Opis:
FT-Raman spectroscopy was used to investigate microstructural changes in the secondary protein structure of soft tissues subjected to increasing levels of macroscopic strain. Main protein bands at 938 cm-1 assigned as v(Cα–C), 1668 cm-1 — amide I and 1268 cm-1 — amide III are sensitive to applied strain and undergo wavenumber shifting. Other main vibrational modes at 1004 cm-1 assigned to the phenyl ring breathing mode and 2940 cm-1 (n(CH3,CH2)) remain unaltered. Spectroscopic results were compared with the mechanical relations obtained from the standard protocol of uniaxial tensile tests carried out in a testing machine. A clear correlation between Raman band shifting and the level of mechanical stress was established. Initially the load is transferred through elastin and then gradually also by collagen. It was proved that transferring loads by soft tissues involves changes in structural protein conformation. This process was described in detail for a tendon. It was also confirmed that mechanical properties of soft tissues depend on collagen and elastin fiers orientation.

The findings of preserved soft body parts including pedicle in the linguloid brachiopods are extremely rare in the fossil record of which the early Cambrian Chengjiang (southern China) and Burgess Shale (British Columbia) faunas are the most important. However, these characteristic Cambrian soft−bodied faunas largely disappeared from the fossil re− cord well before the end of the Cambrian. Here we describe the first record of the pedicle in a linguloid brachiopod from the post−Cambrian strata, preserved with remarkable fidel− ity. Contrary to the Chengjiang and Burgess Shale−type fau− nas which are commonly preserved as essentially two−di− mensional aluminosilicate or degraded organic carbon films or pyritized compressed fossils, the specimens now recov− ered from the Ordovician of China show a three−dimen− sional, pyritized pedicle with preserved external morphol− ogy, in detail. The presence of streamlined shell shape and burrowing shell sculpture in our specimens supports an as− sumption for infaunal mode of life of the genus. Since all linguloid brachiopods of the early Cambrian are inter− preted as epifaunal or semi−infaunal, it seems that the here described Ordovician linguloid is the oldest representative of fully infaunal brachiopods. Apparently, the long vermi− form and flexible linguloid pedicle has appeared as a func− tionally optimized construction.

Being bitten by a dog can have serious health effects. That is why, never underestimate even the smallest soft tissue injuries inflicted by aggressive animals. This incident may have an impact on the further condition of a patient. From our first aid will also depend the aesthetic and functional effect of the scar on the face. We should pay attention to the use of antibiotic prophylaxis. The aim of the study was to perform the analysis of the soft tissue bitten injuries made by dogs in patients treated in the years 2004‑2009 in the Clinic of Cranio-Maxillo-Facial and Oncological Surgery in Łódź. The most frequent attacked areas were analyzed in the cases of single and multiple face wounds. The dependence of the dog attacks and the alcohol consumption by the victims. The use of an early antibiotic prophylaxis and the number of the infectious complications. Material and methods. The material studied is a group of 26 patients, including 17 women and 9 men. In the majority patients were older than 20 years old. Results. The analysis of our data shows that most of the victims were aged 19-30 and 51-60 years. 14 patients have been mutilated on one area of the face, the remaining patients at least two areas. Most injuries underwent upper or lower lip. In all cases, the initial supply has been applied to the wounds. Antibiotic prophylaxis was used in 23 patients. In one of the other three cases, patient who have not been applied to the prevention of complications in the form of phlegmon face. Half of the attack dogs have been known to the victims. All patients had implemented prevention of tetanus, or held-to-date vaccinations. In eight cases, patients reported that at the time of the event they were under the influence of alcohol. Conclusions. Primary supply of bitten wounds of face at the moment seems to be the standard. In our study, in cases where patients has been treated with an antibiotic, there was no case of infection in the wound. Late complication in the form of phlegmon occurred in one patient who had not used prophylaxis. As the most of the authors note lower lip is the most vulnerable for the bite in the case of adult people. Analysis of our data is consistent with these reports. It has also been found that people under the influence of alcohol are often attacked by unknown dogs.

The paper contains the wavelet approach to registered backscattered RF signals from two different cases. First, the wavelet analysis has been performed for RF signals registered from soft tissue phantoms. The second case is the wavelet analyses of RF scattered signals from regions of healthy and BCC changed human skin. The three phantoms made from tissuemimicking material with different structures have been measured. We claim that there are visible differences in the statistical parameters of wavelets coefficients of signals between healthy and BCC changed skin regions as well as between phantoms without scatterers and with different number of strong small scatterers.

It is well known that the nonlinear propagation increases the absorption of acoustic waves in the medium thus increasing the temperature effects. According to the recently developed new theoretical approach it is possible to determine in a simple way the effective absorption in the case of nonlinear propagation basing on the pulse spectrum analysis [Wójcik, 1996]. In this way it was possible to find the corresponding absorption values occuring in ultrasonography. In this case a classical PVDF membrane hydrophone was used to demonstrate and to measure nonlinear effects. Analysing the obtained wave spectra it was possible to determine the increase of the effective tissue absorption and hence to find the rate of heat generation per unit volume which is crucial for temperature elevations In this way possible temperature increases for the case of nonlinear and linear propagation can be determined.

The pre-operative preparation of a surgeon is increasingly often extended to individual patient’s anatomical models depicting the affected organ and the existing lesion in relation to the surrounding structures (e.g. blood vessels). Among the models, there are models of the skeletal system, in which the FFF technology is used. The models of parenchymal organs are more difficult to reproduce due to the material limitations in the FFF technology, which is why alternative technologies such as PolyJet and SLA are used. Due to the high price of models, they are employed less frequently, especially in the Polish hospitals. The research presented in the article deals with the modification of the existing methodology of rapid pre-operative models production with the use of various low-budget rapid manufacturing techniques, which use materials of low hardness, similar to the human soft tissues. The aim was to create a methodology and determine the parameters of materials that allow for the cheap production of functional pre-operational aids.

Sepsis is a severe generalised infection caused usually by pathogenic bacteria. It is often the cause of hospitalization and death in patients treated in intensive care and other hospital wards. Latest research brought to better understanding of patomechanisms, took place significant development of therapy heading to improvement of general patients condition treated as a basis and additionally supported by local therapy. The aim of the study was to evaluate the possibility of using the solution Decasan in the comprehensive treatment of patients necrosis of soft tissues. Material and methods. The study included 192 patients (W/M 103/89; average -aged 53.35 ± 5.36 years). According to the classification of septic states (Chicago, 1991), patients were divided into three groups: first – patients to the local form of the infection, the second – with Systemic Inflammatory Response Syndrome (SIRS), which lasted up to 72 hours and the third – patients with various forms of sepsis, SIRS in which lasted 72 hours. Results. As a result of our studies carried out in patients where the wound was made decontamination solution Decasanu, received: pain reduction, decrease tissue swelling, early debridement of the wound and the appearance in the wound granulation, to reduce delays wound healing. Proposed algorithms treatment of various forms of sepsis, pointed out the essential elements, ie.: a comprehensive approach to the treatment of infection by early surgical intervention, intensive supportive therapy (fluid resuscitation), antibiotic therapy directed to microorganisms that cause infections and topical antiseptics therapy (solution Decasanu). Conclusions. The preparation antiseptic Decasan can be safely used for disinfection of skin, mucous membranes and wounds in the foci of infections caused by bacteria, fungi and protozoa.

Background: Patients with foreign bodies in upper digestive tract not infrequently trigger many diagnostic and treatment challenges, especially when foreign bodies translocate and are lodged outside the esophagus. Case report: We present a case of a foreign body in esophagus 56-years old woman who had developed persistent sensation of an obstacle in her throat after eating fish (Atlantic cod). She has initially dismissed her symptoms and refused medical treatment. Subsequently, a neck CT done one week later showed a 20-milimeter long fish bone in the soft tissues on the left side of her neck (between pharynx and vertebral column). Few attempts of endoscopic removal were unsuccessful. Despite antibiotic prophylaxis and due to the fish bone translocation into soft tissues of the neck and its location close to a common carotid artery and an internal jugular vein a decision was made to remove it from the external approach. The foreign body was successfully removed without any esophageal damages. Conclusions: Foreign bodies in digestive tract may result in many life-threatening complications. The fundamental management is based on the endoscopic removal of a foreign body and the antibiotic prophylaxis. In case of foreign bodies lodged in soft tissues open surgery is recommended. Opis:
Therapeutic and surgical applications of focused ultrasound require monitoring of local temperature rises induced inside tissues. From an economic and practical point of view ultrasonic imaging techniques seem to be the most suitable for the temperature control. This paper presents an implementation of the ultrasonic echoes displacement estimation technique for monitoring of local temperature rise in tissue during its heating by focused ultrasound The results of the estimation were compared to the temperature measured with thermocouple. The obtained results enable to evaluate the temperature fields induced in tissues by pulsed focused ultrasonic beams using non-invasive imaging ultrasound technique.

Opis:
This paper deals with application of 3D scanning technology in medicine. Important properties of 3D scanners are discussed with emphasize on medical applications. Construction of medical 3D scanner according to these specifications is described and practical application of its use in medical volumetry is presented. Besides volumetry, such 3D scanner is usable for many other purposes, like monitoring of recovery process, ergonomic splint manufacturing or inflammation detection. 3D scanning introduces novel volumetric method, which is compared with standard methods. The new method is more accurate compared to present ones. Principles of this method are discussed in paper and its accuracy is evaluated and experimentally verified.

Descriptions of blast-related injuries have long been established in the textbooks. Since, for obvious reasons, it is difficult to perform ballistic studies on human tissues, such research is usually conducted on appropriate models, i.e., gelatin blocks and soap blocks - each of these materials has distinctive properties, which make them suitable for unique applications. The work aims to present the possibilities for analyzing the effects of explosion on the human body using a ballistic soap model. The tests performed allow to conclude that a shock wave affects the surface of the ballistic soap block, generating hemispherical craters, which begin to overlap when the distance between the explosive charge and block is reduced, until they form one semi-cylindrical hollow (when linear charge is used), which represents a temporary cavity. The results obtained allow for an assessment of the extent of post-explosion lesions.
We współczesnym piśmiennictwie tematycznym istnieje zgodność co do rodzajów obrażeń ciała będących skutkiem wybuchu. Ponieważ badania balistyczne tkanek ludzkich z oczywistych względów są trudne do przeprowadzenia, zazwyczaj prowadzi się je na odpowiednich modelach, tj. blokach żelatynowych i blokach mydlanych, których właściwości są jednak różne i przez to ich zastosowania są inne. Celem pracy było przedstawienie możliwości badania skutków wybuchu na ciało ludzkie przy pomocy modelu z mydła balistycznego. Uzyskane wyniki pozwalają na wyciągnięcie wniosku, że fala uderzeniowa działa na powierzchnię takiego bloku generując uszkodzenia o półkulistym kształcie, które przy zmniejszaniu się odległości od ładunku zaczynają się nakładać na siebie, aż do całkowitego zlania się w jedno uszkodzenie o półwalcowatym kształcie (przy ładunku liniowym), co odwzorowuje jamę tymczasową. Uzyskane wyniki pozwalają na oszacowanie rozległości uszkodzeń powybuchowych.

Cutaneous and soft tissue involvement is a very rare manifestation of multiple myeloma. We are describing a woman with the huge multiple plasma cells infiltrations of the soft tissues and the skin of the right shoulder and the arm that had occurred 2.5 years after the diagnosis of multiple myeloma. In the case of the described patient there is a distinct correlation between the appearance of the nodular lesions in soft tissues and the underlying osteolytic changes found in the skeleton. We assumed that the fracture of the right humerus in the past was the beginning of the plasma cells dissemination in the adherent tissues. The skin changes in our patient appeared in the advanced stage of multiple myeloma. There was only transient improvement of cutaneous changes after the chemotherapy, despite the complete remission of the underlying disease in the bone marrow. Despite of the intensive treatment (chemotherapy, radiotherapy) the patient died 7 months after the first appearance of the skin changes.

The paper analyzes the main areas of application of mathematical methods inmedical diagnostics, formulates principles of diagnostics basedon fuzzy logic; developed mathematical models and algorithms that formalize the process of making diagnostic decisions based on fuzzy logic with quantitative and qualitative parameters of thepatient's condition; developed mathematicalmodels of membership function. Mathematical modelsand algorithms have been developed that formalize the process of making diagnostic decisions based on fuzzy logic with quantitative and qualitative parameters of the patient's condition; developed mathematical models of membership functions, formalizing the representation of quantitative and qualitative parameters of the patient's condition in the form of fuzzy sets, used in models and algorithms for diagnosis and finding a diagnosis of assessing the intensity of reactive postoperative edema in patients of all study groups. An expert system was implemented for solving the problems of medical diagnosis based on fuzzy logic when assessing the intensity of reactive swelling of soft tissues, which develops in the postoperative period in patientsof all study groups against the background of diabetes.The paper analyzes the main areas of application of mathematical methods in medical diagnostics, formulates the principles of diagnostics based on fuzzy logic.
W pracy analizowane są główne kierunki zastosowania metod matematycznych w diagnostyce medycznej, formułowane są zasady diagnostyki opartej na logice rozmytej; opracowywane są modele matematyczne i algorytmy formalizujące proces podejmowania decyzji diagnostycznych w oparciuo logikę rozmytą z ilościowymi i jakościowymi parametrami stanu pacjenta; opracowywane są modele matematyczne funkcji przynależności. Opracowano modele matematyczne i algorytmy formalizujące proces podejmowania decyzji diagnostycznych w oparciu o logikę rozmytą z ilościowymi i jakościowymi parametrami stanu pacjenta; opracowano modele matematyczne funkcji przynależności formalizujące reprezentację ilościowych i jakościowych parametrów stanu pacjenta w postaci zbiorów rozmytych wykorzystywanych w modelach i algorytmach diagnozowania i znajdowania rozpoznania nasilenia reaktywnego obrzęku pooperacyjnego u pacjentów wszystkich grup badawczych. Wdrożono system ekspercki do rozwiązywania problemów diagnostyki medycznej oparty na logice rozmytej w ocenie nasilenia reaktywnego obrzęku tkanek miękkich, który rozwija się w okresie pooperacyjnymu pacjentów wszystkich grup badawczych z cukrzycą. W artykule przeanalizowano cechy zastosowania metod matematycznych w diagnostyce medycznej, sformułowano zasady diagnostyki opartej na logice rozmytej.

Opis:
Introduction: Sleep is a physiological state essential for the proper functioning of the body. One of the reasons for its disturbance is obstructive sleep apnea syndrome (OSAS). Aim: The aim of this research is the histological evaluation of the soft palate in patients who suffered from various types of OSAS. Material and method: The study group consisted of patients with sleep-disordered breathing (SDB) in the form of primary snoring or OSAS. Patients with chronic tonsillitis, without a history of SDB, were included in the comparative group. Fragments of the mucous of the uvula (study group) and the palatoglossal pillar (comparative group) were obtained during surgery for histological evaluation. Using histological, histochemical and immunohistochemical methods, we assessed the inflammation and its severity (CD3, CD20, CD68), the structure of nerve fibers (S-100) and the size of blood vessels (CD34) in the examined tissue. Results: Patients with OSAS developed a local inflammatory process in the palatal tissues (stronger expression of CD3, CD20, CD68 in patients with OSAS). The severity of the immunohistochemical reaction with CD3 correlated with the stage of OSAS. A higher degree of fibrosis and a higher expression of CD34 and S-100 receptors were observed in patients with OSAS compared to snoring patients and patients from the comparative group. Conclusion: Most likely, snoring due to chronic tissue vibration leads to damage to the nerve fibers in the soft palate, which can intensify episodes of hypopneas during and increase the chance for sleep apneas.
