Clinical significance is a crucial element in the diagnosis of mental illness, yet, it is practically untheorised and significantly under-researched. This article takes up the question of how the criterion of clinical significance is translated into psychiatric practice. More particularly, it examines how psychiatrists account for the threshold between health and depression. The paper is anchored in the constructionist view of discourse underpinned by the assumptions of critically oriented discourse analysis. It is based upon a convenience sample of 39 semi-structured interviews with specialistpsychiatrists in south-western Poland. There is no discursive space for clinical significance in psychiatrists’ accounts. There is no boundary, no decision to be taken as to whether the symptoms are clinically significant. Depression, invariably constructed in terms of diagnostic criteria, is always represented as fully developed, appearing out of thin air, with no period of falling ill. The article raises the issue of the validity and usefulness of psychiatric diagnosis.

Objectives. To compare the level of secondary traumatic stress disorder symptoms, demographic (age) and work- -related variables (years of experience, average number of trauma patients over the past 12 months, and clinical supervision) in a sample of trauma therapists and psychiatrists working with trauma victims. Method. The study included 160 participants, 80 therapists and 80 psychiatrists, who worked with trauma victims. Participants filled out the PTSD Questionnaire: Factorial Version to assess the level of secondary traumatic stress disorder symptoms and the Berlin Social Support Scales to measure social support. They also provided demographic and work-related information. Results: The results of our study revealed higher rates of secondary traumatic stress disorder (STSD) symptoms among psychiatrists compared to therapists. We also noticed significant differences with respect to age, years of experience and actually received support between therapists and psychiatrists. Conclusions: Mental health specialists should be aware of the potentially negative impact of working with trauma victims in the form of secondary traumatic stress disorder symptoms

Podejście do problemów psychicznych człowieka można podzielić na trzy zasadnicze okresy: w pierwszym chorobami psychicznymi zajmowali się tylko duchowni, w drugim ich rola była negowana i pomijana oraz trzeci będący okresem współpracy medycyny i teologii. Przeprowadzone przez autora badania diagnostyczne ukazały potrzebę rozwijania współpracy pomiędzy lekarzami zajmującymi się zdrowiem psychicznym i kapłanami dbającymi o zdrowie duchowe człowieka. Lekarz psychiatra nie powinien zastępować duchownego, ani na odwrót. Każdy w granicach swych kompetencji powinien wypełniać rolę, do której został powołany. Kapłan powinien dbać o życie duchowe człowieka dzięki któremu nawiązuje on relację z Bogiem a psychiatra o życie „duszewne”, czyli psychofizyczne, w którym mózg i układ nerwowy kieruje wszystkimi funkcjami życiowymi ciała.
We can divide various of attitudes concerning human mental illnesses into three basic categories: firstly, only priests dealt with it; secondly, their role is denied and omitted; and thirdly, medicine and theology co-operate together. The diagnostic tests made by the author have shown the need for cooperation between doctors dealing with mental health issues and priests taking care of spiritual health. A psychiatrist should not replace a priest or vice versa. Each, within the limits of his competences, should fulfil the role for which he has a vocation. The priest should take care of a patient’s spiritual life thanks to which a person forms his relationship with God, whereas psychiatrist should care for psychophysical life, in which the brain and the nervous system direct all life functions.

Objectives The aim of this research is to present the search for psychosocial predictors explaining the strength of perceived stress in psychiatrists and the search for associations between stress and occupational burnout in psychiatrists. The authors also searched for associations between organizational, workload, family and non-professional – social factors and the intensity of occupational stress. Material and methods The group of 131 doctors – specialists in psychiatry, aged 27–86 years – took part in the study. The participants completed 3 questionnaires, i.e., the Sociodemographic Questionnaire, the Link Burnout Questionnaire (LBQ) and the Perceived Stress Scale (PSS-10). Results: The results indicated that significant predictors of the development of occupational stress for psychiatrists were psychophysical exhaustion and lack of a sense of professional efficacy. Taking holidays (several times a year, pursuing the passions and interests), and having a stable family relationship (marriage or civil partnership) were also prominent among the predictors of stress. Conclusions Occupational stress and burnout in the work of the surveyed psychiatrists appeared to be a high intensity phenomenon. These results indicate the need to spread preventive health care also in the work of psychiatrists. Mainly in terms of maintaining the right balance between work and rest, which can help to reduce stress levels and protect against further development of burnout syndrome. Family resources such as a stable marriage or partnership may also be a protective factor against the build-up of stress and burnout in psychiatrists (mainly female). In addition, professional resources (work experience measured by number of years of work) is also important in explaining stress levels in psychiatrists.
