Opis:
Periprosthetic joint infection is one of the complications developing after surgical implantation of the prosthetic joint. Combined with ageing of society, number of arthroplasties will grow. Knowledge of risk factors of PJIs (periprosthetic joint infections) is essential for surgeons performing such interventions. There are many groups of risk factors. Patient’s race does not clearly predispose to developing PJIs. Age is not a key factor of occurrence of PJIs. Sex of patient plays important role (more PJIs in male patients )when there are no additional risk factors such as comorbidities – in that case females are suffering from PJIs more often. Alcohol intake is not clearly correlated with increased risk of PJIs while tobacco smoking and higher BMI increased that risk significantly. Cardiovascular, pulmonary tract, renal, gastrointestinal, rheumatological and oncological diseases are important PJIs risk factors as well as as diabetes, immunosuppression, preoperative infection and depression. Such complications are also bond with higher ASA score, some of the lab test results. Last group of risk factors is related with treatment – complications during surgery, other conditions like hypothermia, longer hospital stay, blood transfusion and reoperations. It is vital to acknowledge these risk factors and manage to remove these related to surgeon while encouraging patient to deal with these related to him.

Opis:
Discopathy of the lumbosacral spine are all pathological changes within the intervertebral disc, causing characteristic clinical symptoms, which mainly include pain in the lower part of the spine, often radiating to the lower limbs but also tingling, numbness and limitation of movements of torso. Treatment of patients with discopathy is usually conservative. Physiotherapy, kinesitherapy and therapeutic massage treatments are used to reduce pain. Subjective evaluation of the effectiveness of the physiotherapeutic procedures used in the group of patients with pain in the lumbosacral spine. The study included 140 people diagnosed with discopathy, attending rehabilitation procedures in Tarnów physiotherapy offices. The research method was a questionnaire of own design supplemented with the VAS scale to assess pain before and after rehabilitation. The data was subjected to statistical analysis. Among the treatments used in the examined group of patients dominated spine and abdominal muscle exercises, therapeutic massage, TENS currents and manual therapy. The average intensity of pain before rehabilitation was = 7.21 points. ± 1.54 points, while after the rehabilitation it was equal to = 3.44 points. ± 1.97 points (p <0.001). Outpatient physiotherapy reduced pain in people suffering from discopathy of the lumbosacral spine. Comprehensive use of TENS currents, spine and abdominal muscle exercises, therapeutic massage and manual therapy does not affect the total elimination of pain in the lumbosacral spine, it only allows for their significant reduction. Physiotherapeutic therapy applied in patients with lumbosacral spine discopathy significantly improved their quality of life.

Hypospadias is the second most common congenital anomaly after undescended testis worldwide. The prevalence of hypospadias in Europe stand at approximately 18.6 per 10,000 births and the number of registered cases between 2001 and 2010 in 23 EUROCAT registries is stable. Endocrine disrupting chemicals and potential EDCs can be found in various materials such as pesticides, metals, additives or contaminants in food and personal care products. Majority of studies indicate EDCs as a potential risk factor of hypospadias. According to some researches, pregnant women should avoid cosmetics with EDCs and exposure to other products containing them. However, because of incoherent experts’ opinion further valuable studies are needed to confirm this thesis.

Aggression is the growing problem in society. Therefore it is important to identify potential factors preventing aggression. In recent years scientists have observed decreased concentrations of omega-3 acids in people prone to aggression. This discovery suggested that the key to reducing aggression could be omega-3 acids supplementation. The aim of this study is to describe whether omega-3 acids consumption may result in reduction of aggressive behavior. Standard up-to-date criteria were followed for review of the literature data. A search for English-language articles in PubMed database was performed. Papers published between 2014 and 2017 were reviewed. In randomized study scientist discovered that omega-3 acids supplementation results in 42-68% reduction of parental complaints about aggressive behaviors in children. The improvement of behavior persisted for six months after cessation of treatment. Scientists in 2015 found out that in the prison population low levels of omega-3 acids in the blood correlate with more aggressive behavior. Patients in treatment group were characterized by more than 30% reduction in aggressive behaviors comparing to placebo group. In another review study we can notice the same conclusions. Aggression against others and impulsivity were lower in the group receiving omega-3 acids. A greater susceptibility to aggression has been observed among patients who have lower levels of omega-3 acids in their blood. Scientists have demonstrated positive omega-3 effects on the reduction of aggressive behavior. Omega-3 supplementation is safe and well tolerated and it can be used in combination with other methods of aggression treatment.

Preeclampsia is a frequent and dangerous complication of pregnancy. It is a significant cause of death for the mother and baby. In the course of this disease, the blood flow in the placenta is impaired, what leads to foetus hypoxia. It is characterized by high blood pressure and frequently large amount of protein in urine. To describe whether acetylsalicylic acid is effective in prevention of preeclampsia. Standard up-to-date criteria were followed for review of the literature data. A search for English-language articles in PubMed database was performed. Papers published in 2017 were reviewed. In double-blind trial published in 2017 in The New England Journal of Medicine researchers divided women who were at high risk for preeclampsia into two groups. One group was receiving acetylsalicylic acid and other was receiving placebo. Preeclampsia was observed in 1.6% participants in the acetylsalicylic acid group, compared with 4.3% in the placebo group. From other study we found out that acetylsalicylic acid has a dose-response effect. Greater reduction in the risk of preeclampsia was reported with the acetylsalicylic acid dosage >75 mg/d. When acetylsalicylic acid was included in treatment at >16 weeks, there was less reduction of preeclampsia without relationship with acetylsalicylic acid dosage. Other meta-analysis shows that there was no statistically significant difference in occurrence of preeclampsia between women who started taking acetylsalicylic acid before and after 16th week of pregnancy. Acetylsalicylic acid is a good agent to prevent of preeclampsia. It should be prescribed to women in high risk groups regardless of the duration of pregnancy even though it may not reduce the risk of preeclampsia as much as before 16th week of gestation.

Osteoarthritis (OA) of the knee joints is nowadays a significant medical and economic problem in the world. The percentage of people diagnosed with degeneration of the knee joint is increasing rapidly. Not only does it cause negative social consequences, but also generates large treatment costs. The treatment consists in taking preventative measures and educating patients, as well as pharmacological therapy, physiotherapy and, as a last resort – surgery. Suitable and comprehensive measures can decrease symptoms of gonarthrosis. The research was conducted among 110 patients diagnosed with OA of the knee joints in one of the rehabilitation clinics in Podkarpacie voivodeship. A self-construction questionnaire, the KOOS scale and the VAS scale were the research tools. The respondents were informed about the anonymity of the survey. The research was carried out twice before and after the rehabilitation. Among the respondents, the highest percentage of people with gonarthrosis involved women (74.5%). Overweight and obesity were important factors correlating with the occurrence of gonarthrosis. The cryotherapy (66.4%), exercise in relief (42.7%), followed by laser therapy, magnetronics and individual exercises were considered the most helpful treatments. Electrotherapy and massage were considered as the least effective. Having analysed the result of the KOOS questionnaire and the VAS scale provided to the patients before and after the treatment, the positive effects of physiotherapy have been noted. The patients have noticed improvement in daily activities, quality of life, severity of symptoms and pain relief (p <0.05). The research indicates that physiotherapy is an effective measure, therefore it should be included in the treatment of patients diagnosed with OA.
