Opis:
An objective of this article is a review of contemporary knowledge on various environmental factors, that influence prevalence and course of allergic diseases, like asthma, allergic rhinitis, atopic dermatitis and also contact dermatitis. Surrounding climate may directly influence each patient, but also determines type of flora and fauna within particular geographical regions and thus affects sources of airborne and food allergens. Epidemiological studies suggest that there is a strong relationship between air pollution and development and exacerbation of asthma and other allergic diseases – main attention has been concentrated on gaseous materials such as ozone (O3) and nitrogen dioxide (NO2), as well as particulate matter (PM), generated by car traffic and industry. Diesel exhaust particulate (DEP) has the ability to bind proteins and may serve as a potential carrier of allergens, penetrating deep into respiratory tract. Among the most extensively studied environmental factors influencing allergy are airborne allergens: dust mites, pollens, fungi and animal dander. Foods may elicit both true IgE-mediated allergy and also various non-immunological reactions, associated with direct release of mediators or toxic activity. It has been estimated, that over 85 000 chemicals are recognized in the human environment and they may act as contact allergens or irritants, causing allergic or non-allergic contact dermatitis. Among them metals, fragrances, preservatives, botanicals and paraphenylenediamine are considered as the most significant. Infections have always been associated with etiopathogenesis of allergic diseases and they may contribute to exacerbation of their course.

Some aspects of the skin infestation by Demodex folliculorum. The importance of demodicids (Demodex folliculorum and D. brevis) infestation and their effect on skin lesions has been described based on literature data and our own clinical and parasitological investigations. Hair follicie mites have been detected in 45% of patients with rosacea, 27% of patients with perioral dermatitis, 28% of patients suffering from seborrhoeic dermatitis and in 3 out of 7 patients with chronic blepharitis. Clinical picture of demodecosis included erythemato-papulous and pustulous (rosacea-like) skin lesions together with erythemato-desquamative changes of the face.

Opis:
Introduction: Atopic dermatitis is one of the most common skin disorders in children. There is only scarce literature data on the prevalence of contact allergy in children with atopic dermatitis. Objectives: To assess the prevalence of contact allergy among children with atopic dermatitis, seborrhoeic dermatitis and in a population of healthy children. Material and methods: Patch tests were performed in 104 children aged 1-20 years treated for atopic dermatitis in the Department of Dermatology, University of Medical Sciences, Poznań, and also in 2 control groups: 15 subjects with seborrhoeic dermatitis (15-20 years) and 36 healthy children (1-20 years). Results: In children with atopic dermatitis, contact allergy was observed in 47/104 patients (45.2%). With regards to the 3 age subgroups, positive patch test results were detected in 30/43 in children aged 1-5 years (69.8%), 13/36 in children aged 6-14 years (36.1%) and in 4/25 adolescents 15-20 years of age (16%). The highest proportion of positive patch tests was detected in the youngest subgroup of healthy children. Comparative analysis revealed type IV hypersensitivity statistically significantly more frequent in children with atopic dermatitis than in the 2 control groups. Conclusions: The statistically significant positive results in the highest proportion of patch tests in the youngest age subpopulation of children with atopic dermatitis, and detection of contact allergy most commonly in the youngest subgroup of healthy children, may suggest nonspecifically positive results associated with the immaturity of the epidermal barrier during the first years of life. Concentrations of contact allergens included in current pediatric sets of patch tests seems to be too high and should be verified.

Opis:
Daily average Alnus pollen counts (1996-2005) from Worcester (UK) and Poznań (Poland) were examined with the aim of assessing the regional importance of Alnus pollen as an aeroallergen. The average number of Alnus pollen grains recorded annually at Poznań was more than 2.5 times that of Worcester. Furthermore, daily average Alnus pollen counts exceeded the thresholds of 100, 500 and 1,000 grains/m3 more times at Poznań than Worcester. Skin prick test results (1996-2005) and allergen-specifi c IgE (asIgE) measurements using the CAP (Pharmacia) system (2002-2005), were supplied by the Allergic Diseases Diagnostic Centre in Poznań. The annual number of positive skin prick tests to Alnus pollen allergens was signifi cantly related (p<0.05) to seasonal variations in the magnitude of the Alnus pollen catch recorded at Poznań (r=0.70). The symptoms of patients with positive skin prick tests to Alnus pollen allergens were: 51% pollinosis, 43% atopic dermatitis, 4% asthma, 1% chronic urticaria and 1% eczema. On a scale of 0-6, 20.5% of patients examined for serum asIgE in relation to Alnus pollen allergens had asIgE measurements in classes 5 and 6. Alnus pollen is generally considered to be mildly allergenic. However, the amount of Alnus pollen released into the atmosphere in places such as Poznań may increase its impact on the population and make it one of the more important aeroallergens present.
