Introduction: Abdominal pain requires rapid diagnosis and treatment, especially in emergency circumstances. Sometimes the diagnosis of the disease cannot be accomplished with laboratory and imaging methods, and an invasive procedure such as diagnostic laparoscopy may be required to obtain a diagnosis. Although diagnostic laparoscopy can be performed for postdiagnosis treatment purposes, laparotomy is inevitable in some cases. Aim: The aim of the study is to evaluate the role of diagnostic laparoscopy in diagnosis and treatment and to retrospectively examine the factors that force the surgeon to perform a laparotomy. Material and methods: Patients over the age of 18 who underwent diagnostic laparoscopy in the general surgery clinic of Sakarya University Training and Research Hospital between January 2013 and December 2019 were retrospectively evaluated. Patients under 18 years of age and patients diagnosed before surgery were excluded. Demographic data of the patients, whether there was a conversion from laparoscopy to laparotomy, postoperative morbidity, and mortality were recorded. Results: The data of 347 patients in total were evaluated retrospectively between the specified dates. As many as 216 of the patients were previously diagnosed, with laparoscopic procedures performed for staging purposes and they were not included in the study. The remaining 131 patients were included in the study. Sixty-eight patients were women and 63 were men. In total 79.4% of the patients had diagnostic laparoscopies performed due to emergency circumstances, and 20.6% for abdominal pain evaluation. While the procedure was concluded laparoscopically in 64.9% of the patients, the operation was continued by performing laparotomy in 35.1%. Conclusion: Despite the increase in the variety and frequency of imaging modalities used, laparoscopic intervention is an essential approach in both diagnosis and treatment when the diagnosis is doubtful, especially in appropriate emergency cases.

Major Vascular injury during laparoscopy is most deadly complication of laparoscopy. This report is review major vascular injuries based on surgeon’s relation and literature. The incidence of MVI is 0,04–0,1%. Extremely important is to learn proper technique of insuflation. According to patient’s physique surgeon should consider best technique of insuflation, take caution against slim people, and induct implements with proper angle. We should avoid excesive force during trocar and Verres’s needle insertion, we also should avoid redundant movement after Verres needle is inserted in abdomen cavity. Elevation of the anterior abdominal wall at the time of Veress or primary trocar insertion is routinely recomended. Major vascular injury is seriously underestimated problem of laparoscopic operations.

Impulsem do opracowania niniejszego artykułu był ponad czteroletni okres czasu, który upłynął od wdrożenia stereotaktycznego manipulatora wieloczynnościowego do usprawnienia operacji laparoskopowych. Urządzenie to zdało egzamin w praktyce chirurgicznej, zarówno w technice laparoskopowej, do której w założeniach było przewidywane, jak również, znalazło szerokie zastosowanie podczas operacji z otwartymi powłokami jamy brzusznej. W ramach pracy przedstawiono budowę własnej konstrukcji manipulatora usprawniającego pewne operacje laparoskopowe. Składa się ono z dwu głównych części, z których jedna służy do unoszenia powłok brzusznych a druga do mechanicznego mocowania narzędzia chirurgicznego oraz przytrzymywania struktur tkankowych. Wykonane urządzenie przeszło próby kliniczne w czasie operacji laparoskopowych u pacjentów z obciążeniami internistycznymi, u których tradycyjna odma gazowa jest przeciwskazana. Urządzenie to stosowano zarówno do unoszenia powłok w warunkach laparoskopii bezgazowej (laparolift) jak i do mechanicznego umocowania narzędzia chirurgicznego w czasie operacji z odmą z użyciem CO2 a także do operacji bez odmy na otwartych powłokach brzusznych zastępując jednego asystenta ("trzecia ręka chirurga"). Wykonane operacje w pełni potwierdziły przydatność kliniczną wykonanego manipulatora.
The new construction for supporting laparoscopy surgery, called "multifunction stereotactic manipulator" was presented. It consists of two parts. The first one laparolift is useful for laparoscopic surgery without pneumo-peritoneum. This part has changeable tools which in various anatomical cases enable its use. The second part of the unit, called "the third hand of surgeon", is used in laparoscopy operations and classic surgery to help fix the internal tissue structures during operation. Both parts of device can work together or separately. Preliminary clinical applications confirmed its usefulness.

It has recently been shown that micropauses during long surgical procedures can be beneficial for surgeons’ precision and fatigue. The aim of the study was to evaluate the impact of micropauses on surgical precision measured by a simple smartphone application. Material and methods. Two surgeons performed 40 simple laparoscopic procedures (appendectomy and cholecystectomy) with or without micropauses. After the operation the precision of surgical movements was measured by a simple smartphone application in which the number of successful trials and their mean time were used as a precision surrogate. Results. Mean number of successful trials was significantly higher for appendectomy than for cholecystectomy (5.59 vs 4; p = 0.032). There was a difference between participating surgeons both in terms of number of successful trials (5.80 vs 3.55; p = 0.01) and a mean time of all successful trials (10.03 vs 6.28; p = 0.001). No other statistically significant differences were identified. Conclusion. Micropauses had no influence on surgical precision as evaluated after short laparoscopy procedures. The only differences were surgeon-dependent and intervention-dependent.

We report a case of patient with stage IIIb gastric cancer qualified for laparoscopy - assisted gastrectomy and our first impressions about this procedure. Total gastrectomy with complete omentectomy and extended lymphadenectomy (D2) was performed laparoscopically. The intestinal continuity was restored in a Roux-en-Y mode extracorporeally through the abdominal access system. The orogastric tube with anvil of the circular stapler was transorally introduced into the esophagus. Subsequently, intracorporeal stapling esophagojejunostomy was performed. There were no complications after the operation and the patient was discharged in good shape. Oncological radicality was sufficient and patient has undergone chemotherapy treatment.

Adnexal tumors are among the most frequent disorders of the female genital system. According to current estimates, they affect up to 10% of all women. Recently, the role of laparotomy in surgical techniques in gynecology increased considerably, while laparoscopy performed for an adnexal tumor is one of the commonest gynecologic procedures. The aim of this study was a retrospective analysis of 307 laparoscopic procedures performed due to adnexal tumors. In this population of patients, in 2 instances the procedure was converted from laparoscopy to laparotomy because of a suspected malignancy, subsequently confirmed by histological study, while in 2 other cases malignancy was diagnosed post-hoc and a repeat surgery was necessary. The incidence of various types of ovarian lesions was analyzed, whereby the most frequently encountered conditions included simple, endometrioid and dermoid cysts, in decreasing order. No serious laparoscopy- associated complications were noticed and all patients were discharged from hospital on the first postoperative day. The authors highlight the importance of precise diagnosis of adnexal tumors prior to surgery and role of sonographic study, as the basic diagnostic tool used to determine the nature of lesion. Sensitivity and specificity of this study in the diagnosis of adnexal tumors is discussed, as well as predictive value of the non-specific marker CA-125, which proved useless when suspecting endometrial tumors. At present, laparoscopy is the technique of choice in the diagnosis and treatment of benign genital lesions in the females. Nevertheless, adequate qualification of patients to surgical treatment is still the key issue.
Guzy przydatków należą do najczęstszych chorób żeńskiego układu rozrodczego. Szacuje się, że występują one nawet u 10% populacji kobiet. W ostatnich latach wykorzystanie laparoskopii w technikach operacyjnych w ginekologii znacznie się zwiększyło, a wykonywanie laparoskopii z powodu guzów przydatków należy do najczęściej wykonywanych operacji ginekologicznych. Celem niniejszej pracy była retrospektywna analiza 307 operacji wykonywanych z powodu guzów w obrębie przydatków. Spośród analizowanych chorych w dwóch przypadkach dokonano konwersji do laparotomii z powodu podejrzenia o proces nowotworowy złośliwy, w dwóch kolejnych przypadkach zmianę złośliwą rozpoznano pooperacyjnie i konieczna była kolejna interwencja. W pracy przeanalizowano częstość występowania poszczególnych rodzajów zmian w jajnikach. Najczęściej stwierdzano torbiele proste, w kolejności endometrialne i dermoidalne. Nie zanotowano poważnych powikłań związanych z wykonywanymi zabiegami, a wszystkie pacjentki opuściły szpital w pierwszej dobie po operacji. W pracy zwrócono szczególną uwagę na dokładną diagnostykę guzów przydatków przed kwalifikacją do zabiegu. Omówiono przydatność badania ultrasonograficznego jako podstawowego badania przy opracowywaniu charakteru zmian. Krytycznie przedyskutowano czułość i specyficzność tego badania w diagnostyce. Ustosun kowano się do wartości predykcyjnej niespecyficznego markera CA-125, który jest nieprzydatny w podejrzeniu guzów endometrialnych. W chwili obecnej laparoskopia jest metodą z wyboru w diagnostyce i leczeniu niezłośliwych zmian narządu rodnego. Jednak na szczególną uwagę zasługuje właściwa kwalifikacja do zabiegu operacyjnego.

Adnexal torsion is a rare emergency condition and its diagnosis is challenging as the clinical presentation is nonspecific. About half of the cases of adnexal torsion are not identified in a timely manner. It is important to undergo prompt surgery to preserve ovarian function. Our article describes the clinical presentation of adnexal torsion and early surgical intervention to preserve the adnexa in a young woman.
Skręt przydatków jest rzadkim stanem nagłym, a jego diagnoza jest wyzwaniem, ponieważ objawy kliniczne są niecharakterystyczne. Około połowy przypadków skrętu przydatków pozostaje nierozpoznana w odpowiednim czasie. Podjęcie szybkiego leczenia operacyjnego jest istotne, aby zachować funkcję jajnika. W artykule opisano objawy kliniczne skrętu przydatków i wczesną interwencję chirurgiczną pozwalającą na zachowanie przydatków u młodej pacjentki.

Wziernikowanie jamy otrzewnowej chorego – czyli laparoskopia jest jedną z technik chirurgicznych, która w ostatnich dwóch dekadach XX wieku odmieniła codzienną praktykę chirurgiczną na świecie. Historia laparoskopii sięga początków XIX wieku, jednak najbardziej dynamiczny jej rozwój przypada na drugą połowę XX wieku. Zawdzięczamy to zarówno pracy pionierów technik małoinwazyjnych jak i rozwojowi techniki. Początki laparoskopii w Krakowie przypadają na początek lat 90. XX wieku. Pierwsze dwa tego typu zabiegi wykonano przez polskich chirurgów przy okazji pobytu w Krakowie jednego z polskich pionierów tego typu zabiegów – E. Stanowskiego, 11.06.1992 przez A. Bobrzyńskiego w II Klinice Chirurgii Akademii Medycznej w Krakowie oraz dzień później - 12.06.1992 przez A. Cienciałę w Klinicznym Oddziale Chirurgii Ogólnej Wojskowego Szpitala Klinicznego w Krakowie. Technika ta stopniowo zyskiwała na popularności i dzisiaj jest standardowo wykorzystywana w małopolskich oddziałach chirurgicznych. Chirurdzy jednak nie przestali poszukiwać coraz to nowych technik operacyjnych, które mają zminimalizować jeszcze bardziej uraz operacyjny oraz zapewnić pacjentowi maksymalnie skuteczną i efektywną operację. Techniki te wywodzą się w dużym stopniu bądź stanowią swego rodzaju rozwinięcie klasycznej laparoskopii. Wśród nich należy wymienić SILS (Single Incision Laparoscopic Surgery), NOTES (Natural Orifi ce Translumenal Endoscopic Surgery), operacje hybrydowe oraz roboty chirurgiczne.
One surgical technique that has changed every-day surgical practice in the last two decades of the 20th century is laparoscopy. This technique dates from the beginning of the 19th century, but the most dynamic development was in the second half of the 20th century. We are indebted to the work of pioneers of minimally invasive surgical techniques and the development of technology. Laparoscopy has developed especially since the early 1990s. The fi rst laparoscopic procedure was performed in Cracow by A. Bobrzyński at the Second Surgery Department of Cracow Medical University (11.06.1992), while the second was performed the next day by A. Cienciała at the Surgery Department of the Fifth Military Clinical Hospital (12.06.1992) with the assistance of E. Stanowski, a Polish pioneer of this technique. This technique became more popular and now is very common in Cracow surgery wards. Surgeons have not stopped searching for new operational techniques which could minimize surgical trauma and maximize the efficacy of operations. These techniques are derived from classic laparoscopy and the following should be mentioned: SILS (Single Incision Laparoscopic Surgery), NOTES (Natural Orifice Translumenal Endoscopic Surgery), hybrid operations and robotic surgery.

This study examines the risk of morbidity for colorectal surgery undergoing patients. The main aim was to identify important risk factors that influence post-operative complications – morbidity, and to create a model to predict possible complications for a patient before surgery. The source data file contains information about 1177 patients who underwent colorectal surgery between 2001 and 2009 at the University Hospital Ostrava, Czech Republic. According to the surgeons’ judgment the following seven independent variables were included in the analysis: Gender, BMI, American Society of Anaesthesiology (ASA) Classification, Stage of Disease, Number of Previous Operations, Surgical Technique and Operation Severity. Discriminant analysis was used for the data evaluation; statistical software SPSS 18 and NCSS 2004 were used for the calculations.

Introduction. Leiomyosarcoma is the most common benign tumor of the bladder. However, it is only a small percentage of all proliferative changes of this organ. Aim. Presentation based on two clinical cases of diagnostics and surgical treatment from laparoscopic access of bladder leiomyoma. Description of the cases. Case 1. The work contains a diagnosis based on transvaginal ultrasound (TVUS), cystoscopy and computed tomography (CT), as well as partial laparoscopic bladder re section of leiomyoma in young women. Case 2. The article demonstrates the important problematic areas associated to the ergonomics of surgical instruments during their usage. For this purpose, an analysis of cholecystectomy surgery with the use of laparoscopic instruments was conducted. There was identified the individual operations performed by surgeon, as well as the particular laparoscopic instruments. The prepared in this way study material, allowed for evaluation of design features of tools used in laparoscopic surgery from functional and ergonomic point of view and developing special research procedure for ergonomic assessment. Particularly, the critical states within the selected body postures in the system were identified: an operator (surgeon) and a surgical tool, as well as the areas of ergonomics corrective intervention in the process of using laparoscopic instruments.

Opis:
Laparoscopic single access technique is a next step in development of minimally invasive surgery. The aim of the study was to present results of different laparoscopic single incision procedures and evaluate application of this technique. Material and methods. 102 patients (15 males and 87 females) who underwent laparoscopic single incision procedure from 15th October 2009 to 31st December 2012 were included in the study. Results. In the analyzed period we performed 72 cholecystectomies (70.6%), 8 left adrenalectomies (7.8%), 3 right adrenalectomies (2.9%), 7 splenectomies (6.9%), 5 spleen cysts unroofings (4.9%), 2 appendectomies (2%), 1 Nissen fundoplication procedure (1%), 1 removal of the adrenal cyst (1%) and 3 concomitant splenectomies and cholecystectomies (2.9%). There were 3 technical conversions to multiport laparoscopy, but no conversion to open technique. Complications were observed in 5 patients (4.9%). Average operation time was 79 min (SD=40), average hospitalization time 2.4 day (SD=1.4). Conclusions. Laparoscopic single incision technique is a safe method and can be used as a reasonable alternative to multiport laparoscopy in different minimally invasive procedures especially in young patients to whom an excellent cosmetic effect is particularly important.

Inguinal hernias generally present with groin lump and pain. Although inguinal hernias can be diagnosed clinically in most cases, patients without a groin lump pose a considerable diagnostic challenge. The first-line diagnostic imaging tool in these cases is ultrasound (US) and the recommended surgical procedure is laparoscopic-endoscopic repair. This retrospective study aims at evaluating postoperative results and complication rates of TEP technique in patients with occult contralateral hernias diagnosed with US in comparison to patients with clinically diagnosed hernias. A retrospective study was conducted to evaluate the outcomes of TEP procedure in patients with radiologically diagnosed occult contralateral hernias in comparison to patients with clinically diagnosed hernias. All hernias included in this study were repaired by TEP technique and secured with an extraperitoneal mesh. Demographic data, patient characteristics and perioperative information were obtained by reviewing medical records. A total number of 109 patients were enrolled in the study. The majority of patients were male and the mean age was 48.9 ± 14.6 years. In 56 cases, hernias were repaired unilaterally, while the remaining 53 were repaired bilaterally. Right-sided hernias were more common than left-sided hernias. The morbidity rate was 7.1% in unilateral repairs and 3.8% in bilateral repairs. The recurrence rate was 3.6% for unilateral repairs and 5.7% for bilateral repair. Some studies report that the incidence of clinical contralateral inguinal hernias identified after primary unilateral surgery is approximately 10%. If these contralateral hernias were diagnosed prior to the primary surgery, the risk of performing another operation could be avoided. Laparoscopic surgery enables bilateral hernia repair without any additional incisions, presenting similar morbidity rates when compared to unilateral repair. There was no significant difference between unilateral and bilateral TEP repair in terms of intraoperative and postoperative surgical complications. These results suggest that laparoscopic inguinal hernia repair is a safe and effective surgical technique for both unilateral and bilateral procedures. In order to prevent second operation, all patients with suspected inguinal hernia should undergo an US examination before surgery.

The aim of the study was to present early outcomes of liver resection using laparoscopic technique. Material and methods. Retrospective analysis of patients who underwent liver resection using laparoscopic method was conducted. The analyzed group included 23 patients (11 women and 12 men). An average patient age was 61.3 years (37 – 83 years). Metastases of the colorectal cancer to the liver were the cause for qualification to the procedure of 15 patients, metastasis of breast cancer in 1 patient and primary liver malignancy in 5 patients. The other 2 patients were qualified to the liver resection to widen the surgical margins due to gall-bladder cancer diagnosed in the pathological assessment of the specimen resected during laparoscopic cholecystectomy, initially performed for other than oncology indications. Results. Hemihepatectomy was performed in 11 patients (9 right and 2 left), while the other 12 patients underwent minor resection procedures (5 metastasectomies, 4 nonanatomical liver resections, 1 bisegmentectomy, 2 resections of the gall-bladder fossa). An average duration of the surgical procedure was 275 minutes 65 – 600). An average size of the resected tumors was 28 mm (7 – 55 mm). In three cases conversion to laparotomy occurred, caused by excessive bleeding from the liver parenchyma. Postoperative complications were found in 4 patients (17.4%). Median hospitalization duration was 6 days (2 – 130 days). One patient (4.3%) was rehospitalized due to subhepatic abscess and required reoperation. Histopathology assessment confirmed radical resection (R0) in all patients in our group. Conclusion. Laparoscopic liver resections seem to be an interesting alternative in the treatment of focal lesions in the liver.

Laparoscopic surgery has become a well approved method of abdominal hernias treatment in recent years. Due to the advancement of laparoscopy and the use of improved synthetic materials laparoscopic surgery is characterized not only by low complication but also by a short period of recovery after surgery. The aim of the study was a retrospective analysis of the results of laparoscopic abdominal hernia surgeries (IPOM). Material and methods. Between year 2007 and 2012, 65 patients aged between 29 to 76 underwent laproscopic abdominal hernia surgeries due to either primary or postoperative abdominal hernias. All patients were examined in perioperative period, after 12 and 24 months after surgery in search of complications, pain and reccurence. Recovery period was also estimated. Results. In most cases postoperative pain was estimated from 1 to 4 on VAS scale. The most frequent complications were seromas that occured in 3 patients. The other complications were pneumothorax, wound hematoma and wound infection that occured once each. One patient required reoperation due to wound hematoma. Chronic postoperative pain was diagnosed in 3 patients and 4 recurrences were stated. Conclusions. Laparoscopic therapy of abdominal hernias is a safe operative method characterized by low recurrence and complication rates as well as short hospital stay and quick recovery. This technique is restricted by high material costs and the lack of full refund for the procedure.

For many years, laparoscopic cholecystectomy remains the method of choice for both the treatment of symptomatic cholelithiasis, and chronic and acute cholecystitis (1). The experience of the surgeon grows with each laparoscopic procedure, which enables to operate in case of difficult anatomical conditions and associated anatomical variants. The aim of the study was to present a case of a 47-year old male patient with total situs inversus and several months history of recurrent left epigastric pain, radiating to the left scapula, being accompanied by nausea and vomiting. The study presented the operative technique of laparoscopic cholecystectomy and postoperative period data. In conclusion, laparoscopic cholecystectomy in a patient with total situs inversus is possible and safe, providing relevant precautions. The main issues certainly include a good and feasible plan of the operation, discussion concerning the possible intraoperative and postoperative complications, a good plan considering the localization of the trocars, as well as an experienced surgical team. One should also not forget that early conversion to classical cholecystectomy is not considered as failure, but might prevent accidental damage of the biliary ducts and long-term complications.

The abdominoperineal resection of the rectum is a classical operation performed in case of patients diagnosed with rectal cancer. The development of laparoscopic techniques in recent years, introduced yet another method of treatment, considering patients with rectal cancer- laparoscopic abdominoperineal resection of the rectum. The aim of the study was to present initial treatment results considering the above-mentioned patients. Material and methods. The study group comprised 25 patients (16 male and 9 female) diagnosed with low-rectal cancer, subjected to surgery by means of the above-mentioned method. Mean patient age amounted to 66 years. Three (12%) patients required conversion to classical surgery (laparotomy), while one patient required reoperation, due to presacral vascular bleeding. Complications were observed in 10 (40%) patients. Average hospitalization was 7 days. In case of all patients the radial margin was negative, and mean number of removed lymph nodes amounted to 9.6. Mortality was not observed during the perioperative period. Due to the initial character of the study analysis (mean observation period in case of 68% of patients was shorter than 2 years), oncological results were not subject to evaluation. Conclusions. Laparoscopic abdominoperineal resections are considered as technically difficult operations, requiring significant experience of the operating team. However, they enable the patient to take advantage of the many assets of minimally invasive surgery, with comparable rates of postoperative complications.

Background: Repair of large, upper thoracic, cuff-induced, tracheo-esophageal fistula (TEF) is technically demanding and is conventionally performed by open surgery. Minimal access approach is, hitherto, unreported. Technique & Case: Minimally invasive repair of TEF involving fistula isolation – by thoracoscopic oesophageal exclusion, and simultaneous establishment of alimentary continuity – by laparoscopy-assisted sub-sternal colonic transposition, is described. The technique was successfully employed in repairing a large (4.5 centimetres), cuff-induced, upper thoracic TEF, in a 25-year-old woman. The rationale behind the technique, its pros and cons are analysed and contrasted against conventional techniques of TEF repair. Conclusion: Large upper thoracic, cuff-induced TEF can be successfully repaired employing minimal access.

Introduction: The prevalence of obesity in Poland and worldwide is constantly rising. High effectiveness of bariatric surgery has been proven in literature. It is recommended that bariatric procedures should be done by highly qualified surgeons with the appropriate, up-to-date medical equipment. Aim: The purpose of the study is to establish Polish recommendations and standards for the use of medical equipment for bariatric surgery centers. Materials and methods: The review of the present recommendations of the worldwide organizations and societies (including EAES, IFSO, SAGES) and guidelines was made. On the basis of current literature and authors’s clinical experience we proposed standardized protocol for bariatric surgical equipment. Conclusions: Relevant equipping of bariatric surgery centers and implementation of standardized perioperative and surgery protocols will result in significant improvements in bariatric treatment. This will ensure patients safety, a shorter length of hospital stay and considerably reduce the risk of morbidity. Moreover, it will contribute to the efficacy of the bariatric and metabolic surgery procedures, in accordance with the highest globally accepted standards.

The authors reported a case of small bowel obstruction secondary to internal herniation through a defect of the broad ligament. This is a rare cause of small bowel obstruction. A 42 year-old woman without previous abdominal surgery presented with episodes of abdominal pain, nausea and vomiting. Due to rather unspecific and changing symptoms and the fact that objective findings were normal the patient was discharged and admitted a couple of times before a computed tomography-scan showed small bowel obstruction. Acute diagnostic laparoscopy showed internal herniation of a loop of small bowel trapped in a 2 cm defect in the right side of the broad ligament. The defect was gently expanded bluntly and it was then possible to retract the strangulated part of the small bowel which afterwards was vital. The defect was sutured laparoscopically. The postoperative recovery was uneventful. This case demonstrates that a preoperative diagnosis is difficult and a high index of suspicion is necessary to prevent diagnostic delay. Internal herniation through a defect in the broad ligament should be considered as a differential diagnosis in female patients presenting with symptoms of small bowel obstruction without a history of prior abdominal surgery, especially in parous women.

Introduction: In December 2019 following an outbreak of Novel coronavirus infection (COVID-19) in Wuhan, China, it spread rapidly overwhelming the healthcare systems globally. With little knowledge of COVID-19 virus, very few published reports on surgical outcomes; hospitals stopped elective surgery, whilst emergency surgery was offered only after exhausting all conservative treatment modalities. Aim: This study presents our experience of outcomes of emergency appendectomies performed during the pandemic.Methods: Prospectively we collected data on 132 patients in peak pandemic period from 1st March to 5th June 2020 and data compared with 206 patients operated in similar period in 2019. Patient demographics, presenting symptoms, pre-operative events, investigations, surgical management, postoperative outcomes and complications were analysed. Results: Demographics and ASA grades of both cohorts were comparable. In study cohort 84.4% and 96.7% in control cohort had laparoscopic appendicectomy. Whilst the study cohort had 13.6% primary open operations, control cohort had 5.3%. Mean length of stay and early post-operative complications (<30 days) were similar in both cohorts apart from surgical site infections (p = 0.02) and one mortality in study cohort. Conclusion: In these overwhelming pandemic times, although conservative treatment of acute appendicitis is an option, a proportion of patients will need surgery. Our study shows that with careful planning and strict theatre protocols, emergency appendicectomy can be safely offered with minimal risk of spreading COVID-19 infection. These observations warrant further prospective randomised studies.
