural areas.
