Background The objective of the study has been to evaluate the pathogenic bacteria contamination of touch surfaces in hospital wards. Material and Methods Samples were taken from frequently touched surfaces in the hospital environment in 13 units of various types. Culturing was carried out on solid blood agar and in growth broth (tryptic soy broth – TSB). Species identification was performed using the analytical profile index (API) biochemical testing and confirmed with matrix assisted laser desorption ionization time-flight mass spectrometry (MALDI-TOF-MS) system. Results The total of 161 samples were taken for the study. Fifty-two of them, after 24 h of culture on a solid medium, demonstrated bacterial growth and further 60 samples had growth after prior multiplication in TSB. Overall, 69.6% of samples exhibited growth of 19 bacterial species. Pathogenic species – representing indicator organisms of efficiency of hospital cleaning – was demonstrated by 21.4% of samples. Among them Acinetobacter spp., Enterocococci spp. and Staphylococcus aureus were identified. Coagulase-negative staphylococci (CNS) were predominant. The proportion of various groups of bacteria significantly varied in respective hospitals, and in various types of wards. Disturbing observation is a large proportion of resistance of isolated CNS strains as a potential reservoir of resistance genes. Conclusions The results show that touch surfaces in hospital units are contaminated by both potentially pathogenic and pathogenic bacterial species. In connection with the reported, also in Poland, frequent omission or incorrect execution of hand hygiene by hospital staff, and probably patients, touch surfaces still constitute important reservoir of pathogenic bacteria. Improving hand hygiene compliance of health-care workers with recommendations is necessary for increasing biological safety of hospital environment. Med Pr 2017;68(3):459–467

Introduction: Patients’ rights must be respected at every stage of therapy, including during biological drug therapy. For clinicians, it is key to be involved in the decision-making process with regard to the choices of medication and possible drug substitution. In Poland, the law encourages automatic drug substitution and does not recognise disparities in biological drugs. Aim: The main aim of the paper is to describe the present legal situation depicting the scope of autonomy of a hospitalised patient. Methods: An analysis was conducted of the Polish regulations, the doctrine and administrative decisions and European Medicines Agency guidance documents. Results: In Poland, patients who require therapy with advanced technologies such as biopharmaceuticals, may obtain access to a medicine within a special drug reimbursement programme in a hospital. Hospitals are supplied with the drugs necessary for drug therapy programmes via public procurement. This means that hospital procurement procedures decide which drug a patient will receive. It is not the decision of the health care provider. In view of this, the Polish Patient Ombudsman, in a decision confirmed by the Provincial Administrative Courts, pointed out that the selection of a drug for therapy should depend on current medical knowledge rather than on the result of a tender carried out by a hospital. Conclusions: Polish solutions based on the lack of an obligatory requirement to consult a substitution with a treating physician deviate from the standard practices followed in numerous EU countries and the US.

Oddziały intensywnej terapii charakteryzują się najwyższym ryzykiem wystąpienia zakażeń u pacjentów. Zapalenia płuc to jedna z ich najczęściej występujących form, obarczona wysokim ryzykiem zgonu. Dla poprawy bezpieczeństwa pacjentów wdrażane są specyficzne pakiety procedur, tzw. bundle care, obejmujące optymalne dla zapobiegania zapaleniom płuc rozwiązania. Jednak ich stosowanie wiąże się z ryzykiem przeniesienia drobnoustrojów z pacjentów na personel, co w przypadku takich patogenów jak SARS-CoV-2 może mieć poważne konsekwencje zdrowotne dla personelu. Przy ich wdrażaniu konieczne jest zatem użytkowanie środków ochrony inwazyjnej i przestrzegania odpowiednich zasad izolacji, które w dobie obecnej pandemii powinny być uzupełnione o specyficzne elementy. Praca prezentuje przegląd artykułów dotyczących optymalizacji opieki nad pacjentami i bezpieczeństwa personelu w ramach tzw. bundle care w pandemii COVID-19.
Intensive care units are characterized by the high risk of infections in patients. Pneumonia is one of the most common forms of infection with a high risk of death. Hence, to improve patient safety, specific packages of procedures, the so-called “bundle care,” are recommended by experts in the field. The usage of selected protective procedures carries the risk of transmitting microbes from patients to staff, which in the case of pathogens such as SARS-CoV-2 can have serious health consequences for staff. Therefore, medical staff of intensive care units should strictly follow recommendation concerning healthcare workers safety and the rules of isolation, which in the current pandemic should be supplemented with specific elements. The paper presents an overview of the optimization of patient care and staff safety within the so-called “bundle care” in the COVID-19 pandemic.

ObjectivesThe purpose of this research is to explore the appropriate behaviors and characteristics of Patients for Patient Safety (PFPS) against the Chinese cultural background, especially the types and internal logical relationships of behaviors of PFPS.Material and MethodsIn this research, Glazer’s methodology principle of the traditional grounded theory was adopted, and the methods of objective sampling, theoretical sampling and snowball sampling were employed. Considering the diversity of the interviewed subjects’ gender, age, professional title, qualification and demographic characteristics, representatives of hospital management staff, doctors, nurses, patients and their family members from different provinces and cities across the country were selected for semi-structured in-depth interviews to assess the behaviors of PFPS against the Chinese cultural background. In addition, some PFPS reports were collected from network media to supplement the interview data. All interviews were recorded and collated into Word text documents. Qualitative research data analysis software Nvivo 12 was used to sort out the collected data, and the theme was separated out through the strategy of substantive coding and theoretical coding.ResultsIn this research, the appropriate behaviors of PFPS were taken as the research content; 6 categories of appropriate behaviors of PFPS were separated out; and a model diagram of PFPS was constructed accordingly.ConclusionsA model diagram of the appropriate behaviors of PFPS against the Chinese cultural background was constructed to provide theoretical guidance for relevant research and practice.

Since one of the main mode of the COVID-19 transmission is through close contact with the infected person (<1.5 m distance) and body fluids (saliva, blood, aerosol particles), the dentists have become one of the groups being at most risk of being infected. The aim of this article is to present, based on the research search (publications from the last year 2020 and from the beginning of the current year 2021), the general overview of the situation during the COVID-19 pandemic with regard to patients and dentist as well as supporting personal protection. Authors identified 7 questions related to this topic. They address among others such issues as identification of the COVID-19 patients coming to the dental practice; protection of both the dentist and the patient from infection and a need for guidelines on the scope of emergency dental services during the pandemic. Opinions on the subject, expressed by the experts in the field, including general guidelines were also taken into consideration. In general, based on the literature overview, the COVID-19 pandemic has completely transformed the healthcare system, including dentistry. The identification of the COVID-19 patients coming to the dental practice has become a necessity (including taking epidemiological survey, temperature measurement). Currently, since “the group zero” in significant part has completed the full vaccination process, the health personnel (including dentists) might feel safer. The efficacy of the vaccines is high and provides a certain sense of security.

Czynnik ludzki, choć na ogół uważany za rzecz subiektywną, stanowi istotny aspekt bezpieczeństwa działań, szczególnie w obszarach, gdzie dochodzi do interakcji z drugim człowiekiem. Nie inaczej jest w anestezjologii, w której dodatkowo nakładają się jeszcze procedury wykorzystujące aparaturę kontrolno-pomiarową. Nie bez znaczenia jest tu również komunikacja wewnątrzzespołowa, zaliczana do istotnych czynników wchodzących w zakres prezentowanego pojęcia. Świadomość, jak wielką rolę odgrywają te interakcje w zapewnieniu bezpieczeństwa pacjenta, jest jednym z istotnych czynników unikania błędów, niejednokrotnie trudnych do jednoznacznego określenia.
The human factor, although generally considered subjective, is an important aspect of operational safety, especially in areas where there is interaction with other people. It is no different in anesthesiology, where additional procedures involving the use of control and measurement equipment also overlap. Not without significance is also intra-team communication, which should also be included among the important factors included in the scope of the presented concept. Awareness of how important a role these interactions play in ensuring patient safety is one of the important factors in avoiding errors that are often difficult to clearly define.

Introduction. European states observe an increased demand for healthcare services and diminishing numbers of nurses working in the healthcare system. A divergence that may result in future shortages of hospital nursing staff. Aim. Definition of factors that influence readiness to recommend the hospital as place of employment by nurses employed in Polish hospitals. Material and method. Polish part of the RN4CAST protocol, the study uses logistic regression model on data from 2605 questioners from nurses working in 30 Polish hospitals. Results. About half of the responders declared readiness to recommend employment in their hospital. The recommendation depended on working conditions, quality of care, patient safety, and manager interest in professional development of personnel. Higher level of emotional burnout reduced the probability of recommendation. Conclusions. Working conditions, quality of care, patient safety, and potential of professional development are the main factors deciding about the eagerness to recommend employment in one’s hospital.

The safety of the interior of ambulances is dubious and, in the event of sudden impact during emergency transport, potentially perilous to patients they carry. The workplace ergonomics of the interior of the passenger cabin is lacking. This article discusses an improved ergonomic interior design based on study findings, observations and subjective perception. It suggests design aspects and safety concepts aimed at increasing the safety of patients and paramedic staff inside the ambulance as a mobile workstation.

Sektor ochrony zdrowia należy do gałęzi gospodarki szczególnie wrażliwych na wysoką jakość. Dlatego też zakłady opieki zdrowotnej muszą traktować proces podwyższania jakości priorytetowo. Doskonalenie i ocena jakości nie są proste, ponieważ na jakość składa się zarówno poprawność realizacji usługi medycznej w świetle aktualnej wiedzy i przyjętych standardów, jak i wartości ważne dla pacjenta. Także formy doskonalenia jakości mogą przyjmować różnorodny kształt. Jedną z zewnętrznych form oceny pracy szpitali jest akredytacja. Głównym jej założeniem jest dobrowolna ocena na podstawie ustalonych, a przede wszystkim jawnych standardów. Pozwala na zgromadzenie danych, które dotyczą poszczególnych obszarów oraz zakresów działalności szpitala, i umożliwia wyciągnięcie wniosków z przeprowadzonego procesu akredytacyjnego, co w konsekwencji może prowadzić do poprawy jakości świadczonych usług i bezpieczeństwa pacjenta. Niniejszy artykuł poświęcony jest problemom poprawy jakości usług medycznych i bezpieczeństwa pacjenta w kontekście akredytacji szpitali w Polsce.
The healthcare system is one of the branches of economy particularly sensitive to high quality. Therefore, healthcare facilities must prioritize the process of increasing quality. Improvement and evaluation of the quality is not simple, since the quality consists of both correct implementation of medical services in the light of current knowledge and accepted standards and values which are important for the patient. Forms of improving quality can also take various shapes. One of the external forms of assessment is accreditation of hospitals. Its main objective is a voluntary assessment on the basis of beforehand determined and public standards. It allows to collect data relating to specific areas and the scope of activities of the hospital and also allows to draw conclusions from accreditation process which in turn can lead to improved quality of services and patients safety. This article is devoted to problems of improving the quality of medical services and patient safety in the context of accreditation of hospitals in Poland.

Liposomal doxorubicin is a newer form of chemotherapeutic agents that, due to its own special properties, preferably accumulates in cancer tissue. On the other hand, it shows lower affinity to cardiomyocytes and in this way is less cardiotoxic. As a result of that, there is the possibility to use liposomal form of doxorubicin until disease progression or chemotherapy intolerance in palliative setting, without treatment cessation after reaching the maximum cumulative dose of conventional doxorubicin. In this article we describe the case of a female patient diagnosed with breast cancer who was primary treated with adjuvant treatment, including chemotherapy and in whom a disease recurrence occurred after seven years of observation. As a primary palliative treatment the patient received chemotherapy based on liposomal doxorubicin and cyclofosphamide with a very good tolerance. The initial response was partial remission in lungs and in mediastinal lymph nodes. During the whole course of therapy there were no pathological changes in electrocardiogram, no signs and no symptoms of congestive heart failure, and the left ventricular ejection fraction was within normal limits.

Objective. The aim of the study was to determine the effects of pregabalin premedication on preoperative sleep quality, intraoperative haemodynamic parameters, anesthesia consumption, blood loss, and postoperative pain control in patients over 65 years of age who had undergone surgery for spinal stenosis. The study also aimed to determine whether pregabalin premedication was safe for that age group and compare its effects with those of diazepam. Materials and method. The retrospective study was conducted in a single centre after approval by the local Ethics Committee. The study sample consisted of 95 patients over 65 years of age who had undergone surgery for spinal stenosis. The sample was divided into two groups: Pregabalin premedication patients (PG; n = 45) and Diazepam patients (D; n = 51). Demographic data, American Society of Anesthesiologists (ASA) scores, comorbidities, indications for operation, pre- operative sleep quality, total anaesthesia consumption during surgery, intraoperative haemodynamic parameters and blood loss, time to leaving the post- anaesthetic care unit (Aldrete’s > 9), postoperative visual analog pain scale (VAS), total fentanyl consumption via a patient-controlled analgesia (PCA) device, additional demand for analgesic, first mobilization, first oral intake, and side-effects were recorded. Results. Participants did not significantly differ by demographic data and duration of surgery. The PG group had better sleep quality, lower systolic and diastolic blood pressure, heart rate, intraoperative bleeding, postoperative VAS scores, additional analgesic consumption, and total fentanyl consumption via a PCA, and earlier mobilization and oral intake than the D group. No serious side-effects were observed in the groups. Conclusion. Pregabalin premedication is a safe and effective alternative medication for patients over 65 years of age.

It has all started 10 years ago, at my first WON CA Europe conference in Istanbul in 2008, where I became amazed by so many general practitioners, who performed and presented their scientific work there. However it took me 7 years since the idea to start my PhD studies until it´s final completion in 2017. My PhD journey wasn´t straight, but rather twisty, with 2 interruptions. In 2011 I started to earn my basic research skills at 2 courses. The first one was the European General Practice Research Network (EGPRN) course in Nice and the second University of Crete´s research workshop in Slovakia lead by Professor Christos Lionis. The easiest part of my PhD studies was the clinical one – administering questionnaire and performing spirometry with my patients at my rural general practice. I also enjoyed teaching medical students at undergraduate as well as postgraduate level. I gave lectures at national conferences and published articles about general medicine in Slovak scientific journals, focusing on prevention, patient safety and respiratory diseases, especially COPD. I also contributed to two medical text books. My research was presented as posters or oral presentations at 3 WONCA Europe conferences, where I found a great space for sharing research ideas and results. Final results of my PhD thesis are going to be presented at Krakow conference. Even though I was a fruitful author of publications, reaching the goal of an international publication was the most difficult part for me, not achievable without a help of experienced colleague, Austrian general practitioner, Professor Gustav Kamenski.

Opis:
Ambulance vehicles play a vital role in sustaining the life of injured persons and should a provide safe transportation route to the medical institution. Transportation of injured patients in severe/critical conditions should be carried out with high caution, as there is no guarantee that patients’ health will not be harmed. The goal of this study is to minimize exposure to the external factors such as random shocks, sharp jumps, vibrations caused by irregular roads, speed breakers, weather, etc., that could influence the tasking ability of the medical team and further threaten the life of the already injured patient. This topic has not been widely researched and still requires implementation of novel standards that should improve the safety of the patient. This article aims to define the biomechanics of cabin occupant safety, introduce ways of collecting live data and develop new mechanisms that would allow safer transportation of patients without any meaningful health deterioration causing by the above-mentioned external factors. This study will identify safety hazards in the ambulance environment and determine the effectiveness of suggested countermeasures to mitigate any further injury or deterioration of the patient’s health.

Opis:
Przedmiotem artykułu uczyniono koncepcję patient safety jako normę soft law na tle konwencyjnych zobowiązań władz krajowych. W artykule przedstawiono stanowisko, że bezpieczeństwo pacjenta należy uznać za niezbędny fundament wysokiej jakości opieki zdrowotnej. Podejście władz krajowych w zakresie zapewnienia tego prawa konwencyjnego powinno opierać się na kompleksowych działaniach zapobiegawczych oraz na systematycznej analizie informacji pochodzących od różnych typów podmiotów uprawnionych: zgłoszeń, skarg i roszczeń pacjentów, jak również w oparciu o badanie zgłaszanych przez personel medyczny incydentów i wypadków. Zgodnie z zaleceniem Rec (2006)7 Komitetu Ministrów z dnia 24 maja 2006 r. w sprawie zarządzania bezpieczeństwem pacjenta i zapobiegania zdarzeniom niepożądanym w opiece zdrowotnej dla państw członkowskich istnieje prawna potrzeba ciągłej oceny bezpieczeństwa pacjentów, ciągłej poprawy bezpieczeństwa i przewidywania sytuacji, czy i kiedy nastąpi naruszenie bezpieczeństwa pacjenta. W artykule poruszono kwestę charakteru zobowiązań państwa w tym zakresie.
The article deals with the concept of patient safety as a soft law mechanism in the light of international and domestic standards. It was shown that patient safety should be recognized as a necessary foundation of health care systems, and should be based on a preventive attitude and systematic analysis and feedback from different reporting systems: patients’ reports, complaints and claims, as well as systematic reporting of incidents, including complications, by healthcare personnel. According to the Recommendation Rec(2006)7 of the Committee of Ministers on the management of patient safety and prevention of adverse events in healthcare to member states, there is a legal need to assess patient safety on an ongoing basis, implement a learning organization, demonstrate ongoing safety improvement and determine when lapses in patient safety occur. The article deals with the issue of state obligations in this regard.
