Purpose The aim of this study was to determine frequency and distribution of sports- and recreation-related injuries (SRIs) affecting children and adolescents who visited a northern tier regional medical center emergency department during a 15-year period. Material and methods A descriptive epidemiologic design was employed to retrospectively examine age, gender, month and year of injury, location of injury, sport/recreational activity, mechanism of injury, type and severity of injury, hospital admission and length of stay. Frequency of reported injuries were compared in categories of single factors using Chi-square tests of homogeneity. The impact of risk factors – gender, age class, and sport/activity – on incidence ratios were analyzed via Poisson regression. All statistical analyses were run in R. Results Findings heretofore unreported or inconsistent with previous emergency department (ED) studies include a peak injury occurrence of SRIs during September; a preponderance of head/neck injuries and fractures and a higher percent of admitted patients; frequent occurrence of ice hockey injuries; increased severity of injury during ages 10-14; and a trend during 2000-2014 showing increased injury rate of various types of recreational injuries. Findings consistent with previous ED studies included variable distribution of injuries by age, gender and sport/activity; increased frequency of SRIs during ages 12-15 years and during the warmer months of the year; and trends of increased frequency of SRIs affecting females, increased injury rate of closed head injuries, and decreased occurrence of bicycle injuries during the study period. Further to these findings, several suggestions are made to inform and guide local injury prevention efforts and further research. Conclusions Our study results provide information on a region-specific occurrence and distribution of SRIs in a northern tier hospital catchment area that can be valuable to guide regional injury prevention efforts and further research to evaluate specific patterns identified and success of prevention efforts.

Objectives Transient exposure with acute effect has been shown to affect the risk of occupational injuries in various industrial settings and at the healthcare workplace. The objective of this study has been to identify transient exposures related to occupational injury risk in an Italian teaching hospital. Material and Methods A case-crossover study was conducted among the employees of the University Hospital of Udine who reported an occupational injury, commuting accident, or incident involving biological risk in a 15-month period in the years 2013 and 2014. The matched-pair interval approach was used to assess the role of acute sleep deprivation whereas the usual frequency approach was used for other 13 transient exposures. Results Sleep hours were not associated with the risk of injuries whereas a significant risk increase was associated with fatigue, rush, distraction, emergency situations, teaching to or being taught by someone, non-compliant patients, bloody operative/work field, excess noise, complex procedures, and anger. Conclusions We identified transient exposures that increased the risk of occupational injuries in an Italian teaching hospital, providing indications for interventions to increase workers’ safety at the healthcare workplace. ObjectivesPolice officers run a risk of injury that is higher than in most other occupations. This study aims to quantify injury prevalence and identify common musculoskeletal injuries (MSIs) among police officers, using injury data from a municipal police service in Alberta, Canada.Material and MethodsThis is a descriptive study based on a secondary data analysis of the MSIs reported to the police service over a 41-month period; January 1, 2013 – June 2, 2016. Data from 1325 active police officers were examined, and injury prevalence was reported according to sex, injury diagnosis, the body part injured, and the work area.ResultsThe prevalence of strains and sprains was very high, at 89.2%. The back and shoulder were most frequently affected. Overall, injury proportions did not differ significantly across work areas. The injury risk was age-related but no significant differences in injuries between sexes were identified.ConclusionsMinor injuries such as strains and sprains occur frequently in the police occupation. Future research should focus on specific risk factors for MSIs in police officers in order to aid prevention.

Biomechanical analyses seek to improve understanding of the mechanisms of knee injury and to find ways to reduce knee injury incidence. Many clinical biomechanics researchers use a standard kinematic adopted from Newington Children’s Hospital. Biomechanical studies include the knee joint configurations, where joint architecture immutably constrains and guides movement outcomes. Investigators will default to reporting resultant joint deformation without considering the influence of joint architectural configurations on kinematic responses. The purpose of this study was to develop a new joint angular kinematic method that accounts for influence of dynamic joint architectural configuration on deformation values. Twenty subjects performed unloaded dynamic flexion/ extension and 45° cutting maneuver. The knee deformation angles obtained with the new method proposed were compared with the values that obtained using the standard method. One way repeated measurement ANOVA’s was used to compare knee deformation angles values from the standard method that uses a static trial and the new method that uses a dynamic trial. The proposed method distinguishes between dynamic joint architectural configuration and joint deformation. Loaded standard abduction/adduction (β) and rotation (γ) angles were 3.4 ±1.8° and 11.2 ±3.8°, respectively. Using the new method, the β and γ angles decrease to 1.5 ±1.4° (<0.05) and 7.1 ±1.8° (<0.05) during cutting. The new method accounted for dynamic joint architectural configuration produced loaded β and γ angles that had smaller magnitudes than the standard method, suggesting that previous studies may have overestimated β and γ angles. Injury management strategies could be influenced by a consideration for dynamic joint architectural configuration. Such a consideration could influence ligament repair strategies. Future studies should account for dynamic configuration when establishing the influence of joint deformation on graft design and appropriate isometry values during reconstruction.

Background The profession of a firefighter is fraught with a significant risk of injuries. Firefighters operate in hazardous zones to mitigate threats and combat fires. Aim of work is to evaluate the accident rate of the officers of the Polish State Fire Service in connection with their service, considering direct participation in rescue operations, and other official duties between the years 2015–2022. Material and Methods An 8-year analysis was conducted from reports covered the number of accidents, the number of injured firefighters, the age of the officers, the circumstances of the event, and the type and location of the injury. This includes fire and rescue operations, exercises, training, official trips, sports activities, and technical work related to equipment. Results Between 2015 and 2022, there were 12 588 individual and group accidents in various circumstances. Most firefighter accidents occurred during rescue and firefighting operations, as well as during sports activities which dominate as the cause of injuries among firefighters in the observed period (37.8%), and injuries related to rescue and fire-fighting activities constitute 28.5%. Other work activities of firefighters cause 33.7% of injuries. In 2020 was a clear reduction in the number of accidents per 1000 employed firefighters in each observed parameter (total, interventions, sport) while maintaining the average level of firefighter employment (M±SD 30 099±224). Conclusions The most frequent circumstances of injury are slippery and challenging spaces, surfaces, and carelessness. The most common type of body injury is a dislocation and sprain concerning the ankle joint, foot. The most frequent causes of work-related injuries for firefighters, as the results indicate, are participation in team sports and fire-fighting and rescue operations. The most commonly occurring medical consequences resulting from a firefighter’s injury are fractures, sprains, and contusions of various body areas. Firefighters are most burdened with injuries in the age group 26–35 years old.

INTRODUCTION: Road accidents lead to injuries which are one of the most common causes of hospitalisation in trauma centres. Proper placement and number of EMS units determine quick arrival in the scene of accident and need for transport to the adequate healthcare entity. MATERIAL AND METHODS: An analysis was carried out of the profile of the patient, the distance to the scene of accident as well as the allocation of accident casualties to healthcare entities. A statistical analysis was conducted by means of the Chi-Square Test of Independence and the Kruskal-Wallis test by ranks. RESULTS: Distances between the scenes of accidents and places where EMS units were stationed was from 0.5 to 37 kilometres (mean: 9.8 km). 170 patients participated in the accidents. Most frequently, these were males at the age from 21 to 40 (mean: 35.77 ± 15.45). The most common diagnoses according to ICD-10 were injuries to the head (42.7%) and injuries of lower limbs (12%). In most cases, the accident casualties were transported to the Emergency Department in Łuków. 21 patients in total were placed in hospital facilities outside the catchment area. The Polish Medical Air Rescue transported the 10 patients in the most severe condition. CONCLUSIONS: Road accidents are most frequently experienced by middle-aged men. Injuries to the head and of lower limbs are the most common injuries. The placement of EMS units in the area under analysis is optimal, but the mean distance covered by the ambulance to reach to scene of accident is nearly 10 kilometres. Despite the presence of the Emergency Department in Łuków, one in ten injured patient warrants transport outside the catchment area and almost 6% of patients qualify for transport by air.

Introduction. Agriculture is known to be a dangerous industry in Korea, as well as in other countries. According to earlier studies, the root cause of occupational injury can be identified with errors in the various systems of safety, and such identification is helpful for the prevention of occupational injury. Objective. The aims of this study were to examine the root causes of cases of agricultural injury in Korea, based on insurance claims and identification of errors in systems of safety. Materials and method. Using the Korean Mutual Aid Insurance’s injury claim database, 277 injury cases were identified, of which 68 were contacted. Root causes were categorized, using the logic tree diagramming method and the systems of safety described in the literature. Results. Seventy-five percent of all injuries were attributable to falls, strangulation, amputation and collision from flying and falling objects. 194 root causes were found for all injuries. The percentages of errors in all the systems of safety for each root cause were 24.7% – training/procedure, 20.3% – design, 11.9% – mitigation, 9.3% – human factor, 6.2% – maintenance/ inspection, and 1.0% – warning/notification. The percentage of root causes which could not be categorized due to a shortage of information was 18.6%. Conclusions. It was found that most agricultural injuries were caused by a complex layer of root causes which were classified as errors in the systems of safety. This result indicates that not only training and personal protective equipment, but also regulation of safety design, mitigation devices, inspection/maintenance of workplaces, and other factors play an important role in preventing agricultural injuries. The identification of errors will help farmers to implement easily an effective prevention programme.

The objective of the study was to describe the characteristics of agriculture and forestry related injury cases admitted to an Emergency Department (ED), and to asses factors related to injury severity and hospital admission. Retrospective analysis of ED case records in Teaching Hospital No 1 in Lublin, from January 2004 to December 2005 were utilized. Inclusion criteria: patient >14 years old, with agriculture and forestry related injuries. Univariate and bivariate descriptive analyses and multiple logistic regressions were performed. 3791 cases were included, 63.1% males, 53.3% cases younger than 30 years and 47.1% of the patients sustained injuries related to machines or falls. After adjusting for age, sex and the presence of multiple injuries, animal related injuries, followed by machine related injuries and falls, were at higher risk of a more severe injury (OR: 1.77, 1.61 and 1.50, respectively). This groups also showed a higher likelihood of hospital admission (OR: 2.03, 192 and 2.00, respectively). Patients attended to in the ED during night hours (OR: 2.06) were also at a higher risk of hospitalization. It was concluded that animal related injuries, agriculture machine operators, and falls, besides accounting for two thirds of analyzed cases, are the mechanism of injury with a greater risk of a more severe injury and higher likelihood of a hospitalization.

Objectives The aim of this study was to evaluate the relationship between high air temperatures and occupational injuries (OIs) occurred during the summer seasons 2000–2013 in agricultural workers from the Autonomous Province of Trento (APT), North-Eastern Italy. Material and Methods Data about OIs for the APT from 2000 to 2013 occurring during the warm season (N = 7325) was provided by the National Institute of Insurance for Occupational Illness and Injury. Daily average and daily maximum temperatures values for the specific geographical site of events were retrieved. Daily temperatures were then assessed in 3 time lags: for the day of the event (lag 0), and for the previous 24 h (lag 1) and 48 h (lag 2). Daily temperatures were then categorized in 3 exposure groups (< 75th, 75–95th and > 95th percentiles). The risk of OIs was assessed as odds ratio (OR) calculated through a Poisson regression model controlled for age, sex, ethnicity and time period, and assuming OI rates for days on which temperature was comprised in < 75th percentile exposure groups as the referent ones. Results Estimated incidence of OIs during the study period was 3.4±2.3 events/day. The peak of work-related accidents occurred on days characterized by severe thermal conditions, and in particular during heat waves (incidence rate ratio = 1.09, 95% confidence interval (CI): 1.02–1.17, p = 0.0165). Days having temperatures higher than 95th percentile, assessed as daily average, both on current days (OR = 1.119, 95% CI: 1.008–1.242) and in lag 1 (OR = 1.125, 95% CI: 1.013–1.249), as well as daily maximum temperatures, were at the highest risk of work-related injuries (OR = 1.144, 95% CI: 1.029–1.272). Conclusions In conclusion, presented findings recommend policymakers to develop appropriate warning/alert systems for agricultural workers regarding high environmental temperatures. W artykule wskazano na duże ryzyko urazów, powodowanych kontaktem z ruchomą piłą przenośnych pilarek łańcuchowych w kontekście szerokiego zastosowania tych maszyn przez profesjonalnych użytkowników i amatorów. Przedstawiono nowe wytyczne norm zharmonizowanych, które pokazują sposoby ograniczenia dostępu operatorów do ruchomej piły łańcuchowej, jak również metody sprawdzania zgodności z tymi wytycznymi. Wybrano proste metody oceny bezpieczeństwa tych maszyn, które mogą być zastosowane przez ich potencjalnych użytkowników. Opisy zilustrowano odpowiednimi rysunkami. Zwrócono jednocześnie uwagę, że w niektórych przypadkach do oceny bezpieczeństwa niezbędne jest jednak zastosowanie bardziej złożonej i skomplikowanej aparatury pomiarowej, np. stanowiska do badania odbicia i hamulca pilarki.
In this paper was pointed to high injury risk caused by contact with movable chain of chain saws and wide availability for DIYers and professional users. New guidelines of harmonized standards were presented, which show limitation ways of operator access to the moving saw chain but also checking methods of agreement assessment with this guidelines. It was chosen simple safety assessment methods of machines, which can be performer by potential users and also descriptions through suitable drawings were illustrated. It was turned attention, that in some cases to safety assessment more complicated and compiled measurement apparatus have to be applied e g. testing stands for kickback and chain brake.

Gunshots with non-powder weapons are relatively common in children, and most of them occur unintentionally. This paper describes the second known case of an adult male in whom a shot from an airgun resulted in the presence of a projectile within the maxillary sinus. A 47-year-old man was shot inadvertently about 10 years earlier and was asymptomatic at the time of reporting. The presence of a foreign body detected on a routine orthopantomogram was confirmed by a medical interview and a three-dimensional radiological examination. The pellet located in the right maxillary sinus was removed from intraoral access through the anterior wall of the sinus. The course of the surgical procedure was complicated by the projectile dislocation in relation to the position determined by radiological examinations. Postoperative care and a 4-year follow-up period were uneventful. The maxillary sinus may be considered a favorable location for a persistent projectile, which may reside in the maxillofacial region asymptomatically for many years. The foreign body may move loosely in the maxillary sinus, which should be taken into account when planning an operation.

Introduction: Multitrauma is defined as injury involving two or more different body parts, with a condition that at least one of these injuries is life-threatening. They represent serious traumas, requiring treatment in the intensive care units and frequently surgical intervention. Aim: The objective of this study was epidemiological and clinical analysis of patients treated in 2015 year in Multitrauma Centre of the University Teaching Hospital no 1 in Szczecin, and comparison the results with outcomes of similar study conducted in the same Centre in 2007 year. Material: Clinical material comprised medical notes of 82 patients, 52 men (63%) and 30 women (37%), with a mean age of 44 years, who sustained multitrauma injuries. An analysis included causes of traumas, spectrum of injuries, involvement of body parts, methods and outcomes of the treatment. Results: The most common cause of multitrauma was traffic accident – 45 cases (55%), followed by fall from height – 22 (27%) and other mechanism – 15 (18%). The most frequent component of multitrauma made bone fractures (spine, pelvis, limbs) – 64 cases (78%), followed by head traumas – 63 (77%), chest – 53 (65%) and abdominal 30 (36%) injuries. A total of 48 patients (58%) required surgical intervention, the most frequently fixation of bone fractures – 24 patients (29%), repair of abdominal and head injuries – 18 (22%) either. Of 82 treated patients 64 (78%) survived and 18 (22%) died. A mean period of stay in Multitrauma Centre was 23 days for survived patients and 17 days for those who died. Comparing to similar analysis conducted 8 years earlier, a change in involvement of particular body parts comprising multitrauma injury was observed: number of head injuries increased of 14%, number of chest traumas and bone fractures decreased of 21% and 11%, respectively. The survival rate improved of 10%.

Objectives We explored the association of workplace characteristics with occupational injuries and adverse events in an Italian teaching hospital. Material and Methods This ecological study was conducted using data routinely collected in the University Hospital of Udine, Northeastern Italy. Poisson regression models were used to investigate, at the hospital unit level, the association between 5 outcomes, including: occupational injuries, patient falls, medication errors, other adverse events and near-misses, and various characteristics of the units. Results The proportion of female workers in a unit, the average number of sick-leave days and of overtime hours, the number of medical examinations requested by employees, and being a surgical unit were significantly associated with some of the outcomes. Conclusions Despite ecological nature of the study, which does not allow for inferences to be drawn at the individual level, the results of our study provide useful clues to support strategies and interventions directed towards healthier work environments and better patient care in hospitals.

Opis:
The objective of this study was to prevent occupational musculoskeletal injuries. Our workers stacked boxes of ceramics weighing 10–27 kg, making low back pain common in our enterprise. In all the stacking stations, recommended weight limits (RWL) were separately calculated using the revised National Institute for Occupational Health lifting equation. Since the boxes weighed significantly more than the RWL, we developed a new ergonomic design that completely changed the stacking process. The load put on the workers’ waist vertebrae in the new and the old stacking methods was compared to evaluate the success of the new ergonomic design, using Newton’s third law of motion. Thanks to the new ergonomic design, the load on the workers’ vertebrae decreased by 80%. Due to its simple technology and its very low cost compared to robots, the new ergonomic design can be commonly used in enterprises with repeated and constraining stacking.
