Objectives: Evaluation of the allergenic properties of the metal knee or hip joint implants 24 months post surgery and assessment of the relation between allergy to metals and metal implants failure. Materials and Methods: The study was conducted in two stages. Stage I (pre-implantation) - 60 patients scheduled for arthroplasty surgery. Personal interview, dermatological examination and patch testing with 0.5% potassium dichromate, 1.0% cobalt chloride, 5.0% nickel sulfate, 2.0% copper sulfate, 2.0% palladium chloride, 100% aluminum, 1% vanadium chloride, 5% vanadium, 10% titanium oxide, 5% molybdenum and 1% ammonium molybdate tetrahydrate were performed. Stage II (post-surgery) - 48 subjects participated in the same procedures as those conducted in Stage I. Results: Stage I - symptoms of "metal dermatitis" were found in 21.7% of the subjects: 27.9% of the females, 5.9% of the males. Positive patch test results were found in 21.7% of the participants, namely to: nickel (20.0%); palladium (13.3%); cobalt (10.0%); and chromium (5.9%). The allergy to metals was confi rmed by patch testing in 84.6% of the subjects with a history of metal dermatitis. Stage II - 10.4% of the participants complained about implant intolerance, 4.2% of the examined persons reported skin lesions. Contact allergy to metals was found in 25.0% of the patients: nickel 20.8%, palladium 10.4%, cobalt 16.7%, chromium 8.3%, vanadium 2.1% Positive post-surgery patch tests results were observed in 10.4% of the patients. The statistical analysis of the pre- and post-surgery patch tests results showed that chromium and cobalt can be allergenic in implants. Conclusions: Metal orthopedic implants may be the primary cause of allergies. that may lead to implant failure. Patch tests screening should be obligatory prior to providing implants to patients reporting symptoms of metal dermatitis. People with confi rmed allergies to metals should be provided with implants free from allergenic metals.

The purpose of this study was to assess miners’ perceptions about the safety climate of their workplace. To achieve the research aim, the relationship between demographic characteristics and occupational hazards was first determined, and finally, the relationship of safety climate with occupational hazards and health-seeking behavior was discovered. The data was collected through a self-reporting questionnaire. The results revealed that the subjects have to deal with severe occupational hazards, and they possess poor health-seeking behavior. A safety climate assessment showed that only one of the seven dimensions (i.e. safety communication, learning, and trust in co-worker safety competence) was at a satisfactory level. With respect to the first objective, we found that age, education, and experience were statistically significant with occupational hazards, while marital status had no significant impact on occupational hazards. Regarding the second objective, three dimensions (management safety justice, safety communication, learning, and trust in co-worker safety competence, and workers’ trust in the efficacy of safety systems) were significant predictors of occupational hazards. The study reflects that workers’ participation is the main factor in setting up an adequate safety climate within the organization. Suggestions provided in this study could provide useful information to managers and safety practitioners to improve safety performance and promote the safety climate in the organization.

BackgroundReaching an appropriate physical activity level (PAL) in adolescence is an important public health problem. This study aimed to evaluate factors associated with PAL and changes in PAL in late adolescence.Material and MethodsThe sample involved 411 adolescents (201 girls) who were 16 years of age at study baseline; the participants completed a structured validated questionnaire at baseline (the beginning of the third grade of high school) and again at follow-up (the end of the fourth grade; when they were 18 years of age). Variables were obtained at both testing waves and included the following predictors: sociodemographic indices (sex, socioeconomic status, parental education), sports factors (participation in individual and team sports, competitive sports achievement, experience in sports), and PAL (the primary outcome), which was measured using the Physical Activity Questionnaire for Adolescents.ResultsA significant decrease in PAL between baseline and follow-up testing was evidenced (t-test = 6.17, p < 0.001). A logistic regression model calculated with a dichotomized outcome (normal PAL vs. low PAL), and sex as a covariate, showed a significant influence of participation in team sports and maternal education on PAL, both at baseline (OR = 1.56, 95% CI: 1.11–1.87; OR = 1.38, 95% CI: 1.02–1.90) and at follow-up (OR = 1.45, 95% CI: 1.01–1.90; OR = 1.35, 95% CI: 1.08–1.70, for team sports and maternal education, respectively).ConclusionsThe study confirmed certain associations between the studied variables and PAL, but there was no significant influence of the observed indicators on changes in PAL in late adolescence. Introduction and objective. Musculoskeletal disorders are frequently met in dentistry. Objectives. To show the efficiency of rehabilitation and to make correlations among patients’ pain levels, their overall health status, and the number of days of work absenteeism. Materials and method. A total of 390 dentists diagnosed with low back pain, scapulohumeral periarthritis, cervicobrachial neuralgia, hand osteoarthritis, tendinitis or tenosynovitis of the upper limb, carpal tunnel syndrome, spinal deformities and fibromyalgia, were followed in a 2-year prospective study. For each ailment the patients were divided into two groups. Group 1 followed both medical and rehabilitation treatment, while group 2 followed medical treatment. The patients were assessed by the visual analogue scale (VAS), the Health Assessment Questionnaire adapted for Dentists (HAQD) and the number of days of absenteeism. Results. VAS scores did not significantly differ between the two groups at the beginning of the study but were significantly lower at final assessment. HAQD scores were significantly lower at one-year and two-year assessments in Group 1. The number of days of absenteeism did not differ significantly between the two groups at the initial assessment. Nevertheless, the number of days of absenteeism was significantly higher for Group 2 patients at the end of the study. For increased values of the visual analogue scale at the beginning and at the end of the study, the significantly increased numbers of days of absenteeism and of health assessment questionnaire scores were associated. Conclusions: Improvements of functional parameters and increase in work productivity were recorded in dentists who followed physical therapy.

Background: Cystic fibrosis(CF),despite much progress in therapy, remains the disease which affects nutrition. Nutrition is an important prognostic factor of the outcome of the disease. We want to evaluate physical development, nutrition and body composition in CF children. Material and methods: 75 children diagnosed with CF (9 months to 18 years old) were included into the study. 33 healthy children (9 months to 18 years old) constituted the control group. The study consisted of 2 stages. In the first the differences between groups were investigated. The second, took place a year later. At each time point the following measurements were performed: height, body mass, skin fold, arm circumference; BMI, FFM%, FM% and Frisancho index. FFM(fat free mass), FM(fat mass), muscle mass, TBW(total body water) were evaluated by mans of BIA(bioimpedance). Results: CF children were shorter than healthy children. Stunting affected 18,67% of CF patients at first examination and 21,6% a year later. Underweight was diagnosed in 28% of patients at the beginning and in 41.2% a year after. Underweight was the result of both little FM and scarce muscle mass. Conclusions: Many children with cystic fibrosis suffers from short stature and underweight, which progresses within time. FFM decreases with the disease progress

Introduction Research into work reintegration following invasive cardiac procedures is limited. The aim of this prospective study was to explore predictors of job satisfaction among cardiac patients who have returned to work after cardiac rehabilitation (CR). Material and methods The study population consisted of 90 cardiac patients who have recently been treated with coronary angioplasty or heart surgery. They were evaluated during their CR and 12 months after the discharge using validated self-report questionnaires measuring job satisfaction, work stress-related factors, emotional distress and illness perception. Information on socio-demographic, medical and occupational factors has also been collected. Results After adjusting for demographic, occupational and medical variables, baseline job satisfaction (p < 0.001), depression (p < 0.01) and ambition (p < 0.05) turned out to be independent, significant predictors of job satisfaction following return to work (RTW). Patients who had a partial RTW were more satisfied with their job than those who had a full RTW, controlling for baseline job satisfaction. Conclusions These findings recommend an early assessment of patients’ psychosocial work environment and emotional distress, with particular emphasis on job satisfaction and depressive symptoms, in order to promote satisfying and healthy RTW after cardiac interventions.

Multiple therapeutic approaches of the treatment of pilonidal sinuses have been described in the literature, but there are still controversies and lack of standardization. Vacuum Assisted Closure (VAC) therapy has potential beneficial effect on the wound healing after the sinus resection. The aim of the study was to analyze the results of VAC therapy in the treatment of pilonidal sinuses. Material and methods. After randomization in the control group (9 men) the simple excision of the pilonidal cyst was performed with the standard wound dressing. In the VAC group (10 men) the same surgical procedure was performed, but after the excision the VAC dressing with mobile VAC Freedom device was used. Both groups were treated in an outpatient setting under local anesthesia. The wound size, time of surgery, time of wound healing time of recovery and pain after the surgery (VAS score) were compared. Results. In VAC treated group the wound size and time of surgery were similar to control group. Time of wound healing, recovery and the pain after surgery in days 4-7 were reduced in comparison to the standard treated group. Conclusions. VAC therapy can be easily used in an outpatient setting, mobile device is highly accepted, operation of the equipment is simple. VAC therapy significantly decreases the time of wound healing and absenteeism from work as well as the postoperative late pain.

Objective: The purpose of this study was to compare the outcome of the great saphenous vein (GSV) sparing versus stripping during Trendelenburg operation for varicose veins. Methods: This was a prospective randomized study of primary varicose vein patients who underwent Trendelenburg operation. Data of patients operated on over a period of 16 months was collected, including: below knee GSV diameter by Duplex Ultrasound and revised venous clinical severity score (rVCSS), calculated preoperatively and postoperatively at 2nd, 4th, and 8th week. Results: A total of 36 patients undergoing Trendelenburg operation were included in the study. Nineteen patients underwent GSV sparing while 17 underwent stripping of GSV till just below the knee after juxtafemoral flush ligation of the great saphenous vein. There was a significant decrease in the below-knee GSV diameter (19% after 2 months) and rVCSS (60.8%) in the sparing group. The stripping group also showed an almost similar decrease in below-knee GSV diameter (19.6% after 2 months) and rVCSS (66.3%). However, no significant difference was found between the two groups in terms of change in GSV diameter (P = 0.467) and rVCSS (P = 0.781). Conclusion: Trendelenburg procedure with sparing of GSV can be done routinely for operative management of varicose veins, where surgery is needed.

BackgroundInsufficient physical activity levels (PAL) during adolescence is a major public health concern, which is even more pronounced during the COVID-19 pandemic due to restricting movement opportunities. This study aimed to identify PAL changes and examine the age-specific determinants of PAL in younger and older adolescents during the COVID-19 lockdown.Material and MethodsThis study included 859 high-school students from Bosnia and Herzegovina stratified into 2 age groups: younger (N = 420, 14–16 years of age), and older adolescents (N = 439, 16–18 years of age). Participants were tested over 2 testing waves: before the COVID-19 lockdown (January 2020) and during the COVID-19 lockdown (April 2020). Variables included PAL assessed by the Physical Activity Questionnaire for Adolescents, sports factors, and parental factors.ResultsThe PAL significantly declined as a result of lockdown measures in the total sample (from 2.76±0.79 to 2.50±0.82). Larger absolute and relative decline of PAL was evidenced in younger adolescents. Sport participation positively influenced PAL before lockdown, with no significant influence during the lockdown. Older adolescents whose mothers were better educated were less likely to be in high risk group with regard to a large decline of PAL as a result of COVID lockdown (OR = 0.50, 95% CI: 0.21–0.84).ConclusionsResults of the study suggest that parental education influences health-related behaviors and that parental education is a protective factor against a decrease in PAL during the COVID-19 pandemic. Main educational agents (i.e., school and parents) should pay more attention to provide children and adolescents adequate information and develop their health literacy, which will hopefully positively impact children’s PAL even in challenging situations similar to COVID-19 lockdown. An organizational change among 82 postal workers was studied with the aim of evaluating the effects on the work environment, work ability, and musculoskeletal complaints. The study was undertaken in 2 suburbs of Stockholm, Sweden. Psychological work demands were estimated to be reduced at the 1-year follow-up but physical work demands had changed very little. In an observation study in a subgroup of older workers, the risk of overexertion at work and musculoskeletal complaints was reduced. In spite of that, most of the older participants (>35 years) had unchanged or increased musculoskeletal symptoms. This shows the need for early preventive measures.

Introduction: Treatment of the pilonidal sinus, due to various surgical methods, remains the current topic of discussion during surgeon meetings worldwide. The newest methods of treatment consist in the excision and simultaneous closure of the wound with cleft lift procedures. One of such methods is Bascom II procedure which has been widely used in the world but is less popular in Poland. Aim of the study: To evaluate the results of treatment of pilonidal cysts using Bascom II procedure performed by one operator. Material and method: 50 patients (40 men, 10 women), Avg. 30.6 years of age. All patients treated with Bascom II procedure in one surgical ward by one operator (resident during specialisation training) with the use of uniform surgical care for all operational protocols. The following were assessed: BMI, average hospitalisation time, pain, post-operative complications, wound healing time, patient’s quality of life, and the recurrences rate after surgery. The follow-up period ranged from 12 to 52 months. Results: Average BMI 27.13 kg / m2. Avg. time of hospitalisation 2.95 days. Post-operative pain in the first 24 hours was on average 4.55 points. (+/- 2.24 points); on the 10th day on average 2.04 points (+/- 1.58 points); in the 30th day on average 0.76 points (+/- 1.1 points). Pain after healing 0.14 points (+/- 0.40 points). Post-oerative complications occurred in 28.57% of patients [partial wound dehiscence (16.32%), serum leak (10.2%), hematoma (6.12%), total wound dehiscence (0%), wound infection (0%)]. The average time of full healing was 2.94 weeks. Recurrence occurred in one patient (2.04%). Conclusions: Bascom II procedure is characterised by a low recurrence rate and short wound healing time. It is a a safe, effective and patient-accepted method of treatment. Statistically, it significantly improves the patient’s quality of life one year after surgery in terms of both physical and mental health.

The Vulnerable Elders Survey (VES-13) is a simple function based frailty screening tool that can be also administered by the nonclinical personnel within 5 minutes and has been validated in the out- and in patient clinic and acute medical care settings. The aim of the study was to validate theaccuracy of the VES-13 screening method for predicting the frailty syndrome based on a CGA in polish surgical patients. Material and methods. We included prospectively 106 consecutive patients ≥65, that qualify for abdominal surgery (both due to oncological and benign reasons), at the tertiary referral hospital.We evaluated the diagnostic performance of VES-13 score comparing to the results from the CGA, accepted as the gold standard for identifying at risk frail elderly patients. Results. The prevalence of frailty as diagnosed by CGA was 59.4%. There was significantly higher number of frail patients in the oncological group (78% vs. 31%; p<0.01). According to the frailty screening methods, the frailty prevalence was 45.3%. The VES-13 score had a 60% sensitivity and 78% specificity in detecting frailty syndrome. The positive and negative predictive value was 81% and 57%, respectively. The overall predictive capacity was intermediate (AUC=0.69) Conclusions. At present, the VES-13 screening tool for older patients cannot replace the comprehensive geriatric assessment; this is due to the insufficient discriminative power to select patients for further assessment. It might be helpful in a busy clinical practice and in facilities that do not have trained personal for geriatric assessment.

Introduction and objective. Low levels of serum vitamin D have been associated with increased occurrence of ischemic heart disease (IHD), but findings are inconsistent, e.g. seasonal variation is not always considered in the analyses. The aim of this study was to investigate whether vitamin D is associated with IHD among rural middle-aged men in Sweden when seasonal variation is taken into account. Materials and method. A nested case-control study was performed within a population-based cohort of Swedish farmers and rural residents followed for 20 years. Outcomes were from national health registers. For 276 cases (IHD at any time during follow-up), and for 276 aged-matched controls free of cardiovascular diseases or diabetes, serum levels of vitamin D (25-hydroxyvitamin D3) were analyzed with liquid chromatography/tandem mass spectrometry (LC/MS/MS), in stored blood samples drawn at the baseline survey in 1990–91. The odds ratio (OR) with 95% confidence intervals (CI) of IHD for vitamin D-quartiles (based on measured as well as adjusted for month of sampling) was evaluated in logistic regression models; potential confounders were taken into account. Results. The measured mean vitamin D levels were 27.6 ng/ml(standard deviation (sd)8.2 ng/ml) and 28.8 ng/ml (sd 9.0) among cases and controls, respectively. Compared to the quartile with the lowest month-adjusted vitamin D levels, the risk of IHD was significantly lower in the highest quartile (OR=0.46, 95% CI 0.25–0.84, p=0.012) in the fully-adjusted model. Conclusion. Rural Swedish men in the highest quartile of serum vitamin D had halved relative risk of IHD, compared to those in the lowest quartile.

This study examined relationships between the components of physical fitness and athletic performance in youth pole-vaulters in a secondary school setting. Twenty-five adolescents of both genders, between the ages of 15 and 18, participated in the study. This case study was grounded on qualitative and quantitative approaches of data analysis, measuring physical abilities self-assessment based on the questionnaire developed by Borg et al. [11] and measuring high-performance effect linking age and gender with the levels of the pole vault performance. The results indicate that, overall, the six dimensions of fitness (coordination, strength, flexibility, speed, endurance, and body self-image) are correlated with physical fitness (r varies between 0.29 and 0.65, p<0.05). The significance is very high in teenage boys for perceived strength and perceived endurance respectively (r=0.61, p <0.001 and r=0.65, p<0.001), whereas in teenage girls global physical fitness is highly correlated with endurance (r=0.63, p<0.001). Moreover, while endurance and flexibility have become a determinant of performance in the girl group in the perceived physical fitness category, strength and perceived speed are the most important components in the boy group. An effect of age was only considered for strength, endurance and perceived fitness. No effect of body self-image on pole vault performance was identified, with non-significant correlation in the two genders, girls and boys respectively (r=0.11, r=0.16). The significant role of physical fitness levels in determining motor exercise performance in adolescence may have further implications for gender roles and talent prospecting as well as potential physical condition benefits for exploring new sports.
