Dementia, the plague of the aging populations, is highly prevalent in East Asia, with currently over 9,8 million dementia patients. Alzheimer’s disease (AD), the most common cause of dementia, awaits for more detailed research in terms of understanding patomechanism of the disease, but more importantly, in supporting the treatment of this incurable disease in modern medicine. Therefore, alternative therapies assisting traditional therapeutic options become more popular, with the comeback of traditional medications, such as Traditional Japnese Medicine (TJM) - Kampo. The aim of this study was to present the use and effectiveness of TJM in treatment of symptoms of AD. Among articles in PubMed Medline database in years 2008-2018, 8 articles were selected for further analysis. Most of the research (38%) consisted of observational studies using different neuropsychiatric scales and methods of assessment of dementia. Studies presented significant improvements of behavioral symptoms with additional therapy with Yokukansan, Orengedoku-to, Ninjin’yoeito and other Kampo formulas in AD patients. Reviewed research presents effectiveness of Kampo treatment in supporting of cognitive and behavioral symptoms reduction, as well as protection of beta-amyloid induced toxicity. However, more research involving bigger cohorts is needed in order to confirm results on the larger scale, in order to introduce the beneficial aspects of Kampo in AD populations worldwide.

As the “innervated universe” of our body, the brain, holds the secrets determining humanity, over centuries there was a huge necessity to understand its complexity and ongoing processes. Surgical interventions for neurological diseases entered their Enlightenment Era with Modern Neurosurgery principles, works of Harvey Cushing and Ernest Sachs. Despite over two centuries of extensive research in various neuroscientific aspects, neurological conditions still await for innovative treatment options. Modern medical world complements Western medicine with traditional therapeutic options known and passed from generations to generations, with one of the most broad and oldest traditions - Traditional Chinese Medicine. The aim of this study is to present the use and effectiveness of acupuncture in treatment and rehabilitation of neurosurgical conditions. Therefore substantial articles on use of acupuncture in neurosurgery have been analyzed. Among 184 articles in PubMed Medline database from years 2008-2018, 9 articles were selected for further analysis. Most of the studies focused on reduction of need of general anesthesia in neurosurgical cases and also on influence on anti-inflamatory and immunological reactions. Another group of research investigated acupuncture as supportive therapy for thalamic hemorrhage and cerebral vasospasm post-SAH. Studies complemented and compared treatment of specific symptoms in neurosurgical conditions, such as post operative nausea and vomiting, anxiety. Reviewed research presents improvements in usage of acupuncture for neurosurgical conditions. Hopefully with introduction of quality standards of acupuncture research, as well as increased interest of medical and patient environments in supportive therapies, the scientific based proofs of acupuncture effectiveness will provide assistance in treatment of neurosurgical conditions.

Transplantation, one of the most intensively developing fields of medicine, is giving patients hope for the chance of "second life" and health. The 21st century medical achievements heralded to achieve more difficult challenges, such as face transplant or lower extremity transplant. Among the futuristic dreams of the scientists, one of the projects - the Head to Body Transplant, stands out as one of the most controversial surgeries of the century. The aim of this study was to present the current state of art in medical aspect on cephalic anastomosis, as well as ethical implications. Among articles in PubMed Medline database related to human cephalic anastomosis, head transplant, head to body transplant keywords, 15 articles were selected for further analysis. Reviewed research presents significant development in conduction of the human cephalic anastomosis project. Most of the crucial parts of the surgery, such as deep profound hypothermia, spinal cord anastomosis - the detachment and fusion with the fusiogenes particles, have been practiced on animal models. The more newest revelations are revealed to the public, the more ethical and technical concerns are raised, primarily in terms of one-center study aprroach, infavourable benefit-cost ratio and futuristic apprehensions of infertility, transsexual or even unnatural life prolongation reasons for performing this type of surgery.

Patient safety still remains as one of the biggest challenges for healthcare professionals. Surgical adverse events comprise 8% of all iatrogenic complications, half of them being easily preventable with simple checkup methods. Especially in pediatric surgery, where not only meaningful differences in anatomy or treatment response lie, but more importantly limited communication with the patient occurs, additional precautions have to be undertaken. In 2008, as a result of Safe Surgery Saves Lives campaign, Surgical Safety Checklist was introduced. A standardized checklist of all crucial perioperative steps is meant to be performed in every operating theater, ideally under all circumstances. The aim of our study was to present the current views and effectiveness of implementation of surgical safety checklists in pediatric surgery settings. We analyzed substantial articles on implementation and challenges of surgical checklist in pediatric surgery from period 2008-2018. Within 310 articles of PubMED database, 10 substantial articles on pediatric surgery safety were identified and reviewed. 70% of articles discussed the implementation of the checklist and post-implementation improvements, 20% included healthcare and parents attitude towards safety checklists. One article presented the variation of the surgical safety checklist in pediatric surgical and ambulatory settings. Most of the articles noted the prevention of adverse events correlated with the usage of the checklist, as well as positive attitude of healthcare providers and patients family towards checklist implementation was noted. Main challenge was the fidelity of the completion, especially in emergency settings. The Surgical Safety Checklist unifies the process of avoiding human error in surgery at all costs. Reviewed research presents improvements in prevention of adverse events in pediatric surgery, as well as innovative solutions for issues related mainly to pediatric patients, such as inclusion of guardians or even patients in safety check process, or implementing procedural or bedside safety checklists.

Transplantologia to jedna z najbardziej intensywnie rozwijających się dziedzin medycyny, której pełny rozkwit rozpoczął się na przełomie XIX i XX wieku wraz z pierwszymi przeszczepami organów zakończonymi sukcesem, dającymi pacjentom nadzieję na „drugie życie” i zdrowie. Obecnie, po odkryciu chirurgicznych i immunologicznych aspektów transplantacji, najbardziej dotkliwym problemem jest brak organów. Naukowcy stawiają przed medycyną również coraz to nowe wyzwania wraz z wprowadzeniem procedury przeszczepu twarzy, przeszczepu u noworodka oraz planowanej procedury przeszczepu głowy. Celem poniższej pracy jest przedstawienie najnowszych wyzwań technicznych związanych z transplantacją: hodowli organów oraz 3D bioprintingu, przeszczepu głowy, ksenotransplantacji. Liczne badania stwierdzają niezwykły postęp w pracach nad syntetyzowaniem organów z iPSC – pierwsze próby regeneracji serca, wątroby, kości i rogówki zwiastują początek medycyny regeneracyjnej i nowej ery w transplantologii, w której możliwość użycia syntetycznych organów wyeliminuje potrzebę ludzkich dawców. Niezbędna wydaje się w planowaniu powyższych osiągnięć technika 3D bioprintingu, dzięki której będzie możliwe harmonijne odtworzenie anatomicznych i histologicznych struktur i zależności. Intensywne badania prowadzone są również w aspekcie przeszczepu głowy, w którym najważniejszymi dylematami operatywy są m.in. głęboka hipotermia, anastomoza rdzenia kręgowego oraz następcze uszkodzenie drogi rdzeniowo-wzgórzowej, niepokoją również liczne argumenty natury etycznej i moralnej, związane z przeprowadzeniem zabiegu. Mimo iż w transplantologii obserwuje się ogromny postęp techniczny w ostatnim dwudziestoleciu, to powyższe procedury czeka jeszcze wiele wyzwań natury technicznej oraz etycznej.
Organ transplantation is one of the most intensively developing fields of medicine, the blooming period of which began at the turn of the century with the first successful organ transplants, giving patients hope for a second chance and health. Currently, after the discovery of the surgical and immunological aspects of transplantation, the most pressing problem is the lack of organs. Scientists are also facing new challenges with the introduction of face transplantation procedures, transplantation in the newborn, and the planned procedure for head transplant. The purpose of the study is to present the latest technical challenges of transplantation: organ culture and 3D bioprinting, head transplantation, and xenotransplantation. Numerous studies show remarkable progress in the synthesis of iPSC organs – the first attempts of regeneration techniques of the heart, liver, bones and cornea mark the beginning of regenerative medicine and a new era in transplantation, where synthetic organs replace the need for human donors. The 3D bioprinting technique seems necessary in the planning of the above-mentioned achievements, which will enable a harmonious reconstruction of anatomical and histological structures and dependencies. Intensive research is also being carried out from the aspect of head graft, where the most important dilemmas of surgery are, e.g. deep hypothermia, anastomosis of the spinal cord, and subsequent damage to the spinothalamic tract. Additionally, numerous ethical and moral arguments related to the surgery are also the cause of disquiet. Although, in the last two decades transplantation has been experiencing tremendous technical progress, there are still many technical and ethical challenges related with these procedures.

X-linked adrenoleukodystrophy is a genetic disease correlated with mutation of ATP-binding cassette, which results in errors of peroxisomal beta oxidation and accumulation of impaired very long chain fatty acids. This leads to degeneration of adrenal glands, spinal cord and myelin sheaths. Despite nearly 100 years of ALD history, the treatment is limited to few therapeutic options, mainly Lorenzo’s Oil and Hematopoetic stem cell transplant. Available therapy can only slow the progression of the disease in early stages. The aim of our study was to present the newest therapeutic options in X-ALD. Substantial articles on new treatment of X-ALD from period 2007-2018 in the Asian, European and American regions have been analyzed. Among 219 articles in PubMed Medline database related to therapy and treatment of X-ALD, 13 articles were selected for analysis, reviewed and divided into two main groups: cause-related treatment (11 articles) and symptoms-related treatment (2 articles). Within cause-related treatment, the usage of known medications such as eg. pioglitazone, natural phenols - resveratrol, gene therapy with adenoassociated virus serotype 9 or combined therapies (Hematopoetic Stem Cell Gene Therapy) have been reviewed. Symptoms related treatment attempted to reduce spasticity and secondary dystonia in X-ALD patients. Reviewed research presents progress in development of treatment options for X-ALD, however still in primary in vitro and animal tested stages. Secondary neurological symptoms medication is awaiting for better solutions for ALD patients, in order to improve their Quality of Life and allow symptomless course for a longer time.

Year 2018 marks as the decade of introduction of WHO Surgical Safety Checklist into the world of modern surgery. Surgical safety checklist, a set of items vital within the perioperative patient’s safety checkup, easy to be performed in any of the operating theatres and under any circumstances, is proven to be an effective tool in fight against preventable adverse events in surgical care. The aim of our study was to present the results of implementation of the SSC to surgical wards in European Region -in terms of utilization and effectiveness. We analyzed substantial articles on implementation and challenges of surgical checklist in European centres from period 2008-2017. Within 308 articles of PubMED database, 8 substantial articles on utilization and effectiveness of SSC were identified and reviewed. 50% of research was performed in Central Europe. 75% of articles measured the effectiveness with one method, either documents/checklist evaluation or questionnaires among health care professionals. Presented research showed significant changes in patient safety, decrease in post-surgical complications and mortality rates. The utilization of the checklist did not reach the ideal 100% in all procedures, ranging between 55-95%, with maximum compliance of 80% of all items. Main challenges in completion and participation of professionals in the studies were lack of training and audit options, as well as limited financial resources towards ensuring patient safety in surgical wards. Despite 10 years of introduction to the surgical field, the data on utilization and effectiveness of SSC is still insufficient among countries of the European region. Potential ways of improvement, such as training, monitoring and financial resources could result in further reduction of complication rates and mortality.

Despite successful introduction of rules of asepsis, antisepsis, antibiotic treatment, medicine still fights with human errors in medicine. One of the crucial and sensitive to miscommunication are surgical situations, where plenty of multidisciplinary team members are involved in successful provision of the treatment. That is why starting from 2008 a useful tool supporting provision of safe surgery - Surgical Safety Checklist (SSC) - was introduced by World Health Organization. In single worldwide studies in beginning of implementation, SSC was proven to be effective in many both high- and low income settings. The aim of our study was to review the effectiveness of the SSC based on the review of research in low- and middle income countries in years 2008-2017. Within 302 articles of PubMed Medline database, 12 articles were chosen for further analysis, with majority research held in Sub-Saharan Africa (42%), and Upper Middle Income Countries group (42%). The review proved the effectiveness of implementation of SSC in all income groups, especially in terms of reduction of surgical site infection, postoperative sepsis and improvement of communication in healthcare. Promising results of the few studies should encourage more research in this field, which may be difficult to conduct due to the lack of necessary resources in the LMIC.
