Introduction. There are reports in literature which indicate the connection between impacted third molars and occurrence of symptoms of craniomandibular dysfunctions and headaches. Objectives. The aim of this study was evaluation of the outcome of patients who reported specific symptoms of craniomandibular dysfunction and had impacted mandibular third molars. Materials and method. The research material consisted of 10 women who reported to the Department of Craniomandibular Disfunctions of the Medical University in Lublin, Poland, with pain and acoustic symptoms in the Temporomandibular joint (TMJ) area. During preliminary therapy, the patients used a silicone occlusal device; ionotherapy was ordered and the patients were recommended to eliminate parafunctions. Results. Clicks before treatments were present in 6 patients, after treatment with silicone occlusal device and ionotherapy with Profenid gel in 5 patients, while two years after extraction of the impacted teeth the clicks were no longer present, and differences in the presence of clicks analyses by means of the Q-Cochran test were statistically significant between examinations 1m vs.3 (Q=10.33; p<0.01) and examinations 2 vs.3 (Q=8.40; p<0.05). Conclusions. The study showed that extraction of the mandibular third molars can cause regression of some symptoms of craniomandibular disorders.

Opis:
Background. Structures of temporomandibular joints and ear have a common phylogenetic origin. Muscular imbalance in the area of masticatory muscles may lead to tinnitus. Objectives. A comparison of muscular tension and masticatory muscles asymmetry between women with tinnitus and without tinnitus. Material and methods. From a group of 96 examined responders, we found 32 women (aged 22 ± 2 years) who exhibited no temporomandibular joint disorders and who had not been treated otolaryngologically. The examined women were then placed within two groups: with tinnitus (16 people) and without tinnitus (16 people). Next, we examined the muscular activity in the temporomandibular muscles by way of sEMG at rest, when the teeth were clenched and when the mouth is opened maximally. To assess muscular asymmetry, the asymmetry index was used. Results. Despite the lower bioelectrical muscular tension of masticatory muscles at rest and during the examined functions in people with tinnitus, the results were not important statistically (p > 0.05). However, we observed crucial statistical differences in masseter muscle asymmetry in people with tinnitus when clenching teeth (p = 0.042), and in the number of people with muscle asymmetry in the area of the masticatory muscles at rest (p = 0.012) and when teeth are clenched (p = 0.013), in comparison to a healthy group. Conclusions. There is a relationship between changes in the asymmetry index of the masticatory muscles and the occurrence of tinnitus. In order to confirm the results, we recommend further examination in this area and longer observation time.

Background. The number of patients with temporomandibular joint disorders (TMD) is increasing in clinical dental practice. Our study aimed to determine whether a correlation between Parkinson’s disease and TMD symptoms can be established. Material and methods. The anamnestic and clinical dysfunctional indices and the correlations related to the quality of life (SF 36, QoL) of Parkinson’s disease patients (PG, n=35) and healthy controls (CG, n=42) were examined in a cross-sectional study. Statistical analysis was carried out using SPSS 24.0 software. Results. The mean value (7.0±7.7) of the PG in the OHIP-14 (Oral Health Impact Profile) questionnaire was significantly higher (p<0.001) than that of the CG (2.0±3.7). The ratio of belonging to the asymptomatic (Ai0) group was higher in the CG (71.4%) than in the PG (45.7%). The number of moderate or severe symptoms (DiII and DiIII) was higher in the PG (37.1%) than in the CG (2.4%). The mean value of QoL of the PG (60.9±20.1) was significantly lower (p<0.001) than that of the CG (75.9±15.3). Conclusions. Results of the study support that patients with Parkinson’s disease have a higher incidence of TMD. Using the SF-36, we were able to quantify lower QoL of the PG.
Wprowadzenie. W stomatologicznej praktyce klinicznej coraz częściej widuje się pacjentów z zaburzeniami czynności stawu skroniowo-żuchwowego (ang. temporomandibular joint disorder, TMD). Celem badania było ustalenie, czy istnieje zależność między objawami TMD a chorobą Parkinsona. Materiał i metody. W badaniu przekrojowym wzięto pod uwagę anamnestyczne i kliniczne wskaźniki dysfunkcji oraz korelacje związane z jakością życia (kwestionariusz oceny jakości życia SF-36) u pacjentów z chorobą Parkinsona (PG, n=35) i u osób zdrowych (CG, n=42). Analizę statystyczną przeprowadzono przy użyciu oprogramowania SPSS 24.0. Wyniki. Średnia wartość uzyskana na podstawie profilu wpływu zdrowia jamy ustnej (ang. Oral Health Impact Profile, OHIP-14) dla PG (7,0±7,7) była znacznie wyższa (p<0,001) niż wśród CG (2,0±3,7). Wskaźnik przynależności do grupy bezobjawowej (Ai0) okazał się wyższy w przypadku GK (71,4%) w porównaniu do PG (45,7%). Liczba umiarkowanych lub ciężkich objawów (DiII i DiIII) była większa u PG (37,1%) aniżeli u CG (2,4%). Średnia wartość jakości życia dla PG (60,9±20,1) była natomiast istotnie niższa (p<0,001) niż w przypadku CG (75,9±15,3). Wnioski. Wyniki badania potwierdzają, że pacjenci z chorobą Parkinsona częściej cierpią z powodu TMD. Dzięki zastosowaniu SF-36 określono ilościowo niższą jakość życia PG.

Objectives This study investigated whether playing wind instruments has adverse effects on musculoskeletal functions among junior high school students who play in music clubs. Material and Methods The study included 210 junior high school students (35 boys, 175 girls) belonging to 1 of 4 different school clubs that practiced playing wind instruments more than 6 days/week. The mean age of the participants was 14 years. The study was performed using a questionnaire survey and an electromyographic examination of jaw and cervical muscle activities during playing wind instruments. Results The prevalence of temporomandibular disorders (TMD) among the children playing woodwind (WW) or brass wind (BW) instruments was higher than in those playing non-wind (NW) instruments. Long duration of playing WW with a reed mouthpiece or BW with a small mouthpiece was suggested to affect the incidence of TMD, which was more marked in girls than in boys, irrespective of height or weight. Muscle activity in the masseter muscle during playing an instrument was significantly higher in the BW with a small mouthpiece group than in the NW group (p < 0.05). In cervical muscles, muscle activity of both the sternocleidomastoid and trapezius muscles was higher during playing BW than in the case of other instruments, and activity in the sternocleidomastoid muscle was significantly higher in the BW with a small mouthpiece group than in the case of other instrument groups (p < 0.05). Conclusions Playing wind instruments may have adverse effects on musculoskeletal functions among junior high school students playing in music clubs as compared with playing NW instruments. The prevalence of TMD among the students playing wind instruments was higher than in those playing other instruments. Long duration of playing those instruments affects musculoskeletal function, and this effect is more marked in girls than in boys, irrespective of height or weight.
