Opis:
Head and neck cancers constitute the sixth most common malignant tumours worldwide, and are one of the majors problems of global public health. In the oral cavity there are several types of oral cancers, but around 90% are squamous cell carcinoma. Many different risk factors play a role in the etiology of head and neck cancer. The aim of this study is presentation from the aspect of molecular, viral and bacterial infection. Knowledge about all factors which influence the development of these malignances is essential for diagnostics and successful treatment.

Opis:
Methylation of specific lysine residues in histone tails has been proposed to function as a stable epigenetic marker that directs biological functions altering chromatin structure. Recent findings have implicated alteration in heterochromatin formation as a contributing factor in cancer development. In order to verify whether changes in the overall level of H3K4 histone methylation could be involved in oral squamous carcinoma, the levels of H3K4me1, me2 and me3 were measured in oral squamous carcinoma, leukoplakias and normal tissues. The levels of H3K4me2 and me3 were significantly different in oral squamous cell carcinoma in comparison with normal tissue: the level of H3K4me2 was increased while that of H3K4me3 decreased. No significant differences could be found between the two types of tissues in the level of H3K4me1. A similar trend was found in the leukoplakias that appeared more like the pathological than normal tissue. These results support the idea that alteration of chromatin structure could contribute to oncogenic potential.

Opis:
Purpose of the study. To evaluate the impact of tumour location, local and regional advancement, histological differentiation, status of the surgical margins and radiotherapy on the disease-free time and overall survival rates in patients with oral squamous cell carcinoma. Material and methods. A retrospective analysis of 67 patients treated with surgery (61 pts.), radiotherapy (6 pts.) and their combination (28 pts.). Follow time on average 40 months. The probabilities of survival were assessed using the Kaplan-Meier estimates, the differences were calculated with the log-rank test. An analysis of the infl uence of the neck recurrences on the prognosis was additionally performed. Relationship between independent categorical variables as: primary local advancement, location of the tumour, histological grading and lymph node metastases was evaluated with Fisher´s Exact Test. Results. Disease-free time rate amounted to 40,1%. There was no independent prognostic importance of primary location, T-staging and N-staging, histological grading of the tumour or radiation on disease-free time, just opposite to the status of the resection margins. However, the number of neck metastases was directly proportional to the tumour dimension and poor histological differentiation. Overall survival rate amounted to 87,5%. Posterior location in the oral cavity, involvement of cervical lymph nodes, surgical margins with the presence of tumour cells, poor histological differentiation and necessity of irradiation negatively correlated with the survival. Conclusions. A complete resection of the tumour was the most important independent prognostic parameter for the disease-free and overall survivals in oral squamous cell carcinomas in this study. An adjuvant radiation therapy could improve the results of treatment of oral squamous cell carcinoma also in cases were so far considered only for surgical management.

Introduction and aim. Tumour necrosis factor-alpha (TNF-α) belongs to the cytokine family TNF/TNFR. As a multifunctional cytokine, TNF-α plays a significant role in diverse and a variety of cellular events such as cell survival, proliferation, differentiation, and death. As a pro-inflammatory cytokine, TNF-α acts as a bridge between inflammation and carcinogenesis. Receptor for advanced glycation end products (RAGE) are cellular receptors belonging to the immunoglobulin superfamily. As one of the primary mediators of innate immunity, acute and chronic inflammatory disorders, and certain cancers, RAGE signaling plays an important role. The aim of the present study is to analyze the prognostic significance of salivary TNF-α and RAGE in oral squamous cell carcinoma. Material and methods. A study was conducted testing saliva samples collected from ten patients with well-differentiated and moderately differentiated oral squamous cell carcinomas. To determine the levels of TNF-α and RAGE in unstimulated saliva from patients, an ELISA kit from RAY BIOTECH was used for the study, and the readings were read at 450 nm. Statistical analysis was performed using SPSS software. Version 23 of SPSS was used to plot the standard curve. Statistical comparisons were done using Mann-Whitney U test and ROC analysis. Results. Salivary TNF-α and RAGE in patients were considered to be induced by radiotherapy at a higher level in moderately differentiated squamous cell carcinoma when compared to well differentiated squamous cell carcinoma. Thus, there is an increase in the induced Salivary TNF-α and RAGE levels by radiotherapy with increase in the histological stages of oral squamous cell carcinoma. The statistical analysis also proved the same. Conclusion. Hence salivary TNF-α and RAGE may be used as a biomarker for oral cancer to predict the prognosis.

Introduction and aim. Angiogenesis, which is accomplished by capillary sprouting, is the process by which new vessels are created from pre-existing ones. In tumor, once their initial blood supply is depleted, a tumour is unable to grow without additional blood flow. Additionally, a tumor’s microvasculature, or microvessel density (MVD), increases along with its capacity to produce angiogenesis. We aimed to observe the relationship between the expression of vascular endothelial growth factor (VEGF) and MVD (using CD34) in oral squamous cell carcinoma (OSCC). Material and methods. The expression of VEGF and CD34 antibodies was analysed using immunohistochemistry method on 50 cases of histopathologically proved OSCC. The expression was correlated with clinicopathological parameters. Results. A significant correlation was observed between VEGF expression and gender, LVSI. No correlation between any other factors and the difference in VEGF expression was statistically significant. Similarly, the MVD expression was not found to be statistically significant in any of the pathological parameters. Conclusion. VEGF positivity as well as MVD were found to be independent of the tumor pathology. Tumor MVD was found to be independent of the expression of VEGF. Further studies in a larger study group may establish a significant association so that antiangiogenic targeted therapy may be initiated.

The aim of the study was to assess the expression of MGMT, p16, and APC genes in tumors and matching surgical margin samples from 56 patients with primary OSCC. We also analyzed the association of the clinical variables with the expression of the studied genes. After RNA isolation and cDNA synthesis gene expression levels were assessed by quantitative reverse transcription (qRT)-PCR. Two-sided parametrical Student's t-test for independent groups with equal/unequal variances showed no statistically significant differences in genes' expression in tumor compared to margin samples. No association was found between the genes' expression and clinical parameters, except for MGMT, whose low expression was probably associated with smoking (0.87 vs 1.34, p=0.065). 'Field cancerization' is an area with genetically or epigenetically altered cells and at the same time a risk factor for cancer. Disturbances in gene expression could also be the source of damages leading to cancerization. In conclusion, it is important to mention that the field remaining after a surgery may pose an increased risk of cancer development. It may be suggested that the diagnosis and treatment of cancers should not be concentrated only on the tumor itself, but also on the cancer field effect. Therefore, further molecular analysis on surgical margins and additional research regarding their assessment are required.

Aim: The aim of this study is to demonstrate the possible correlation between the expression of examined protein markers - p53, EGFR, PCNA, p44/42 in the mass of the tumor and the clinical stage of disease. Material: 48 patients of the Department and Clinic of Oral and Maxillofacial Surgery, Lublin diagnosed with oral cancer. The control group consisted of 10 patients diagnosed with leukoplakia lesions in the oral cavity. The methods: Immunohistochemical analysis using the detection system DAKO K5007 Cat - Dako REAL ™ Detection System, Peroxidase DAB +, Rabbit / Mouse. Results: Based upon the statistical results, significant correlation between p53 protein and tumor staging; however, a correlation between the level of expression of EGFR, p44/42, PCNA and staging was not likewise revealed. Conclusions: Looking for oral squamous cell carcinoma markers remains an actual issue. Identification of specific markers of oral cancer could be used in screening the population, determining prognosis and response to treatment.
