Heart rate is constantly changing under the influence of many control signals, as manifested by heart rate variability (HRV). HRV is a nonstationary, irregularly sampled signal, the spectrum of which reveals distinct bands of high, low, very low and ultra-low frequencies (HF, LF, VLF, ULF). VLF and ULF components are the least understood, and their analysis requires HRV records lasting many hours. Moreover, there are still no well-established methods for the reliable extraction of these components. The aim of this work was to select, implement and compare methods which can solve this problem. The performance of multiband filtering (MBF), empirical mode decomposition and the short-time Fourier transform was tested, using synthetic HRV as the ground truth for methods evaluation as well as real data of three patients selected from 25 polysomnographic records with a clear HF component in their spectrograms. The study provided new insights into the components of long-term HRV, including the character of its amplitude and frequency modulation obtained with the Hilbert transform. In addition, the reliability of the extracted HF, LF, VLF and ULF waveforms was demonstrated, and MBF turned out to be the most accurate method, though the signal is strongly nonstationary. The possibility of isolating such waveforms is of great importance both in physiology and pathophysiology, as well as in the automation of medical diagnostics based on HRV.

Opis:
Cyfrowe mapy topoklimatyczne Warszawy są efektem pierwszego etapu badań mającego na celu stworzenie bazy danych o środowisku fizyczno-geograficznym tej aglomeracji. Przy tworzeniu bazy danych zastosowano metody GIS. Wykonano mapy elementów meteorologicznych (temperatury powietrza, prędkości wiatru i całkowitego promieniowania słonecznego) oraz mapy zanieczyszczenia powietrza. Przy zastosowaniu modelu MENEX obliczono ilość ciepła zmagazynowanego przez organizm człowieka i promieniowania słonecznego pochłoniętego przez człowieka. W końcowym etapie wykonano syntetyczne mapy, np. obciążenia cieplnego człowieka, topoklimatyczną i biotopoklimatyczną.
Digital topoclimatic maps of Warsaware the effect of the first step of the studies dealing with creation of the data base of physico-geographical environment of this agglomeration. GIS methods are used in this purpose. At present there were made basic maps of meteorological elements (air temperature, wind speed and global solar radiation) as well as air pollution maps. With the use of the MENEX model there were calculated maps of net heat storage of the human organism and solar radiation absorbed by man. Finally some synthetic maps were made e.g. heat load of man, topoclimatic and biotopoclimatic (which takes into account heat load of man and air pollution).

The paper presents general features of the climate and bioclimate of Uniejów assessed from the point of view of the possibility to apply them to climate therapy. The authors have used multiannual data from meteorological stations located in the vicinity of Uniejów. It was found that the climate and bioclimate of the city are suitable for climate therapy. The most useful is the period from the beginning of May to the middle of October when the majority of patients can practice one of several forms of climate therapy (aerotherapy, heliotherapy, terrain therapy).

Bickerstaff’s brainstem encephalitis (BBE) is a rare, immune-dependent disease of brainstem characterized by progressive ophthalmoplegia, ataxia and central nervous system involvement, which may take the form of impaired consciousness and/or pyramidal signs. BBE requires differentiation with many neurological conditions and it is extremely difficult to make the proper diagnosis. We describe the case of a 63-year-old female patient admitted to the Department of Neurology due to speech changes, balance disorder, diplopia and weakness of the right limbs. On admission, neurological examination revealed depressed gag and palatal reflexes, central facial nerve palsy on the right side of the face and paresis of the right limbs. On the second day, progression of neurological deficit was observed – quantitative disturbances of consciousness, absence of gag and palatal reflexes and bilaterally positive Babinski sign. Head MRI did not confirm the presence of recent ischemic changes in the brain. From the fourth day, the patient's general and neurological condition was very severe. She was unconscious with ophthalmoplegia and narrow, non-reactive pupils. Multimodal therapy was administered during hospitalization, observing a gradual improvement in the patient's condition from the thirteenth day of hospitalization. Due to the ambiguous clinical picture, the following diseases were considered in the differential diagnosis: ischemic stroke of the brainstem region, venous sinus thrombosis, posterior reversible encephalopathy syndrome, reversible cerebral vasoconstriction syndrome, paraneoplastic brainstem encephalitis and Bickerstaff’s brainstem encephalitis. On the thirty-fifth day, the patient was discharged home. The patient maintained verbal contact with clear speech, proper eye movement and persistent four-limb paresis. After 5 months, the patient was re-admitted to the Department of Neurology to assess the neurological condition and undergo medical check-ups.

Opis:
AISI 316L steel was subjected to nitrocarburizing under glow discharge conditions, which was followed by DLC (diamond-like carbon) coatings deposition using the same device. The coatings were applied under conditions of direct current and pulsed glow discharge. In order to determine the influence of the produced nitrocarbon austenite layer and the type of discharge on the microstructure and mechanical properties of the coatings, the following features were analysed: surface roughness, coating thickness, structure, chemical composition, adhesion and resistance to frictional wear. For comparison purposes, DLC coatings were also deposited on steel without a nitrocarburised layer. The obtained results indicate a significant influence of the type of glow discharge on the roughness, hardness, nitrogen content and of the nitrocarburised layer on the resistance to wear by friction and adhesion of the produced coatings.
