The presented article takes up the subject of conflict and violence involving elderly residents of social care homes for chronically ill. Firstly, it discusses the legal regulations appropriate to these situations, to further evaluate these regulations and recommend improvements, based on qualitative research conducted in three social care homes in Podlasie. Particular attention was payed to changes in legal regulations regarding placing residents in care homes, legally expanding the staff’s rights with regard to preventing aggression, filling staff vacancies in care homes, investments in improving staff qualifications, improving communication and information regarding payments for care services, improving the image of the institutions and mutual respect, and thought-through distribution of residents within the facility.

Research results presented in the article focus on the correlation between attitudes towards people with mental disorders and chosen personality traits. Research involved 150 people – employees of social care homes. Job characteristics and the nature of contact with social care homes dwellers constituted the criterion for placing the analysed person into one of the subgroups. Three subgroups of equal size have been recognised: employees at social – therapeutic department(I), medical staff (nurses) (II), economic department employees (III). The following methods have been used: The Scale for Defining Attitudes Towards People with Mental Disorders, Raven's Progressive Matrices, Study of Values by G. W. Allport, P. E. Vernon, G. Lindzey, Stait-Trait Anxiety Inventory by C. D. Spielberger, J. Strelau, M. Tysarczyk, K. Wrześniewski, The Questionnaire: ”What are you like?” by P. Sears. Within the analyzed group people from subgroup I manifested the most positive attitudes, people from subgroup II were slightly less favourably inclined, and people from subgroup III showed the least positive approach. The application of multiple regression equation showed that among the analysed variables trait anxiety has significant meaning for the type of attitudes both in group I and II. In subgroup III a significant influence on manifested attitudes is exerted by the following variables: economic values, intellectual level, artistic values and trait anxiety.

In recent years, demographical changes have posed a significant interest for social work. The primary aim of social work as a science and as a profession is to ensure the social participation of all people on all levels of society: micro, meso and macro. Social work is therefore connected not just with individuals and families, but also with communities and society as a whole. This paper presents how this characteristic of social work in homes for the older people infl uence on the transformation of Slovenian institutional care from primary residential care to centres of care for the older people. Th e signifi cant impact is in the diff erence of social work between socially and medically oriented homes. Th e diff erence lies in social work methods as well as in the roles of the social worker in different areas of work with the residents, relatives and staff . This approach enables the provision of holistic care for the older people in institutions and in the community. Slovenian homes for the older people with existing and planned forms of assistance in the community demonstrate that institutional care is not necessarily linked only to the classical care in an institution. The transformation of homes enables the development of new forms of care for older adults both inside and outside each home for the older people

The article aims to assess the demand for the places in social homes for the elderly among persons aged 75+ up to the year 2035. To this end, the proportion of the disabled in the selected subpopulation is estimated to approximate the demand for the beds. The findings indicate that a significant increase in the demand for institutional support is likely, driven by the growing subpopulation of older persons in Poland.

In his article, the author discusses the findings of the audits conducted by NIK in seven poviats (Polish local self-government units) managing social welfare homes, in six social welfare centres (OPS) and in 13 social welfare homes (DPS). In poviats, the audit covered the financing of DPS and supervision of them, while in OPS – the availability of statutory forms of assistance to older people were audited, as well as the regularity of assistance services provided at the place of residence, and of sending persons to DPS. The audit covered the years 2012-2014, while in order to define the trends observed, the statistical data of 2011 were used.

The granting of non-cash benefits within the social welfare system, in particular care services and specialist care services, as well as services in the form of placement in a social welfare home has repeatedly been the subject of interest of administrative courts. The analysis of the judicial practice of these courts leads to the conclusion that the award of the services in question is based on the principle of subsidiarity. This principle plays a key role here. It is respected both at the stage of granting these benefits and their use by authorized persons. The case law used in this study confirms that the indicated services can only be awarded to persons who meet the requirements set out in the Law. The common element here is the inability to satisfy one's own efforts through the inability to use the help of people from the immediate environment, especially the family, which may result from various reasons. The indicated principle is also taken into account at the stage of using the services. It manifests itself, among others. in determining by the authority, the type, scope and form of assistance provided. In practice, this means that the subjective feelings of the person concerned, as well as their expectations do not affect the decision of the institution, which obviously is guided by the individual needs of the person, however determined objectively, while also taking into account the general principles and objectives of social assistance, such as also its ability to provide help. The subsidiarity principle is also reflected in the need to pay for services (as well as in the use of exemptions). Those using the services are primarily required to make this payment. The liability of other entities is exceptional, which also complies with the subsidiarity principle.

Assisted living institutions such as nursing homes were established to answer a vital social need. Such services are needed by the elderly, disabled, and chronically ill. The social demand for these institutions is growing due to a deficit of care functions in families, among other factors. Undoubtedly, assisted living institutions play a vital role by providing help and assistance to less-independent individuals, and for many, they are a chance for survival or a dignified life because they offer food and shelter. However, nursing homes as assisted living institutions are not limited to satisfying the needs of their residents. There is something surprising and worrying about their functioning. To use R.K. Merton’s terminology, they operate according to latent functions that maintain social order and social control. To better understand the problem, it is worth looking at Erving Goffman’s popular theory of total institutions and Niklas Luhmann’s social systems theory.

The necessity is emphasized to distinguish between the traditional model of data acquisition reported by a patient in doctor’s office and the more valuable and desired model to become acquainted with the core of the problem by going to a patient’s domicile. In the desired model it is possible to come across various determinants of health during home visits. Family members can be approached and there is a possibility to evaluate the relationships between the patient and his loved ones. One can visually assess one’s living conditions and predictable environmental hazard. For several years, the desired model has been put into practice by general practitioners and home health care nurses. Recently this model is also promoted by “health care therapists” who are members of “teams of home health care”. The authors, being convinced of the merits of “home and environmental model” of practical medicine, have developed a method of recording and illustrating data collected during visits in patient’s home. The elaborated tool helps to communicate and exchange information among general practitioners, home health care nurses, social workers of primary health care centers and specialists. The method improves the formulation of the plan of further therapeutic steps and remedial interventions in psycho-social relations and living conditions of patients.
Autorzy pracy podkreślają na wstępie, iż należy rozróżnić tradycyjny model obsługi problemu zdrowotnego, zgłaszanego przez pacjenta w gabinecie lekarza, na ogół przy jego biurku od bardziej wartościowego sposobu zaznajomienia się z istotą problemu poprzez udanie się do miejsca zamieszkania chorego. W trakcie takiej wizyty domowej jest możliwe zapoznanie się z rozmaitymi uwarunkowaniami stanu zdrowia pacjenta. Można poznać członków rodziny i ocenić relacje jakie zachodzą pomiędzy chorym a jego osobami najbliższym. Można naocznie ocenić jego warunki bytowe i ewentualne szkodliwości środowiskowe. Od kilkunastu lat ten drugi model czasami realizują lekarze rodzinni i tzw. pielęgniarki środowiskowe (środowiskowo – rodzinne). Ostatnio propaguje się poszerzenie takiego sposobu zaznajamiania się z problemami zdrowotnymi przez zalecenie go tzw. terapeutom środowiskowym, którzy są członkami tzw. „zespołów leczenia środowiskowego”. Autorzy pracy, będąc przekonani o zaletach owego „środowiskowego modelu” medycyny praktycznej opracowali metodę odnotowywania i ilustrowania danych pozyskiwanych w trakcie wizyt w domu chorego. Opracowanie przez nich narzędzie ułatwia przekazywanie i dyskutowanie danych pomiędzy pielęgniarka środowiskową, a lekarzami ośrodków podstawowej opieki zdrowotnej i specjalistami jak i pracownikami socjalnymi. Metoda doskonali ustalanie planu dalszych działań terapeutycznych i interwencji korygujących relacje psychospołeczne i warunki bytowe pacjentów.
