Opis:
To reduce human casualties associated with explosive ordnance disposal, a wide range of protective wear has been designed to shield against the blast effects of improvised explosive devices and munitions. In this study, 4 commercially available bomb suits, representing a range of materials and armor masses, were evaluated against 0.227 and 0.567 kg of spherical C-4 explosives to determine the level of protection offered to the head, neck, and thorax. A Hybrid III dummy, an instrumented human surrogate [1], was tested with and without protection from the 4 commercially available bomb suits. 20 tests with the dummy torso mounted to simulate a kneeling position were performed to confirm repeatability and robustness of the dummies, as well as to evaluate the 4 suits. Correlations between injury risk assessments based on past human or animal injury model data and various parameters such as bomb suit mass, projected area, and dummy coverage area were drawn.

Background. A spinal cord injury (SCI) leads to patho-physiological changes that can affect physical and psychological performance. The aim of this observational study was to evaluate the relationship between exercise capacity, functioning and quality of life in patients 12 weeks after traumatic paraplegia participating in early rehabilitation. Material and methods. 13 patients participated in this study and performed cardio-pulmonary exercise testing (CPET) on an arm-crank ergometer to determine peak exercise capacity (VO2peak). Data from the spinal cord independence measure (SCIM) were used to assess different areas of functioning. The 12-item short form survey (SF12) questionnaire was applied to measure quality of life. Spearman correlations were used to relate VO2peak with SCIM data and results from the SF12 questionnaire. Results. VO2peak ranged between 12.6 and 28.1 ml/kg/min. A significant relationship was found between VO2peak and the physical component of the SF12 questionnaire, whereas no correlations were found with either SCIM sub or total score, or with the mental component or the total score of the SF12 questionnaire. Conclusions. 1. Patients with traumatic paraplegia showed fair to average exercise capacity after 12 weeks of early rehabilitation. 2. A significant relationship between VO2peak and subjectively rated physical fitness exists at this time point. 3. The implementation of an individual fitness program tailored to the patients' needs based on CPET results is highly recommended in order to improve functioning and quality of life.

Opis:
A case of a 46-year-old car driver struck with great force by a tram through the driver’s door is presented. The main trauma consisted in chest injury with multi-rib fracture along with rupture and dissection of the thoracic aorta. Immediate medical rescue actions consisted only in procedures necessary to support vital functions; the patient survived owing to being promptly transported to the Emergency Department to undergo thoracotomy and laparotomy with massive blood transfusion. Polytrauma angio-CT scan revealed a posttraumatic thoracic aorta lesion which in turn was treated by deployment of an endovascular thoracic stent graft. This way, the immediate risk of death was averted, and the remaining traumatic lesions and conditions could be treated. Patient was discharged to a Rehabilitation Center on the 49th day of treatment. The authors stress that trauma resulting from accidents with this particular mechanism, i.e. lateral car crash on the driver’s side with the driver’s door being staved in by the tram, should be managed by immediate transport of the patient to the Emergency Center. In such cases early drainage of the pleural cavity can deteriorate patient’s status by increasing the bleeding from the ruptured aorta.

ObjectivesThis study aimed to assess the surgical outcomes of patients with work-related upper extremity musculoskeletal disorders (UE-MSDs) who failed conservative treatment.Material and MethodsThis was a retrospective study of 17 patients who had work-related UE-MSDs and underwent the following surgeries and follow-up evaluations: decompression, external and internal neurolysis of the upper trunk of the brachial plexus and the long thoracic nerve (LTN), and a partial resection of the anterior and middle scalene muscle. A detailed history of clinical presentation including pain, physical and clinical examinations of the extent of scapular winging (ESW), and upper extremity anatomical postures, such as active forward arm flexion and shoulder abduction, were recorded before and after 3 months of the surgery. Nerve conduction velocity and electromyography examination reports were obtained to assess the sensory or motor loss of the nerve injury before their operation.ResultsAll 17 patients included in this report showed some improvement anatomically in the scapula appearance and functionally in their shoulder movements. More specifically, 9 (53%) patients got a restored to near healthy appearance of the scapula, and 11 (65%) patients recovered a full range of motion, 180° post-surgically. Overall, the mean shoulder flexion and abduction improved to 157±37.5° and 155±40.2° after the surgery from 106±30.2° and 111±34.8°, respectively (p < 0.0001). The ESW score also significantly improved from a mean of 1.5±0.51 to 3.5±0.71 (p < 0.0001). The post-surgical improvements were statistically highly significant (p < 0.0001).ConclusionsDecompression and neurolysis of the upper brachial plexus and the LTN, and the partial release of the scalene muscle contracture procedures have fostered improvements in the shoulder anatomical appearance and movements in all 17 patients.

The term "deadly dozen" has been known for years as a list of chest injuries that pose a potential threat to life. In pre-hospital conditions, each of twelve injuries should be considered, but making a decision to implement medical procedures before a patient transportat may be a challenge for emergency services. The authors point to six types of chest injuries for which there are possible benefits linked to transport delays, in order to perform the necessary actions to stop the dying process. Endotracheal intubation, mechanical ventilation, chest puncture, oxygen therapy, and occlusive dressings are available in most pre-hospital care systems all over the world. Thanks to these measures, optimal patient protection during transfer to the hospital is possible in selected situations.
Pojęcie „deadly dozen” jest znane od lat jako lista urazów klatki piersiowej stanowiących potencjalne zagrożenie życia. W warunkach przedszpitalnych należy rozważyć każde z dwunastu obrażeń, jednak podjęcie decyzji o wdrożeniu zabiegów medycznych jeszcze przed transportem może stanowić wyzwanie dla służb ratunkowych. Autorzy wskazują na sześć typów obrażeń klatki piersiowej, dla których istnieją możliwe korzyści wynikające z opóźnienia transportu w celu wykonania niezbędnych czynności, które mają na celu przerwanie procesu umierania. Intubacja dotchawicza, wentylacja mechaniczna, nakłucie klatki piersiowej, tlenoterapia, czy opatrunki okluzyjne są dostępne w większości systemów pomocy przedszpitalnej na świecie. Dzięki tym zabiegom jest możliwe w wybranych sytuacjach optymalne zabezpieczenie pacjenta na czas transportu do szpitala.

First responders and military personnel are particularly susceptible to behind armor blunt thoracic trauma in occupational scenarios. The objective of this study was to develop an armored thorax injury risk criterion for short duration ballistic impacts. 9 cadavers and 2 anthropomorphic test dummies (AUSMAN and NIJ 0101.04 surrogate) were tested over a range of velocities encompassing low severity impacts, medium severity impacts, and high severity impacts based upon risk of sternal fracture. Thoracic injuries ranged from minor skin abrasions (abbreviated injury scale [AIS] 1) to severe sternal fractures (AIS 3+) and were well correlated with impact velocity and bone mineral density. 8 male cadavers were used in the injury risk criterion development. A 50% risk of AIS 3+ injury corresponded to a peak impact force of 24,900 ± 1,400 N. The AUSMAN impact force correlated strongly with impact velocity. Recommendations to improve the biofidelity of the AUSMAN include implementing more realistic viscera and decreasing the skin thickness.
