Intraoral irradiation, the treatment choice for well defined oral-cavity tumors, is done using intraoral cone (IOC) systems. In this study, an IOC system was developed for a Neptun 10PC linac. Beam parameters necessary to plan an intraoral electron treatment were evaluated for two applicators, a flat and a beveled end. Measurements were performed using a Scanditronix (p-Si) diode field detector in a Scanditronix (RFAplus) 3-D (three-dimensional) water phantom. Percent depth dose distributions, beam profiles, and leakage dose distributions for the developed cone system are presented.

Introduction: The aim of the study was to analyse the risk factors for oral cavity and oropharynx cancer in peopled examined under the Head and Neck Cancer Awareness Week in 2016, Lodz. Material and methods. In Lodz, 21st September 2016, under the Head and Neck Cancer Awareness Week, 106 people, including 67 women aged 29–77 and 39 men aged 23–84, underwent preventive examinations in the hospital department. Prior to the laryngological examination, the patients were asked to answer questions that referred to their education, medical case history, symptoms, smoking habits with the number of cigarettes per day, alcohol intake, the number of lifetime sexual partners, oral sex engagement, incidents of head and neck cancer in the family history. Results: The major part of the examined patients were women and men with the secondary and high level of education, 47,76% and 35,82%, and 58,97% and 35,91% respectively. The patients were informed by mass media about the planned preventive medical examinations – 80,60% women and 79,49% men. The most common symptoms reported by women were: hoarse voice in 61,19% cases, dysphagia in 32,84% cases and burning sensation and/or pain in the oral cavity in 29,85% cases. The examined male patients mainly showed hoarse voice (46,15%), other symptoms (43,59%) and dysphagia (25,64%). 28,35% women and 28,20% men smoked cigarettes, while passive smokers were 22,38% and 25,64% respectively. Alcohol consumption was reported by 67,16% women and 82,05% men, rather occasionally. Having oral sex was noted in 25,37% women and 38,46% men, mostly with multiple sexual partners. Among the studied patients, 13,43% women and 5,12% men suffered from malignant cancer, including 2,98% women and 2,56% men who reported head and neck carcinoma in the medical interview. On the basis of the interview and ENT examination, 11,94% women and 17,94% men were qualified for the extended oncological diagnostics. Conclussion. The Fourth Head and Neck Cancer Awareness Week shows the increased interest in preventive screening, especially oncological screening, and thus the necessity of such preventive activities in the future.

Opis:
Fungi, mainly yeast-like fungi of the Candida genus, and especially the species Candida albicans, occupy an important place among microorganisms that colonize the oral cavity. The aim of the paper was to assess the susceptibility of different strains to some antifungal drugs. The study material consisted of swab samples taken from the buccal mucosa of 376 healthy individuals of both genders, from neonate to18 years. C. albicans strains were isolated in 79 persons. To evaluate drug resistance phenotypes of the isolated C. albicans strains FungitestTM (Bio-Rad) was used. The study showed that in different age groups the percentage of the tested C. albicans isolates susceptible to all the tested azole drugs varied from 76% to 95%. Drug resistant strains were found only in the eldest age group (4.8%). Phenotypic diversity (drug resistance types) of the studied isolates indicates a wide range of adaptation possibilities enabling colonization of various ecological niches in the oral cavity ecosystem. The presence of C. albicans strains intermediate or resistant to azole drugs among the tested isolates may be due to overuse of that drug group in general therapy and in the oral cavity treatment.

Introduction. Among the microorganisms colonizing the oral cavity ontocenosis, an important role is played by fungi: mainly yeast-like fungi of the Candida genus, and especially by the species C. albicans. Objective. The aim of this study was to evaluate the frequency of oral colonization by Candida species in healthy individuals of different age groups, and to identify the species of the isolates. Materials and method. The fungi were isolated from the oral cavities of 654 healthy individuals of both genders, representing different age groups, and the species of the isolates were determined. Initial identification of the yeast-like fungi was made based on the macroscopic appearance of the colonies on Sabouraud’s medium, and the growth of coloured colonies on ChromAgar Candida medium. API 20 C AUX microtest (bioMerieux) was used to identify the most frequently found species of Candida spp. Results. The frequency of Candida isolation from the oral cavity ontocenosis in healthy individuals of different age groups was assessed as 30.6%. The oral cavity ontocenosis was colonized mainly by the yeast-like fungi of C. albicans species: they were found in 24.5% of the tested population, while the frequency of oral cavity colonization by non-albicans Candida spp. was 6.1%. Seven species of non-albicans Candida spp. were identified: C. glabrata, C. inconspicua, C. famata, C. tropicalis, C. parapsilosis, C. lusitaniae, and C. kefyr. Conclusions. In the Polish population, 30.6% of the oral cavity ontocenosis is colonized by Candida yeast-like fungi, while C. albicans is the fungus that colonizes oral cavity ontocenosis most frequently. The frequency of oral cavity ontocenosis colonization with Candida spp (including C. albicans) increases with age.

Introduction. Metastatic renal cell carcinoma can spread from the primary renal cell carcinoma to other organs, practically to all tissues. Distant metastases – to the tissue of the head and neck – are transferred through the blood and lymph vessels. A case of such a metastasis localized on the palate is relatively rare and therefore poses interesting material for description. Objective. Analysis of a case of cell renal carcinoma meastasis of the palate in a 67-year-old patient, based on clinical observations and medical publications. Conclusion. Clear cell carcinoma metastases to distant organs are often the first sign of a covertly progressing disease of which the primary tumour is localized in the kidney. The treatment is surgical, and prognosis is poor in the case of distant metastases since tumours recur even after a few years.

Opis:
Foods rich in sugar are an excellent substrate for the microorganisms that inhabit the initial sections of the gastrointestinal tract, and one of the most commonly available sources of sugar is the sweetened drink. Students represent an interesting sub-population; the large number of classes and associated stress levels promote fixing of unhealthy behaviors, e.g. tendency to consume a lot of sweetened drinks, for example cola-type or energetic drinks. Aim of this study was to determine the relationship between the amount of sugar consumed in beverages and the prevalence of fungi in the oral cavity. The investigated material consisted of oral washings. Participants completed original questionnaire regarding beverages consumed. The relationship between the consumption of sweetened beverages and risk of the presence of fungi in the oral cavity was determined. Fungi were isolated from 68.1% of examined subjects. Seven species of the genus Candida were observed. Higher prevalence of fungi was seen in the oral cavity of subjects who declared consumption of beverages containing sugar. 37.8% of respondents were found to consume with beverages doses of sugar exceeding the recommended daily requirement. Significantly greater prevalence of oral cavity fungi was noted in those exceeding the recommended GDA (76.3%), compared to of those who were not (68.7%). There were positive correlations between occurrence of fungi and consumption of sweetened carbonated drinks or adding sugar to coffee and tea. The addition of sugar to coffee/tea and sugar consumption above the recommended daily amount significantly increases the risk of colonization of the oral cavity by fungi. Students, due to invalid nutritional habits especially excessive consumption of beverages containing large amounts of sugar, belong to a group with a predisposition to the occurrence of fungi in the oral cavity.

Opis:
The occurrence of trichomonads in oral cavity of HIV patients is not well known. HIV patients often suffer from oral lesions (candidiosis, advanced caries) and it remains unclear if any oral parasites can affect that, therefore the aim of the study was verification of species that can occur in HIV patients” oral cavity. Diagnosis of oral trichomonads can be performed by conventional methods (microscopic observation of wet and stained preparations and cultivation) but these are time consuming and insufficient for proper species differentiation, therefore in order to detect and identify species of parasites precisely, molecular methods such as PCR (Polymerase Chain Reaction) and sequencing of its product, were applied. 54 HIV patients (18 females and 36 males at the age of 20-54) were examined. All of them were addicted to intravenous drugs for at least 6 years. Saliva, smears and spittle samples were collected and used for cultivation, preparations and molecular diagnosis. For PCR amplification a pair of primers (T1 and T2) specific for ITS 1 — 5.8 SrRNA — ITS 2 region was designed. The oral trichomonads were detected in saliva samples of 3 HIV patients; these were males at the age of 25, 27 and 44. The identification of species by PCR and sequencing of the PCR products showed the trichomonads belonging to Trichomonas tenax. Infection of HIV patients’ oral cavity caused by T. tenax is rather related with inflammatory processes than with the immunosuppression of these patients but should be considered as a potential factor in pathogenesis of oral disorders in immunosuppressed patients.

Opis:
Cases of fungal infections are being encountered more often in clinical practice. The factors associated with a high risk of mycoses include, among others, corticosteroidotherapy, the administration antibiotics with wide spectrum of antibacterial properties, neutropenia, neoplasms. Fungi may play a role in cancer formation, may act as a complication in the course of treatment, and may mimic a neoplastic process by giving a similar clinical picture. In the case of fungal throat infection, patients complain of increased body temperature, a general feeling of weakness, malaise, headache, spontaneous pain intensifying during swallowing, a feeling of an obstacle in the throat or a cough. A physical examination may reveal congestion of the mucosa followed by a unilateral crater ulceration often covered with fat, as well as a thick coating, which is accompanied by foetor ex ore. The submandibular and neck lymph nodes are often greatly enlarged and painful. These symptoms may resemble those associated with the neoplastic process and changes in the course of systemic diseases (agranulocytosis). A correct diagnosis in these cases is necessary for adequate therapy. Chronic lymphocytic leukemia (CLL) is the most common type of leukemia among adults in Europe and North America. It is estimated that in Poland, CLL affects approximately 1,400 people per year. In this paper, a case of 62-years old patient with CLL with fungal infection of oral cavity and throat is presented.
