Objective. The objective of the study was to analyze the level of knowledge on tumours and their prevention among rural inhabitants. Materials and method. The research involved 500 residents of villages in Podkarpackie Province in south-east Poland. The age of the researched ranges from 18–30 years; mean age – 26.96±0.84 (range [18, 30], median 25.95%CI [18, 9, 29, 01]). The researched group was represented in 47.59% by women in 52.41% by men. In order to obtain the research material, a standardized questionnaire was applied which included questions focused mainly on assessment of the level of knowledge on tumours (causes, symptoms, ways of treatment, prevention), as well as questions with both spontaneous and prompted answers. Results. 34.72% of respondents confirmed the occurrence of tumours in their family; the most frequently occurring was a lung tumour (9.4%). While assessing the degree of relationship it was proved that among parents’ of the respondents, neoplastic disease had occurred in 3.22% of cases, and in 22.36% of cases it affected grandparents. In self-assessment, a low level of knowledge was indicated by 35.35% of respondents: average by 30.45%, hard to determine by 32%, while 2.18% stated that their level of knowledge was high. The most frequently enumerated risk factors were: smoking (36.61%), improper diet (15.03%), and improper lifestyle (9.83%). UV radiation was a risk factor for 16.18%; however, a solarium only for 1.93% of respondents. For 37.94%, a medical examination was a diagnostic method in neoplastic disease. Conclusions. According to self-assessment, every third respondent stated having a low or average level of knowledge. The most frequently used source of knowledge was the Internet, and much more rarely a doctor or a nurse. Very few of the respondents could enumerate the tests applied in the early detection of neoplastic diseases.

Introduction. Inflammatory bowel disease (IBD) is group of global range inflammatory conditions. There has been a regular increase in the number of IBD cases. Patients exclude whole food groups from their diet fearing the emergence of disease symptoms or due to learning from unreliable sources. Doing so, they might deepen the already existing vitamin deficiencies which occur along with the shortage of many minerals. These deficiencies might intensify the disease process or cause a new one. The most common deficits pointed out by numerous researchers concern vitamin D, calcium, cobalamin, folic acid and iron. It is well worth introducing selenium, zinc and ascorbic acid into a diet because of their immunomodulating effect. Important aspect of the healing process is a personalized diet which is designed to compensate for, or prevent vitamin and mineral deficiencies. Aim. BackgroundCarbohydrate metabolism disorders, obesity and a severe course of gastroesophageal reflux correlate with more frequent development of esophageal complications. The aim of this study was to assess the influence of obesity and carbohydrate disorders on the characteristics of gastroesophageal reflux disease (GERD).Material and methodsThe study included 58 patients with excess weight. Anthropometric parameters (including the body mass index, BMI), data regarding GERD (severity of symptoms, gastroscopy and esophageal pH monitoring results) were included in the study. Correlations between obesity and GERD parameters were analyzed. Subjects were divided into a diabetic and a control group and the severity of GERD was compared.ResultsGERD was diagnosed in 40 patients and occurred more frequently in the obese group (73%) than in the overweight group (57%). Increased GERD severity was associated with increased BMI only for postprandial parameters. GERD was diagnosed in most of the group with carbohydrate disorders (78% vs 63% in the non-diabetic group). No differences in the severity of GERD were observed between groups depending on carbohydrate disorders.ConclusionsIn our study, GERD was common in obesity and in diabetic disorders. Increased severity of postprandial reflux was associated with an increased BMI. Diabetic disorders were not associated with more severe GERD.

Introduction. The epidemic of obesity has led to an increase in the occurrence of obstructive sleep apnea and gastro-esophageal reflux disease. The mechanism of development of gastro-esophageal reflux disease is multifactorial, and studies conducted in the last decade have shed new light on the causes of its development in patients with sleep apnea and obesity. Aim. The aim of the study was to discuss the mechanism of development of gastro-esophageal reflux disease is multifactorial, and studies conducted in the last decade have shed new light on the causes of its development in patients with sleep apnea and obesity and the dysfunction of the lower esophageal sphincter. Material and methods. An analysis of literature regarding gastro-esophageal reflux and obstructive sleep apnea. Results. In obstructive sleep apnea and obesity, high levels of cytokines and insulin resistance are observed, resulting in disorders in the levels of ghrelin – a hormone responsible for normal gastrointestinal motility. Conclusion. The effect of deviations in the ghrelin profile on the development of gastro-esophageal reflux disease remains a matter for further research

Introduction. The development of perianal fistulas are a risk factor in colonic and rectal disease. Perianal CD treatment requires a combination of surgical and therapeutic treatments aimed to prevent septic complications, reduce fistula discharge and ultimately heal fistulas. Aim. The purpose of the study was to evaluate the efficacy and safety of biosimilar IFX (CT-P13) and adalimumab in active fistulizing perianal Crohn’s disease (CD) in patients from the Subcarpathian Region (South-Eastern Poland). Material and methods. Thirty patients with CD with perianal fistulas naïve to anti-TNF therapy were enrolled (13 females/ 17males) ranging from 18 to 64 years of age. Twenty-one were treated with biosimilar infliximab (CT-P13), nine were treated with adalimumab (ADA). The treated patients had ileal CD (4), ileo-colonic CD (13) or colonic CD (13). All of them received standard immunosuppression with no additional steroid therapy. Response was evaluated at week 16 and 40 after the first CT-P13 dose, and 16 and 40 weeks after the first ADA dose. Remission was defined as the complete closure of all fistulas and partial response as a reduction (≥50%) in the number of draining fistulas. Results. Treatment outcomes with CT-P13 and ADA were both effective and similar in the percentage of patients with perianal fistula improvement, perianal fistula remission, no effect or observed adverse events. Conclusion. In patients with active fistulizing CD, both CT-P13 and ADA were effective and safe, however a slight superiority of CT-P13 was visible

Opis:
Introduction. Acute pulmonary hypertension leading to right ventricular failure and circulatory collapse is usually caused by a pulmonary embolism. However, in extremely rare cases, similar clinical manifestations can be related to another diseases, such as lymphangitis carcinomatosa. Aim. The purpose of this paper is to report on the case of a 29-year-old male patient presented with rapidly progressing dyspnoea. Description of the case. The diagnosis of pulmonary embolism was made on the basis of echocardiographic signs of pulmonary hypertension and right ventricular (RV) dilatation, and the recommended therapy was introduced. On the suspicion of bronchopneumonia, antibiotics and steroids were applied. However, the previously stated diagnosis of pulmonary embolism was not confirmed by the angio-CT scan, which showed small diffusive lung parenchyma intra-biliary nodules (ground glass opacity) with the peripheral appearance of a tree-in-bud sign. Consecutive CT of pelvis and abdomen along with endoscopy revealed a metastatic gastric cancer with the presence of lymphangitis carcinomatosa and miliary dissemination to the lungs. The presence of pulmonary metastases in the course of disseminated gastric cancer is known in literature as Bard’s syndrome. Conclusion. Extrapulmonary malignancies, particularly gastric cancer, should be taken into consideration in differential diagnosis in patients with an acute right ventricular failure and nonspecific lesions in the respiratory system.

Introduction. Inflammatory bowel disease (IBD) is a group of inflammatory conditions of the colon and small intestine. Aim. The aim of this work was to review the current literature regarding matrix metalloproteinases. The databases such as PubMed, ScienceDirect and Springer were utilized to search the literature for relevant articles. Materials and methods. An analysis of literature. We collected information, data, and examples of the function of metalloproteinases. Results. Herein we show that metalloproteinases play a role in such processes as the immune response, angiogenesis, the epithelial barrier function, fibrosis induced by the inflammatory process, and in the process of carcinogenesis. Conclusions. Further studies on the role of metalloproteinases in the process of carcinogenesis associated with inflammatory bowel diseases are required.

Introduction. Glycosaminoglycans (GAGs) play a widespread role in tissue modelling. GAG polymers may affect several receptor pathways in parallel. Aim. To present difference in concentration of GAG in healthy and cancer tissues. Material and methods. The literature search was performed nd reviewed using selected keywords. Results. We revieved the methods of detection various types of glycans measured by Magnetic Resonance Imaging. Conclusion. MRI methodology provides an efficient tool forstudy of cellular composition. The use T1 aof T2 measurements to study cancer tissue is a promising assay.

Introduction. Infliximab is a monoclonal antibody that acts against tumor necrosis factor TNF-α. The drug is used in the treatment of autoimmune diseases. Aim. This article reviewed the efficacy and safety of infliximab for the treatment in severe ulcerative colitis. This review included studies that evaluated the clinical use of infliximab. Material and methods. This meta-analysis was performed according to systematic literature search of three major bibliographic databases (Scopus, PubMed, and Cochran). Results. Infliximab has been approved by the US Food and Drug Administration (FDA) as a medicine to treat Leśniowski and Crohn’s disease, ulcerative colitis, psoriasis, psoriatic arthritis, ankylosing spondylitis, and rheumatoid arthritis. However, further trials are required to compare other parameters of efficacy such as the clinical response with infliximab. Conclusion. In patients suffering from Crohn’s disease or ulcerative colitis under infliximab maintenance therapy, sustained good trough levels are associated with: better response and remission rates, more mucosal healing and less loss of response.

Introduction. The use of Quantitative Magnetic Resonance Imaging to drug monitoring in vitro or in vivo can provides a powerful means to map the effects of drugs on tissue activity. Aim. The purpose of this study was to measure relaxation time of antibody phantom. For this purpose, infliximab sample was used. Material and methods. The selected methods to detect relaxation time of antibody was Magnetic Resonance Imaging technique. We detected spin-lattice (T1 ) relaxation time and discuss differences where compare to water. Results. The measurements ofspin-lattice (T1 ) relaxation time showed significant differences. The results obtained in phantom indicate that we can use this result for measurements of relaxation time in vitro. Conclusion. Infliximab is approved for severe cases of rheumatoid arthritis, together with methotrexate, for pronounced psoriasis and psoriasis-arthritis, ankylosing spondylitis as well as for chronic inflammatory bowel disease. We conclude, that Quantitative Magnetic Resonance Imaging can be used to monitor drug effects

Introduction. Quantitative and accurate monitoring of tumor makes hyperpolarized carbon (13C) Magnetic Resonance Imaging and Spectroscopy (MRI/S) a powerful tool for in vivo metabolic and structural study. Moreover, the studies of the properties and functions in tumor tissue of the compounds of carbon (C) that are organic, are fundamental to tumor biochemistry. Aim. To review 13C MR spectroscopy to study the absolute configuration of endogenous and polarized alanine. Material and methods. An analysis of literature regarding 13C MR spectroscopy of polarized alanine. Results. Current evidence suggests that the determination of absolute configurations of amino acids play significant role in physiological mechanisms during tumor growth and treatment. Conclusions. Nearly 50% nuclear polarization for 13C can be achieved in various organic molecules when Dynamic Nuclear Polarization DNP is performed in a strong magnetic field and at cryogenic temperatures.

Introduction. Kidney stone formation may be a result of increased urinary oxalate supersaturation. Material and Methods. Eighteen pigs were randomly divided into: Control group, where standard cereal-based feed was supplemented with 4% HP only, Prevention group, where treatment with OxDc slurry started at the end of the adaptation period when pigs were switched to 4% HP diet, Reduction group, where the treatment with OxDc lyo powder started after pigs were already on a 4% HP diet for 6 days. Results. OxDc slurry prevented oxalate excretion in urine. The reduction effect of OxDc lyo feed addition was generally visible during the first two days of the therapy (p<0.05). Both dietary intake of 4% HP and OxDc preparations did not influence weight gain, water or feed intake, urine excretion and creatinine clearance. Conclusions. The capacity of OxDc in preventing induced hyperoxaluria was moderate. Most probably, this is due to the incoherent response of animals to the HP enriched diet dependent on their gut pH, since optimum pH for OxDc is around 5-6. A higher pH essentially reduces the activity of OxDc. The capacity of OxDc in reversing the hyperoxaluria induced by a HP enriched diet was significant during the first 2 days after introducing OxDc to the diet.

Opis:
Aim of the study. In this study, we sought to create a model of reversible hyperoxaluria in pigs by feeding with hydroxyproline (HP). Materials and methods. The experiment included 12 pigs divided into 2 groups (n = 6). The pigs were fed twice a day. At the beginning of the experiment, in the adaptation period, all pigs were given standard feed. In the next 7 days, an increasing amount of hydroxyproline (1–3% HP), was added to the feed. In next 14 days, 4% HP was administered in each pig meal. After 14 days of 4% HP diet, the pigs were randomly divided into 2 groups. For 6 pigs, 4% HP treatment had been continued for the next 14 days while the second group of pigs for the next 14 days received a standard HP free diet. 24h urine samples, blood and fecal samples were collected on particular days. Results. The addition of HP to the diet increased urinary oxalate excretion. A characteristic increase was noted after 12 days of treatment with 4% HP. During the removal period, oxalate excretion decreased in the group without HP in diet, while in the group which continued with a 4% HP diet, oxalate excretion significantly increased. Gross examination of kidneys showed that in the group which had 4% HP diet for 4 weeks, kidneys were fibrotic with enlarged cavities, and had small visible urinary stones. In second group, kidneys were relatively normal looking with no visible stones. Conclusion. Hyperoxaluria is reversible, if HP is removed 14 days after the start of 4% HP diet. Prolonged exposure up to 4 weeks causes pathologic changes in kidneys including crystals, sand and small stone formation.

Autorzy tekstu podważają tezy recenzji, bagatelizujące trwający 16 lat dialog operacyjny ks. Gulbinowicza z SB. Wzmacniają kwestionowane przez Polaka i Suleję tezy z książki „Dialog należy kontynuować…”, w oparciu o niewykorzystane wcześniej materiały źródłowe, wskazując, że obraz niezłomnego hierarchy, który funkcjonował przez lata w sferze publicznej, wymaga skorygowania.
This text is a reply to a review of the source volume “Dialog należy kontynuować…” Rozmowy operacyjne Służby Bezpieczeństwa z ks. Henrykiem Gulbinowiczem z lat 1969–1985. Studium przypadku, selected, prefaced and edited by Rafał Łatka and Filip Musiał (Warsaw–Cracow, 2020), which is actually introduction to the volume by Wojciech Polak and Włodzimierz Suleja. The authors of this text contest the theses of the reviews which challenge Father Gulbinowicz’s 16-year-long operational dialog with the Security Service (Służba Bezpieczenstwa, SB). They reinforce the contested Polak’s and Suleja’s claims from “Dialog należy kontynuować…”, on previously unused source materials, indicating that the image of the relentless hierarch that had functioned for years in the public sphere needs to be corrected.
